前側アーム(11)と、後側アーム(12)と、アーム移動手段(13)と、を備えており、後側アーム(12)は、先端を前側アーム(11)に向けた状態かつその中心軸が後側アーム(12)と前側アーム(11)が接近離間する方向と平行になるように設けられた一対の針状部材(15,16)を有する縫合針部(14)を備えており、前側アーム(11)は、前側アーム(11)と後側アーム(12)が接近したときに、一対の針状部材(15,16)の先端部をそれぞれ収容し得る一対の収容空間(18,19)が設けられた収容部(17)を備えており、収容部(17)の一対の収容空間(18,19)内には、縫合糸(22)によって互いに連結された一対の係合部材(21,21)が収容されている。

Description

縫合装置
 本発明は、縫合装置に関する。さらに詳しくは、内視鏡に取り付けられて消化管内の組織の縫合に使用される縫合装置に関する。
 従来、胃等の消化管に腫瘍などができた場合、その除去は外科手術で行われることが一般的であった。近年、外科手術よりも患者の負担を軽減できしかも体表面に形成される傷を小さくできる低侵襲な手術として、ラパロスコープによる手術も行われるようになっている。
 最近では、ラパロスコープによる手術よりも低侵襲な手術として、軟性内視鏡を使用した手術が行われている。癌が粘膜層に留まっている初期胃がん等であれば、軟性内視鏡によって消化管の内腔から粘膜層を含めた粘膜下層までを剥離することによって癌を切除できる。この場合、体表面には全く傷を残すことなく手術を行うことができるので、患者への負担が小さく、しかも、手術後の回復も早くなる。
 一方、胃壁に形成された粘膜下層よりも深い腫瘍、つまり、固有筋層に到達しているような腫瘍の場合、固有筋層まで除去しなければならない。この場合、腫瘍を除去すると胃壁に開口が形成されてしまうので、腫瘍を除去した後、開口を塞ぐ(縫合する)必要がある。しかし、軟性内視鏡だけでは開口を塞ぐことができないので、軟性内視鏡とラパロスコープを使用した手術も行われている。具体的には、胃壁の腫瘍の切除は軟性内視鏡で実施し、腫瘍を除去した後の開口はラパロスコープで縫合する手術が実施されている。
 軟性内視鏡とラパロスコープを使用した手術は、外科手術と比較すれば低侵襲であるが、それでも、ラパロスコープを腹腔内に挿入するための孔を形成しなければならない。もし、軟性内視鏡によって消化管腔内から腫瘍の切除だけでなく縫合まで行うことができれば、体表面に傷を形成することなく手術を行うことができる。このため、現在、消化管腔内から開口を縫合する縫合器の開発がすすめられている(例えば、特許文献1)。
 この特許文献1の縫合器を使用すれば、腫瘍を切除した際に形成される開口の端面を突き合わせて、外科手術による縫合と同等程度の縫合を実現できる。また、特許文献1の縫合器を使用すれば、腫瘍を切除した開口に限らず、消化管に形成された種々の孔等を縫合して塞ぐことも可能になる。

 ところで、胃壁などから組織を切除した場合、切除した部位に形成される開口から組織が腹腔内に出てしまう恐れがある。切除した組織が癌であれば、癌組織が腹腔内に漏れた場合、癌が他の臓器等に転移してしまう可能性がある。
 もし、胃壁などに開口を形成せずに癌等の組織を切除することができれば、より安全に軟性内視鏡によって消化管腔内から組織の切除などをできるので、かかる方法を実現する機器の開発が望まれている。
 本発明は上記事情に鑑み、消化管内から組織の縫合をより簡単に実施することができる縫合装置を提供することを目的とする。
 第1発明の縫合装置は、前側アームと、該前側アームに対し接近離間可能に設けられた後側アームと、前記前側アームと前記後側アームを接近離間させるアーム移動手段と、を備えており、前記前側アームおよび前記後側アームは、基端部が前記アーム移動手段に連結されており、前記後側アームは、先端を前記前側アームに向けた状態かつその中心軸が該後側アームと前記前側アームが接近離間する方向と平行になるように設けられた一対の針状部材を有する縫合針部を備えており、前記前側アームは、該前側アームと前記後側アームとが接近したときに、前記一対の針状部材の先端部をそれぞれ収容し得る一対の収容空間が設けられた収容部を備えており、該収容部の一対の収容空間内には、縫合糸によって互いに連結された、前記針状部材と係合可能な一対の係合部材が収容されていることを特徴とする。
 第2発明の縫合装置は、第1発明において、前記前側アームおよび前記後側アームが前記アーム移動手段に連結されている位置から、前記一対の針状部材までの、前記後側アームと前記前側アームが接近離間する方向と垂直な面に沿った距離が、5mm以上であることを特徴とする。
