Abstract

본 발명은 (a) 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 (b) HGF를 암호화하는 폴리뉴클레오타이드 및 SDF-1α를 암호화하는 폴리뉴클레오타이드를 유효성분으로 포함하는 말초동맥질환의 예방 또는 치료용 약제학적 조성물에 관한 것이다. 본 발명의 조성물을 이용하는 경우 혈관 내피 세포의 유주 및 혈관 생성의 현저한 촉진을 통하여 HGF, 이의 이형체, SDF-1α 또는 이의 단백질을 암호화하는 폴리뉴클레오타이드를 단독으로 이용한 경우보다 말초동맥질환(예컨대, 허혈성 지체질환)을 효과적으로 예방 또는 치료할 수 있다.

Description

간세포 성장인자 및 기질세포 유발인자1알파를 이용한 말초동맥질환의 예방 또는 치료용 조성물
본 발명은 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 상기 단백질들을 암호화하는 폴리뉴클레오타이드를 유효성분으로 포함하는 말초동맥질환의 예방 또는 치료용 조성물에 관한 것이다.
본 특허출원은 2014년 9월 26일에 대한민국 특허청에 제출된 대한민국 특허출원 제10-2014-0129361호에 대하여 우선권을 주장하며, 상기 특허출원의 개시 사항은 본 명세서에 참조로서 삽입된다. 심혈관 질환(Cadiovascular disease)은 동맥경화 등의 원인으로 혈관이 좁아지거나 폐색되어 발생하는 질환으로, 크게 관상동맥질환(Coronary Artery Disease; CAD)과 말초동맥질환(Peripheral Artery Disease; PAD)으로 크게 나뉠 수 있다. 이 중 말초동맥질환의 대표적인 종류로 허혈성 지체질환(Ischemic Limb Disease)이 있다.
현재까지 심혈관 질환의 치료는 혈관을 확장시키는 약물의 사용이나 외과적 수술이 대부분을 차지하고 있다. 그러나 허혈성 지체질환의 경우 썩어 들어가는 고통이 너무 심해 환자에 따라 항전신성 진통제를 복용하고 심해지면 다리를 절단하거나 사망에 이르는 경우도 있어 근본적인 치료가 필요한 실정이다.
한편, 유전자 치료를 위한 유전자 전달체로서의 발현벡터들은 WO 2000/040737에 기재되어 있다. 또한, WO 2003/078568에는 HGF 유전자를 이용한 허혈성 하지질환치료 효과에 대하여 기재하고 있다.
본 명세서 전체에 걸쳐 다수의 논문 및 특허문헌이 참조되고 그 인용이 표시되어 있다. 인용된 논문 및 특허문헌의 개시 내용은 그 전체로서 본 명세서에 참조로 삽입되어 본 발명이 속하는 기술 분야의 수준 및 본 발명의 내용이 보다 명확하게 설명된다.
본 발명자들은 말초동맥질환을 예방 또는 치료할 수 있는 약물을 개발하고자 예의 연구 노력하였다. 그 결과, 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 상기 단백질들을 암호화하는 폴리뉴클레오타이드를 병용하는 경우 HGF, SDF-1α 또는 이의 단백질을 암호화하는 폴리뉴클레오타이드 각각을 단독으로 이용하는 경우보다 말초동맥질환에 대한 치료 효과가 현저함을 확인함으로써, 본 발명을 완성하게 되었다.
따라서, 본 발명의 목적은 말초동맥질환(Peripheral Artery Disease)의 예방 또는 치료용 약제학적 조성물을 제공하는데 있다.
본 발명의 다른 목적은 말초동맥질환(Peripheral Artery Disease)의 예방 또는 치료방법을 제공하는데 있다.
본 발명의 다른 목적 및 이점은 하기의 발명의 상세한 설명, 청구범위 및 도면에 의해 보다 명확하게 된다.
본 발명의 일 양태에 따르면, 본 발명은 (a) 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 (b) HGF를 암호화하는 폴리뉴클레오타이드 및 SDF-1α를 암호화하는 폴리뉴클레오타이드를 유효성분으로 포함하는 말초동맥질환의 예방 또는 치료용 약제학적 조성물을 제공한다.
본 발명자들은 말초동맥질환을 예방 또는 치료할 수 있는 약물을 개발하고자 예의 연구 노력하였다. 그 결과, 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 상기 단백질들을 암호화하는 폴리뉴클레오타이드를 병용하는 경우 HGF, SDF-1α 또는 이의 단백질을 암호화하는 폴리뉴클레오타이드 각각을 단독으로 이용하는 경우보다 말초동맥질환에 대한 치료 효과가 현저함을 확인하였다.
본 발명의 치료 전략은 크게 단백질 치료 및 유전자 치료 두 가지로 분류될 수 있다. 본 발명의 단백질 치료제 전략에 따르면, HGF 또는 이의 이형체, 및 SDF-1α 단백질을 병용한다. 한편, 본 발명의 유전자 치료제 전략에 따르면, HGF를 암호화하는 하나 이상의 뉴클레오타이드 서열 및 SDF-1α를 암호화하는 하나 이상의 뉴클레오타이드 서열을 투여한다. HGF를 암호화하는 하나 이상의 뉴클레오타이드 서열 및 SDF-1α를 암호화하는 하나 이상의 뉴클레오타이드 서열은 하나의 폴리뉴클레오타이드로 제공되거나 별도의 폴리뉴클레오타이드로 제공될 수 있다. 본 발명의 일 구현예에 따르면, 본 발명의 HGF를 암호화하는 하나 이상의 뉴클레오타이드 서열 및 SDF-1α를 암호화하는 하나 이상의 뉴클레오타이드 서열은 별도의 폴리뉴클레오타이드로 제공된다.
이하, 본 발명에 대하여 상세히 기재한다.
