上記のような円筒状シールドを有する注射針装置においては、 使用に 際して、 釙管をシールドから突出させた所定の状態で、 注射針をシール ドに対して位置規制する機構が必要である。 特に穿刺操作時には、 注射 針がシールド内に押し込まれないように保持可能でなければならない。 また穿刺操作後に、 その状態で注射針を留置させる際に、 注射針をシー ルドに対して所定の位置に保持可能であることが要求される場合もある。 注射針が留置された状態から抜去されないようにシールドを穿刺部に固 定しても、 注射針がシールド内で容易に移動すると、 注射針が穿刺部か ら抜去されてしまう恐れがあるからである。特公平 6— 7 8 6 1号公報、 特開平 5— 3 0 0 9 4 2号公報あるいは米国特許第 4, 1 7 0 , 9 9 3 号明細書に記載された各注射針装置は、 注射針をシールドに対して所定 の状態に保持するための構造を有する。
また、 注射針をシールド内に収納する時には、 注射針がシールドに対 して容易にスライド可能であることが望ましい。 しかしながら、 特公平 6 - 7 8 6 1号公報に記載の構造の場合、 注射針をシールド内に収納す る時には、注射針をシールドに対して保持する力が加わつた状態のまま、 その保持力に抗して注射針をシールド内に引き込まなければならない。 特開平 5— 3 0 0 9 4 2号公報に記載の構造の場合は、 注射針をシール ド内に収納する時に、 注射針を保持する力を弱めることが可能である。 しかしながら、 注射針をシールドに対して保持する力を十分に大きく確 保しながら、 しかも注射針をシールド内に収納する時の力を十分に弱め ることは、 その構造上困難である。 米国特許第 4, 1 7 0, 9 9 3号明 細書に記載の搆造によれば、 そのような条件が充足される。
しかしながら米国特許第 4, 1 7 0 , 9 9 3号明細書に記載の注射針 装置では、 注射針を保持するための構造が障害となって、 注射針がシー ルドから突出した使用状態においてシールドに対する注射針の回動が困 難な構造になっている。 種々の操作に適切に対処できるためには、 注射 針がシールドから突出した使用状態において、 シールドに対して注射針 が回動可能であることが望ましい。 例えば、 穿刺時と穿刺後で針管の刃 面の角度を異ならせるために、 注射針を回動させる場合があるからであ る。 特公平 6— 7 8 6 1号公報または特開平 5— 3 0 0 9 4 2号公報に 記載の構造においても、 注射針がシールドから突出した使用状態におい て、 シールドに対して注射針を回動可能とすることは困難である。 上述の問題は、 翼付き注射針装置を例として説明したが、 翼を持たな い注射針装置についても共通する問題である。 発明の開示
本発明は、 針管がシールドから突出した状態において、 針管のシール ド内への移動が不能であるように位置規制し、 かつその状態で針管をシ 一ルドに対して回動させることを可能とした、 誤穿刺防止用シールド付 き医療用針装置を提供することを目的とする。
本発明の誤穿刺防止用シールド付き医療用釙装置は、 略円筒状のシー ルド筒を本体とする誤穿刺防止用シールドと、 前記シールド筒内に軸方 向に移動可能なように揷入され後端部に輸液チューブを接続可能なハブ と、 前記ハブの先端部に装着された針管とを備え、 前記シールド筒の内 腔に前記針管の先端を収納可能なように構成される。 前記ハブは、 先端 部の外周面の一部に、 前記シールド筒の内径を超えて突出する高さを有 する凸部を有し、 前記シールド筒は、 その内壁に、 先端から後端近傍に 亘つて軸方向に延在するゲート溝が形成され、 前記ゲート溝は、 先端部 において前記ハブ先端部の凸部が嵌合可能な寸法を有する。 前記凸部が 前記シールド筒の先端から露出した状態において、 前記ハブは前記シ一 ルド筒に対して回動可能である。 前記凸部が前記ゲート溝の先端と対向 しない回動位置においては、 前記凸部と前記シールド筒の先端面との係 合により、 前記八ブが前記シールド筒後端部側へ移動不能なように位置 規制され、 前記凸部が前記ゲート溝の先端と対向する回動位置において は、 前記凸部は前記ゲート溝内を摺動可能である。
