Abstract

4種すべてのデングウイルス血清型に対する中和抗体応答を単回投与にて誘導し、一定量以上のウイルス血症を生じない安全性の高いデングワクチンを見出した。薬効(中和抗体応答)と安全性(ウイルス血症)の成績が共に優れる4価デングウイルス製剤を提供する。

Description

デングウイルス弱毒株をバンク化した生ウイルス、及びそれらを抗原とするデングワクチン
 本発明は弱毒化デングウイルスを含むデングワクチンに関する。
 デングウイルス感染症は、特定の感染症としては世界三大感染症(エイズ、マラリア、結核)に次いで発症者が多く、世界規模で蔓延している。
 デングウイルスは、フラビウイルス科に属する1本鎖RNAウイルスであり、抗原性で区別される4種の血清型が存在する。ある種の血清型のデングウイルスに感染すると、獲得免疫により同じ血清型のデングウイルスにはおよそ生涯にわたって再感染することはないとされるものの、一方で異なる血清型のウイルスに対する交差保護は長くても12週間程度しか続かないため、2回以上のデングウイルス感染が生じうる。特に、死に至ることがある重篤な病態である「デング出血熱」や「デングショック症候群」は、2回目以降の感染時に起こりやすいことが知られている。
 デングウイルス感染症には確立された治療法はなく、予防法も限定的である。その他、感染対策としてはデングウイルスの宿主である蚊の駆除や環境整備、個人レベルの対策として忌避剤の使用や適切な服装についての指導に頼っているのが現状である。しかし、これらは費用面や効果面で十分とはいえず、幅広い年齢層への接種が可能で、且つ優れた有効性を示すワクチンによる感染防止又は発症予防が望まれている。

 上述の通り、初感染とは異なる血清型による2回目以降の感染により症状が重篤化するリスクが高いと考えられていることから、同時に4種すべての血清型のデングウイルスへの防御能を賦与しうる4価ワクチンが望まれている。しかしながら数十年に渡るワクチン開発への取り組みがなされたにもかかわらず、4種すべての血清型に対して優れた有効性を示すワクチンは存在しない。
 デングウイルスゲノムは、Cタンパク質、Mタンパク質、Eタンパク質などの構造タンパク質と、ウイルス粒子を構成せず複製時に働く7つの非構造(NS)タンパク質をコードしている。よって、他のウイルスや特定の血清型のデングウイルスをバックボーンとする遺伝子組換えキメラ生ワクチン、又は、不活化全粒子ワクチンやサブユニットワクチン、DNAワクチンなどは、デング由来コンポーネントを欠いており、デングウイルスの自然感染により生体内に生じる様々な免疫抗原の内の一部を提示するにすぎない。
 一方で、自然宿主とは異なる細胞にて継代馴化して宿主細胞域変異を獲得した弱毒化生ワクチンは、麻疹、風疹、おたふくかぜ、ポリオ等で実績があるように、自然感染時と同様の免疫応答を誘導しうることから、デングウイルス感染症においても高い有効性を期待しうる。
 現在開発中であることが報告されている又は上市されたデングワクチンは、複数回投与することで4種全ての血清型のデングウイルスに対する中和抗体を誘導させることが可能であるが、4種すべての血清型に対する中和抗体が獲得されるまでに期間を要し、流行地域ではその期間中に自然感染するリスクがあり、単回投与により4種すべての血清型に対する防御免疫を賦与しうるワクチンがより有用性が高いと言える。
 デングウイルスワクチンの調製にあたって、各血清型の株及び用量の混合組成の選定は容易ではない。単価で投与した場合に高い中和抗体を誘導可能であっても、4価混合し投与すると各血清型のデングウイルスに対する中和抗体レベルが均一とならない(干渉作用)問題がある。また、不十分な中和活性(交差反応、結合)抗体による抗体依存性感染増強(ADE;Antibody-Dependent Enhancement)の問題もある。
 既承認薬又は臨床段階にある主要な開発品は、SanofiのChimeriVax-DEN(Dengvaxia(商標名))、TakedaのDENVax(商標名)及びButantanのTetraVax(商標名)の3つであり、いずれも遺伝子組み換えキメラ生ワクチンである。SanofiのDengvaxiaは、黄熱ワクチン株YF-17Dをバックボーンとする遺伝子組換えキメラ生ワクチンであり、メキシコ、ブラジル及びフィリピンの3ヶ国で9歳から45歳を対象に6箇月間隔3回接種の用法で承認されている。
 DengvaxiaのPhase3試験は、東南アジア5ヶ国において、2歳から15歳の年齢層の約1万人、及び南米5ヶ国にて9~16歳の年齢層の約2万人を被験者とし実施された。その結果、有効性は6割程度に留まり、特に血清型2に対する有効性は約4割と低く十分ではなかった。また、デング既存免疫がない被験者に対する有効性は約4割であった。さらに、Phase3試験の長期フォローアップ及びタイにおけるPhase2b試験の長期フォローアップでの結果では、3年目の時点で9歳未満の年齢層での入院リスクが非接種群に対して1.6倍と高くなっており、特に5歳以下では約5倍と高くなっていた。この点がVaccine Enhancementではないかと問題視されている。有効性が低い原因としては、デング由来コンポーネントの不足や流行株の変異などが考察されている。よって、デング由来コンポーネントをすべて含み、かつ、積極的なクローニングを行っておらずウイルスの亜集団(quasispecies)を含む、継代馴化生ワクチンであればより高い効果が期待できる。
 本発明者らは上記課題を解決するために鋭意研究した結果、薬効と安全性の成績が共に優れる弱毒化4価デング生ウイルスワクチンを見出した。また、単回投与であっても、複数回投与であっても、薬効と安全性の成績が優れる弱毒化4価デング生ウイルスワクチンを見出した。
 野生型のデングウイルス(血清型1~4)を、イヌ初代腎(PDK;Primary Dog Kidney)細胞やミドリザル初代腎(PGMK;Primary Green Monkey Kidney)細胞で継代して得た弱毒株が知られている。本発明は、さらに、アフリカミドリザル腎(Vero)細胞で継代することによって得られる弱毒化デングウイルス、及び、それらをバンク化した生ウイルスを抗原として含む弱毒化デングウイルスワクチンを提供する。
 したがって、本発明は以下を含む。
[1] 次世代シーケンサーを用いて解析した配列番号1のアミノ酸配列を有する血清型1デングウイルス親株03135との相同性が95%以上あり、該親株の483位、568位、1663位のいずれかのアミノ酸が完全に変異しており、さらに以下の(1)~(4)のいずれか一つ以上の変異を有する弱毒化血清型1デングウイルス;

