また、免疫チェックポイント阻害剤は、近年登場した抗がん剤の一群で、がん細胞やリンパ球(T細胞)に発現している、免疫チェックポイントと呼ばれる、免疫にブレーキをかけるタンパク質を阻害する。免疫チェックポイントとして働く分子としては、programmed cell death protein-1(PD-1)、programmed death-ligand 1(PD-L1)およびcytotoxic T-lymphocyte-associated antigen-4(CTLA-4)などが知られている。免疫チェックポイント阻害剤のうちの幾つかは医薬品として承認されており、その適応症には、悪性黒色腫、非小細胞肺がん、腎細胞がん、悪性リンパ腫、多発性骨髄腫、頭頸部がんおよび尿路上皮がんを含む。さらには、大腸がん、乳がん、肝細胞がん、胃がん、食道がん、卵巣がん、小細胞肺がん、中皮腫、子宮体がんなどの癌種に対しても、免疫チェックポイント阻害剤単独または他の抗がん剤との併用による治療が有効である臨床試験結果が報告されている。

マウス抗マウス/ヒトMyl9モノクローナル抗体の作製
マウスMyl9(Genbank Accession No.NP_742116.1、配列番号1)とヒトMyl9(Genbank Accession No.NP_006088.2、配列番号2)に対するモノクローナル抗体を作製するため、マウスMyl9とヒトMyl9に共通するN末端の配列(1~27位)のC末端にシステイン(Cys)を付加したもの(以下、マウス/ヒトMyl9ペプチドという)(配列番号3)にスカシガイヘモシアニン(KLH)を融合したタンパク質(以下、「マウス/ヒトMyl9ペプチド-KLH」という)を以下の工程により調製した。マウスMyl9とヒトMyl9の配列の比較を図1-1Aに示す。
抗体Aのマウス、ヒトMyl9に対する結合能をELISAにて評価した。以下の工程に従って、マウスMyl3(Genbank Accession No.NP_034989.1、配列番号5)、マウスMyl9、ヒトMyl3(Genbank Accession No.NP_000249.1、配列番号6)、ヒトMyl9の各C末端にヒスチジンタグを結合したタンパク質(以下、それぞれマウスMyl3-His、マウスMyl9-His、ヒトMyl3-His、ヒトMyl9-Hisという)を作製した。マウスMyl3とヒトMyl3(図1-1B)、マウスMyl3とマウスMyl9(図1-2C)、ヒトMyl3とヒトMyl9(図1-2D)のアミノ酸配列の比較を示す。
抗体AのマウスCD69細胞外領域タンパク質とマウスMyl9との結合に対する阻害活性の評価を競合ELISAにて行った。まず、マウスCD69細胞外領域タンパク質とマウスMyl9タンパク質の作製を行った。
抗体Aの重鎖および軽鎖のシグナル配列、および可変領域をコードするDNA配列を、5’-RACE(5’-rapid amplification of cDNA ends)法によって増幅した。前記ハイブリドーマから、RNeasy Mini kit(QIAGEN)を用いて全RNAを調製し、DNase(QIAGEN,RNase free DNase set)で処理した。cDNA合成キット(TAKARA)を用いて、前記全RNAから二本鎖cDNAを調製した。オリゴDNA ad29S(ACATCACTCCGT)(配列番号7)およびオリゴDNA ad29AS(ACGGAGTGATGTCCGTCGACGTATCTCTGCGTTGATACTTCAGCGTAGCT)(配列番号8)のアニーリングによって得られた5’アダプターを前記cDNAに付加した。得られたcDNAを、5’フォワードプライマー(5’-PCR4 primer,AGCTACGCTGAAGTATCAACGCAGAG) (配列番号9)および3’リバースプライマー(マウスIgG重鎖の増幅にはGCCAGTGGATAGACTGATGG(配列番号10)を用い、マウスIgκ軽鎖の増幅にはGATGGATACAGTTGGTGCAGC(配列番号11)を用いた)によって増幅した。増幅されたcDNAを、pCR2.1ベクター(Invitrogen/LifeTechnologies)に挿入した。抗体Aの遺伝子配列を、ABI3130XLを用いて解析した(配列番号12~19)。