この構成によれば、 針管がシ一ルドから突出した状態において、 凸部 とシールド筒先端面との係合により、 針管がシールド内へ移動しないよ うに保持させることが可能である。 また、 その状態で針管をシールドに 対して回動させることが可能である。
上記構成において好ましくは、 前記シールド筒は前記ゲ一ト溝の後端 に隣接して形成された係合孔を有し、 前記ゲート溝は、 前記シールド筒 の内壁面からの深さが、 後端部において前記凸部の高さよりも浅くなる ように設定される。 前記凸部は、 前記ゲート溝内を摺動して前記係合孔 に係合可能である。それにより、釙管をシールド筒内に収納した状態で、 針管をシールド筒に対して確実に保持することができる。
前記凸部は、 前記ハブの周方向における、 前記釙管の刃面側およびそ の裏側に対応しない位置に設けることができる。 それにより、 釙管がシ 一ルド内へ移動しないように保持された状態で、 釙管を穿刺操作に都合 の良い状態に設定することができる。 その結果、 穿刺状態で針管がシー ルド内に引き込まれるという不測の事態を防止できる。
また前記凸部は、 前記ハブの周方向における、 前記針管の刃面側に対 応する位置に設けてもよい。 その場合、 凸部をゲート溝に係合させてシ 一ルドに対して針管を回動不能とし、 刃面を上方に向けた際に、 穿刺部 位に対して凸部が当接することがない。 従って、 穿刺に適した状態を確 実に得ることができる。
また好ましくは、 前記シールド筒の側壁から伸びるように設けられた 可撓性を有する係止片と、 前記係止片の位置に対応させて前記シールド 筒に設けられた貫通孔とを更に備え、 前記係止片は側方に突出する突起 を有し、 前記係止片が前記シールド筒に巻き付けられたときに前記突起 が前記貫通孔に揷入されて、 前記シールド筒内での前記ハブの軸方向移 動を規制するように構成される。 それにより、 シ一ルド筒に対してハブ を所望の状態で保持することが可能になる。
また、 前記係止片が前記シールド筒に巻き付けられた状態を保持する ための係止片保持機構を有することが好ましい。
前記誤穿刺防止用シールドは、 前記シールド筒の先端側に結合した一 対の翼部を更に備えた翼付きシールドとすることができる。
好ましくは、 前記各翼部は、 その翼面から突出するように形成された 翼突起を有し、 前記シールド筒は、 前記各翼突起を挿入可能なように側 部の円筒壁に形成された貫通孔を有する。 前記両翼部を前記シールド筒 側面に沿って重ね合わせることにより、 前記貫通孔を通して前記各翼突 起を前記シ一ルド筒内腔に挿入可能であり、 それにより、 前記各翼突起 が前記ハブの前記シールド筒内での軸方向移動を阻止して、 前記シ一ル ド筒先端から前記針管が所定長さ突出した状態で、 前記ハブが前記シ一 ルド筒に保持される。 図面の簡単な説明

(実施の形態 1 )
図 1は、 本発明の実施の形態 1における医療用針装置の平面図、 図 2 は図 1の左側から見た側面図である。 図 3 Aは、 図 1の A— A断面の形 状を示す断面図である。 図 3 Bは、 図 3 Aの状態から収納動作を行った 後の状態を示す断面図である。
1は針管であり、 樹脂製のハブ 2の先端部に固定されている。 ハブ 2 の後端部にはチューブ 3が接続されている。 4はシールドであり、 樹脂 製の略円筒形状を有するシ一ルド筒 4 aを本体とする。 ハブ 2は、 シ一 ルド筒 4 aの内腔に挿入されて、 先端部および後端部をシールド筒 4 a から突出させることが可能な長さを有する。 またハブ 2の外径は、 シ一 ルド筒 4 aの内壁との間に、 シールド筒 4 aに対する回動および軸方向 の移動が可能な程度の隙間が形成される大きさを有する。