[49] 次世代シーケンサーを用いて解析した配列番号4のアミノ酸配列を有する血清型4デングウイルス親株1036をPDK細胞で35~45継代して弱毒株とし、前記弱毒株をさらにVero細胞で継代することを含む、[31]~[38]に記載の弱毒化血清型4デングウイルスの製造方法。
[50] 次世代シーケンサーを用いて解析した配列番号4のアミノ酸配列を有する血清型4デングウイルス親株1036をPDK細胞で40継代して弱毒株とし、前記弱毒株をさらにVero細胞で継代することを含む、[31]~[38]に記載の弱毒化血清型4デングウイルスの製造方法。
 本発明は、4種すべてのデングウイルス血清型に対する中和抗体応答を単回投与であっても誘導でき、上市されている他の弱毒生ウイルスワクチンと同等レベルの安全性(忍容性)を有するデングワクチンを提供するものである。

 本発明によれば、4種すべてのデングウイルス血清型に対する中和抗体応答を単回投与であっても誘導することが可能であり、上市されている他の弱毒生ウイルスワクチンと同等レベルの安全性(忍容性)を有するデングワクチンが提供される。
 以下、本発明の好適な実施形態について詳細に説明する。ただし、本発明は以下の実施形態に限定されるものではない。
 本発明は、臨床分離された野生型デングウイルス(親株)をPDK細胞、又はPGMK細胞とPDK細胞で継代することによって得られた弱毒株を、さらにVero細胞で継代して得られる弱毒化デングウイルスに関するものであり、デングワクチン用として用いられるシード株(VMS(Virus Master Seed)、VWS(Virus Working Seed)等)や製品相当継代株(GLP試験用バルク(Pre-CTM:Pre-Clinical Trial Material)等)、及びそれらを用いて作製されるデングワクチンが含まれる。本発明では、親株、弱毒株、シード株、製品相当継代株のアミノ酸配列を比較検討することにより、親株と弱毒株が識別できる部位及び弱毒株とシード株、製品相当継代株が識別できる部位を特定している。
 本発明の弱毒化血清型1デングウイルスは、臨床分離株である親株03135(配列番号1)を、イヌ初代腎(PDK)細胞にて15~25継代して作出された弱毒株1を、さらにVero細胞で継代することにより得られる。弱毒株1は継代に伴って、親株03135に対して、483位のグルタミン酸、568位のリジン、1663位のアスパラギンのいずれか又はすべてが完全に他のアミノ酸に変異しており、好ましくは、E483K、K568R、N1663Kの変異を有する本発明の弱毒化血清型1デングウイルスは、さらにVero細胞で継代することにより、上記変異に加えて、482位、484位、2353位、2364位のいずれか一つ以上にも変異を有しており、弱毒株1と遺伝的に識別可能である。
 本発明の弱毒化血清型2デングウイルスは、臨床分離株である親株99345(配列番号2)を、PDK細胞にて20~35継代して作出された弱毒株2を、さらにVero細胞で継代することにより得られる。弱毒株2は継代に伴って、親株99345に対して、143位のアスパラギン酸、400位のトレオニン、1102位のアスパラギン酸、1308位のロイシン、1654位のグルタミン酸のいずれか又はすべてが完全に他のアミノ酸に変異しており、好ましくは、D143N、T400K、D1102N、L1308F、E1654Kの変異を有する。本発明の弱毒化血清型2デングウイルスは、さらにVero細胞で継代することにより、2347位又は2828位にも変異を有しており、弱毒株2と遺伝的に識別可能である。
 本発明の弱毒化血清型3デングウイルスは、臨床分離株である親株16562(配列番号3)を、PGMK細胞で20~30継代し、さらにPDK細胞にて2~5継代して作出された弱毒株3を、さらにVero細胞で継代することにより得られる。弱毒株3は、親株16562に対して、209位のイソロイシン、582位のセリン、1211位のフェニルアラニン、1563位のグルタミンのいずれか又はすべてが完全に他のアミノ酸に変異しており、好ましくは、I209L、S582G、F1211L、Q1563Kの変異を有する。本発明の弱毒化血清型3デングウイルスは、さらにVero細胞で継代することにより、671位、687位、764位、1237位のいずれか一つ以上にも変異を有しており、弱毒株3と遺伝的に識別可能である。
 本発明の弱毒化血清型4デングウイルスは、臨床分離株である親株1036(配列番号4)を、PDK細胞にて35~45継代して作出された弱毒株4を、さらにVero細胞で継代することにより得られる。弱毒株4は、親株1036に対して、2187位のロイシン、2354位のフェニルアラニン/ロイシン(混合アミノ酸)のどちらか又は両方が完全に他のアミノ酸に変異しており、好ましくは、L2187F、F/L2354Sの変異を有する。本発明の弱毒化血清型4デングウイルスは、624位、742位、1628位、2286位、2482位、又は2508位に変異を有さず、弱毒株4と遺伝的に識別可能である。
 本発明には、上記の各血清型のデングウイルスを抗原として含むワクチンも含まれる。血清型1~4それぞれの単価ワクチンであっても良く、いずれかの血清型の組合せでも良く、4価ワクチンであっても良い。4価ワクチンの場合、血清型1、2、3、及び4の抗原混合比率は任意であってよいが、特に1:1:1:1、5:3:5:3、5:3:3:3が好ましい。本発明のデングウイルスワクチンをプライミングワクチンとし、他のワクチンをブースターとして組み合わせてもよい。また、他のフラビウイルスワクチンと混合しても良く、他の感染症などのワクチンと混合して用いても良い。
 ワクチンの剤型は適時選択し得る。液状製剤であっても良く、粉末製剤であってもよく、凍結乾燥製剤であっても良い。
 親株16562と親株1036は、特許文献1に開示されており、また、フランスのCNCM(Collection Nationale de Cultures de Micro-organismes)に、それぞれI-2482、I-2483として寄託されている。また、親株03135はGenBankにADJ18295.1として部分配列が登録されており、親株99345はGenBankにADJ18294.1として部分配列が登録されている。本発明の弱毒化デングウイルス(血清型1~4)は、米国のATCC(American Type Culture Collection; 10801 University Boulevard, Manassas, VA 20110 USA)にそれぞれ寄託番号PTA-123506、PTA-123505、PTA-123507、PTA-123508として寄託されている。
