(3)当該生体吸収性支持体が、ポリグリコール酸、ポリ乳酸、グリコール酸と乳酸の共重合体からなる群より選択される材料を含む上記(1)または(2)に記載の創傷被覆材。
(4)当該生体吸収性支持体が不織布に加工されていることを特徴とする上記(1)から(3)のいずれかに記載の創傷被覆材。

・培養や前処理(生理食塩液による浸漬・洗浄の反復)の必要がないため、受傷後速やかに適用できる;
・フィブリン生成による製剤自体の接着性により貼付することが可能なため、縫合は必要としない;
・柔軟性及び伸縮性があり、複雑な形状の創面にも密着できる;
・接着閉鎖効果により、滲出液の貯留が抑えられ、痛みが軽減する;
・閉鎖効果により、出血が抑えられるため、出血を伴う新鮮外傷に有用である;
・トロンビン成分の細胞成長促進活性により、肉芽組織の形成と共に表皮を含む欠損組織の再建を促進する;
・創面に適用後は、適度な強度によって肉芽組織が形成される空間を確保して、創収縮を抑え、瘢痕拘縮を軽減する;
・保湿用被覆材を積層しない場合は、創面を保護しながら治癒に伴って自然に剥離し、創傷被覆材として機能する。選択的に保湿用被覆材を積層して湿潤環境にした場合は、保湿用被覆材以下の部分が細胞増殖の足場となって自己組織に置換され、人工皮膚としても機能する;
・安全性に優れている;
等の性質を有しており、理想的な創傷被覆材となることが明らかになった。
A.トロンビン固定化生体吸収性支持体及びフィブリノゲン成分
B.トロンビン固定化生体吸収性支持体及びフィブリノゲン固定化生体吸収性支持体
C.生体吸収性支持体、トロンビン成分及びフィブリノゲン成分
を基本構成とし、さらにこれに上述した添加剤や安定剤を適宜含むものである。すなわち、本発明のキットは、トロンビンのみが生体吸収性支持体に固定化されている(A)、トロンビンおよびフィブリノゲンのいずれもがそれぞれ生体吸収性支持体に固定化されている(B)、およびトロンビンおよびフィブリノゲンのいずれも生体吸収性支持体に固定化されていない(C)のいずれの態様をも包含する。
市販の生体組織接着剤(製品名:ボルヒール:財団法人化学及血清療法研究所製)のキットに含まれるトロンビンを付属の溶解液により溶解し、1875単位/mLのトロンビン溶液を作製した。1875単位/mLトロンビン溶液を用いて、終濃度1%グリセロール、3%トレハロース、0.18Mヒスチジン、40mM塩化カルシウム及び0.1%ツイン80を含むトロンビン充填液(pH6.0)を調製した。このトロンビン溶液2.5mLを、ポリグリコール酸系生体吸収性合成不織布(製品名:ネオベール/グンゼ(株),5 x 10cm,厚さ0.15mm)上に均一にしみ込ませた。この検体を凍結後、24時間凍結乾燥させたものをトロンビン固定化支持体のサンプルとした(固定化されたトロンビンの量:93.8U/cm2)。
市販の生体組織接着剤(製品名:ボルヒール:財団法人化学及血清療法研究所製)のキットに含まれるフィブリノゲンに、非イオン界面活性剤としてTween80(0.1%)、アルブミン(1.0%)、NaCl(1.75%)、クエン酸3Na(1.2%)、グリシン(1.5%)、D-マンニトール(4.0%)を含有する8%フィブリノゲン溶液を調製した。それぞれのフィブリノゲン溶液2.5mLを、ポリグリコール酸系生体吸収性合成不織布(製品名:ネオベール/グンゼ(株),5 x 10cm,厚さ0.15mm)上に均一にしみ込ませた。この検体を凍結後、24時間凍結乾燥させたものをフィブリノゲン固定化支持体のサンプルとした(固定化されたフィブリノゲンの量:4mg/cm2)。
(1)実験方法
モルモット(雌性、Hartley、5週齢、日本エスエルシー株式会社)にケタミン(第一三共株式会社)、ジアゼパム(武田薬品工業株式会社)で導入麻酔を行い、イソフルラン(メルク製薬株式会社)のマスク吸入で麻酔を維持した。背部皮膚に1.5cm角の全層皮膚欠損創を数箇所作製し、以下の群構成の各処置を施した。第1群及び3群については、適用に要する時間を計測した。術後5日目、8日目、10日目、2週目、3週目に創傷部の写真撮影を行い、創傷部面積を画像解析により求めた。また、術後5日目、10日目、2週目、3週目、6週目に4匹ずつ安楽死させ、処置部を採材して病理組織検査を行った。
第1群:創傷被覆材(本願発明);フィブリノゲン液(ボルヒール(登録商標)、財団法人化学及血清療法研究所)を擦り込んだ後に生体吸収性合成不織布(ネオベール(登録商標)、グンゼ株式会社)を貼り、その上からフィブリノゲン液とトロンビン液(ボルヒール(登録商標)、財団法人化学及血清療法研究所)をスプレー塗布した。
