Abstract

Die Erfindung betrifft eine Hüftgelenkprothese mit einem Pfannenteil (1) und einem in dem Pfannenteil drehbeweglichen Kopfteil (2). Gemäß der Erfindung weist der Pfannenteil (1) auf seiner dem Kopfteil (2) abgewandten Seite eine gleitfähige Fläche (9) zur drehbeweglichen Lagerung des Pfannenteils (1) in der natürlichen Hüftgelenkpfanne (4) auf. Vorzugsweise ist ein die Drehung des Pfannenteils begrenzender Anschlag an dem Kopfteil oder/und am Beckenknochen gebildet. In letzterem Fall weist der beidseitig polierte Pfannenteil (1) eine flanschartige Randabwicklung (11) zum Anschlag gegen den Beckenknochen auf, wobei diese Randabwicklung in einer Randeinsenkung (12) der natürlichen Gelenkpfanne angeordnet ist.

Description

  • Die Erfindung betrifft eine Hüftgelenkprothese mit einem Pfannenteil und einem in dem Pfannenteil drehbeweglichen Kopfteil.
  • Durch Benutzung bekannte solche Hüftgelenkprothesen weisen einen Pfannenteil auf, der zum Einpressen in die ggf. aufgeweitete, natürliche Hüftgelenkpfanne vorgesehen ist. Auf seiner dem Kopfteil abgewandten Seite ist die Oberfläche des Pfannenteils mit einer rauhen Beschichtung versehen, welche das Anwachsen an das Knochengewebe fördert. Beim Einpressen können bleibende Verformungen der den Kopfteil aufnehmenden Kugelfläche des Pfannenteils auftreten, welche die ordnungsgemäße Funktion der Hüftgelenkprothese beeinträchtigen.
  • Der vorliegenden Erfindung liegt die Aufgabe zugrunde, eine neue Hüftgelenkprothese der eingangs erwähnten Art zu schaffen, deren Pfannenteil sich belastungsfrei implantieren lässt.
  • Die diese Aufgabe lösende Hüftgelenkprothese nach der Erfindung ist dadurch gekennzeichnet, dass der Pfannenteil auf seiner dem Kopfteil abgewandten Seite eine gleitfähige Fläche zur drehbeweglichen Lagerung des Pfannenteils in der natürlichen Hüftgelenkpfanne aufweist.
  • Gemäß der Erfindung erfolgt keine feste Verankerung des Pfannenteils im Beckenknochen. Die beidseitig unter Bildung von Gleitflächen polierte Pfanne lässt sich ohne die Gefahr der Verformung in die natürliche Hüftgelenkpfanne einsetzen. Durch die gleitfähige Außenfläche des Pfannenteils ist eine Beanspruchung der Knorpelschicht in der Hüftgelenkpfanne vermieden.
  • In einer besonders bevorzugten Ausführungsform der Erfindung ist ein die Drehung des Pfannenteils in der natürlichen Hüftgelenkpfanne begrenzender Anschlag gebildet, welcher verhindert, dass der Pfannenteil zu weit aus der Gelenkpfanne austritt und mit seinem Rand umliegendes Gewebe beschädigt.
  • Ein solcher, die Drehung des Pfannenteils begrenzender Anschlag kann am Kopfteil oder/und am Beckenknochen gebildet sein.
  • In letzterem Fall weist der Pfannenteil vorzugsweise eine flanschartige Randabwinklung zum Anschlag gegen den Beckenknochen am Rand der Hüftgelenkpfanne auf. Insbesondere kann diese Randabwinklung innerhalb einer an der Hüftgelenkpfanne gebildeten Randeinsenkung angeordnet sein. Eine durch die Randeinsenkung gebildete Schulter bildet den die Drehung des Pfannenteils in der Hüftgelenkpfanne begrenzenden Anschlag.
  • Vorzugsweise erstreckt sich der Pfannenteil über einen solchen Winkel, dass in jeder Drehstellung, in welcher die Randabwinklung gegen die Schulter anschlägt, die Randabwinklung vollständig innerhalb der Einsenkung verbleibt.