 第3発明の縫合装置は、第1または第2発明において、前記縫合針部は、前記一対の針状部材が前記後側アームの軸方向に並ぶように設けられており、前記収容部は、前記一対の収容空間が前記前側アームの軸方向に並ぶように設けられていることを特徴とする。
 第4発明の縫合装置は、第1、第2または第3発明において、前記後側アームには、該後側アームの軸方向に並ぶように複数の前記縫合針部が設けられており、前記前側アームには、該前側アームの軸方向に並ぶように各縫合針部に対応する複数の前記収容部が設けられていることを特徴とする。
 第5発明の縫合装置は、第1、第2、第3または第4発明において、前記後側アームと前記前側アームを、接近離間可能かつ接近離間する方向と直交する面内の相互回転を固定した状態で連結する連結機構を備えていることを特徴とする。
 第1発明によれば、アーム移動手段によって前側アームと後側アームを接近させれば、一対の針状部材の先端部を一対の収容空間内にそれぞれ同時に挿入することができる。すると、一対の針状部材の先端部に、縫合糸によって連結された一対の係合部材を同時に係合させることができる。このため、胃壁などの組織を前側アームと後側アームによって一回挟んで離すだけで、縫合糸の両端部(係合部材と連結されている部分)がいずれも後側アーム側に位置した状態となるように、縫合糸を組織に通すことができる。その状態で、縫合糸の両端部を結紮すれば、通常の外科手術における縫合と同様に組織を縫合することができる。
 第2、第3発明によれば、幅や長さのある組織の縫合が可能となる。
 第4発明によれば、複数個所の縫合を同時にできるので、縫合作業を迅速に行うことができる。
 第5発明によれば、前側アームと後側アームを接近させた際に、一対の針状部材の先端部を一対の収容空間内に確実に収容させることができる。

 つぎに、本発明の実施形態を図面に基づき説明する。
 本発明の縫合装置は、軟性内視鏡に取り付けて、消化管腔内から縫合を行う装置であって、簡便かつ迅速に縫合を実施できるようにしたことに特徴を有している。
 本発明の縫合装置は、軟性内視鏡だけでなく、ラパロスコープの先端に取り付けて使用することもできる。とくに、本発明の縫合装置を軟性内視鏡に取り付けて使用した場合には、消化管腔内において、消化管に開口や貫通孔が形成されていない状態でも、組織の全層縫合等を実施できる。なお、全層縫合とは、消化管の全層(漿膜、漿膜下層、固有筋層、粘膜下層、粘膜)を貫通するように縫合糸を通して縫合することを意味している。
 以下では、本実施形態の縫合装置10を軟性内視鏡に取り付けて使用する場合を代表として説明する。
 なお、装置各部の構造を分かりやすくするために、各図面における各部の相対的なサイズなどは必ずしも実際の装置におけるサイズとは対応させていない。
(内視鏡1の説明)
 図1において、符号2は、本実施形態の縫合装置10が取り付けられる内視鏡1のシャフトを示している。本実施形態の縫合装置10が取り付けられる内視鏡1は、一般的な内視鏡手術に使用される軟性内視鏡である。この内視鏡のシャフト2は、生体の消化管に挿入して使用されるものであれば、その径や長さ、材質などはとくに限定されない。例えば、シャフト2の径は、一般的な内視鏡では10mm程度であるが、5~15mm程度のものでもよい。また、シャフト2の長さは、一般的な内視鏡では1200mm程度であるが、1200~3000mm程度のものでもよい。
(本実施形態の縫合装置10の説明)
 図1に示すように、本実施形態の縫合装置10(以下、単に縫合装置10という場合がある)は、前後一対のアーム11,12と、この前後一対のアーム11,12を作動させるアーム移動手段13と、前後一対のアーム11,12の移動を案内する連結機構40と、を備えている。
 図1に示すように、縫合装置10は、アーム移動手段13を内視鏡1のシャフト2に固定することによって、内視鏡1に固定して使用されるものである。具体的には、縫合装置10は、アーム移動手段13の軸方向がシャフト2の軸方向と略平行となるように取り付けられる。
 このように縫合装置10がシャフト2に取り付けられていれば、シャフト2を屈曲したときなどにアーム移動手段13をシャフト2の屈曲に確実に追従させることができるので、アーム移動手段13がシャフト2の屈曲などの邪魔になることを防ぐことができる。
 しかも、アーム移動手段13は、前後一対のアーム11,12の両方が内視鏡1のシャフト2の先端面2sより前方に位置し、かつ、後側アーム12が前側アーム11に対して内視鏡1の先端面2s側に位置するように、内視鏡1のシャフト2に取り付けられる。