 本発明では、デングウイルスの塩基配列を次世代シーケンサーを用いて解析している。次世代シーケンサーとは、第2世代シーケンサーとも呼ばれ、例えばIllumina社のSBSシーケンサーの場合、数百万本以上のDNA断片の塩基配列を同時並行的に決定することができる。原理は、基本的にはキャピラリー電気泳動を用いた従来のサンガー法と類似しており、DNA断片をテンプレートとし1塩基ずつ再合成する時の蛍光強度を検出し塩基配列を決定するものである。サンガー法では1~96本のDNA断片を同時処理するのに対し、次世代シーケンサーではサンガー法でのキャピラリーに相当する「クラスター」を形成させることで、数百万本以上のDNA断片に対して大量並列処理を行うところに違いがある。これによりシーケンス解析のスピードは飛躍的に向上し、大きなゲノム領域を対象とする研究や高いリード数が要求される低頻度変異の研究や亜集団(quasispecies)の研究ができるようになった。
 次世代シーケンサーを用いた塩基配列解析により、ウイルスの亜集団の研究が可能である。混合塩基がある場合にその混合比を得られ、亜集団を確認できることが大きな特徴である。本明細書では、その塩基配列をアミノ酸配列に変換して表記しており、例えば、弱毒株の第484部位はリジンが30%、アルギニンが70%などのように示される。
 本明細書では、親株や弱毒株などとの比較において変異がある部位は、K482E、K482E/Kなどのように記載する。ここで、「K482E」とは482位のリジンがグルタミン酸に変異していることを意味する。「K482E/K」とは、リジンであった482位に、変異の結果、グルタミン酸に変異したものとリジンに変異したものの両方が混在していることを意味する。
 デングウイルスゲノムはCタンパク質、Mタンパク質、Eタンパク質といった構造タンパク質、そしてウイルス粒子を構成せず複製時に働く7つの非構造(NS)タンパク質をコードしている。デングウイルスゲノムからは、1本のポリタンパク質が翻訳された後、それぞれの構造タンパク質及び非構造タンパク質にプロセシングを受ける。本明細書では、アミノ酸配列中の位置を数える上で、ポリタンパク質の開始コドンを起点としている。
 次世代シーケンサーで測定する際には、まずデングウイルスからRNAを抽出し、Randomプライマーによる逆転写、二本鎖化、アダプター配列付加、増幅を経て、SBSシーケンサーで短鎖配列の集合データを得る。親株のデータからアセンブラーVelvetによりコンティグを作成し、デングウイルスに相同で最も長いコンティグを得る。当該コンティグと相同性の高い遺伝子バンク上の配列をアライメントし、5’末端及び/又は3’末端の配列を補いリシーケンス解析の参照配列とする。リシーケンス解析では、マッパーBWAを用いてシーケンスデータを参照配列にマッピングし、ウイルスゲノム配列を混合塩基も加味して決定する。得られたウイルスゲノム配列から混合アミノ酸を加味してアミノ酸配列を得る。なお、混合アミノ酸には、混合比として全体の10%以上存在するアミノ酸のみを含めるものとする。
 次世代シーケンサーの機種は、MiSeqやHiSeq(共にIllumina社製)などが知られているが、その他の機種であっても構わない。
 デングウイルスの弱毒特性は、プラークサイズアッセイや増殖温度感受性試験、動物実験などにより確認することができるが、これに限定されない。プラークサイズアッセイでは、例えばアカゲザル由来のLLC-MK2細胞を用い、プラークの直径を測定し、親株と比較して直径が小さければ弱毒特性が確認される。デングウイルスの増殖温度感受性試験では、例えばVero細胞を用い、培養温度を変化させ、親株と比較して高温増殖能が減じていれば弱毒特性が確認される。
 デングウイルスの薬効・安全性試験は、NHP(Non-Human Primate)や一部の遺伝子組換え免疫欠損マウスを用いて行われる。NHPの代表例はカニクイザルである。
 中和抗体価測定は、プラークを指標とするPRNT(Plaque Reduction Neutralizing Test)法やウイルス由来タンパク質を抗体で標識するImmunospot PRNT又はFRNT(Focus Reduction Neutralization Titer)法が用いられる。
 血中ウイルス量の測定は、直接的にプラークサイズアッセイを用いるか、間接的にウイルスゲノム断片を検出するRT-qPCR(Real Time Quantitative Polymerase Chain Reaction)法が主に用いるがこれに限定されない。
 以下、実施例により本発明を詳細に説明するが、本発明はこれらの実施例に何ら限定されるものではない。
(1)弱毒株の作製
 タイ国マヒドン大学にて、新規のワクチン候補となる弱毒化デングウイルス(血清型1、血清型2、血清型3及び血清型4)が次の手順で作出された。血清型1の弱毒化デングウイルスは、臨床分離株である03135を親株として、イヌ初代腎(PDK)細胞にて15継代することで作出された。血清型2の弱毒化デングウイルスは、臨床分離株である99345を親株として、PDK細胞にて25継代することで作出された。血清型3の弱毒化デングウイルスは、臨床分離株である16562を親株として、初代ミドリザル腎(PGMK)細胞にて30継代し、さらにPDK細胞にて4継代することで作出された。血清型4の弱毒化デングウイルスは、臨床分離株である1036を親株として、PDK細胞にて40継代することで作出された。なお、血清型1の弱毒化デングウイルスはVero細胞でさらに3継代された。これらの弱毒化デングウイルスを、それぞれ弱毒株1~4と称する。
(2)PreVMSの作製
 VMS(Virus Master Seed)の作製にあたって、まず、preVMSシードを作製した。preVMSシードは、弱毒株1~4をVero細胞にてそれぞれ2継代、3継代、3継代、2継代することで得た。以下に各血清型のPreVMSシードの具体的な作製方法について記載する。
(i)血清型1PreVMSシード(PreVMS1)