  • In einer weiteren Ausführungsform kann der die Drehung des Pfannenteils begrenzende Anschlag durch eine, insbesondere ringförmige, Aufweitung der Außenfläche einer Gelenkkugelkappe des Kopfteils gebildet sein. Hierzu kann die Kugeloberfläche der Kappe in eine zylindrische oder konische Oberfläche übergehen.
  • In einer alternativen Ausführungsform ist der die Drehung des Pfannenteils begrenzende Anschlag durch den Rand einer Ausnehmung in der Außenfläche einer Gelenkkugelkappe des Kopfteils gebildet, gegen welchen ein von der Innenfläche des Pfannenteils vorstehender Ansatz anschlägt.
  • Umgekehrt kann ein solcher Anschlag durch einen von der Außenfläche einer Gelenkkugelkappe des Kopfteils vorstehender Ansatz gebildet sein, gegen den der Rand einer in der Innenfläche des Pfannenteils gebildeten Ausnehmung anschlägt.
  • Die Erfindung soll nun anhand von Ausführungsbeispielen und der beiliegenden, sich auf diese Ausführungsbeispiele beziehenden Zeichnungen näher erläutert werden. Es zeigen:
  • 1 ein erstes Ausführungsbeispiel für eine Hüftgelenkprothese nach der Erfindung, 2 ein bei der Implantation der Hüftgelenkprothese von 1 verwendbares Fräswerkzeug, und
  • 3 bis 9 jeweils ein weiteres Ausführungsbeispiel für eine Hüftgelenkprothese nach der Erfindung.
  • Die in 1 gezeigte Hüftgelenkprothese weist einen Pfannenteil 1 und ein Kopfteil 2 auf. Der Pfannenteil ist in die im Beckenknochen 3 gebildete, natürliche Hüftgelenkpfanne 4 einsetzbar. Der Kopfteil 2 weist eine zum Aufsetzen auf den bearbeiteten Femurkopf vorgesehene Gelenkkugelkappe 5 und einen mit der Kugelkappe verbundenen zentralen Stift 6 für die Verankerung des Kopfteils im Oberschenkelhalsknochen auf.
  • Die kugelförmige Außenfläche 7 der Kugelkappe 5 ist zur Erhöhung der Gleitfähigkeit poliert. Die Außenfläche 7 liegt im implantierten Zustand der Hüftgelenkprothese gegen eine kugelförmige, polierte Innenfläche 8 des Pfannenteils 1 an.
  • Auch die dem Kugelkopf abgewandte kugelförmige Außenfläche 9 des Pfannenteils 1 ist poliert, so dass eine Gleitfläche gebildet ist, mit welcher der implantierte Pfannenteil 1 gegen die Knorpelschicht 10 in der Hüftgelenkpfanne 4 anliegt.
  • Wie 1 ferner erkennen lässt, ist der Pfannenteil 1 mit einer flanschartigen Randabwinklung 11 versehen. Die Randabwinklung 11 findet Platz in einer Randeinsenkung 12 der Hüftgelenkpfanne, wobei die Randeinsenkung 12 in der Hüftgelenkpfanne eine Ringschulter 13 bildet. Im implantierten Zustand des Pfannenteils 1 nimmt die Randabwinklung 11 z.B. die bei 14 durch Strichlinien angedeutete Position ein.
  • Zur Bildung der Randeinsenkung 12 dient ein in 2 gezeigter Fräskopf 15 mit einer kugelförmigen Stirnfläche 16 und einem Schneidelemente aufweisenden Fräsring 17. Durch den Fräsring 17 ist eine der Ringschulter 13 entsprechende Schulter 25 gebildet.
  • Der Pfannenteil 1 der implantierten Prothese ist auf der Knorpelschicht 10 der Hüftgelenkpfanne 4 gleitbeweglich und damit gemäß Doppelpfeil 26 in der Hüftgelenkpfanne drehbar, wobei diese Beweglichkeit dadurch beschränkt ist, dass die Ringschulter 13 für die Randabwinklung 11 einen Anschlag bildet. Dies verhindert, dass der auf der Knorpelschicht 10 gleit- und damit drehbewegliche Pfannenteil 1 aus der Hüftgelenkpfanne 4 heraustreten und in der Umgebung des Hüftgelenks Verletzungen hervorrufen kann, welche zu Beschwerden führen und schließlich einen weiteren chirurgischen Eingriff erforderlich machen.