アレルギー性気道炎症に対する、精製マウス抗マウス/ヒトMyl9抗体(抗体A)のin vivo投与における効果を、OVA誘導性の気道炎症モデルを用いて検証した。
CD4陽性CD45RB強陽性(CD4+CD45RBhigh)Tリンパ球移入炎症性腸疾患モデルの作製についてはPowrie et al., Int.Immunol.,5,1461-1471,1993を参考にした。雌性、8-10週齢のBalb/cマウス(日本チャールズ・リバー)の脾臓を摘出し、スリガラスですり潰し脾臓細胞を分離した。分離した脾臓細胞は、脾臓一個あたり5mLの155mM塩化アンモニウム、10mM炭酸水素カリウム、80μM EDTA-4Na蒸留水を加え5分室温に放置し赤血球を溶解した。脾臓細胞溶液にPBSを2倍容量加え、1500rpmで5分遠心して沈殿を回収した。分離した脾臓細胞からCD4 T cell isolation kit(ミルテニー社製)によりCD4 Tリンパ球を精製した。CD4陽性CD45RB強陽性Tリンパ球を分離するために、精製したCD4 Tリンパ球に対して、フィコエリスリン(PE)標識抗CD4抗体(eBioscience社製)、フルオレセインイソチオシアネート(FITC)標識抗CD45RB抗体(eBioscience社製)を用い二重染色を行った。二重染色の後、FACSAria(ベクトンディッキンソン社製)を用いCD4陽性CD45RB強陽性細胞をソーティングし目的の細胞を回収した。回収された細胞はPBSで洗浄後、2x106細胞/mLの細胞濃度にPBSにて懸濁した。雌性、8週齢のSCIDマウス(日本クレア)の腹腔に、上記で調製したCD4陽性CD45RB強陽性細胞を250μLずつ、すなわち5x105細胞/マウスで移入を行った。各群8匹のCD4陽性CD45RB強陽性細胞を移入したSCIDマウスに、500μgのコントロール抗体(マウスIgG)、500μgの抗体A(抗体はPBS溶液)を、細胞移入後11日目より、週2回投与した。なお投与は尾静脈から行った。また陰性対照群として、CD4 T cell Isolation kit(ミルテニー社製)により精製し、CD45RBの発現強度による分離をしていないマウスCD4 Tリンパ球(全CD4陽性細胞)を5x105/マウスで移入した。細胞移入27日後に剖検を行い体重減少、大腸内便性状をスコア化し評価を行った。便性状のスコアはデキストラン硫酸ナトリウム誘発大腸炎で用いられている便性状のスコア(Cooper et al., Lab. Invest., 69, 238-249, 1993)を使用した。
キメラ抗体およびヒト化抗体の調製
まず、キメラ抗体の発現ベクターを構築した。重鎖として、抗体Aの重鎖のシグナル配列をコードする遺伝子配列(配列番号16)と可変領域をコードする遺伝子配列(配列番号17)を、V234AおよびG237A変異を有し、C末端のリシン残基を欠失したヒトIgG2の定常領域(配列番号44)をコードする遺伝子配列(配列番号45)を含む発現ベクター(pcDNA3.4)に挿入し、軽鎖として、抗体Aの軽鎖のシグナル配列をコードする遺伝子配列(配列番号18)と可変領域をコードする遺伝子配列(配列番号19)を、ヒトIgκの定常領域(配列番号46)をコードする遺伝子配列(配列番号47)を含む発現ベクター(pcDNA3.4)に挿入し、キメラ抗体の発現ベクターを構築した。キメラ抗体を産生するため、Expi293発現システム(Gibco/ThermoFisher)を用いて、前記発現ベクターをExpi293F細胞(Gibco/ThermoFisher)へ形質移入した。上清を回収し、Protein A(GE Healthcare)を用いて精製した。ここでいう、「V234A」とは、234位のバリンがアラニンに置換された変異を、「G237A」とは、237位のグリシンがアラニンに置換された変異を表す。
抗体Aから作製したキメラ抗体とヒト化抗体のヒトMyl9に対する結合能をELISAにて評価した。ヒトMyl3-His、ヒトMyl9-Hisタンパク質は実施例1に記載の方法により調製した。
マウス/ヒトMyl12a、12bタンパク質の調製