 1継代目は、Multiplicity of Infection(MOI)0.001にて弱毒株1を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養12日後に培養容器ごと2回凍結融解した。それから、遠心分離(1500rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。安定剤として上清の20%量のFBS(Fetal Bovine Serum;ウシ胎児血清)を添加後、重曹によりpHを調整し、-60℃以下で保存した。プラークアッセイで測定した結果、感染価は1.6×107PFU/mLであった。
 2継代目は、弱毒株1を1継代したウイルスをMOI0.001にて接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養9日後に培養上清を回収した。培養上清に安定剤として20%終濃度となるようFBSを添加後、重曹によりpHを調整し、一時的に凍結保存した。融解後、0.2μm無菌ろ過し、-60℃以下で保存した。プラークアッセイで測定した結果、PreVMS1の感染価は5.1×106PFU/mLであった。
(ii)血清型2PreVMSシード(PreVMS2)
 1継代目は、MOI0.001にて弱毒株2を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、MEM+2%培地を添加し、37℃・5%CO2下で培養し、培養10日後に培養容器ごと2回凍結融解した。それから、遠心分離(1500rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。安定剤として20%終濃度となるようFBSを添加後、重曹によりpHを調整し、0.2μm無菌ろ過し、-60℃以下で保存した。プラークアッセイで測定した結果、感染価は2.7×105PFU/mLであった。
 2継代目は、弱毒株2を1継代したウイルスをMOI0.001にて接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、MEM+2%培地を添加し、37℃・5%CO2下で培養し、培養11日後に培養容器ごと2回凍結融解した。それから、遠心分離(1500rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。0.2μm無菌ろ過後、安定剤として上清の20%量のFBSを添加し、重曹によりpHを調整した後、-60℃以下で保存した。プラークアッセイで測定した結果、感染価は1.1×104PFU/mLであった。
 3継代目は、弱毒株2を2継代したウイルスをMOI0.001にて接種し、37℃で90分間ウイルスをVero細胞に吸着させた。この後、MEM+2%培地を添加し、37℃で密栓培養し、培養6日後に培養上清を回収した。それから、遠心分離(1500rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。0.2μm無菌ろ過後、安定剤として上清の20%量のFBSを添加し、重曹によりpHを調整した後、-60℃以下で保存した。プラークアッセイで測定した結果、PreVMS2の感染価は7.1×104PFU/mLであった。
(iii)血清型3PreVMSシード(PreVMS3)
 1継代目は、MOI0.001にて弱毒株3を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養10日後に培養上清を回収した。0.2μm無菌ろ過後、安定剤として培養上清の20%量のFBSを添加し、重曹によりpHを調整した後、-60℃以下で保存した。プラークアッセイで測定した結果、感染価は9.8×104PFU/mLであった。
 2継代目は、弱毒株3を1継代したウイルスをMOI0.001にて接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養7日後に培養上清を回収した。それから、遠心分離(1500rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。0.2μm無菌ろ過後、安定剤として上清の20%量のFBSを添加し、重曹によりpHを調整した後、-60℃以下で保存した。プラークアッセイで測定した結果、感染価は1.3×105PFU/mLであった。
 3継代目は、弱毒株3を2継代したウイルスをMOI0.001にて接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養7日後に培養上清を回収した。それから、遠心分離(1500rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。0.2μm無菌ろ過後、安定剤として上清の20%量のFBSを添加後、重曹によりpHを調整し、-60℃以下で保存した。プラークアッセイで測定した結果、PreVMS3の感染価は3.7×105PFU/mLであった。
(iv)血清型4PreVMSシード(PreVMS4)
 1継代目は、MOI0.001にて弱毒株4を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養7日後に培養上清を回収した。それから、遠心分離(1500rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。0.2μm無菌ろ過後、安定剤として20%終濃度となるようにFBSを添加し、重曹によりpHを調整した後、-60℃以下で保存した。プラークアッセイで測定した結果、感染価は1.9×107PFU/mLであった。
 2継代目は、弱毒株4を1継代したウイルスをMOI0.001にて接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養5日後に培養上清を回収した。それから、遠心分離(2000rpm・5分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。0.2μm無菌ろ過後、安定剤として20%終濃度となるようにFBSを添加後、重曹によりpHを調整し、-60℃以下で保存した。プラークアッセイで測定した結果、PreVMS4の感染価は5.5×107PFU/mLであった。
(3)VMSの作製
 PreVMSをさらに1継代し、ウイルスバンクの最上流となるVMS1、VMS2、VMS3及びVMS4を作製した。以下に各血清型のVMSの具体的な作製方法について記載する。
(i)VMS1