 このため、シャフト2を屈曲させたりしても、前後一対のアーム11,12は、常に内視鏡1のシャフト2の先端面2sの前面に配置しておくことができる。すると、シャフト2に設けられたカメラによって前後一対のアーム11,12の動きを確実に視認することができる。したがって、前後一対のアーム11,12を使用して、胃などの消化管内の組織を縫合する際に、操作者による前後一対のアーム11,12の操作を容易にすることができる。
 なお、アーム移動手段13は、必ずしも内視鏡1のシャフト2に沿って設ける必要はなく、アーム移動手段13の先端部だけがシャフト2の先端部に固定されていてもよい。この場合でも、シャフト2の先端部において、アーム移動手段13の軸方向とシャフト2の先端部の軸方向とが略平行となっていれば、操作者による前後一対のアーム11,12の操作を容易にすることができる。
 つぎに、本実施形態の縫合装置10の各部を説明する。
 まず、アーム移動手段13を説明する。
 図1~図4に示すように、アーム移動手段13は、軸方向に沿って延びた長尺な部材であり、内視鏡1のシャフト2に取り付けられるものである。このアーム移動手段13の長さは、内視鏡1のシャフト2の長さと同程度の長さであればよく、とくに限定されない。
 このアーム移動手段13の先端には、筒状の固定部13dが設けられている。この筒状の固定部13dに内視鏡1のシャフト2の先端部を挿入することによって、アーム移動手段13の先端は、内視鏡1のシャフト2の先端部に固定されている。
 また、アーム移動手段13の先端部から手元側までの部分は、アーム移動手段13がシャフト2に沿うように固定されている。例えば、ポリエチレン、ポリ塩化ビニル、アルミなどを素材とするベルト状部材や、ポリエチレン、ポリ塩化ビニル、金属などを素材とする輪状留め具などによって、アーム移動手段13をシャフト2に固定することができる。
 アーム移動手段13は、シャフト2に固定された状態において、シャフト2の曲りなどに追従して変形できる程度の柔軟性を有する素材によって形成されている。つまり、アーム移動手段13は、内視鏡1のシャフト2に取り付けても、内視鏡1の操作の妨げにならないような強度となるように形成されている。
 なお、アーム移動手段13をシャフト2に固定する方法はとくに限定されず、シャフト2の曲りなどの変形を妨げないように固定できる方法であればよい。
 具体的には、アーム移動手段13は、シース13aと、このシース13a内に挿入されたワイヤー13bと、シース13aが内部に挿通されたチューブ13cと、から構成されている。
 チューブ13cはその先端から基端まで貫通孔が形成された筒状の部材であり、このチューブ13cが、上述したように、ベルト状部材などによってシャフト2に固定されている。このチューブ13cの素材はとくに限定されないが、例えば、ポリエチレンやポリ塩化ビニルなどの樹脂で形成されていることが好ましい。
 チューブ13c内には、シース13aが挿通されている。このシース13aは中空なチューブ状の部材であり、チューブ13cの軸方向に沿って移動可能かつチューブ13c内で回転可能となるようにチューブ13c内に挿入されている。このシース13aの素材はとくに限定されないが、シャフト2の曲り(言い換えればチューブ13cの曲り)に追従して変形できるが、軸方向に力が加わっても、伸びたり折れ曲がったりしにくい強度を有するように形成されていることが望ましい。例えば、ステンレスやニッケルチタン合金で形成された金属コイルチューブや補強材で補強された樹脂チューブなどで構成されていることが好ましい。このシース13aの先端には、後側アーム12の基端部が固定されている(図5参照)。一方、シース13aの基端は、シャフト2を操作する操作部近傍まで延びている。したがって、アーム移動手段13のシース13aの基端を移動させることによって、チューブ13cの先端からシース13aが突出する長さを変化させることができる。言い換えれば、シャフト2の軸方向に沿って、シャフト2に対して後側アーム12を移動させることができる。また、シース13aの基端を回転させることによって、後側アーム12をシース13aの中心軸周りに回転させることができる。
 ワイヤー13bは、例えば、金属製のワイヤー等であり、シャフト2の曲り(言い換えればシース13aの曲り)に追従して変形できるものである。このワイヤー13bは、シース13a内をその軸方向に沿って移動できるように設けられている。このワイヤー13bの先端は、前側アーム11の基端に連結されている(図5参照)。一方、ワイヤー13bの基端は、シャフト2を操作する操作部近傍まで延びている。したがって、アーム移動手段13のワイヤー13bの基端を移動させることによって、シース13aの先端からワイヤー13bが突出する長さを変化させることができる。言い換えれば、シャフト2の軸方向に沿って、後側アーム12に対して前側アーム11を移動させることができる。