以下の工程に従って、マウスMyl12a(Genbank Accession No.NP_080340.2、配列番号93)、マウスMyl12b(Genbank Accession No.NP_075891.1、配列番号94)、ヒトMyl12a(Genbank Accession No.NP_001289976.1、配列番号95)、ヒトMyl12b(Genbank Accession No.NP_001138416.1、配列番号96)の各C末端にヒスチジンタグを結合したタンパク質(以下、それぞれマウスMyl12a-His、マウスMyl12b-His、ヒトMyl12a-His、ヒトMyl12b-Hisという)を作製した。マウスMyl9、12a、12b(図7A)、ヒトMyl9、12a、12b(図7B)のアミノ酸配列の比較を示す。
マウス、ヒトMyl12a、12bに対する抗体Aの結合能は以下の工程に従ってELISAにて評価した。マウスMyl3-His、マウスMyl9-His、マウスMyl12a-His、マウスMyl12b-His、ヒトMyl3-His、ヒトMyl9-His、ヒトMyl12a-His、ヒトMyl12b-Hisを、96ウェルプレート(Nunc)のウェル上にコートした。4℃にて一晩インキュベートした後、1xブロックエース(DSファーマバイオメディカル株式会社)により、ウェルを室温にて1時間ブロッキングした。0.02% Tween20/PBSで3回洗浄した後、抗体Aを10μg/mLの濃度から4倍ずつ11段階希釈して、ウェルに添加した。室温にて1時間インキュベートして3回洗浄した後、西洋ワサビペルオキシダーゼ標識抗マウスIgG抗体(Jackson ImmunoResearch Loboratories)を加え、室温にて1時間インキュベートした。5回洗浄した後、ウェルにTMBZ(3,3’,5,5’-テトラメチルベンジジン)溶液を加え、5~20分、室温にてインキュベートした。ウェルに等量の反応停止溶液(2N H2SO4)を加え、マイクロプレートリーダー(Thermo Scientific)により450nmの吸光度を読み取った。

ヒトMyl9、12a、12bタンパク質とヒトCD69細胞外領域タンパク質との結合
ヒトMyl9、12a、12bタンパク質とヒトCD69細胞外領域タンパク質との結合をELISAにて評価した。以下の工程に従って、ヒトCD69タンパク質の細胞外領域(64-199位、配列番号97)のN末端にFlagタグを付加したタンパク質(以下、3xFlag-ヒトCD69ECタンパク質)を作製した。ヒトとマウスのCD69タンパク質の細胞外領域のアミノ酸配列の比較を示す(図10)。
ヒトMyl9、ヒトMyl12aおよびヒトMyl12bとヒトCD69細胞外領域タンパク質との結合能に対する、抗体Aの阻害活性は、以下の工程によりELISAにて評価した。実施例5に従って作製したヒトMyl9-His、ヒトMyl12a-His、およびヒトMyl12b-Hisを、それぞれ96ウェルプレート(Nunc)のウェル上にコートした。4℃にて一晩インキュベートした後、1xブロックエース(DSファーマバイオメディカル株式会社)により、ウェルを室温にて1時間ブロッキングした。0.02% Tween20/PBSで3回洗浄した後、抗体Aまたはコントロール抗体(抗ジニトロフェノール抗体、マウスIgG2c、κ)を30μg/mLから3倍希釈で7段階に50mM NaOAc(pH5.5)/150mM NaCl/0.02% Tween20で希釈し、ウェルに添加した。室温にて1時間インキュベートして3回洗浄し、3xFlag-ヒトCD69ECタンパク質を10μg/mLの濃度になるように、50mM NaOAc(pH5.5)/150mM NaCl/0.02% Tween20で希釈して、ウェルに添加した。室温にて1.5時間インキュベートして3回、50mM NaOAc(pH5.5)/150mM NaCl/0.02% Tween20で洗浄した後、西洋ワサビペルオキシダーゼ標識の抗Flag(M2)抗体(Sigma)を加え、室温にて1時間インキュベートした。5回洗浄した後、ウェルにTMBZ(3,3’,5,5’-テトラメチルベンジジン)溶液を加え、5~20分、室温にてインキュベートした。ウェルに等量の反応停止溶液(2N H2SO4)を加え、マイクロプレートリーダー(Thermo Scientific)により450nmの吸光度を読み取った。