 MOI0.01にてPreVMS1を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養7日後に培養容器ごと2回凍結融解した。それから、遠心分離(3000rpm・10分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。安定剤として20%終濃度となるようにFBSを添加及び0.2%終濃度となるように重曹を添加し、0.2μm無菌ろ過後、-60℃以下で保存した。プラークアッセイで測定した結果、VMS1の感染価は1.49×106PFU/mLであった。
(ii)VMS2
 MOI0.001にてPreVMS2を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養8日後に培養上清を回収した。それから、遠心分離(1800rpm・10分間)にて細胞及び細胞残滓を沈殿させ、上清を回収し、0.2μm無菌ろ過した。安定剤として上清の0.258倍量のFBS及び0.03445倍量の重曹を添加し、-60℃以下で保存した。プラークアッセイで測定した結果、VMS2の感染価は2.07×105PFU/mLであった。
(iii)VMS3
 MOI0.01にてPreVMS3を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養6日後に培養上清を回収した。それから、遠心分離(3000rpm・10分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。安定剤として20%終濃度となるようにFBSを添加及び0.2%終濃度となるように重曹を添加し、0.2μm無菌ろ過後、-60℃以下で保存した。プラークアッセイで測定した結果、VMS3の感染価は6.49×104PFU/mLであった。
(iv)VMS4
 MOI0.01にてPreVMS4を接種し、37℃・5%CO2下で90分間ウイルスをVero細胞に吸着させた。この後、無血清培地を添加し、37℃・5%CO2下で培養し、培養6日後に培養上清を回収した。それから、遠心分離(3000rpm・10分間)にて細胞及び細胞残滓を沈殿させ、上清を回収した。安定剤として20%終濃度となるようにFBSを添加及び0.2%終濃度となるように重曹を添加し、0.2μm無菌ろ過後、-60℃以下で保存した。プラークアッセイで測定した結果、VMS4の感染価は9.39×106PFU/mLであった。
(4)Pre-CTMの作製
続いて、GLP試験用バルク(以下、「Pre-CTM」;Pre-Clinical Trial Material)を作製した。血清型1は、VMS1をVero細胞で4継代することで、Pre-CTM1を得た。血清型2~4は、VMS2~4をそれぞれVero細胞で2継代し、Virus Working Seed(VWS)を作製した。さらにVWS2~4をそれぞれVero細胞で2継代(VMSから数えて合計4継代)し、Pre-CTM2~4を得た。VMSから3継代目までは、培養上清を遠心分離後、その上清に安定剤として20%終濃度となるようにFBSを添加し、0.2%終濃度となるように重曹を添加した。0.2μm無菌ろ過後、-60℃以下で保存した。最終継代となる4継代時は、培養上清に0.2%終濃度となるように重曹を添加し、清澄ろ過及び0.2μm無菌ろ過の後、中空糸膜を用いて濃縮した。濃縮液を0.2μm無菌ろ過した後、-60℃以下で保存した。なお、本実施例ではVWS1を作製せずにPre-CTM1を作製しているが、VMS1をVero細胞で2継代してVWS1を作製した後、さらにVero細胞で2継代してPre-CTM1としても良い。
(5)配列決定
 ウイルスゲノム配列を下記の方法で調べた。各血清型の親株、弱毒株、VMS、Pre-CTMのそれぞれからRNAを抽出し、Randomプライマーによる逆転写、二本鎖化、アダプター配列付加、増幅を経て、Illumina社のSBSシーケンサーで短鎖配列の集合データを得た。親株のデータからアセンブラーVelvetによりコンティグを作成し、デングウイルスに相同で最も長いコンティグを得た。当該コンティグと相同性の高い遺伝子バンク上の配列をアライメントし、5’末端及び/又は3’末端の配列を補いリシーケンス解析の参照配列とした。リシーケンス解析では、マッパーBWAを用いてシーケンスデータを参照配列にマッピングし、ウイルスゲノム配列を混合塩基も加味して決定した(配列番号17~20に親株1~4、配列番号21~24に弱毒株1~4、配列番号25~28にVMS1~4、配列番号29~32にPre-CTM1~4の塩基配列をそれぞれ示す)。得たウイルスゲノム配列から混合アミノ酸を加味してアミノ酸配列を得た(配列番号1~4に親株1~4、配列番号5~8に弱毒株1~4、配列番号9~12にVMS1~4、配列番号13~16にPre-CTM1~4のアミノ酸配列をそれぞれ示す)。なお、混合アミノ酸を表記する場合には、混合比として全体の10%以上存在するアミノ酸のみを表記した。また、アミノ酸配列を比較する際は、各部位の各アミノ酸を比較する時にその各混合比が10%以上乖離していることを閾値とし、差異の有無を判定した。例えば、親株の50位が「アラニン95%、グリシン5%」、弱毒株の50位は「アラニン85%、グリシン14%、プロリン1%」という解析結果を得た場合、親株のアミノ酸表記はA(アラニンのみ)、弱毒株のアミノ酸表記はA/G(アラニンとグリシンの混合アミノ酸)となる。この部位において、親株と弱毒株のアラニンの混合比を比較すると10%以上(10%)の乖離があるため、親株と弱毒株で「差異あり」の判定となる。グリシンについては9%の差異しか認められないが、最低1種類のアミノ酸で差異が認められれば「差異あり」の判定としている。一方、親株の100位が「アラニン94%、グリシン6%」、弱毒株の100位は「アラニン85%、グリシン15%」という場合にも、親株のアミノ酸表記はA、弱毒株のアミノ酸表記はA/Gとなる。しかし、この部位のアラニンとグリシンの混合比をそれぞれ比較すると、共に10%未満(9%)であるため、「差異なし」の判定となる。
(6)アミノ酸配列比較
 親株と弱毒株の配列を比較し、アミノ酸の完全変異が生じている部位を確認した(表1~4)。なお、混合アミノ酸が存在する親株の部位については、弱毒株の同部位において当該親株の混合アミノ酸に含まれない他のアミノ酸に変異している場合に完全変異しているものとみなした。