シールド筒 4 aの先端から露出したハブ 2の先端部には、 外周面の一 部に凸部 1 1が形成されている。 凸部 1 1のハブ 2の外表面からの高さ は、 図 2に示すように、 シールド筒 4 aの内径を超えて凸部 1 1が突出 するように設定されている。シールド筒 4 aの外表面には畝状部 1 2が、 図 1に示すように、 先端から後端近傍に亘つて軸方向に延在して形成さ れている。 畝状部 1 2の内壁にはゲ一ト溝 1 2 aが、 シールド筒 4 aの 先端から後端近傍に亘つて、 軸方向に延在して形成されている。 ゲート 溝 1 2 aは、 先端部において凸部 1 1が進入可能な寸法を有する。 ゲ一 ト溝 1 2 aの後端に隣接して、 シールド倚 4 aの管壁を貫通する係合孔 1 3が形成されている。 係合孔 1 3は、 凸部 1 1が嵌合可能な寸法を有 する。 なお、 ゲート溝 1 2 aを形成するためには、 必ずしも畝状部 1 2 を設ける必要はない。 すなわち、 シ一ルド筒 4 aが十分な厚さを有する 場合は、 ゲート溝 1 2 aをシールド筒 4 aの内壁に直接形成することが できる。
図 3 Aから判るように、 凸部 1 1は、 ゲート溝 1 2 aの先端から進入 してゲート溝 1 2 a内を摺動可能である。更に図 3 Bに示されるように、 凸部 1 1はゲート溝 1 2 aの後端を通過して、 係合孔 1 3に係合するこ とが可能である。 その状態においては、 針管 1の先端はシールド筒 4 a の内腔に収納される。
またゲート溝 1 2 aは、 シ一ルド筒 4 aの内壁面からの深さが、 後端 に向かって浅くなるように傾斜しており、 後端においては凸部 1 1の高 さよりも浅くなつている。 従って凸部 1 1がゲート溝 1 2 a内を摺動す るとき、 後端に近づくに従い、 凸部 1 1の上面がゲート溝 1 2 aの底面 に強い力で当接する状態になる。 従って凸部 1 1は、 ゲート溝 1 2 aの 後端を経由するときには弾性変形により高さ方向に圧縮変形され、 係合 孔 1 3に係合することにより押圧力が開放されて高さが回復する。 その 状態においては、 凸部 1 1の先端面と畝状部 1 2の後端面との当接によ り、 凸部 1 1は先端側への移動、 すなわち逆戻りが不能となる。 また凸 部 1 1の後端面と係合孔 1 3の後端面との当接により、 凸部 1 1の後端 側への移動も阻止される。 このようにして、 凸部 1 1と係合孔 1 3の係 合により、シールド筒 4 に対してハブ 2をロックする機能が得られる。 その結果、 針管 1がシールド筒 4 a内に収納された状態が保持され、 誤 穿刺を防止した状態が得られる。 ゲ一ト溝 1 2 aの傾斜は、 ゲート溝 1 2 aの長さ全体に亘つて設けられても、 あるいは後部にのみ設けられて もよい。
以上の構成から明らかなように、 図 1に示される凸部 1 1がシールド 筒 4 aの先端から露出した状態において、 ハブ 2はシ一ルド筒 4 aに対 して回動可能である。 それにより、 図 4 Aに示すように、 凸部 1 1がシ 一ルド筒 4 aの側方に向いた状態にすることができる。 図 4 Bは、 図 4 Aの状態を断面で示す図である。 八ブ 2がシールド筒 4 aに対して回動 可能であることにより、 シ一ルド 4に対する釙管 1の刃面 1 aの回動位 置を自由に調整可能であり、 穿刺後の種々の操作に対処することができ る。
また図 4 Aの状態においては、 凸部 1 1がゲート溝 1 2 aの先端と対 向していない。 この回動位置においては、 凸部 1 1の後端面とシールド 筒 4 aの先端面との係合により、 八ブ 2はシールド筒 4 aの後端側への 移動を阻止される。 従って、 例えば穿刺操作の際に凸部 1 1の回動位置 をそのように調整することにより、針管 1がシールド筒 4 aから突出し、 シールド筒 4 a内に押し込まれることのない状態を保持することができ る。