第2群:無処置(陰性コントロール);何も処置を行わず開放創とした。
第3群:テルダーミス;真皮欠損用グラフトであるテルダーミス(登録商標、テルモ株式会社)を貼り、ナイロン糸で単結紮縫合により縫い付けた。
(1) 適用時間
テルダーミスの適用時間は平均368.0±27.2秒(n=8)を要したのに対して、創傷被覆材の適用時間は平均44.2±6.1秒(n=9)であった。創傷被覆材は本材自身の接着性のため縫合を必要としないため、テルダーミスに比べ約1/9の時間で適用が可能だった。
創傷面積の温存率は、創傷被覆材群で95.5±5.75%(n=6)、無処置群で73.6±16.21%(n=5)、テルダーミス群で73.1±18.02%(n=5)であった。このように創傷被覆材では、無処置及びテルダーミスに比べて、処置後8日目の創傷の収縮が抑制されていた。
<群間の比較>
創傷被覆材では、無処置及びテルダーミスに比べ、5日目の浮腫、痂皮下の血液及び滲出液の貯留が軽度だった。10日目では、いずれの群も創傷による陥没部は肉芽組織で埋められていたが、創傷被覆材では無処置及びテルダーミスに比べて、元からあるコラ-ゲン層間の距離が最も保たれていた(図1)。
5日目:創傷被覆材の下に薄い滲出物層があり、その下に線維増生及び辺縁部からの上皮伸張(一部)が認められた。
10日目:創傷被覆材は剥離していた。辺縁部から上皮が伸長しており、その下層には血管が新生し、肉芽組織が形成されていた。
2週目:上皮の伸長が顕著となり、完全に被覆している例もあった。
3週目:上皮が完全に被覆していた。肉芽組織にはコラーゲンが認められ、真皮及び皮下組織の修復も進んでいた。
6週目:真皮のコラーゲンの連続性はほぼ回復していた。付属器もほぼ修復していた。
このように、本創傷被覆材は治癒過程において過度の炎症等は認められず、上皮、真皮層ともに速やかに再建された。
(1)実験方法

第1群:創傷被覆材;フィブリノゲン液(ボルヒール(登録商標)、一般財団法人化学及血清療法研究所)を擦り込んだ後に生体吸収性合成不織布(ネオベール(登録商標)、グンゼ株式会社)を貼り、その上からフィブリノゲン液とトロンビン液(ボルヒール(登録商標)、一般財団法人化学及血清療法研究所)をスプレー塗布した。更にその上から保湿用被覆材で被覆した。
8日目:創傷による陥没部は肉芽組織で埋められており、辺縁部からの上皮伸長が認められた
2週目:上皮伸長が顕著となり、完全に被覆している例もあった。
3週目:創傷被覆材の生体吸収性不織布及びフィブリンクロットを足場として、結合織増生、血管新生が認められ、創傷被覆材は自己組織に置換されつつあった(図2)。湿潤環境下では創傷の治癒に伴って創傷被覆材は自然剥離するのではなく、本材自体が自己組織に置き換わることが明らかになった。
Claims (41)
- 有効成分としてトロンビン成分及びフィブリノゲン成分、基材として生体吸収性支持体から構成されることを特徴とする創傷被覆材。
- (1)トロンビン固定化生体吸収性支持体とフィブリノゲン成分、(2)トロンビン固定化生体吸収性支持体とフィブリノゲン固定化生体吸収性支持体、または(3)トロンビン成分、フィブリノゲン成分および生体吸収性支持体、から選択されるいずれか一つの構成であり、トロンビン成分とフィブリノゲン成分が使用直前まで隔離されていることを特徴とする請求項1に記載の創傷被覆材。
- 当該生体吸収性支持体が、ポリグリコール酸、ポリ乳酸、グリコール酸と乳酸の共重合体からなる群より選択される材料を含む請求項1または2に記載の創傷被覆材。
- 当該生体吸収性支持体が不織布に加工されていることを特徴とする請求項1から3のいずれかに記載の創傷被覆材。
- 当該生体吸収性支持体がポリグリコール酸を材料とする不織布である請求項1から4のいずれかに記載の創傷被覆材。
- 添加剤として、血液凝固第XIII因子、プロテアーゼ抑制剤、塩化カルシウム、アルブミン、塩化ナトリウム、クエン酸ナトリウム、非イオン界面活性剤から選択される少なくとも一つを含む請求項1から5のいずれかに記載の創傷被覆材。
- 選択的に保湿用被覆材を構成要件として含む請求項1から6のいずれかに記載の創傷被覆材。
- 当該保湿用被覆材がシリコーンまたはポリウレタンからなる請求項7に記載の創傷被覆材。
- 外傷、外科手術、熱傷などによる皮膚欠損部を修復可能な請求項1から8のいずれかに記載の創傷被覆材。