본 명세서에서 사용되는 용어 "HGF의 이형체(isoform)"는 모든 대립 유전자 변형체들(variants)을 포함하는, 동물에서 자연적으로 생성되는(naturally occurring) HGF 아미노산 서열과 적어도 80% 동일한 아미노산 서열을 갖는 HGF 폴리펩타이드를 의미한다. 예를 들어, HGF 이형체는 HGF의 정상형(normal form) 또는 야생형(wild type), 그리고 HGF의 다양한 변이체(예컨대, 스플라이싱 변이체 및 결손 변이체)를 모두 포괄하는 의미를 갖는다.
본 명세서에서 사용되는 용어 “예방”은 본 발명의 조성물의 투여로 말초동맥질환을 억제시키거나 진행을 지연시키는 모든 행위를 의미한다.
본 명세서에서 사용되는 용어 “치료”는 (a) 말초동맥질환의 발전의 억제; (b) 말초동맥질환의 경감; 또는 (c) 말초동맥질환의 제거를 의미한다.
본 발명의 일 구현예에 따르면, 본 발명의 HGF는 재조합 인간 HGF 단백질을 포함한다. 본 발명의 다른 구현예에 따르면, 본 발명의 HGF는 서열목록 제1서열의 아미노산 서열을 포함한다.
본 발명의 일 구현예에 따르면, 본 발명의 HGF 이형체는 전장 HGF(full length HGF; flHGF) 및 결손된 변이형 HGF(deleted variant HGF; dHGF)를 포함한다.
본 명세서에서 사용되는 용어 "flHGF"는 동물 HGF의 아미노산 1-728 서열, 일 구현예에 따르면 포유동물 HGF의 아미노산 1-728 서열, 다른 구현예에 따르면 인간 HGF의 아미노산 1-728 서열을 의미한다.
본 명세서에서 사용되는 용어 "dHGF"는 동물 HGF 유전자, 일 구현예에 따르면 포유동물 HGF 유전자의 선택적 스플라이싱에 의해 생성되는 HGF 단백질의 결손된 변형체를 의미한다. 본 발명의 다른 구현예에 따르면, 본 발명의 dHGF는 전장 HGF 서열로부터 알파 사슬의 첫 번째 크링글 도메인에서 5 개의 아미노산(F, L, P, S 및 S)이 결손된 723개의 아미노산으로 이루어지는 인간 HGF이다.
본 발명의 일 구현예에 따르면, 본 발명의 전장 HGF는 서열목록 제2서열의 아미노산 서열을 포함하고, 본 발명의 결손된 변이형 HGF는 서열목록 제3서열의 아미노산 서열을 포함한다.
본 발명의 일 구현예에 따르면, 본 발명의 SDF-1α는 서열목록 제4서열 또는 서열목록 제8서열의 아미노산 서열을 포함한다.
하기 실시예에서 입증한 바와 같이, 본 발명의 HGF 및 SDF-1α 단백질을 HUVEC에 병용처리한 경우 각각의 단백질을 단독으로 처리한 경우보다 혈관 내피 세포의 유주 정도 및 혈관 생성 정도를 더욱 효과적으로 촉진한바, HGF 및 SDF-1α 단백질의 병용 투여가 말초동맥질환의 예방 또는 치료에 효과적으로 이용될 수 있음을 확인하였다.
본 발명의 일 구현예에 따르면, 본 발명의 HGF의 이형체는 별도의 뉴클레오타이드 서열에 의해 암호화되거나, 또는 단일의 폴리뉴클레오타이드에 의하여 암호화된다. 본 발명인 약제학적 조성물은, HGF의 이형체가 별도의 폴리뉴클레오타이드에 의하여 암호화되는 경우에 둘 이상의 폴리뉴클레오타이드를 포함하고, HGF의 이형체가 단일의 폴리뉴클레오타이드에 의하여 암호화되는 경우에는 상기 단일의 폴리뉴클레오타이드를 포함하는 하나 이상의 폴리뉴클레오타이드를 포함한다. 본 발명의 폴리뉴클레오타이드는 HGF 이형체의 발현을 조절하는 하나 이상의 조절 서열(예컨대, 프로모터 또는 인핸서)에 작동가능하게 연결될 수 있다.
HGF의 이형체들이 별도의 폴리뉴클레오타이드에 의하여 암호화되는 경우, 두 가지 방식으로 발현 카세트를 구축할 수 있다. 첫 번째 방식에 따르면, 이형체 각각에 대한 CDS(coding sequence)에 발현 조절 서열을 연결하여 발현 카세트를 구축한다. 두 번째 방식에 따르면, "발현조절서열-제 1 이형체 CDS-IRES-제 2 이형체 CDS-전사종결 서열"과 같은 방식으로 IRES(internal ribosomal entry site) 또는 2A 펩타이드를 이용하여 발현 카세트를 구축한다. IRES 는 유전자의 번역이 IRES 서열에서 시작되도록 함으로써 2 개 이상의 관심 유전자가 동일한 구조물에서 발현되도록 한다.
HGF의 이형체가 단일의 폴리뉴클레오타이드에 의하여 암호화되는 경우, 상기 이형체들을 모두 암호화하고 있는 폴리뉴클레오타이드는 단일의 발현조절서열에 작동가능하게 연결된다.
본 발명에 있어서, HGF의 이형체는 둘 이상의 서로 다른 종류의 이형체, 예컨대 flHGF 및 dHGF를 동시에 발현하는 하이브리드(hybrid) HGF 유전자에 의해 암호화될 수 있다.
본 발명의 일 구현예에 따르면, 상기 하이브리드 HGF 유전자는 인간 HGF 유전자의 엑손 1 내지 4에 해당하는 서열, 인간 HGF 유전자의 인트론 4 또는 그의 단편 서열, 및 인간 HGF 유전자의 엑손 5 내지 18에 해당하는 서열을 포함한다.