以上のように、 針管 1がシールド筒 4 aの先端から所定長さ突出した 使用位置において、凸部 1 1とシールド筒 4 aの先端面との係合により、 針管 1を取り付けたハブ 2がシールド筒 4 a内で位置規制される。また、 チューブ 3の端面がシールド筒 4 aの後端に当接すると、 ハブ 2をシー ルド筒 4 aの先端の向きに更に移動することが阻止される。 それにより 釙管 1が、 所定長さ以上シールド筒 4 aから突出しないように位置規制 される。 それらの結果、 ハブ 2を使用位置へ保持する機能が得られる。 使用後に医療用針装置を廃棄する際には、 凸部 1 1の回動位置をゲ一 ト溝 1 2 aに合わせることにより、 ハブ 2をシ一ルド筒 4 aの後端側に 容易に移動させ、 図 3 Bに示したように、 その位置で保持することが可 能である。
ハブ 2の周方向における、凸部 1 1と釙管 1の刃面 1 aの位置関係は、 用途に応じて設定される。 例えば、 図 4 Aにおいて凸部 1 1は、 ハブ 2 の周方向における、 針管 1の刃面 1 aに対して 9 0 ° の角度をなす位置 に設けられている。 このように、 凸部 1 1が刃面 1 a側およびその裏側 に対応しない位置に設けられることにより、 釙管 1がシールド筒 4 a内 へ移動しないように保持された状態で、 針管 1を穿刺操作に都合の良い 状態に設定することができる。 その結果、 穿刺状態で針管 1がシールド 筒 4 a内に引き込まれるという不測の事態を防止できる。 あるいは、 図 5に示すように、 凸部 1 1を刃面 1 aに対応する位置に設けることが好 都合な場合もある。 その場合、 凸部 1 1をゲート溝 1 2 aに係合させて シールド筒 4 aに対して釙管 1を回動不能とし、 刃面 1 aを上方に向け た際に、 穿刺部位に対して凸部 1 1が当接することがない。 従って、 穿 刺に適した状態を確実に得ることができる。
なお、 上述の実施の形態では、 凸部 1 1とゲート溝 1 2が各々 1個の 場合について例示したが、 1個または 2個の凸部 1 1と 2個のゲート溝 1 2の組み合わせとすることも可能である。
上述の基本的な構成に加えて、 図 6 Aに示すように、 シールド筒 4 a に対してハブ 2を係止する保持機構を別途備えることもできる。 図 6 B は、 図 6 Aの B— B断面におけるシ一ルド筒 4 aを示す。 1 4は保持機 構を構成する係止片であり、 シールド筒 4 aの側壁から伸びるように設 けられ、 可撓性を有する。 係止片 1 4は、 図 6 Bに示されるように略円 弧状をなし、 突起 1 4 a、 および把持用リブ 1 4 bが形成されている。 係止片 1 4の先端部には、 係合孔 1 4 cが形成されている。 シ一ルド筒 4 aにおける、 係止片 1 4が設けられた部位の反対側の側壁には、 固定 用突起 1 5が形成されている。
係止片 1 4の機能について、 図 7 A〜(:を参照して説明する。 図 7 A は、 図 6 Aの状態から、 ハブ 2がシールド筒 4 a内に若干引き込まれた 状態を示す。 図 7 Bは、 図 7 Aの C— C断面を示す。 図 7 Cは、 図 7 A の D— D断面におけるシールド筒 4 aを示す。図 7 Bにのみ示されるが、 ハブ 2の外表面には、 環状溝 2 aが形成されている。 図 7 A〜(:に示す 状態において、環状溝 2 aの位置は、係合孔 1 3の位置と合致している。 これらの図に示されるように、 係止片 1 4は、 その可撓性によりシー ルド筒 4 aに巻付けることが可能である。 係止片 1 4をシールド筒 4 a に巻付けることにより、 突起 1 4 aは、 係合孔 1 3を通してシールド筒 4 aの内腔に突出する。 それにより、 突起 1 4 aの先端部が環状溝 2 a と係合し、 シールド筒 4 aに対してハブ 2を係止する作用が得られる。 ただし、 環状溝 2 aは必ずしも設ける必要はない。 つまり、 ハブ 2の外 表面に突起 1 4 aの先端が当接し押圧することにより、 摩擦力でハブ 2 を保持することも可能である。