 次に、弱毒株、VMS、Pre-CTMの配列を比較し、弱毒株に対してVMS及びPre-CTMを識別できる部位を確認した(表5~8)。

 これにより、血清型1のVMS及びPre-CTMは、親株03135に対して483位のグルタミン酸がリジンに、568位のリジンがアルギニンに、1663位のアスパラギンがリジンに完全変異しており、これらの部位において弱毒株と同じ変異であることが確認された。さらに、482位のリジンがグルタミン酸/リジンの混合アミノ酸に、484位のリジンがアルギニン/リジンの混合アミノ酸に、2353位のイソロイシン/トレオニンの混合アミノ酸がトレオニンに、2364位のアラニンがトレオニン/アラニンの混合アミノ酸に変異しており、これらの部位において弱毒株との違いが確認された。
 血清型2のVMS及びPre-CTMは、親株99345に対して143位のアスパラギン酸がアスパラギンに、400位のトレオニンがリジンに、1102位のアスパラギン酸がアスパラギンに、1308位のロイシンがフェニルアラニンに、1654位のグルタミン酸がリジンに完全変異しており、これらの部位において弱毒株と同じ変異であることが確認された。さらに2347位のプロリンがプロリン/ロイシンの混合アミノ酸に、2828位のトレオニンが、トレオニン/メチオニンの混合アミノ酸に変異しており、これらの部位において弱毒株との違いが確認された。
 血清型3のVMS及びPre-CTMは、親株16562に対して209位のイソロイシンがロイシンに、582位のセリンがグリシンに、1211位のフェニルアラニンがロイシンに、1563位のグルタミンがリジンに完全変異しており、これらの部位において弱毒株と同じ変異であることが確認された。さらに、671位のリジン/アルギニンの混合アミノ酸がリジンに、687位のアラニンがバリンに、764位のトレオニンがイソロイシン/トレオニンの混合アミノ酸に、1237位のアラニンがトレオニンに変異しており、これらの部位において弱毒株との違いが確認された。
 血清型4のVMS及びPre-CTMは、親株1036に対して2187位のロイシンがフェニルアラニンに、2354位のフェニルアラニン/ロイシンの混合アミノ酸がセリンに完全変異しており、これらの部位において弱毒株と同じ変異であることが確認された。さらに624位、742位、1628位、2286位、2482位、2508位が変異しておらず、これらの部位において弱毒株との違いが確認された。
(7)VMSの試験管内弱毒特性
 VMSの試験管内弱毒特性を確認するために、LLC-MK2細胞を用いたプラークサイズアッセイを行った。プラークの直径を測定し、測定結果により表9のように区分した。

 その結果、VMS1、VMS2、VMS3及びVMS4のプラークサイズの分布は、それぞれの親株に対して小さく、弱毒特性を示すことを確認した(表10)。

(8)VMSの増殖温度感受性試験
 VMS1、VMS2及びVMS4については35℃及び39℃、VMS3については35℃及び38℃で、それぞれ一定日数の間Vero細胞で培養し、低温培養時と高温培養時での増殖量の差を確認した。その結果、それぞれの親株と比較して、VMS1、VMS2、VMS3及びVMS4すべてにおいて増殖温度感受性が高く、弱毒特性を示すことを確認した(表11)。

1.カニクイザルを用いた4価デングウイルス製剤の薬効・安全性評価試験1
 デングウイルス感受性実験動物は、NHP(Non-Human Primate)や一部の遺伝子組換え免疫欠損マウスに限られるため、NHPであるカニクイザルを用いて、薬効(中和抗体応答)及び安全性(ウイルス血症)の評価を実施した。なお、中和抗体応答はワクチン開発上の主要評価項目の一つであり、ウイルス血症はヒトでの病原性及び症状の重篤化に起因する要素の一つである。
(1)4価デングウイルス製剤(5555製剤)の調製
 実際のワクチン投与時に近い反応を観察するため、VMS1、VMS2、VMS3及びVMS4をVero細胞で4継代した後(VMSVP1、VMSVP2、VMSVP3、VMSVP4)、4価デングウイルス製剤を調製した。その際、各VMSVPの投与量が、5log10PFU/mL、1.0mL/doseとなるように調製した(5555製剤)。この5555製剤を3頭のカニクイザルの前腕部皮下に麻酔下で投与した(5555群)。投与時をDay0とし、Day0、1、2、3、4、6、8、10、14、30、60に麻酔下で採血を実施した。
(2)Immunospot PRNT法による中和抗体価測定
  Immunospot PRNT(Plaque Reduction Neutralizing Test)法により、Day0、14、30、60の採血で得た血清中の中和抗体価を測定した。その結果、全3個体、4種すべての血清型についてDay14以降で中和抗体が陽転しており、少なくともDay60までの間は陽性を維持していた(陽転基準は10PRNT50以上;表12及び表13)。