상기 인트론 4를 포함하는 하이브리드 HGF 유전자는 7113bp 길이이며, 서열목록 제7서열의 뉴클레오타이드 서열을 포함한다. 상기 하이브리드 HGF 유전자는 선택적으로 HGF cDNA의 엑손 4와 5 사이에 인트론 4의 단편을 포함할 수 있다. 본 발명의 특정 구현예에 따르면, 상기 하이브리드 HGF 유전자는 서열목록 제5서열의 뉴클레오타이드 서열을 포함한다.
본 발명의 “HGF 이형체”, 하이브리드 HGF 유전자(예컨대, HGF-X7)에 관하여서는 WO 2003/078568에 보고된 바 있으며, 상기 특허문헌의 개시 내용은 그 전체로서 본 명세서에 참조로 삽입된다.
본 발명에서 이용 가능한 HGF 이형체의 아미노산 또는 뉴클레오타이드 서열은 야생형 인간 HGF 이형체의 서열과 실질적인 동일성을 나타내는 아미노산 또는 뉴클레오타이드 서열도 포함하는 것으로 해석된다. 상기 실질적인 동일성은, 야생형 인간 HGF 이형체의 아미노산 또는 뉴클레오타이드 서열과 임의의 다른 서열을 최대한 대응되도록 얼라인하고, 당업계에서 통상적으로 이용되는 알고리즘을 이용하여 얼라인된 서열을 분석한 경우에, 최소 80%의 상동성, 보다 바람직하게는 최소 90%의 상동성, 가장 바람직하게는 최소 95%의 상동성을 나타내는 아미노산 서열 또는 뉴클레오타이드 서열을 의미한다. 서열비교를 위한 얼라인먼트 방법은 당업계에 공지되어 있다. 얼라인먼트에 대한 다양한 방법 및 알고리즘은 Smith and Waterman, Adv. 본 발명의 일 구현예에 따르면, 본 발명의 SDF-1α를 암호화하는 폴리뉴클레오타이드는 서열목록 제6서열을 포함한다.
본 발명의 일 구현예에 따르면, 본 발명의 말초동맥질환은 허혈성 지체질환(Ischemic Limb Disease)이다. 하기 실시예에서 입증한 바와 같이, 본 발명의 조성물은 하지 허혈을 유도한 마우스 모델에서 pCK 투여군, pCK-SDF-1a 투여군 및 pCK-HGF 투여군의 다리 상태가 나빠지는 것과는 상이하게, 정상상태를 계속적으로 유지하게 하는 효과가 있다.
본 발명의 일 구현예에 따르면, 본 발명의 폴리뉴클레오타이드는 네이키드 DNA(naked DNA) 또는 유전자 운반체에 포함되어 있는 뉴클레오타이드이다. 본 발명의 조성물은 유전자 치료 분야에서 통상적으로 공지된 다양한 운반 방법을 통하여 생체 내에 적용될 수 있으며, 상기 유전자 운반체는 예컨대 벡터, 플라스미드 및 바이러스 벡터를 포함하나, 이에 한정되는 것은 아니다.
(i) 플라스미드(벡터)
본 발명의 폴리뉴클레오타이드를 운반하는 운반체로서 플라스미드(벡터)가 이용될 수 있다. 벡터에 포함되는 폴리뉴클레오타이드는 적합한 발현 카세트 내에 존재하는 것이 바람직하다. 상기 발현 카세트에서 폴리뉴클레오타이드는 프로모터에 작동적으로 연결되는 것이 바람직하다.
본 명세서에서 사용된 용어 "작동적으로 연결된"은 핵산 발현조절 서열(예: 프로모터, 시그널 서열 또는 전사조절인자 결합 위치의 어레이)과 다른 핵산 서열 사이의 기능적인 결합을 의미하며, 이에 의해 상기 조절 서열은 상기 다른 핵산 서열의 전사 및/또는 해독을 조절하게 된다.
본 발명에 있어서, 폴리뉴클레오타이드 서열에 결합된 프로모터는 일 구현예에 따르면 동물세포, 다른 구현예에 따르면 포유동물 세포, 특정 구현예에 따르면 인간 세포에서 작동하여 뉴클레오타이드 서열의 전사를 조절할 수 있는 것으로서, 포유동물 바이러스로부터 유래된 프로모터 및 포유동물 세포의 지놈으로부터 유래된 프로모터를 포함하며, 예컨대, CMV(cytomegalo virus) 프로모터, 아데노바이러스 후기 프로모터, 백시니아 바이러스 7.5K 프로모터, SV40 프로모터, HSV의 tk 프로모터, RSV 프로모터, EF1 알파 프로모터, 메탈로티오닌 프로모터, 베타-액틴 프로모터, 인간 IL-2 유전자의 프로모터, 인간 IFN 유전자의 프로모터, 인간 IL-4 유전자의 프로모터, 인간 림포톡신 유전자의 프로모터 및 인간 GM-CSF 유전자의 프로모터를 포함하나, 이에 한정되는 것은 아니다. 본 발명의 일 구현예에 따르면, 본 발명에서 이용되는 프로모터는 인간 CMV(hCMV)의 IE(immediately early) 유전자로부터 유래된 프로모터 또는 EF1 알파 프로모터이며, 다른 구현예에 따르면 CMV IE 유전자의 프로모터/인핸서 및 엑손 1 전체서열부터 엑손 2 의 ATG 개시코돈 직전까지를 포함하는 5'-UTR (untranslated region)이다.