 以上のごとき構成であるので、アーム移動手段13のシース13aの基端およびワイヤー13bの基端を操作して、シース13aやワイヤー13bを軸方向に沿って移動させれば、前側アーム11と後側アーム12を移動させることができる。つまり、シャフト2の先端面2sに対して、前側アーム11や後側アーム12の相対的な位置を変化させることができる。例えば、シース13aとワイヤー13bを同じ量だけ同じ方向に移動させれば、シャフト2の先端面2sに対して、前側アーム11および後側アーム12を同じ量だけ同じ方向に移動させることができる。
 しかも、シース13aとワイヤー13bを独立して移動させることができるので、前側アーム11と後側アーム12を互いに接近離間させることができる。例えば、ワイヤー13bの移動を固定して、シース13aを移動させれば、後側アーム12を前側アーム11に対して接近離間させることができる。逆に、シース13aの移動を固定して、ワイヤー13bを移動させれば、前側アーム11を後側アーム12に対して接近離間させることができる。
 なお、アーム移動手段13の外径(つまり、チューブ13cの外径)は、本実施形態の縫合装置10を取り付けた内視鏡1を消化管内(またはオーバーチューブ内)に挿入することができる程度であればよく、とくに限定されない。例えば、アーム移動手段13の外径は、アーム移動手段13とシャフト2の外径を合わせた径が、11~15mm程度となる大きさが好ましく、11~14mm程度となる大きさがより好ましい。

 図5および図6に示すように、アーム移動手段13の先端には、前後一対のアーム11,12が連結されており、前後一対のアーム11,12は連結機構40によって連結されている。つまり、前側アーム11と後側アーム12は、連結機構40によって、接近離間可能かつ接近離間する方向と直交する面内の相互回転を固定した状態となるように連結されている。
 具体的には、後側アーム12の基端には、断面略方形に形成された略軸状の連結機構40の後側連結部材42が設けられている。この後側連結部材42には、前側アーム11に設けられた前側連結部材41が係合している。前側連結部材41は、係合溝41gが設けられており、この係合溝41gの3つの内面が後側連結部材42の3つの外面と略面接触した状態(または略面接触した部分ができる状態)となるように係合している(図6参照)。
 以上のごとき構造であるので、前後一対のアーム11,12が接近離間する際に、前側連結部材41の係合溝41gの3つの内面が後側連結部材42の3つの外面と略面接触した状態で移動する。したがって、前側アーム11と後側アーム12が接近離間した際に、前側アーム11が後側アーム12に対して回転することなく(つまり接近離間する方向と直交する面内の相互回転を固定した状態で)移動させることができる。
 なお、連結機構40の構成は上記のごとき構成に限られない。連結機構40の構成は、前側アーム11と後側アーム12が接近離間可能かつ両者が接近離間する方向と直交する面内の相互回転を固定した状態となるように連結できる構成であればよい。例えば、前側連結部材41(または後側連結部材42)に軸方向に沿って線状の溝を設け、この線状の溝に係合する突起を後側連結部材42(または前側連結部材41)に設けても、同様の効果を得ることができる。
(前後一対のアーム11,12の説明)
 つぎに、前後一対のアーム11,12を説明する。
(後側アーム12について)
 まず、後側アーム12について説明する。
 図5に示すように、後側アーム12は、略短冊状であって前面12aと背面12bが互いに平行な平坦面に形成されたものである。ここでいう短冊状とは、長さに対して幅が短く、また、厚さも長さに対して短い形状のものを意味しており、短冊状には、長さ方向に沿って若干湾曲しているもの(図1~図4参照)と、長さ方向に沿って真っ直ぐなものの両方を含む概念である。
 この後側アーム12の基端には、前述したシース13aの先端が連結されている。具体的には、後側アーム12の基端部に連結された連結機構40の後側連結部材42とシース13aの先端が連結されている。後側アーム12は、シース13aとの連結部分においてシース13aの中心軸と後側アーム12の前面12aおよび背面12bとが直交するように、シース13aに連結されている。以下では、シース13aと後側アーム12の連結部分におけるシース13aの中心軸方向を、単に、シース13aの中心軸という場合がある。
 なお、後側アーム12の基端部に設けられた連結機構40の後側連結部材42も、その軸方向がシース13aの中心軸と略平行となるように配設されている。
(縫合針部14)
