- the present invention pertains to the field of ophthalmic implants and relates to a device comprised basically of a structure having as its main purpose to shape the curvature of the cornea thereby correcting the morphological deformations and refractive errors associated with ecchyctic diseases of the cornea .
- the intrathromal implant uses small devices, comprised of rings or ring segments commonly produced in polymethylmethacrylate, which are introduced between the layers of the corneal stroma on each side of the pupil (i.e. off-axis), by means of a small incision made in the periphery of the cornea.

- Historically such devices were developed for the treatment and correction of myopia; however such procedures are rare so that the main application of these devices has basically been restricted to the treatment of keratoconus - non-inflammatory degenerative eye disease, which causes structural changes in the cornea that end up leaving it thinner and surface in a more conical shape (ectasia) than its conventional gradual curvature thus causing impairment to the quality of vision.
- Symptoms resulting from keratoconus include distortion of images, generation of multiple images, photophobia, myopia and irregular astigmatism.
- one or two ring segments are implanted within the corneal stroma, in order to leave the cornea flatter and thus reduce the visual effects cited.

- - Intacs (Addition Technology Inc.): comprises ring segments with thicknesses between 0.25mm and 0.45mm and arc of 150 °. It also has variable radius between 2.5mm and 3.5mm, hexagonal cross section, internal diameter of 6.77mm and external diameter of 8.1mm;
- - Keraring (Mediphacos): ring segments with thickness between 0.15mm and 0.35mm, 90 °, 120 °, 160 ° and 210 ° arcs. It has a variable radius of 2.5mm, 2.75mm or 3mm;
- US6966972 discloses a hybrid structured ring being formed of a central core of rigid material coated with a more flexible material and, similarly, US 6214044 also emphasizes the constructive composition of the rings, comprising high and low modulus materials of elasticity, while US5888243 discloses rings with hollow inner region.
- US8394140 discloses implants comprised of rings which may have a circular cross-section or substantially elliptical in shape but only provide an annular conformation which is not capable of broadly conforming to the total curvature of the cornea thus exhibiting limited results.
- keratoconus is treated exclusively by flattening the marked curvature of the cornea that it causes without, however, inducing the cornea to have a desired predetermined curvature in order to maximize the quality of vision in each case.
- existing intracorneal implants cover a very limited corneal area, having a reduced width, usually less than 1mm, so as not to compromise the diffusion of nutrients to the corneal stroma.
- the current state of the art lacks solutions to improve the treatment of corneal ectasias by using means to induce the surface of the cornea to acquire a predetermined curvature that can provide better quality of vision.
- an object of the present invention to provide an implantable device capable of reshaping the curvature of the cornea according to its own curvature.
- the present invention achieves the above objects by means of an implantable device for shaping the curvature of the cornea, said device being intended to be surgically implanted within human corneas between its layers and configured to change the curvature of the cornea for treatment of corneal ectasias.
- the implantable device for shaping the cornea curvature comprises a substantially circular structure with a convex surface and a cross section with a radius of varying curvature proportional to the radius of curvature whether to print treated t ⁇ ôrr ⁇ eá FRA 'served' wherein said convex surface comprises at least one opening for biological exchanges between the layers of the cornea, and wherein the substantially circular structure dictates comprises the shape of a circular segment or of a spherical shell provided with at least one aperture on its surface.
- a corneal intrastromal device which comprises a material having a modulus of elasticity greater than the modulus of elasticity of the human cornea, preferably with modulus of elasticity between 0.6 and 8500 MPa.
- said intrastromal device will be produced with polymers or copolymers of methylmethacrylate or other biocompatible polymers, or optionally with biocompatible metal materials such as platinum, titanium, etc.
- the corneal intrastromal device of the present invention comprises at least one circular segment with a convex surface provided with apertures, said circular segment having in its cross-section a radius of curvature varying between 4.5 millimeters and 17 , 0 millimeters.
- the device of the present invention comprises at least one circular body having a convex surface provided with at least one central opening, said convex surface also comprising cavities distributed along its surface to allow for stromal nutrition.

- the device comprises a structure whose main purpose is to shape the curvature of the cornea according to its own curvature thereby correcting eyes visual disturbances accompanying the corneal ectasias, differing from the existing ones by having a curved and more elongated profile, besides having leaked spaces along its body for the possibility of the exchange of gases and nutrients along the cornea.
- the device object of the present invention can be produced in two basic formats: the first, shown in Figures 1 and 2, basically comprises an open circular structure in the form of a phalanx segment and the second symbol, shown in Figures 3, 4 and 5, comprises a substantially spherical or aspherical shell shaped structure.
- Figures 1.1 to 1.5 it is possible to observe a first embodiment for said instraestromal device, basically defined by a circular segment 1 with a smoothly convex surface 11 provided with central spans 13. It should be noted that the length of the arches can be varied, commercialization of different models with arches of different lengths.

- the alternative configuration shown in Figure 5 is basically defined by two circular segments 51, each defined by convex surfaces 52 with internal cavities 53, the circular segments 51 being joined together by means of a central ring 54 which interconnects by means of rods 55.
- intra-sternal implant devices presented in the present application, it is possible to promote corneal modeling according to the curvature of the implant and with a profile closer to the spherical, very similar to that of the healthy eye and with conventional corneal topography.
- the more elongated profile of the device especially that shown in Figures 2.1 to 2.4, also contributes to the patient's cornea deforming according to the shape of the implant.
- Another important advantage of the device object of the present invention compared to known similar ones concerns the cavities 13, 33, 43 and 53 which enable the occurrence of gas and nutrient exchanges along the cornea thereby contributing to maintain the health of the cornea.
- a conventional ring adds volume to the periphery of the cornea, making it more flat.
- the proposal presented here contains differentials that enhance its benefits.
- the largest differential is the fact that its cross section presents a previously defined curvature, as can be seen in the profile shown in Figure 1.5, 2.4 and 3.4.
- the convex surface and greater width of the implant over the conventional ones cause the cornea to deform according to the format of the device. With this, the cornea acquires a new radius of curvature, allowing to improve the quality of vision of patients suffering from corneal ectasias.
- the greater width of the implant or even the possibility of construction of the shell are only possible because the proposals have empty spaces along their bodies, and these spaces allow the continuous exchange of gases and nutrients between the inner and outer layers of the cornea, which contributes significantly to its biocompatibility.
- the cornea in order to ensure that the cornea is adapted to the shape of the device, it is produced from a material having a modulus of elasticity so that it is stiffer than the cornea, so that the opposite effect does not occur, that is, the implant deforms according to the inadequate curvature of the cornea.
- the modulus of elasticity of the cornea is approximately 0.29 ⁇ 0.06 MPa
- biocompatible metal materials such as platinum, titanium, etc. may be used.
- implants should be produced from transparent materials for visible light.
- intra-stromal implant object of the present invention has been represented herein in the form of preferred and exemplary, but not limiting, embodiments. In this sense, it should be remembered that there may be other variations in the structure proposed for the object, but always maintaining the same objective of altering the curvature of the cornea according to the curvature of the implant.