본 발명에서 이용되는 발현 카세트는 폴리아네닐화 서열을 포함할 수 있으며, 예를 들어 소성장 호르몬 터미네이터(Gimmi, E. R., et al., Nucleic Acids Res. 17:6983-6998 (1989)), SV40 유래 폴리 아데닐화 서열(Schek, N, et al., Mol. Cell Biol. 12:5386-5393 (1992)), HIV-1 polyA(Klasens, B. I. F., et al., Nucleic Acids Res. 26:1870-1876 (1998)), β-글로빈 polyA(Gil, A., et al, Cell 49:399-406 (1987)), HSV TK polyA(Cole, C. N. and T. P. Stacy, Mol. Cell. Biol. 5:2104-2113 (1985)) 또는 폴리오마바이러스 polyA(Batt, D. B and G. G. Carmichael, Mol. Cell. Biol. 15:4783-4790 (1995))를 포함하나, 이에 한정되는 것은 아니다.
본 발명의 다른 구현예에 따르면, 본 발명의 유전자 운반체는 pCK, pCP, pVAX1 또는 pCY 벡터를 이용할 수 있으며, 본 발명의 특정 구현예에 따르면 pCK 벡터를 이용할 수 있다. 상기 pCK 벡터는 WO 2000/040737에 상세히 개시되어 있으며, 상기 특허문헌의 개시 내용은 그 전체로서 본 명세서에 참조로 삽입된다.
(ii) 레트로바이러스
레트로바이러스는 자신의 유전자를 숙주의 지놈으로 삽입시키고, 대량의 외래 유전 물질을 운반할 수 있으며, 감염시킬 수 있는 세포의 스펙트럼이 넓기 때문에 유전자 전달 벡터로서 많이 이용되고 있다.
레트로바이러스 벡터를 구축하기 위하여, 본 발명의 폴리뉴클레오타이드 서열은 레트로바이러스의 서열 대신에 레트로바이러스 지놈에 삽입되어 복제 불능의 바이러스를 생산한다. 비리온을 생산하기 위하여, gag, pol 및 env 유전자를 포함하지만 LTR (long terminal repeat)와 ψ서열은 없는 패키징 세포주를 구축한다 (Mann et al., Cell, 33:153-159 (1983)). 본 발명의 폴리뉴클레오타이드 서열, LTR 및 ψ서열을 포함하는 재조합 플라스미드를 상기 세포주에 이입하면, ψ서열은 재조합 플라스미드의 RNA 전사체의 생산을 가능하게 하며, 이 전사체는 바이러스로 패키징되고, 바이러스는 배지로 배출된다(Nicolas and Rubinstein "Retroviral vectors," In: Vectors: A survey of molecular cloning vectors and their uses, Rodriguez and Denhardt (eds.), Stoneham: Butterworth, 494-513 (1988)). 재조합 레트로바이러스를 함유하는 배지를 수집하고 농축하여 유전자 전달 시스템으로 이용한다.
2 세대 레트로바이러스 벡터를 이용한 유전자 전달이 발표되었다. Kasahara 등(Science, 266:1373-1376 (1994))은 몰로니 뮤라인 류케미아 바이러스의 변이체를 제조하였고, 여기에서 EPO (erythropoietin) 서열을 엔벨로프 부위에 삽입하여 새로운 결합 특성을 갖는 키메릭 단백질을 생산하였다. 본 발명의 폴리뉴클레오타이드 서열도 이와 같은 2 세대 레트로바이러스 벡터의 구축 전략에 따라 레트로바이러스에 탑재될 수 있다.
(iii) 아데노바이러스
아데노바이러스는 중간 정도의 지놈 크기, 조작의 편의성, 높은 타이터, 광범위한 타깃세포 및 우수한 감염성 때문에 유전자 전달 벡터로서 많이 이용되고 있다. 지놈의 양 말단은 100-200bp 의 ITR (inverted terminal repeat)를 포함하며, 이는 DNA 복제 및 패키징에 필수적인 시스-엘리먼트이다. 지놈의 E1 영역 (E1A 및 E1B)은 전사 및 숙주세포 유전자의 전사를 조절하는 단백질을 암호화한다. E2 영역 (E2A 및 E2B)은 바이러스 DNA 복제에 관여하는 단백질을 암호화한다.
현재 개발된 아데노바이러스 벡터 중에서, E1 영역이 결여된 복제 불능 아데노바이러스가 많이 이용되고 있다. 한편, E3 영역은 통상적인 아데노바이러스 벡터에서 제거되어 외래 유전자가 삽입되는 자리를 제공한다(Thimmappaya, B. et al., Cell, 31:543-551 (1982); 및 Riordan, J. R. et al., Science, 245:1066-1073 (1989)). 따라서, 본 발명의 폴리뉴클레오타이드 서열은 결실된 E1 영역 (E1A 영역 및/또는 E1B 영역) 또는 E3 영역에 삽입되는 것이 바람직하다. 또한, 상기 뉴클레오타이드 서열은 결실된 E4 영역에도 삽입될 수 있다. 본 명세서에서 바이러스 지놈 서열과 관련하여 사용되는 용어, "결실" 은 해당 서열이 완전히 결실된 것뿐만 아니라, 부분적으로 결실된 것도 포함하는 의미를 가진다. 또한, 아데노바이러스는 야생형 지놈의 약 105% 까지 패킹할 수 있기 때문에, 약 2 kb 를 추가적으로 패키징 할 수 있다(Ghosh-Choudhury et al., EMBO J., 6:1733-1739 (1987)). 따라서, 아데노바이러스에 삽입되는 상술한 외래 서열은 아데노바이러스의 지놈에 추가적으로 결합시킬 수도 있다.
아데노바이러스는 42개의 상이한 혈청형 및 A-F의 서브그룹을 갖는다. 이 중에서, 서브그룹 C 에 속하는 아데노바이러스 타입 5가 본 발명의 아데노바이러스 벡터를 얻기 위한 가장 바람직한 출발물질이다. 아데노바이러스 타입 5 에 대한 생화학적 및 유전적 정보는 잘 알려져 있다. 아데노바이러스에 의해 운반되는 외래 유전자는 에피좀과 동일한 방식으로 복제되며, 이에 숙주세포에 대해 유전적 독성이 매우 낮다. 따라서, 아데노바이러스 유전자 전달 시스템을 이용한 유전자 치료가 안전할 것으로 판단된다.
