Description
In the treatment of mental illnesses with electroconvulsive devices, an alternating current surge is passed through the patient's skull, which, in sufficient dosage, leads to immediate unconsciousness and triggers an epileptic seizure. To avoid surgical incidents, it is desirable to keep the treatment dose as little as possible above the threshold value required to induce convulsions. However, this threshold value differs from patient to patient and shows fluctuations that are still unexplained for one and the same patient.
To set the dose, the treatment duration and current intensity are set to different values in the treatment device. However, this setting does not allow any reliable conclusions to be drawn about the dose actually used, which can easily deviate from the set values as a result of errors in the device and in the treatment circuit as well as fluctuations in the operating voltage.
According to the invention, all these uncertainties can be kept under control in that the cramping device is provided with a device for measuring and subsequently reading off the actual treatment dose. The product of treatment current and treatment time is advantageously used as a measure of the dose used and is displayed. For this purpose, the peak voltage can be measured on a capacitor which, via a charging resistor, is parallel to a resistor through which the treatment current flows.
The invention is illustrated by way of example in the drawing. The electroconvulsive device shown consists essentially of an alternating voltage source which is connected to the treatment circuit with the aid of a timer; furthermore from a measuring device for determining the dose applied in each case.
The time switch 2 is actuated by a push button 1, which can consist of a tube toggle circuit in a known manner, and which holds a relay 3 with its contacts 3a and 3d for an adjustable long time in its working position. Contact 3a is in the patient circuit.
The dose used is measured in such a way that the product of treatment current and treatment time is measured and displayed by a tube voltmeter 4. For this purpose, the alternating voltage drop occurring at series resistors 5, 6 in the patient circuit is rectified by a full-wave rectifier 7 and sent to the measuring device. A voltage corresponding to the treatment current occurs at the full-wave rectifier and is applied to the dose measuring device through the relay contact 3d. The measuring or holding capacitor 8 of the tube voltmeter is short-circuited via a resistor 10 before the treatment current flows through the contact 9a of a changeover relay 9. At the beginning of the current flow time in the patient circuit, a short current impulse is sent through the relay 9 via a capacitor 11, thereby canceling the short circuit of the capacitor 8. It is then gradually charged by the voltage taken from the full wave rectifier, which is dependent on the patient current, until the relay 3 drops out at the end of the treatment period. Its voltage is then a measure of the dose used and is displayed on the tube voltmeter. The display is maintained by the holding capacitor 8 beyond the duration of the current pulse.
The relay 9 is brought back to its old position after a fixed adjustable time by a breakover circuit with capacitor 12, charging resistor 13 and glow tube 14. As a result, the measuring capacitor 8 is discharged and the display of the dose disappears.
The measured dose can also be used to determine the head resistance by comparing it with the then occurring dose display by switching on a resistor 15 instead of the head resistance. On the other hand, the switch-on direction of the resistor 15 to secure and control the dose display.
The invention is not restricted to the example shown. In particular, the dose can also be controlled in another way, e.g. instead of measuring the product of the treatment current and current flow time by measuring these two quantities individually or by writing down the treatment pulse.