  • In dem gezeigten Ausführungsbeispiel tritt der Pfannenteil in Drehpositionen, in welchen er mit seiner Randabwinklung 11 gegen die Ringschulter 13 anliegt, an keiner Stelle aus der eingesenkten Hüftgelenkpfanne 4 hervor, d.h., die der Anschlagstelle diametral gegenüberliegende Stelle der Randabwinklung 11 verbleibt innerhalb der Randeinsenkung 12. So sind Verletzungen in der Umgebung des Implantats weitgehend ausgeschlossen.
  • In den folgenden Figuren sind gleiche oder gleichwirkende Teile mit derselben Bezugszahl wie in den 1 und 2 bezeichnet, wobei den betreffenden Bezugszahlen der Buchstabe a, b usw. beigefügt ist.
  • Ein Pfannenteil 1a der in 3 gezeigten Hüftgelenkprothese erstreckt sich über einen Winkel von etwa 180°. Eine flanscharfige Randabwinklung 11a des in die Hüftgelenkpfanne 4a eingesetzten Pfannenteils ragt bei diesem Ausführungsbeispiels aus der Hüftgelenkpfanne heraus und nimmt z.B. die bei 14a durch Strichlinien angedeutete Position ein. In dieser Position ist die Randabwinklung etwa 2 mm von einer bei 13a durch den Beckenknochen gebildeten Anschlagschulter 13a entfernt.
  • Auch bei dem Ausführungsbeispiel von 3 ist der Bewegungsspielraum des Pfannenteils 1a der implantierten Hüftgelenkprothese durch Anschlag der Randabwinklung 11a gegen die Randfläche 13a begrenzt und Verletzungen des umliegenden Gewebes durch zu weites Heraustreten des Pfannenteils aus der Hüftgelenkpfanne sind ausgeschlossen.
  • Bei dem Ausführungsbeispiel von 4 ist ein Anschlag für einen Pfannenteil 1b durch eine Ringaufweitung 18 an einer Gelenkkugelkappe 5b eines Kopfteils 2b gebildet. In dem betreffenden Ausführungsbeispiel weist die Ringaufweitung 18 eine zylindrische Mantelfläche 19 auf, welche an die kugelförmige polierte Außenfläche 7b der Gelenkkugelkappe 5b angrenzt. Der in der Hüftgelenkpfanne 4b gleitbewegliche Pfannenteil 1b kann sich auf der Außenfläche 7b maximal bis zu dieser Ringaufweitung 18 bewegen. Ein vollständiger Austritt des sich über einen Winkel von etwa 150° erstreckenden Pfannenteils 1b aus der Hüftgelenkpfanne 4b ist dadurch ausgeschlossen.
  • Bei dem Ausführungsbeispiel von 5 ist in der kugelförmigen Außenfläche 7c einer Gelenkkugelkappe 5c eine Ausnehmung 19 gebildet, deren Rand 20 einen Anschlag für einen zentralen, von der Innenfläche 8c eines Pfannenteils 1c vorstehenden Ansatz 21 bildet.
  • Bei dem Ausführungsbeispiel von 6 ragt von einer polierten Außenfläche 7d eines Kappenteils 5d zentral ein Ansatz 22 vor. Der Ansatz 22 greift in eine Ausnehmung 23 in der Innenfläche 8d eines Pfannenteils 1d ein.
  • Bei dem Ausführungsbeispiel von 7 ist eine Innenfläche 8e eines Pfannenteils 1e exzentrisch zur Außenfläche 9e angeordnet, wobei der Kugelmittelpunkt der Innenfläche 8e gegenüber dem Kugelmittelpunkt der Außenfläche 9e zur Hüftgelenkpfanne 4e hin verschoben ist. Exzentrizität könnte auch in umgekehrter Richtung bestehen, d.h. der Pfannenteil am Boden dicker als am Rand sein, wie dies in 9 dargestellt ist. In beiden Fällen sorgt die Form des Pfannenteils dafür, dass ein Austritt des Pfannenteils aus der Hüftgelenkpfanne erschwert ist. Ergänzend könnten Anschläge, wie sie z.B. in den vorangehenden Figuren gezeigt sind, vorgesehen sein.