(3)RT-qPCR法による血中ゲノム量測定
 RT-qPCR(Real Time Quantitative Polymerase Chain Reaction)法により、Day1、2、3、4、6、8、10、14に採取した血清中のウイルスゲノム量を測定したところ、全3個体について全て定量限界未満であった(定量限界:1型は500、2型、3型及び4型は1000;単位はgenome copy/mL serum)。
 上記結果より、良好な薬効及び安全性が確認された。
1.カニクイザルを用いた4価デングウイルス製剤の薬効・安全性評価試験2
 血清型別に異なる配合量(混合用量)を調製した4価デングウイルス製剤の薬効・安全性への影響を確認する目的で、以下の実験を行った。
(1)4価デングウイルス製剤(5353製剤、5333製剤)の調製
 VMSVP1及びVMSVP2、VMSVP3、VMSVP4のそれぞれの含有量を5log10PFU/dose、3log10PFU/dose、5log10PFU/dose、3log10PFU/doseとした製剤(5353製剤)と、VMSVP1の含有量を5log10PFU/dose、VMSVP2、VMSVP3及びVMSVP4の含有量を3log10PFU/doseとした製剤(5333製剤)を調製し、カニクイザル3頭を用いて中和抗体応答及びウイルス血症を評価した(それぞれ5353群、5333群)。
(2)Immunospot PRNT法による中和抗体価測定
 中和抗体応答について、5353群の一頭が4型に対してのみDay14では陽転していなかったことを除き、5353群及び5333群共にDay14以後で中和抗体が陽転しており、Day60までの間、陽転が維持されていた(表6~表9)。また、Day14に4型の陽転が認められなかった5353群の一頭についても、Day30以後は陽転しており、その後Day60まで陽転が維持されていた(表14~17)。

(3)RT-qPCR法による血中ゲノム量測定
 Day4にて5333群の一頭のみから、血清型1及び血清型4についてわずかに定量限界を上回るウイルスゲノムが検出された(それぞれ1632、2808genome/mL)が、5333群及び5353群の他の個体については定量限界未満であった。また、Day4にてウイルスゲノムが検出された一頭についても、血清型1の親株及び血清型4の親株をそれぞれ等量単価接種した場合の個体別ピークウイルス量の幾何平均(それぞれ75001(21864~592650)、9853(3006~32315)genome/mL、N=3)に比して、低い値であった。
 上記結果より、5353製剤及び5333製剤は、5555製剤と同等の薬効・安全性が示され、製剤中における血清型別のウイルス配合量の違いによっては、薬効・安全性に大きな影響はないことが確認された。
 5555製剤、5353製剤及び5333製剤とは異なる弱毒化デングウイルス由来のデングウイルスを含む製剤についても評価する目的で以下の実験を行った。
(1)弱毒株2B及び弱毒株4Bの作製
 上述の実施例1(1)弱毒株の作製において、血清型2の臨床分離株99345を親株として、PDK細胞で35継代して作出した弱毒化デングウイルスを弱毒株2Bと称する。また、血清型4の臨床分離株1036を親株として、PDK細胞にて45継代して作出した弱毒化デングウイルスを弱毒株4Bと称する。
(2)LAVVP2B及びLAVVP4Bの作製
 弱毒株2B及び弱毒株4BをVero細胞で継代し、それぞれLAVVP2B(Live Attenuated Virus Vero Passaged 2B)及びLAVVP4Bを得た。
(3)VMSVP123LAVVP4及びVMSVP13LAVVP24の調製
 VMSVP1、VMSVP2、VMSVP3及びLAVVP4Bによる4価デングウイルス製剤(各5log10PFU/dose;「VMSVP123LAVVP4」と呼称)及びVMSVP1、LAVVP2B、VMSVP3及びLAVVP4Bによる4価デングウイルス製剤(各5log10PFU/dose;「VMSVP13LAVVP24」と呼称)を調製し、カニクイザル各3頭を用いて中和抗体応答及びウイルス血症を評価した(それぞれVMSVP123LAVVP4群、VMSVP13LAVVP24群)。
(4)PRNT法による中和抗体価測定
 PRNT法により、VMSVP123LAVVP4群及びVMSVP13LAVVP24群の中和抗体価を測定した。その結果、両製剤とも5555製剤と同等の中和抗体応答を誘導した(表18及び表19)。