さらに、 上記の状態では、 図 7 Cに明瞭に示されるように、 係止片 1 4の先端部の係合孔 1 4 cと固定用突起 1 5が係合する。 それにより、 係止片 1 4がシールド筒 4 aに巻付いた状態が保持される。
以上のような保持機構を備えるのは、 次の理由による。 すなわち、 図 6 Aの状態では、凸部 1 1の回動位置がゲート溝 1 2 aに合致しており、 ハブ 2をシ一ルド筒 4 aの後端側に移動させることが可能である。 針管 1をこのような角度に保持したい場合があり、 従ってこの状態でシール ド筒 4 aに対してハブ 2を係止し、 針管 1がシ一ルド筒 4 a内に引き込 まれないようにするために、 保持機構を設ける。 また、 図 7 Aに示され る状態でシールド筒 4 aに対してハブ 2を係止したい場合もある。 この 状態では、 凸部 1 1がゲート溝 1 2 a内に入り込んでいるため、 ハブ 2 および針管 1の回動を規制することができるからである。
なお、 係止片 1 4の突起 1 4 aをハブ 2に係合させるためには、 上述 のような係合孔 1 3を用いる構成に限定されることはない。 つまり、 係 止片 1 4の位置を係合孔 1 3とは異ならせて、 突起 1 4 aをシールド筒 4 a内に突出させるための貫通孔を別途設けた構成とすることもできる。
(実施の形態 2 )
図 8は、 本発明の実施の形態 2における医療用釙装置の平面図、 図 9 は図 8の左側から見た側面図である。 図 1 0は、 同装置の平面形状を示 す断面図である。 この実施形態は、 実施の形態 1におけるシールド筒 4 aに左右の翼を付与して、 翼付きシールドとした構成に関する。 シール ド筒およびハブの基本的な構成は実施の形態 1と同様である。 従って、 実施の形態 1における要素と同様の要素については、 同一の参照符号を 付して説明する。
1は針管であり、 樹脂製のハブ 2の先端部に固定されている。 ハブ 2 の後端部にはチューブ 3が接続されている。 2 4は翼付きシールドであ り、 樹脂製の略円筒形状を有するシールド筒 2 4 aと、 左右の翼部 5、 6からなる。 ハブ 2は、 シールド筒 2 4 aの内腔に揷入されて、 先端部 および後端部をシールド筒 2 4 aから突出させることが可能な長さを有 する。 またハブ 2の外径は、 シールド筒 2 4 aに対する回動および軸方 向の移動が容易に可能な程度に、 シ一ルド筒 2 4 aの内壁に対して隙間 が形成される大きさに設定されている。
左右の翼部 5、 6は、 シールド筒 2 4 aの先端部、 すなわち針管 1が 突出する側の端部に設けられている。 翼部 5、 6は、 各々シールド筒 2 4 aの外周面の両側部に結合し、 シールド筒 2 4 aの軸を中心として互 いに対称な形状を有する。 翼部 5、 6の基端部領域には、 それぞれ翼突 起 7、 8が形成されている。 翼突起 7、 8に対応するシールド筒 2 4 a の左右の側部壁には、 貫通孔 9、 1 0が形成されている。
シールド筒 2 4 aの先端から露出したハブ 2の先端部には、 外周面の 一部に凸部 1 1が形成されている。 シ一ルド筒 2 4 aの外表面には畝状 部 1 2が形成され、 畝状部 1 2の内壁にはゲート溝 1 2 aが形成されて いる。 凸部 1 1およびゲート溝 1 2 a等の構成および作用は実施の形態 1と実質的に同様である。
