Description
【発明の属する技術分野】
この発明は、2個の椎体間に挿入するための可変の軸方向長さを有する間隔保持具であって、スリーブ状の第1の部材と、全体長さを調節するための、この第1の部材内を案内されかつ第1の部材に対して軸方向で動くことができる第2の部材とを備えた、間隔保持具に関するものである。

【従来の技術】
上記の種類の間隔保持具がEP 0 977 528 A 1で公知である。ここでは、上記両方の部材を畳み込んだ状態で2個の椎体間に入れ、この後手動で所望の長さへと引き離し、次に引き離した状態で固定する。

【発明が解決しようとする課題】
この発明の目的は、先に記載した種類の間隔保持具であって、極めて簡単なやり方で所望の長さへの調節を手術者が遂行できるものを提供することである。

【課題を解決するための手段】
上記の目的は、先に記載した種類の間隔保持具であって、両方の部材がレバーによって互いに接続され、レバーにおける一方の旋回点が一方の部材と接続され、レバーにおける他方の旋回点が他方の部材と接続されることを特徴とする、間隔保持具によって解決される。

【発明の実施の形態】
図1が最もよく示すように、間隔保持具1はスリーブ状の第1の部材2と、この第1の部材内を案内されるスリーブ状の第2の部材3とを含む。これら両方の部材は、図1および図2に示すように最大限に押し込まれ、図3に示すように最大の長さで離れるように動かされる。

図1から図3に示すように、内側の第2の部材3は、外側の第1の部材2の方を向いた第2の部材の外壁に、留め具4を有する軸方向に延びる部分を含み、留め具4は、互いに対し軸方向で隣り合って配置され互いに隣接する複数の窪み5を有し、外側の第1の部材2は、留め具に嵌り込むことができる固定部材6を含む。この固定部材は、両方の部材を所望の位置で互いに対して固定するのに用いられる。

図2および図4で最もよく示されるように、間隔保持具1は、両方の部材を軸方向の位置で互いに対して調整するためのレバー機構を含む。第1の実施例では、このレバー機構は第1のレバーアーム7、第2のレバーアーム8、調整ねじ9、およびねじ付スリーブ10を含む。図2で最もよく示されるように、外側にある第1の部材2は側部に開口11を含み、図1で最もよく示されるように、開口11の横方向の大きさは調整ねじ9の頭部12の直径よりも小さい。図2に示すようなやり方で、調整ねじ9をスリーブ状部材の内部空間へ、軸方向に対して直交するように入れる。ねじ付スリーブ10はこのねじに対してねじ止めされる。ねじ付スリーブ10は、スリーブ状の部材の軸方向で延びる、ねじ付スリーブの両側に、出っ張り13,14を含む。出っ張り13は第1のレバーアーム7との接続に用いられる。このレバーアームは、その第1の端部15で、第2の部材3の壁における2つの対向する場所に装着されるシャフト16によって、このシャフトで旋回できるように装着される。シャフト16は間隔保持具の縦軸に対して垂直に延びる。第1のレバーアームは、シャフト16との接続部と対向する第2の端部で、ピンまたはシャフト18を介して、シャフト18について旋回できるように、出っ張り13と接続される。ピンまたはシャフト18はシャフト16と平行に延びる。

図2に示すように、第2のレバーアーム8は、調整ねじ9について見て第1のレバーアーム7に対して対称に形成または配置される。このレバーアームは、端部15に対応して対向する第1の端部19で、シャフト20を介しこのシャフトで旋回できるように装着される。シャフト20は、第1の部材2の、互いに対向する側壁に端部が装着され、シャフト16と平行に延びる。第2のレバーアームはその端部19に対向する端部で、ピンまたはシャフト21によって、シャフト21で旋回できるように出っ張り14と接続される。

図1および図2に示すように、開口11は、間隔保持具の縦軸と平行の方向に延びる長円形の穴として形成される。この長円形の穴に関して、調整ねじ9がここで往復して動くことができ、このため図2に示す収縮した位置と図3に示す拡張した位置との間で、ねじが長円形の穴内で往復して動くことができるように、この長円形の穴が位置付けられる。

操作に関して、間隔保持具は、図2に示した、軸方向の長さが最小である縮められた状態で、椎骨間に挿入される。次に長さは、調整ねじ9の頭部12の対応するスリットまたは六角開口部へのねじ回しによる係合によって、所望の長さにされるが、これは、調整ねじ9を回転させて、図2に示す、ねじ付スリーブ10が調整ねじ9の自由な端部にある最も端の位置から、頭部に向かってねじ付スリーブ10を動かすことによって行なわれる。ここで両方のレバー7,8は、その収縮した位置から、図3に示す最大限に伸ばした位置へと動かされる。このようにして、両方のスリーブ状の部材2および3は、図2に示す縮めた状態から、図3に示す拡張した状態またはあらゆる中間状態へと動かされる。調整ねじ9とねじ付スリーブ10とのねじ案内により、両方の部材はさしあたり、調整ねじ9の回転によって達した状態に留まる。この状態が最終的なものと見なされると、固定部材6を形成する固定ねじを締めることによる完全な固定が行なわれるが、この目的のためにこの固定ねじは留め具4の窪み5に嵌り込む。