(5)RT-qPCR法による血中ゲノム量測定
 ウイルス血症については、VMSVP123LAVVP4群から比較的高い量の4型のウイルスゲノムがDay1にて検出(10066genome/mL、GMT、N=3)されており、VMSVP13LAVVP24群から比較的高い量の4型のウイルスゲノムと、定量限界をわずかながら上回る量の1型及び2型のウイルスゲノムがDay1にて検出(それぞれ12340、547、1337genome/mL、GMT、N=3)された。これらのことから、VMSVPはLAVVPに比して高度に弱毒化されていることが示唆された。
 興味深いことに、VMSVP13LAVVP24群の2頭より、Day1にて血清型1のウイルスゲノムがわずかながらも検出されており、VMSVPによる4価デングウイルス製剤の組み合わせの素性の良さを示唆していると考えられた。
(1)4価デングウイルス製剤(5555製剤)の中和抗体価長期観察試験
 良好な薬効・安全性を示したVMSVP1、VMSVP2、VMSVP3及びVMSVP4を用いて調製した4価デングウイルス製剤(5555製剤)について、中和抗体価の長期評価を行う目的で、全6頭のカニクイザル(ID No.1~6)に5555製剤を単回接種し、接種月をMonth0とし、Month1、2、3、5、6、7、8、9、10、12、14、16、18、20、23、25と、2年以上に渡り中和抗体価(Focus Reduction Neutralization Titer50;以下「FRNT50」)を測定した。
 その結果、Month1より全ての個体で中和抗体の陽転(FRNT50>10)が認められた。また、中和抗体価の幾何平均値は1~4型いずれに対しても100以上であった。中和抗体価はその後2年以上に渡り同程度に維持されており、下降のトレンドは認められなかった(図1~図4)。
 このことから、4価デングウイルス製剤(5555製剤)は、少なくとも2年以上は薬効が認められ、また下降のトレンドが認められないことから、より長期にわたって中和抗体価が維持されると期待される。
(2)接種回数及び接種間隔の検討
 4価デングウイルス製剤(5555製剤)の接種回数と接種間隔の評価を行う目的で、1箇月間隔2回接種、6箇月間隔2回接種のスケジュールで、カニクイザル6頭に本製剤を接種した。
 その結果、1箇月間隔2回接種、6箇月間隔2回接種のいずれの場合でも、中和抗体価は単回接種の場合とほぼ同程度に上昇し、また、2回接種以降で中和抗体価に大きな変化は認められなかった。したがって、本デングウイルス弱毒株を含むワクチンは1回の接種により中和抗体応答を十分に誘導し得ることが示唆された。(図5~図8;図5~図8は、各5log10PFU/doseの4価デングウイルス製剤を、カニクイザルに単回接種(N=6)、1箇月間隔2回接種(N=6)及び6箇月間隔2回接種(N=6)したときの中和抗体価の幾何平均値を示した。垂直バーは±SDを示す。)
(3)投与用量試験
 さらに異なる用量での反応性を確認する目的で、各5log10、各3log10、各2log10及び各1log10FFU/doseの4価デングウイルス製剤を調製し、カニクイザルに単回接種した。接種週をWeek0とし、Week2及びWeek5の中和抗体価を測定した。
 その結果、Week2において全個体の中和抗体が陽転し、またWeek5においても陽性が維持されていた。したがって、各1log10FFU/doseという低抗原量からの中和抗体応答が確認できた。(図9~図12;図9~図12は、各5log10FFU/dose(N=1)、各3log10FFU/dose(N=3)、各2log10FFU/dose(N=3)、各1log10FFU/dose(N=3)の4価デングウイルス製剤を、カニクイザルに単回接種したときのWeek0、Week2及びWeek5の中和抗体価の幾何平均値を示した。垂直バーは±SDを示す。)
(1)GLP薬効試験
 実施例1で調製したPre-CTMを4価混合し、5555Log10FFU/dose、4444Log10FFU/dose、3333Log10FFU/doseの4価デングワクチン製剤を調製した。調製した4価デングワクチン製剤又は媒体をカニクイザルに皮下単回接種し、中和抗体誘導能及びチャレンジウイルス(親株)に対する防御能を評価した。
 中和抗体価はImmunospot PRNT法で得た。5555Log10FFU/dose接種群(N=24)、4444Log10FFU/dose接種群(N=24)、3333Log10FFU/dose接種群(N=24)のすべての個体でDay14より中和抗体陽転が認められた(表20~28)。一方、媒体接種群では中和抗体陽転は認められなかった(表29、30)
 チャレンジウイルス(親株)に対する防御能評価のため、各群を6頭ずつ均等にさらに群分けし、Day60にそれぞれの血清型の単価親株を皮下チャレンジした。チャレンジ量は、血清型ごとにそれぞれ5Log10PFU/dose、5Log10PFU/dose、6Log10PFU/dose、5Log10PFU/doseとした。チャレンジ1、2、3、4、6,8,10、14日後に血清中のウイルスゲノム量を測定したところ、媒体接種群(各N=3)ではウイルスゲノムが多く検出されたが(表31)、ワクチン接種群ではいずれの群においてもウイルスゲノムは検出されなかった。

(1)サル神経毒力試験
 VMSを被験物質としてWHO protocol(Technical Report Series No.978、2013)に従ったサル神経毒力試験を実施した。1群あたり12頭のカニクイザルを割り当て、VMSの各血清型それぞれ5Log10FFU/dose、5Log10PFU/dose、5Log10FFU/dose、5Log10FFU/doseと、参照ウイルスとしてYellow Fever 17D 5Log10PFU/doseを頭蓋内に投与した。投与後、30日間の臨床スコアリング及び30日経過時点で脳組織及び脊髄に限って剖検を実施した。その結果、神経毒性に関する問題のある臨床的及び組織病理学的所見は認められなかった。
(1)GLP反復投与毒性試験
 実施例1で調製したPre-CTMを4価混合し、7777Log10FFU/dose(5mL)、5555Log10FFU/dose(0.5mL)の4価デングワクチン製剤を調製した。4価デングワクチン製剤又は媒体をそれぞれ雌雄6頭ずつのカニクイザルの皮下に4週間隔で3回投与する反復投与毒性試験を実施した。被験物質の毒性及び可逆性を評価する目的で、カニクイザルの半数は最終投与3日後、残り半数は最終投与28日後に剖検した。
 評価項目は、一般状態(Clinical signs)、行動・神経機能(general behavior and neurobehavioral function)、局所反応(skin reaction)、体重(body weight)、摂餌量(food consumption)、体温(body temperature)、眼科的検査(ophthalmology)、心電図(electrocardiography)、呼吸数(respiratory rate)、尿検査(urinalysis)、血液学的検査(hematology)、血液生化学的検査(blood chemistry)、剖検(necropsy)、器官重量(organ weights)、病理組織学的検査(histopathology)、精巣細胞数評価(quantitative examination of testes)とした。
 反復毒性試験の投与期間及び回復期間中に死亡した個体は、いずれの投与群においても認められなかった。また、投与期間及び回復期間中に、一般状態、行動・神経機能、局所反応、体重、摂餌量、体温、眼科的検査、心電図、呼吸数、尿検査、剖検、器官重量、精巣細胞数評価において、被験物質に関連する変化は雌雄共にいずれの投与群においても認められなかった。
 血液学的には被験物質投与により白血球、好中球、リンパ球、単球、好塩基球、大型非染色細胞の個数増加が一部個体で見受けられたものの、接種部位の炎症又は免疫応答に起因する二次的な変化であり、毒性学的に有意なものではないと考えられた。病理組織学的には接種部位のみにおいて血管周囲に炎症性細胞浸潤が極小程度から中程度見受けられたものの、それらは3回目の投与から4週のうちに回復又は回復傾向を示した。
 以上より、7777Log10FFU/doseが当該試験におけるno-observed-adverse-effect levelと結論付けられた。
 本発明は、デングウイルスを用いたワクチンとして利用可能である。