(iv) AAV 벡터
아데노-관련 바이러스 (AAV)는 비분열 세포를 감염시킬 수 있고, 다양한 종류의 세포에 감염할 수 있는 능력을 갖고 있기 때문에 본 발명의 유전자 전달 시스템으로 적합하다. AAV 벡터의 제조 및 용도에 대한 상세한 설명은 미국 특허 제 5,139,941 호 및 제 4,797,368 호에 상세하게 개시되어 있다.
유전자 전달 시스템으로서의 AAV 에 대한 연구는 LaFace et al, Viology, 162:483486 (1988), Zhou et al., Exp. Hematol. (NY), 21:928-933 (1993), Walsh et al, J. Clin. Invest., 94:1440-1448 (1994) 및 Flotte et al., Gene Therapy, 2:29-37 (1995)에 개시되어 있다. 최근에, AAV 벡터는 낭포성 섬유증의 치료제로서 임상 I 을 실시하고 있다.
전형적으로, AAV 바이러스는 두 개의 AAV 말단 리피트가 옆에 위치되어 있는 목적의 유전자 서열을 포함하는 플라스미드(McLaughlin et al., J. Virol., 62:1963-1973 (1988); 및 Samulski et al., J. Virol., 63:3822-3828 (1989)) 및 말단 리피트가 없는 야생형 AAV 코딩 서열을 포함하는 발현 플라스미드 (McCarty et al., J. Virol., 65:2936-2945 (1991))를 동시 형질전환 시켜 제조된다.
(v) 다른 바이러스 벡터
다른 바이러스 벡터들도 본 발명의 폴리뉴클레오타이드 서열을 생체 내로 운반하는 데 이용할 수 있다. 배시니아 바이러스 (Puhlmann M. et al., Human Gene Therapy 10:649-657 (1999); Ridgeway, "Mammalian expression vectors," In: Vectors: A survey of molecular cloning vectors and their uses. Rodriguez and Denhardt, eds. Stoneham: Butterworth, 467-492 (1988); Baichwal and Sugden, "Vectors for gene transfer derived from animal DNA viruses: Transient and stable expression of transferred genes," In: Kucherlapati R, ed. Gene transfer. New York: Plenum Press, 117-148 (1986) 및 Coupar et al., Gene, 68:1-10 (1988)), 렌티바이러스 (Wang G. et al., J. Clin. Invest. 104(11):R55-62 (1999)) 또는 헤르페스 심플렉스 바이러스 (Chamber R., et al., Proc. Natl. Acad. Sci USA 92:1411-1415 (1995)) 로부터 유래된 벡터들도 상기 폴리뉴클레오타이드를 세포 내로 운반할 수 있는 운반 시스템으로 이용할 수 있다.
(vi) 리포좀
리포좀은 수상에 분산된 인지질에 의해 자동적으로 형성된다. 외래 DNA 분자를 리포좀으로 성공적으로 세포 내로 운반한 예는 Nicolau 및 Sene, Biochim. Biophys. Acta, 721:185-190 (1982) 및 Nicolau et al., Methods Enzymol., 149:157-176 (1987)에 개시되어 있다. 본 발명의 폴리뉴클레오타이드 서열을 내포한 리포좀은 엔도사이토시스, 세포 표면에로의 흡착 또는 플라즈마 세포막과의 융합 등의 기전을 통해 세포와 상호작용하여 세포 내로 폴리뉴클레오타이드 서열을 운반한다.
본 발명에서 본 발명의 폴리뉴클레오타이드 서열이 네이키드(naked) 재조합 DNA 분자 또는 플라스미드(벡터)에 탑재된 경우에는, 미세 주입법(Capecchi, M.R., Cell, 22:479 (1980); 및 Harland 와 Weintraub, J. Cell Biol. 101:1094-1099 (1985)), 칼슘 포스페이트 침전법(Graham, F.L. et al., Virology, 52:456 (1973); 및 Chen 과 Okayama, Mol. Cell. Biol. 7:2745-2752 (1987)), 전기 천공법(Neumann, E. et al., EMBO J., 1:841 (1982); 및 Tur-Kaspa et al., Mol. Cell Biol., 6:716-718 (1986)), 리포좀-매개 형질감염법(Wong, T.K. et al., Gene, 10:87 (1980); Nicolau 및 Sene, Biochim. Biophys. Acta, 721:185-190 (1982); 및 Nicolau et al., Methods Enzymol., 149:157-176 (1987)), DEAE-덱스트란 처리법(Gopal, Mol. Cell Biol., 5:1188-1190 (1985)) 및 유전자 밤바드먼트(Yang et al., Proc. Natl. Acad. Sci., 87:9568-9572 (1990)) 방법에 의해 폴리뉴클레오타이드 서열을 세포 내로 이입시킬 수 있다.
본 발명의 폴리뉴클레오타이드 서열이 바이러스 벡터에 기초하여 제작된 경우에는 당업계에 공지된 다양한 바이러스 감염 방법에 따라 폴리뉴클레오타이드 서열을 세포 내로 운반할 수 있다. 바이러스 벡터를 이용한 숙주 세포의 감염은 상술한 인용문헌에 기재되어 있다.
본 발명의 다른 구현예에 따르면, 본 발명의 유전자 운반체는 벡터이다.
본 발명의 어떠한 구체예에 따르면, 본 발명의 벡터는 플라스미드이고, 본 발명의 특정 구현예에 따르면, 본 발명의 플라스미드는 pCK이다. pCK를 이용하여 HGF의 둘 이상의 이형체를 발현하는 단일 폴리뉴클레오타이드를 포함하는 재조합 벡터들은 WO 2000/040737 및 WO 2003/078568에 자세히 기재되어 있다.