 一方、後側アーム12の先端部には、縫合針部14が設けられている。この縫合針部14は、後側アーム12の軸方向に沿って並ぶように配置された、一対の針状部材15,16を備えている。一対の針状部材15,16は、いずれも軸部bと、この軸部bの先端に設けられた、この先端よりも外径が大きい部分(やじり状部a)とを有している。やじり状部aは、その基端の外径が軸部bの先端の外径よりも大きく、軸部bとの連結部分に段差ができるように形成されている。このやじり状部aは、後述するように、一対の針状部材15,16が一対の係合部材21,21と係合した際に、一対の針状部材15,16から一対の係合部材21,21が脱落することを防ぐ役割を有する。
 また、縫合針部14の一対の針状部材15,16は、いずれもその先端が前側アーム11に向いた状態かつ、その軸方向が後側アーム12の前面12aと直交するように後側アーム12に取り付けられている。言い換えれば、一対の針状部材15,16は、いずれもその中心軸が後側連結部材42の軸方向(つまりシース13aの中心軸)と略平行となるように後側アーム12に取り付けられている。
(前側アーム11について)
 図5に示すように、前側アーム11は、略短冊状であって前面11aと背面11bが互いに平行な平坦面に形成されたものである。この前側アーム11は、実質的に、後側アーム12と同じ形状に形成されている。この前側アーム11の基端部には、前述した連結機構40の前側連結部材41が設けられている。
 この前側アーム11には、ワイヤー13bの先端が連結されている。具体的には、後側アーム12の後側連結部材42を貫通する貫通孔42hにワイヤー13bが挿通されており、そのワイヤー13bの先端が前側アーム11の基端部に設けられた連結部材11cに連結されている。
 この前側アーム11の先端部には、一対の収容空間18,19を有する収容部17が形成されている(図6参照)。図7に示すように、収容部17の一対の収容空間18,19は、前側アーム11の前面11aと背面11bとの間を貫通する貫通孔である。一対の収容空間18,19は、いずれも、その中心軸が前後一対のアーム11,12が接近離間する方向(つまりシース13aの中心軸と平行な方向)と平行となるように形成されている。なお、一対の収容空間18,19は、いずれも前側アーム11の側面と連通する開口が形成されており、この開口を通して、後述する縫合器具20の縫合糸22は一対の収容空間18,19に出入りすることができるようになっている。
 この一対の収容空間18,19は、その背面11b側(大径部a)の内径が前面11a側(小径部b)の内径より大きい段付き孔である。そして、各収容空間18,19は、小径部bの内径がやじり状部aの外径よりも大きくなるように形成されているが、かかる形状に形成されている理由は後述する。
 そして、一対の収容空間18,19は、その中心軸が一対の針状部材15,16の中心軸とそれぞれほぼ同軸となるように設けられている。
 以上のごとき構成であるので、前後一対のアーム11,12を接近させれば、一対の針状部材15,16のやじり状部aを一対の収容空間18,19内にそれぞれ挿入することができる。
 なお、一対の収容空間18,19の中心軸と一対の針状部材15,16の中心軸がそれぞれほぼ同軸とは、前後一対のアーム11,12を接近させた際に、一対の収容空間18,19に縫合針部14の一対の針状部材15,16が挿入できる程度のずれは許容されること意味している。
 なお、上記例では、前側アーム11および後側アーム12の前面および背面が平坦面の場合を説明したが、前側アーム11および後側アーム12の前面および背面は必ずしも平坦面である必要はない。
 また、後側アーム12において、一対の針状部材15,16を設ける位置はとくに限定されない。しかし、後側アーム12の大きさが体内に挿入するために支障がない範囲であれば、一対の針状部材15,16と連結機構40との距離がある程度離れている方が、組織の端縁から縫合位置までの距離を長くできるという利点が得られる。この観点から、連結機構40の位置(言い換えれば、前側アーム11および後側アーム12がアーム移動手段13に連結されている位置)から一対の針状部材15,16までの、アーム移動手段13の軸方向(言い換えれば、後側アーム12と前側アーム11が接近離間する方向)に垂直な面に沿った距離L(図5参照)は、5mm以上が好ましく、10~20mmがより好ましい。なお、本実施形態では、一対の針状部材15,16のそれぞれについての距離Lは互いに等しいが、一対の針状部材15,16についての距離Lが互いに異なる場合には、その両方が上記の範囲にあることが好ましい。