  • 8 zeigt ein Ausführungsbeispiel ohne jede Maßnahme zur Begrenzung der Beweglichkeit eines Pfannenteils 1f in einer Hüftgelenkpfanne 3f. Eine Innenfläche 8f ist konzentrisch zur Außenfläche 9f eines Pfannenteils 1f angeordnet. Die Gefahr des Austritts des Pfannenteils aus der Gelenkpfanne ist dadurch gemildert, dass er sich über einen verhältnismäßig kleinen Winkel erstreckt und der Rand des Pfannenteils vom Rand der Hüftgelenkpfanne zurückgezogen ist. Dennoch könnte der Winkel auch größer sein und bis zu 180° betragen.

Claims (11)

  1. Hüftgelenkprothese mit einem Pfannenteil (1) und einem in dem Pfannenteil drehbeweglichen Kopfteil (2), dadurch gekennzeichnet, dass der Pfannenteil (1) auf seiner dem Kopfteil (2) abgewandten Seite eine gleitfähige Fläche (9) zur drehbeweglichen Lagerung des Pfannenteils (1) in der natürlichen Hüftgelenkpfanne (4) aufweist.
  2. Hüftgelenkprothese nach Anspruch 1, dadurch gekennzeichnet, dass der Pfannenteil zur Bildung der Fläche (9) auf seiner dem Kopfteil abgewandten Seite poliert ist.
  3. Hüftgelenkprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass Einrichtungen zur Begrenzung der Drehung des Pfannenteils (1) in der Hüftgelenkpfanne vorgesehen sind und ggf. ein die Drehung begrenzender Anschlag (13;18;20;22) gebildet ist.
  4. Hüftgelenkprothese nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass der die Drehung des Pfannenteils (1) begrenzende Anschlag an dem Kopfteil (2) oder/und am Beckenknochen (3) gebildet ist.
  5. Hüftgelenkprothese nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass der Pfannenteil (1) eine flanscharfige Randabwinklung (11) zum Anschlag gegen den Beckenknochen (3) am Rand der natürlichen Hüftgelenkpfanne (4) aufweist.
  6. Hüftgelenkprothese nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass die Randabwinklung (11) zum Anschlag gegen eine am Rand der Hüftgelenkpfanne durch eine Randeinsenkung (12) gebildete Schulter (13) vorgesehen ist.
  7. Hüftgelenkprothese nach Anspruch 6, dadurch gekennzeichnet, dass die Randabwinklung (11) in jeder Drehstellung, in welcher sie gegen die Schulter (13) anschlägt, vollständig innerhalb der Randeinsenkung verbleibt.
  8. Hüftgelenkprothese nach einem der Ansprüche 1 bis 7, dadurch gekennzeichnet, dass der die Drehung des Pfannenteils begrenzende Anschlag durch eine, insbesondere ringförmige, Aufweitung (18) der Außenfläche (7b) einer Gelenkkugelkappe (5b) des Kopfteils (2b) gebildet ist.
  9. Hüftgelenkprothese nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass der die Drehung des Pfannenteils (1c) begrenzende Anschlag durch den Rand einer Ausnehmung (19) in der Außenfläche (7c) einer Gelenkkugelkappe (5c) eines Kopfteils (2c) gebildet ist, gegen den ein von der Innenfläche (8c) des Pfannenteils (1c) vorragender Ansatz (21) anschlägt.
  10. Hüftgelenkprothese nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass der die Drehung des Pfannenteils (1d) begrenzende Anschlag durch einen von der Außenfläche (7d) einer Gelenkkugelkappe (5d) vorragenden Ansatz (22) gebildet ist, gegen den der Rand (24) einer in der Innenfläche (8d) des Pfannenteils (1d) gebildeten Ausnehmung (23) anschlägt.
  11. Hüftgelenkprothese nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, dass die Innenfläche (8e,8g) exzentrisch zur Außenfläche (9e,9g) des Pfannenteils (1e) angeordnet ist.