上述の実施例での調整ねじ9の長さに関すると、調整ねじがその自由な端部で第2の部材3の中空の内部空間に達し、かつ第2の部材3の対向する壁に嵌り込まず、こうして第2の部材の動きが全く妨げられないように、調整ねじの長さを選択する。図に示すように、第2の部材3は好ましくは開口22を含み、開口22は、間隔保持具の縦軸と平行に長手方向に伸びる長円形の穴として形成され、さらに、調整ねじ9の自由な端部が、その上でスライドするねじ付スリーブ10および両方の出っ張り13および14、ならびにこれと接続された両方のレバーアーム7および8の端部とともに、図2および図3に示す往復する動きにより、長円形の穴の中で自由に往復して動くことができるように、開口22の長さおよび幅を構成する。こうすれば調整ねじ9がより大きな長さを有することができ、こうしてねじ付スリーブ10の動程が増大し、両方の部材または間隔保持具の拡張能力が大きくなるという目的が達成される。

図5および図6に示す変形した実施例では、第2の部材3、調整ねじ9、第1のレバーアーム7、出っ張り13、および両方のシャフト16,18は、第1の実施例における対応する要素と同様に形成される。

第2の実施例では、長円形の穴11の代わりに、調整ねじを案内する円形の穴23が第1の部材2′の壁に設けられ、その直径は、調整ねじがこの穴の中で回転可能に案内されるように選択される。ねじ付スリーブ10′はただ1つの出っ張り13を含む。

操作に関して、図6に示す畳み込んだ状態と、図5に示す拡張した状態との間の調節は、第1の実施例と同様に調整ねじ9の回転によって、ねじ付スリーブ10′を、図6に示した自由な端部における最も端の状態から頭部へ向けてねじ止めして、所望の程度まで拡張が行なわれる、またはレバー7がほとんど垂直の位置へ動かされるように行なわれる。最終的な固定は、第1の実施例と同様、留め具4の窪み5との協働で固定ねじ6をしっかりと締めることによって行なわれる。

図から見て取れるように、第1および第2の部材の両方の壁は各々、円周方向に配された複数の菱形の開口24を含むように形成される。図に示すように、互いに対向する自由な端部25,26は各々鋸歯状に形成され、これにより、隣接する椎体壁に食い込んで回転しないように安定させることが容易となる。壁にある開口は術後の内生(ingrowing)を容易にする。

以上に記載した実施例では、調整ねじ9、ねじ付スリーブ10およびレバーアーム7,8あるいは7の調整機構は各々、ねじ付スリーブ10を頭部12へ向けて最大限に動かした際に最大の伸展が起こり、ねじ付スリーブ10が頭部12から可能な限り最大の距離を有する際に可能な限り折り畳まれるように形成される。しかしこの機構を逆にして、ねじ付スリーブ10が頭部12からその最大の距離を有する際に可能な限り最大の伸展に達するようにすることもできる。頭部12からの距離が最小のときに高さはその可能な限り最小の値を取る。このような実施例を図7に示す。

Claims (6)
- 可変の軸方向長さを有する、2個の椎体間に挿入するための間隔保持具(1)であって、スリーブ状の第1の部材(2)と、全体長さを調節するための、前記第1の部材内を案内され、第1の部材(2)に対して軸方向で動かすことができる第2の部材(3)とを備え、前記間隔保持具(1)は、両方の部材(2,3)がレバー(7,8)によって互いに接続され、前記レバー(7,8)における一方の旋回点(16)が第1の部材(3)と接続され、前記レバー(7,8)における他方の旋回点(18)が他方の部材(2)と動作的に接続されることを特徴とする、間隔保持具。
- 前記レバー(7,8)が2本のアームを有するように形成され、旋回点(16,20)から離れた両方の端部が、第1の部材(2)の壁の長円形の穴(11)内を案内される調整ねじ(9)上を案内されるねじ付スリーブ(10)と接続されることを特徴とする、請求項1に記載の間隔保持具。
- 他方の旋回点(18)が、第1の部材(2)の壁の穿孔(23)内を案内される調整ねじ上を案内されるねじ付スリーブ(10′)と接続されることを特徴とする、請求項1に記載の間隔保持具。
- 穴(11,23)と対向する側で、第2の部材(3)の壁が、間隔保持具の縦軸と平行の長手方向に延びる長円形の開口を有し、前記長円形の開口には調整ねじ(9)の自由な端部が自由に入り込むことを特徴とする、請求項1から請求項3のいずれかに記載の間隔保持具。
- 第2の部材(3)が、第1の部材(2)の方を向く外壁上で、互いに対して軸方向で隣り合って配置され互いに隣接する複数の窪み(5)を伴う留め具(4)を有する、軸方向に延びる部分を含み、第1の部材(2)が、所望の長さでの固定のための、留め具(4)と協働する固定部材(6)を含むことを特徴とする、請求項1から請求項4のいずれかに記載の間隔保持具。
- 第1および第2の部材(2,3)の壁が、内生を向上させるための、表面上に配された開口(24)を含むことを特徴とする、請求項1から請求項4のいずれかに記載の間隔保持具。