 さらに、一対の針状部材15,16は、その素材や長さ、軸径はとくに限定されない。具体的には、一対の針状部材15,16は、縫合する対象に突き刺してその対象を貫通させることができ、しかも、対象を貫通した状態から逆方向に移動させて対象から引き抜くことができる程度の長さおよび強度を有するものであればよい。例えば、本実施形態の縫合装置10によって胃壁を縫合する場合であれば、その長さは後側アーム12の前面からその先端までの長さが胃壁を貫通できる長さであればよく、その素材は金属が強度の点で好ましい。より具体的には、一対の針状部材15,16の先端から後側アーム12の前面までの長さH(図5参照)が、7~20mm程度が好ましく、7~10mm程度がより好ましい。また、一対の針状部材15,16の軸径は、軸部bの先端部の軸径(やじり状部aとの連結部分)として、1~2mm程度が好ましく、やじり状部aの最大径は軸部bの先端部の軸径よりも0.1~1mm程度が大きい方が好ましい。
(縫合器具20について)
 また、図7に示すように、本実施形態の縫合装置10は、縫合器具20を備えている。この縫合器具20は、円環状に形成された一対の係合部材21,21と、一対の係合部材21,21を連結する縫合糸22と、から構成されている。
 この縫合器具20の一対の係合部材21,21は、前側アーム11の収容部17の一対の収容空間18,19内にそれぞれ配置されるものである。
 この一対の係合部材21,21は、一対の収容空間18,19内に配置されたときに、その表裏を貫通する貫通孔が一対の収容空間18,19の小径部bを貫通する孔の上方に配置し得る大きさに形成されている。具体的には、一対の係合部材21,21は、その外径が一対の収容空間18,19の大径部aの内径よりも小さくかつ小径部bの内径よりも大きく形成されている(図7(B)参照)。つまり、一対の係合部材21,21が一対の収容空間18,19内に配置されると、一対の係合部材21,21の外縁と大径部aの内面との間にわずかな隙間しか形成されないような大きさに、一対の係合部材21,21は形成されている。
 そして、一対の係合部材21,21の貫通孔21hは、一対の針状部材15,16のやじり状部aを挿通させることはできるが、やじり状部aが完全に貫通孔21hを挿通すると一対の針状部材15,16から一対の係合部材21,21が抜け落ちない構造に形成されている。具体的には、一対の係合部材21,21は、その内径が一対の針状部材15,16のやじり状部aの外径よりも小さいが一対の針状部材15,16の軸部bの先端(つまりやじり状部aとの連結部分)の軸径よりも大きくなるように形成されている。
 この一対の係合部材21,21の素材はとくに限定されない。しかし、上述したような貫通孔21hを形成しても、一対の針状部材15,16のやじり状部aを挿通させつつ、一対の針状部材15,16に係合した状態を維持できるような性質を有する素材が好ましい。つまり、一対の係合部材21,21は、ある程度は弾性変形可能な素材によって形成されていることが望ましい。例えば、金属製の板や軸材等を使用すれば、上述したような性質を有する一対の係合部材21,21を形成することができる。
(本実施形態の縫合装置10の作動の概略)
 以上のごとき構成であるので、本実施形態の縫合装置10では、アーム移動手段13を操作して前側アーム11と後側アーム12を接近させれば、一対の針状部材15,16を、同時に一対の収容空間18,19に挿入することができる。
 すると、一対の収容空間18,19内に配置されている縫合器具20の一対の係合部材21,21に、一対の針状部材15,16のやじり状部aを同時に挿通させることができる。そして、一対の針状部材15,16のやじり状部a全体が一対の収容空間18,19の小径部bに挿入されるまで前側アーム11と後側アーム12を接近させれば、一対の針状部材15,16を、その軸部bまで一対の係合部材21,21に貫通させることができる。
 この状態で、アーム移動手段13を操作して、前側アーム11と後側アーム12とを離間させれば、一対の係合部材21,21を一対の針状部材15,16とともに一対の収容空間18,19から離脱させることができる。なお、縫合糸22は、上述した開口を通して一対の収容空間18,19から離脱させることができる。
 すると、一対の針状部材15,16に一対の係合部材21,21が係合した状態となるから、一対の係合部材21,21を連結する縫合糸22によって、一対の針状部材15,16が連結された状態とすることができる(図8(C)参照)。
 したがって、本実施形態の縫合装置10によれば、前側アーム11と後側アーム12の間に物体を配置した状態で、前側アーム11と後側アーム12を1回接近離間させるだけで、縫合糸22を、その両端が物体の同じ側に位置するように物体を貫通させることができる。言い換えれば、縫合糸22の両端間の部分が物体に引っ掛かった状態となるように、縫合糸22を物体に貫通させることができるのである(図8(C)参照)。
(本実施形態の縫合装置10による生体の縫合について)