なお凸部 1 1は、 ハブ 2の周方向における、 針管 1の刃面 l a側およ びその裏側に対応しない位置に設けられている。 従って、 刃面 l aが翼 - 5、 6に垂直な方向を向いた状態で、 凸部 1 1がゲ一ト溝 1 2 aに対向 することが防止される。 通常、 穿刺の際は刃面 1 aが翼 5、 6に垂直な 方向を向いた状態にするので、 この構成によれば、 穿刺の際に針管 1が シールド筒 2 4 a内に押し込まれることを回避できる。
図 1 1 Aおよび 1 1 Bは、 翼部 5、 6に設けた翼突起 7、 8の作用を 示す。 同図は、 図 8の E— Eにおける断面図である。 翼部 5、 6を用い る操作は、 穿刺に際して行われる。
図 1 1 Aに示す状態から、 翼部 5、 6をシールド筒 2 4 aの外表面に 沿って上方に持ち上げて、 図 1 I Bに示すように、 翼突起 7、 8を貫通 孔 9、 1 0に挿入し、 シールド筒 2 4 aの壁を貫通して内腔に突出させ る。 その結果、 八ブ 2の外表面に翼突起 7、 8の先端部が当接する。 そ の当接の押圧力に基づく摩擦力で、 ハブ 2、 すなわち、 針管 1が翼付き シールド 2 4に保持される。 従って、 針管 1をしつかりと確保した状態 で穿刺操作を行うことができる。 両翼部 5、 6を指で強く挟み込むこと により、 翼突起 7、 8に十分な押圧力を与えて八ブ 2を強力に保持する ことも可能である。
なお、 シ一ルド筒 2 4 aの外表面の底部は、 患者の皮膚上に安定に配 置されるように、 平坦に形成されていることが好ましい。 また図示では シールド筒 2 4 aの内腔は円形であるが、 内腔の上側をハブ 2の外表面 形状に対応する曲面に形成し、 内腔面の底部は平坦に形成してもよい。 産業上の利用の可能性
本発明の誤穿刺防止用シールド付き医療用釙装置は、 穿刺操作を行う ことが可能なように針管がシールドから突出した状態において、 釙管が シールド内へ移動不能となるように保持させることが可能である。 しか もその状態で、 針管をシールドに対して回動させることが可能である。

3 . 前記凸部は、 前記ハブの周方向における、 前記針管の刃面側およ びその裏側に対応しない位置に設けられている請求項 1に記載の誤穿刺 防止用シールド付き医療用釙装置。
4 . 前記凸部は、 前記八ブの周方向における、 前記針管の刃面側に対 応する位置に設けられている請求項 1に記載の誤穿刺防止用シールド付 き医療用針装置。
5 . 前記シ一ルド筒の側壁から伸びるように設けられた可撓性を有す る係止片と、 前記係止片の位置に対応させて前記シールド筒に設けられ た貫通孔とを更に備え、 前記係止片は側方に突出する突起を有し、 前記 係止片が前記シールド筒に巻き付けられたときに前記突起が前記貫通孔 に挿入されて、 前記シールド筒内での前記ハブの軸方向移動を規制する ように構成された請求項 1に記載の誤穿刺防止用シールド付き医療用釙
6 . 前記係止片が前記シールド筒に巻き付けられた状態を保持するた めの係止片保持機構を有する請求項 5に記載の誤穿刺防止用シールド付 き医療用針装置。
7 . 前記誤穿刺防止用シールドは、 前記シールド筒の先端側に結合し た一対の翼部を更に備えた翼付きシールドである請求項 1に記載の誤穿 刺防止用シールド付き医療用針装置。
8 . 前記各翼部は、 その翼面から突出するように形成された翼突起を 有し、 前記シールド筒は、 前記各翼突起を挿入可能なように側部の円筒 壁に形成された貫通孔を有し、
前記両翼部を前記シールド筒側面に沿って重ね合わせることにより、 前記貫通孔を通して前記各翼突起を前記シールド筒内腔に挿入可能であ り、 それにより、 前記各翼突起が前記ハブの前記シールド筒内での軸方 向移動を阻止して、 前記シールド筒先端から前記針管が所定長さ突出し た状態で、 前記ハブが前記シールド筒に保持される請求項 7に記載の誤 穿刺防止用シールド付き医療用釙装置。