본 발명의 조성물은 약제학적으로 허용되는 담체를 포함할 수 있다. 본 발명의 조성물에 포함되는 약제학적으로 허용되는 담체는 제제시에 통상적으로 이용되는 것으로서, 락토스, 덱스트로스, 수크로스, 솔비톨, 만니톨, 전분, 아카시아 고무, 인산 칼슘, 알기네이트, 젤라틴, 규산 칼슘, 미세결정성 셀룰로스, 폴리비닐피롤리돈, 셀룰로스, 물, 시럽, 메틸 셀룰로스, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 활석, 스테아르산 마그네슘 및 미네랄 오일 등을 포함하나, 이에 한정되는 것은 아니다. 본 발명의 약제학적 조성물은 상기 성분들 이외에 윤활제, 습윤제, 감미제, 향미제, 유화제, 현탁제, 보존제 등을 추가로 포함할 수 있다. 적합한 약제학적으로 허용되는 담체 및 제제는 Remington's Pharmaceutical Sciences(19th ed., 1995)에 상세히 기재되어 있다.
본 발명의 일 구현예에 따르면, 본 발명의 약제학적 조성물은 비경구로 투여한다. 예컨대, 정맥내 투여, 복강내 투여, 피하 투여, 피내투여, 척수내 투여, 척수강내 투여, 뇌실내 투여, 뇌실질내 투여, 두개강내 투여, 근육내 투여 또는 국부 투여를 이용하여 투여할 수 있다. 본 발명의 다른 구현예에 따르면, 본 발명의 약제학적 조성물은 근육내, 척수내, 척수강내, 뇌실내, 뇌실질내, 두개강내 투여할 수 있다.
본 발명의 약제학적 조성물은 주사로 제제화되어 투여될 수 있다. 본 발명의 약제학적 조성물의 적합한 투여량은 제제화 방법, 투여 방식, 환자의 연령, 체중, 성, 질병 증상의 정도, 투여 시간, 투여 경로, 배설 속도 및 반응 감응성과 같은 요인들에 의해 다양하며, 보통으로 숙련된 의사는 목적하는 치료에 효과적인 투여량을 용이하게 결정 및 처방할 수 있다.
본 발명의 일 구현예에 따르면, 본 발명의 HGF, 이의 이형체, 및 SDF-1α은 각각 10 ng 내지 100 mg의 투여량으로 투여되며, 상기 단백질을 암호화하는 폴리뉴클레오타이드는 각각 1 μg 내지 100 mg의 투여량으로 투여된다. 상기 HGF, 이의 이형체, SDF-1α 또는 이를 암호화하는 폴리뉴클레오타이드의 투여가 일회를 초과하여 반복될 때 투여량은 매회 동일하거나 다를 수 있다.
본 발명의 약제학적 조성물은 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약제학적으로 허용되는 담체 및/또는 부형제를 이용하여 제제화 됨으로써 단위 용량 형태로 제조되거나 또는 다용량 용기 내에 내입시켜 제조될 수 있다. 이때 제형은 오일 또는 수성 매질중의 용액, 현탁액 또는 유화액 형태이거나 엑스제, 분말제, 과립제, 정제 또는 캅셀제 형태일 수도 있으며, 분산제 또는 안정화제를 추가적으로 포함할 수 있다.
본 발명의 다른 양태에 따르면, 본 발명은 (a) 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 (b) HGF를 암호화하는 폴리뉴클레오타이드 및 SDF-1α를 암호화하는 폴리뉴클레오타이드를 유효성분으로 포함하는 조성물을 이를 필요로 하는 개체에게 투여하는 단계를 포함하는 말초동맥질환의 예방 또는 치료방법을 제공한다.
본 명세서에서 사용된 용어, "투여"또는"투여하다"는 본 발명의 조성물의 치료적 유효량을 상기 조성물을 필요로 하는 개체(즉, 대상체)에 직접적으로 투여함으로써 대상체의 체내에서 동일한 양이 형성되도록 하는 것을 말한다. 따라서, 용어 "투여"는 본 발명의 조성물을 병변 부위에 주입하는 것을 포함하므로, 용어"투여하다"는"주입하다"와 같은 의미로 사용된다.
조성물의“치료적 유효량"은 조성물을 투여하고자 하는 개체에게 치료적 또는 예방적 효과를 제공하기에 충분한 조성물의 함량을 의미하며, 이에 "예방적 유효량"을 포함하는 의미이다. 본 명세서에서 사용된 용어, "개체"는 제한 없이 인간, 마우스, 래트, 기니아 피그, 개, 고양이, 말, 소, 돼지, 원숭이, 침팬지, 비비 또는 붉은털 원숭이를 포함한다. 구체적으로는, 본 발명의 개체는 인간이다.
본 발명인 말초동맥질환의 예방 또는 치료방법의 경우, 본 발명의 일 양태인 말초동맥질환의 예방 또는 치료용 약학적 조성물을 투여하는 단계를 포함하는 방법이므로, 중복되는 내용에 대해서는 본 명세서 기재의 과도한 복잡성을 피하기 위해 생략하도록 한다.
본 발명의 특징 및 이점을 요약하면 다음과 같다:
(a) 본 발명은 말초동맥질환(Peripheral Artery Disease; PAD) 예방 또는 치료용 약학적 조성물을 제공한다.
(b) 본 발명의 조성물을 이용하면 혈관 내피 세포의 유주 및 혈관 생성의 현저한 촉진을 통하여 HGF, 이의 이형체, SDF-1α 또는 이의 단백질을 암호화하는 폴리뉴클레오타이드를 단독으로 이용한 경우보다 말초동맥질환(예컨대, 허혈성 지체질환)을 효과적으로 예방 또는 치료할 수 있다.

이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로서, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 당업계에서 통상의 지식을 가진 자에 있어서 자명할 것이다.