 上記のごとき構成を有するので、本実施形態の縫合装置10を内視鏡1のシャフト2に取り付けておけば、胃壁などの組織を胃の内部から縫合することができる。
 以下、本実施形態の縫合装置10を使用して、胃壁を内反させた状態で縫合する縫合作業を、図8に基づいて説明する。なお、図8では、動きを分かりやすくするために、適宜構成の記載を削除している。
 まず、胃内に、本実施形態の縫合装置10を取り付けた内視鏡1のシャフト2を挿入する。そして、縫合すべき個所の胃壁を内視鏡に挿通した内視鏡用鉗子で内側に引き込む等の方法で内方に凹ませて、胃内部に突起状の部分(突起部PP)を形成させる。
 その状態で、アーム移動手段13を操作して、前側アーム11を後側アーム12から離間させる。そして、縫合針部14の一対の針状部材15,16の先端と、前側アーム11の背面11bとの間に、突起部PPを挟むことができる程度の隙間を形成する。その状態で、シャフト2を操作して、縫合針部14の一対の針状部材15,16の先端と前側アーム11の背面11bとの間の隙間に突起部PPを配置する(図8(A))。
 そして、アーム移動手段13を操作して、前側アーム11の移動を固定した状態で後側アーム12を前側アーム11に向かって移動させる。すると、縫合針部14の一対の針状部材15,16が突起部PPを貫通し、その先端が一対の収容空間18,19に挿入されるので、一対の針状部材15,16のやじり状部aを縫合器具20の一対の係合部材21,21とそれぞれ係合させることができる(図8(B))。
 なお、図8(B)では、後側アーム12と前側アーム11とを接近させる際に、後側アーム12を前側アーム11に接近させる場合を説明したが、前側アーム11を後側アーム12に接近させてもよいし、両者をともに移動させて両者を接近させてもよい。
 一対の針状部材15,16のやじり状部aに一対の係合部材21,21を係合させたら、アーム移動手段13を操作して、後側アーム12を前側アーム11から離間させる。このとき、一対の針状部材15,16は、突起部PPを貫通した孔を通ってそれぞれ元の位置に戻る。すると、縫合糸22の両端が固定されている一対の係合部材21,21がいずれも突起部PPの一方の側(図8(C)では上側)に位置した状態となる。すると、縫合糸22によって、針状部材15から突起部PPを貫通して針状部材15の反対側に出てから、突起部PPを貫通し針状部材16に戻る輪が形成される(図8(C)参照)。
 上記のごとき縫合糸22の輪が形成されると、突起部PPの対向する壁面同士を接触させた状態で縫合糸22を結紮する。具体的には、縫合糸22において、針状部材15(つまり一方の係合部材21)から突起部PPに向かって延びている部分と、針状部材16(つまり他方の係合部材21)から突起部PPに向かって延びている部分を結紮する。この結紮は、市販されているクリップなどを利用することができる。例えば、クリップなどを内視鏡1の鉗子口から供給して縫合糸22に取り付ければ、結紮することができる。また、日本国特許5294181号に記載されている結紮器具によって結紮してもよい。
 最後に、縫合糸22において、結紮した部分よりも一対の針状部材15,16側に位置する部分を切れば、突起部PPの対向する壁面同士を接触させた状態で固定することができるのである。
(本実施形態の縫合装置10を使用した手術について)
 また、本実施形態の縫合装置1を使用すれば、以下のような方法で、胃壁内に形成された腫瘍を胃壁に開口を形成することなく、切除することができる。
 図9に示すように、まず、胃壁において腫瘍TMが形成されている部分(腫瘍壁TW)において、腫瘍TMの周辺の粘膜等を切除して、腫瘍TMを囲むように筋層が露出された部分(露出部EA)を形成する。
 ついで、胃を若干萎縮させた状態で、露出部EAと腫瘍TMが形成されている部分と対向する胃壁(対向壁OW)を縫合糸SLなどによって連結する。例えば、この連結には、日本国特許5294181号に記載されている縫合装置を使用することができる。なお、露出部EAと対向壁OWの連結は、少なくとも2か所以上で実施する。
 露出部EAと対向壁OWが連結された後に、胃に二酸化炭素ガスを送気して胃を膨張させる。すると、腫瘍壁TWと対向壁OWとが離間する。このとき、露出部EAと対向壁OWが連結されているので、露出部EAに囲まれている部分(つまり腫瘍TM)は、露出部EAの外側の部分に比べて、膨張が制限される。つまり、腫瘍TMは、縫合糸SLによって対向壁OWに吊り上げられたような状態となる(図9(A))。
 腫瘍TMが吊り上げられた状態になると、腫瘍TMの部分を頂点として、胃壁が内方に凹んだ状態となる。すると、上述したような突起部PPの縫合と同様の方法で、本実施形態の縫合装置10によって露出部EAの位置を縫合することができる(図9(B))。