抗体Aから作製したキメラ抗体およびヒト化抗体の、ヒトMyl9とヒトCD69細胞外領域タンパク質との結合能に対する阻害活性は、以下の工程によりELISAにて評価した。実施例5に従って作製したヒトMyl9-Hisを、96ウェルプレート(Nunc)のウェル上にコートした。4℃にて一晩インキュベートした後、1xブロックエース(DSファーマバイオメディカル株式会社)により、ウェルを室温にて1時間ブロッキングした。0.02% Tween20/PBSで3回洗浄した後、抗体A、キメラ抗体、ヒト化抗体、またはコントロール抗体(ヒトIgG2、κ、Sigma)を30μg/mLから3倍希釈で7段階に50mM NaOAc(pH5.5)/150mM NaCl/0.02% Tween20で希釈し、ウェルに添加した。室温にて1時間インキュベートして3回洗浄し、3xFlag-ヒトCD69ECタンパク質を10μg/mLの濃度になるように、50mM NaOAc(pH5.5)/150mM NaCl/0.02% Tween20で希釈して、ウェルに添加した。室温にて1.5時間インキュベートして3回、50mM NaOAc(pH5.5)/150mM NaCl/0.02% Tween20で洗浄した後、西洋ワサビペルオキシダーゼ標識の抗Flag(M2)抗体(Sigma)を加え、室温にて1時間インキュベートした。5回洗浄した後、ウェルにTMBZ(3,3’,5,5’-テトラメチルベンジジン)溶液を加え、5~20分、室温にてインキュベートした。ウェルに等量の反応停止溶液(2N H2SO4)を加え、マイクロプレートリーダー(Thermo Scientific)により450nmの吸光度を読み取った。

10%FBS、ペニシリン/ストレプトマイシンを含むRPMI1640培養液で培養したマウス大腸がん細胞株CT26.WT(ATCC number CRT-2638)をリン酸緩衝生理食塩水にて1.0 × 107 cells/mLの濃度の細胞懸濁液を調製し、6週齢のマウス(Balb/c、雌、日本チャールズリバー)の右背部皮下に0.1mLの用量で移植した。移植から6日後に電子デジタルノギス(デジマチックTMキャリパ、株式会社ミツトヨ)を用いて腫瘍の短径、長径を計測し、以下の計算式で腫瘍体積を算出した。

マウス大腸がん細胞株CT26.WTの皮下腫瘍組織を遺伝子発現解析した結果を図15に示す。CT26細胞由来腫瘍組織から、TRIzol(登録商標)Reagent(ThermoFisher scientific)を用いて核酸を抽出した後、RNeasy mini kit(Qiagen)を用いて全RNAを調製した。全RNAから、SureSelect Strand Specific RNAライブラリー調製試薬(Agilent Technologies)を用いてRNASeq解析用のLibraryを作製し、HiSeq4000(illumine)にてシーケンシングを行った。シーケンスにより得られたFastqファイルからTPM(Transcript Per Million)の方法によりノーマライズを行い、Myl9、Myl12aおよびMyl12bの発現量を測定した。腫瘍組織ではMyl9の発現量は低いが、Myl12aおよびMyl12bの発現量は高いことが示された。
投与初日の腫瘍体積をもとに、各群の腫瘍体積の平均値がほぼ等しくなるように群分けを行った。抗体Aおよび抗PD-1抗体(BioXCell、Catalog#:BE0146)はリン酸緩衝生理食塩水にて2mg/mLに調製し、0.1mL/マウスの投与量で7日に2回の頻度で計4回(がん細胞移植後6、9、13、16日目)腹腔内に投与した。コントロール群にはリン酸緩衝生理食塩水を0.2mL/マウスの投与量で7日に2回の頻度で計4回(がん細胞移植後6、9、13、16日目)腹腔内に投与した。実験は1群7-8匹で行った。
試験最終日(30日目)までのコントロール群(A)、抗体A投与群(B)、抗PD-1投与群(C)、抗体Aと抗PD-1抗体併用投与群(D)それぞれの腫瘍体積を計算し経日的な変化の推移を図16に示す。図16Bに示すように、抗体A単独では腫瘍の増殖をほとんど抑制しなかった。一方、抗PD-1抗体単独ではコントロール群と比較して若干の腫瘍増殖抑制(図16C)が見られた。一方、抗体Aと抗PD-1抗体を併用投与すると、顕著な腫瘍増殖抑制が見られ、腫瘍が消失する個体(2匹/10匹)も見られた(図16D)。以上の結果から、抗体Aと抗PD-1抗体の併用投与による相乗効果が強く示唆された。