실시예
실시예 1. HUVEC (Human Umbilical vein endothelial cell)의 세포 유주에 HGF와 SDF - 1α의 조합이 미치는 효능 확인실험방법
인간의 탯줄로부터 정맥의 내피세포만을 취하여 단일 세포화한 뒤 배양하여 얻은 HUVEC을 Lonza에서 구매하였다.
HUVEC의 세포 유주에서 HGF(서열목록 제1서열, R&D systems Cat No. 294-HG-025/CF, 미국) 및 SDF-1α(서열목록 제4서열, R&D systems Cat No. 350-NS-010/CF, 미국)가 미치는 영향을 확인하기 위해 트랜스웰(Corning, cat#3422)을 1% 젤라틴으로 코팅한 후 웰당 2×104개의 세포를 파종하였다. 세포가 붙을 수 있도록 한 시간 동안 배양한 후, 각각 다음과 같이 실험군을 편성하였다(50 ng/ml HGF; 50 ng/ml SDF-1α; 25 ng/ml HGF + 25 ng/ml SDF-1α). 각 실험군에 해당 단백질을 두 시간 동안 처리하였고, 세포 유주의 정도를 측정하기 위하여 세포를 크리스탈 바이올렛으로 염색한 뒤 현미경을 이용하여 트랜스웰을 이동한 세포의 개수를 측정하였다.
그 결과, 50 ng/ml HGF를 단독으로 처리한 경우 대조군에 비하여 1.8배 세포 유주가 증가하였고, 50 ng/ml SDF-1α를 단독으로 처리한 경우 대조군에 비하여 1.5배 세포 유주가 증가하였다. HGF 및 SDF-1α를 각 25 ng/ml씩 같이 처리한 경우에는 세포 유주가 대조군에 비하여 2.5배 증가하여 각각을 단독으로 처리한 것보다 세포 유주에 대한 효과가 더 좋은 것을 확인하였다(도 1).
실시예 2. 마트리젤 플러그 분석( Matrigel plug assay)에서 HGF와 SDF - 1α의 조합이 혈관생성에 미치는 효능 확인
마트리젤 플러그 분석를 이용하여 혈관 생성에 HGF와 SDF-1α가 미치는 영향을 확인하였다.
5주령의 C57BL/6 마우스를 다음과 같이 나누어 실험군을 편성하였다(PBS; 300 ng HGF; 150 ng HGF + 150 ng SDF-1α). 400 μl 마트리젤 매트릭스(Corning, cat#356231)에 헤파린 1 유닛을 넣고 각각 실험군에 해당하는 단백질을 첨가하였다. 이렇게 만들어진 마트리젤 혼합물을 마우스 복부에 피하주사 하였다. 7일 후에 마우스를 희생시킨 뒤 이식한 마트리젤 매트릭스를 분리하였고, 혈관 생성 정도를 정량화하기 위해 각각의 마트리젤에 포함된 헤모글로빈(hemoglobin) 수치를 Drabkin’s assay를 통하여 측정하였다.
그 결과, 300 ng HGF를 넣은 군의 경우 PBS를 넣은 군에 비해 헤모글로빈의 수치가 약 1.8배 증가하였고, HGF 및 SDF-1α를 각각 150 ng씩 넣은 군의 경우 PBS군에 대비하여 헤모글로빈의 수치가 약 2.3배 증가하여, HGF 단독 투여 시보다 혈관 생성 정도가 향상된 것을 확인하였다(도 2a 및 2b).
실시예 3. 하지 허혈( hindlimb ischemia ; HLI ) 마우스 모델에의 HGF SDF -1α의 투여 효능 확인
실시예 3-1: 플라스미드 DNA의 제조

하지 허혈 마우스 모델에 대한 실험에 앞서, 사용할 플라스미드 DNA를 다음과 같은 방식으로 제조하였다. pCK 벡터는 발현하고자 하는 대상을 인간 사이토메갈로바이러스(hCMV)의 인핸서/프로모터의 조절 하에 있도록 구축된 벡터이며, Lee et al., Biochem. Biophys. Res. Commun. 272:230 (2000); WO 2000/040737에 상세히 개시되어 있다. 본 발명에서 이용한 pCK-HGF 플라스미드는 WO 2003/078568에 기재되어 있는 방법에 따라 인간 HGF 유전자의 엑손 4 및 5 사이에 인간 HGF 유전자의 인트론 4의 단편 서열이 삽입된 하이브리드 유전자(즉, HGF-X7 유전자; 서열목록 제5서열)를 pCK 벡터로 삽입하여 제작하였다.
pCK - SDF - 플라스미드의 제조
인간 SDF-1α의 유전자 정보(NCBI Reference Sequence: NM_199168.3)를 바탕으로 양 끝에 NheI과 NotI 제한효소 서열을 추가하여 유전자 합성을 하였다. 합성된 인간 SDF-1α 조각을 NheI과 NotI을 이용하여 pCK 벡터에 삽입하였다. pCK 벡터에 삽입된 인간 SDF-1α 유전자의 서열은 서열목록 제6서열과 같다.
실시예 3-2: 하지 허혈( HLI ) 마우스 모델 제조 및 플라스미드 DNA의 투여
HLI 마우스 모델은 인간의 중증 하지 허혈(Critical Limb Ischemia; CLI)을 모방하는 가장 대표적인 마우스 모델이다[1, 2]. 상기 마우스 모델의 제작방법은 다음과 같다. Balb/c 마우스 7주령 수컷을 졸레틸(zoletil)과 럼펀(rumpun) 혼합액으로 마취시킨 후, 허벅지 쪽 피부를 약 1 cm 절개시켰다. 그 후, 허벅지 안쪽에 있는 대퇴부 동맥(femoral artery)의 위치를 찾아내어 6-0 굵기의 실로 동맥의 약 1 cm 정도의 길이를 잘 묶어주고, 그 사이의 조직을 잘라내어 혈관을 제거하였다. 이러한 방법을 통해 허벅지 아래로 내려가는 혈관을 제거함으로써 허혈 조건을 유도할 수 있다. HLI 유도와 동시에, 제거한 혈관 근처의 근육에 평가하고자 하는 플라스미드 DNA를 투여하였다. 이후, 절개한 부분을 잘 봉합해주고 마우스가 마취로부터 잘 회복하는지 지켜보았다.