 すると、縫合した部分では、胃壁の外面同士が接触した状態となるので、露出部EAにおいて、縫合した部分よりも内方の位置で露出部EAを切断すれば、腫瘍TMを切除することができる(図9(C))。このとき、縫合した部分では胃壁の外面同士が縫合されているので、胃壁に開口が形成されることがない。つまり、胃壁に開口を形成することなく腫瘍TM等を除去できるので、腫瘍TMが癌であったとしても、癌細胞が腹腔内に播種されてしまうことを防止できる。
(縫合針部14について)
 縫合針部14は一対の針状部材15,16を備えているが、一対の針状部材15,16の軸間距離は、とくに限定されない。縫合糸22によって縫合部分を適切に縫合できる程度の距離に設けられていればよい。例えば、2~4mm程度にしておけば、外科手術における縫合と同等程度の縫合状態を実現することが可能となる。
 また、上記例では、縫合針部14の一対の針状部材15,16が、後側アーム12の軸方向に沿って並ぶように配設されている場合を説明した。しかし、一対の針状部材15,16は、必ずしも軸方向に沿って並ぶように配設されていなくてもよい。例えば、後側アーム12の軸方向に対して傾けて配設したり、直交するように配設したりしてもよい。なお、前側アーム11の収容部17の一対の収容空間18,19は、縫合針部14の一対の針状部材15,16に対応するように配置される。
 さらに、縫合針部14は、後側アーム12の軸方向に沿って複数並べて設けてもよい。この場合、前側アーム11と後側アーム12を一回接近離間させるだけで、複数個所を同時に縫合することが可能となる。すると、複数個所の縫合が必要な場合でも、縫合を短時間で実施することが可能となる。
 例えば、縫合針部14が一か所の場合、一回縫合するたびに、縫合装置10の交換や縫合器具20を収容部17の一対の収容空間18,19に充填する作業が必要となるので、その作業の分だけ縫合時間が長くなる。
 しかし、縫合針部14を後側アーム12の軸方向に沿って複数並べておけば、複数個所を一度に縫合することが可能となるので、縫合時間が短くできる。具体的には、縫合針部14を2つ設けていれば、縫合針部14が一か所の場合に比べて、ほぼ半分の時間で縫合を完了することができる。すると、患者や施術する医師の負担を軽減できる。
 なお、複数の縫合針部14を設けた場合には、縫合針部14に対応した数の収容部17が設けられ、各縫合針部14の一対の針状部材15,16に対応するように、各収容部17の収容空間18,19が配置される。
 本発明の縫合装置は、消化管内において組織同士の縫合するための器具として適している。

Claims (5)

  1.  前側アームと、
    該前側アームに対し接近離間可能に設けられた後側アームと、
    前記前側アームと前記後側アームを接近離間させるアーム移動手段と、を備えており、
    前記前側アームおよび前記後側アームは、基端部が前記アーム移動手段に連結されており、
    前記後側アームは、
    先端を前記前側アームに向けた状態かつその中心軸が該後側アームと前記前側アームが接近離間する方向と平行になるように設けられた一対の針状部材を有する縫合針部を備えており、
    前記前側アームは、
    該前側アームと前記後側アームが接近したときに、前記一対の針状部材の先端部をそれぞれ収容し得る一対の収容空間が設けられた収容部を備えており、
    該収容部の一対の収容空間内には、
    縫合糸によって互いに連結された、前記針状部材と係合可能な一対の係合部材が収容されている
    ことを特徴とする縫合装置。
  2.  前記前側アームおよび前記後側アームが前記アーム移動手段に連結されている位置から、前記一対の針状部材までの、前記後側アームと前記前側アームが接近離間する方向と垂直な面に沿った距離が5mm以上である
    ことを特徴とする請求項1記載の縫合装置。
  3.  前記縫合針部は、
    前記一対の針状部材が前記後側アームの軸方向に並ぶように設けられており、
    前記収容部は、
    前記一対の収容空間が前記前側アームの軸方向に並ぶように設けられている
    ことを特徴とする請求項1または2記載の縫合装置。
  4.  前記後側アームには、該後側アームの軸方向に並ぶように複数の前記縫合針部が設けられており、
    前記前側アームには、該前側アームの軸方向に並ぶように各縫合針部に対応する複数の前記収容部が設けられている
    ことを特徴とする請求項1、2または3記載の縫合装置。
  5.  前記後側アームと前記前側アームを、接近離間可能かつ接近離間する方向と直交する面内の相互回転を固定した状態で連結する連結機構を備えている
    ことを特徴とする請求項1、2、3または4記載の縫合装置。