HLI 마우스 모델은 군당 6마리씩 편성하여 하기의 플라스미드를 각각 투여하였다: 200 μg pCK; 200 μg pCK-HGF; 200 μg pCK-SDF-1α; 200 μg pCK-HGF + 200 μg pCK-SDF-1α)의 플라스미드를 투여하였다. 투여 후 각각 3일, 10일, 17일, 22일 및 27일에 다리의 상태를 관찰하여 일정한 기준에 의해 점수를 매겨 수치화하였다. 이때 사용한 기준은 하기와 같다 [3]: 0 = 정상 상태; 1 = 발톱의 괴사; 2 = 발가락의 괴사; 3 = 발 조직의 괴사.
그 결과, HLI를 유도한 후 pCK 투여군의 경우 다리의 평균적인 상태가 서서히 나빠지기 시작하여 약 2주 후의 점수가 1.83 정도로 올라갔다. pCK-HGF 투여군의 경우 역시 시간이 지남에 따라 점수가 0.66-0.83 정도로 올라갔다. 반면, pCK-HGF 및 pCK-SDF-1α 병용 투여군의 경우 HLI 유도 후 점수가 0으로 유지되었다(도 3).
결과적으로, 상기의 실시예 1-3을 통하여 HGF 및 SDF-1α 의 병용 투여, HGF 및 SDF-1α를 각각 암호화하는 폴리뉴클레오타이드를 병용 투여한 경우 각각의 단백질 또는 폴리뉴클레오타이드를 단독으로 투여한 경우보다 혈관 내피 세포의 유주 및 혈관 생성 촉진 효과, 그리고 허혈성 지체질환에 대한 치료 효과가 현저함을 확인할 수 있었다.

이상으로 본 발명의 특정한 부분을 상세히 기술하였는 바, 당업계의 통상의 지식을 가진 자에게 있어서 이러한 구체적인 기술은 단지 바람직한 구현 예일 뿐이며, 이에 본 발명의 범위가 제한되는 것이 아닌 점은 명백하다. 따라서, 본 발명의 실질적인 범위는 첨부된 청구항과 그의 등가물에 의하여 정의된다고 할 것이다.

Claims (14)

  1. (a) 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 (b) HGF를 암호화하는 폴리뉴클레오타이드 및 SDF-1α를 암호화하는 폴리뉴클레오타이드를 유효성분으로 포함하는 말초동맥질환(Peripheral Artery Disease; PAD)의 예방 또는 치료용 약제학적 조성물.
  2. 제 1 항에 있어서, 상기 HGF는 서열목록 제1서열의 아미노산 서열을 포함하는 것을 특징으로 하는 조성물.
  3. 제 1 항에 있어서, 상기 HGF의 이형체는 전장 HGF(full length HGF; flHGF) 및 결손된 변이형 HGF(deleted variant HGF; dHGF)를 포함하는 것을 특징으로 하는 조성물.
  4. 제 3 항에 있어서, 상기 전장 HGF는 서열목록 제2서열의 아미노산 서열을 포함하고, 상기 결손된 변이형 HGF는 서열목록 제3서열의 아미노산 서열을 포함하는 것을 특징으로 하는 조성물.
  5. 제 1 항에 있어서, 상기 SDF-1α는 서열목록 제4서열 또는 서열목록 제8서열의 아미노산 서열을 포함하는 것을 특징으로 하는 조성물.
  6. 제 1 항에 있어서, 상기 HGF를 암호화하는 폴리뉴클레오타이드는 인간 HGF 유전자의 엑손 1 내지 4에 해당하는 서열, 인간 HGF 유전자의 인트론 4 또는 그의 단편 서열, 및 인간 HGF 유전자의 엑손 5 내지 18에 해당하는 서열을 포함하는 것을 특징으로 하는 조성물.
  7. 제 6 항에 있어서, 상기 HGF를 암호화하는 폴리뉴클레오타이드는 서열목록 제5서열인 것을 특징으로 하는 조성물.
  8. 제 1 항에 있어서, 상기 SDF-1α를 암호화하는 폴리뉴클레오타이드는 서열목록 제6서열의 뉴클레오타이드 서열을 포함하는 것을 특징으로 하는 조성물.
  9. 제 1 항에 있어서, 상기 말초동맥질환은 허혈성 지체질환(Ischemic Limb Disease)인 것을 특징으로 하는 조성물.
  10. 제 1 항에 있어서, 상기 폴리뉴클레오타이드는 네이키드 DNA(naked DNA) 또는 유전자 운반체에 포함되어 있는 뉴클레오타이드인 것을 특징으로 하는 조성물.
  11. 제 10 항에 있어서, 상기 유전자 운반체는 벡터인 것을 특징으로 하는 조성물.
  12. 제 11 항에 있어서, 상기 벡터는 플라스미드인 것을 특징으로 하는 조성물.
  13. 제 12 항에 있어서, 상기 플라스미드는 pCK인 것을 특징으로 하는 조성물.
  14. (a) 간세포 성장인자(Hepatocyte Growth Factor: HGF) 또는 이의 이형체, 및 기질세포 유발인자1알파(Stromal Cell derived Factor 1α: SDF-1α); 또는 (b) HGF를 암호화하는 폴리뉴클레오타이드 및 SDF-1α를 암호화하는 폴리뉴클레오타이드를 유효성분으로 포함하는 조성물을 이를 필요로 하는 개체에게 투여하는 단계를 포함하는 말초동맥질환의 예방 또는 치료방법.
