Abstract

本発明は、マクロライド系免疫抑制剤についての血液検査を簡便および/または高精度に行うことを提供する。具体的には、本発明は、以下を含む、マクロライド系免疫抑制剤についての血液検査方法を提供する: (a)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む血液試料を酸またはアルカリで処理すること;ならびに (b)抗体を用いて、血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。

Description

マクロライド系免疫抑制剤についての血液検査方法
 本発明は、マクロライド系免疫抑制剤についての血液検査方法などに関する。
 タクロリムス(FK506)、シクロスポリンAをはじめとするマクロライド系免疫抑制剤は、臓器移植後の拒絶反応回避、自己免疫疾患の治療等に有効であるが、適切な治療のためには血中薬物濃度を適切な治療濃度域に保つことが求められるため、治療期間中は血中薬物濃度を監視する必要がある。
 投与されたタクロリムスおよびシクロスポリンAの大部分は、患者血中の赤血球画分に存在している。したがって、TDM(Therapeutic Drug Monitoring)における血液試料としては、赤血球画分を含む全血が用いられている。
 ところで、全血等の血液試料が薬物に対する結合タンパク質を含む場合、血液試料中の薬物濃度を測定するためには、適切な前処理により血液試料中の薬物をその結合タンパク質から解離させることが必須である。このような前処理では、一般的に、有機溶媒、変性剤、界面活性剤等の化学物質が用いられている。
 例えば、このような前処理を記載する先行技術として、以下を挙げることができる。
 特許文献1には、両イオン性界面活性剤およびサポニンの使用が記載されている。
 特許文献2には、ノニオン系界面活性剤(例、サポニン)またはオクチルフェノキシポリエトキシエタノールを含む溶血剤、および胆汁酸塩界面活性剤からなる抽出剤の使用が記載されている。
 特許文献3には、ジメチルスルホキシド(DMSO)、グリコール(例、エチレングリコール)および亜鉛塩類(例、硫酸亜鉛)を含む試薬の使用が記載されている。
 特許文献4には、プロテアーゼ、グリコール(例、エチレングリコール)およびアルコール(例、メタノール)の使用が記載されている。
 特許文献5には、グリコール(例、エチレングリコール)、アルコール(例、メタノール)および塩(例、塩化ナトリウム)を含む抽出試薬試料の使用が記載されている。
 特許文献6には、サポニン、メタノール、エチレングリコールおよび硫酸亜鉛の使用が記載されている。
 特許文献7には、溶血剤および界面活性剤〔例、イオン性界面活性剤(例、SDS)、非イオン性界面活性剤(例、Triton X-405)〕の使用が記載されている。
 特許文献8には、遊離剤(例、疎水性薬物の類似体)、および選択的可溶化剤(例、エチレングリコール、グリセロール、1-メトキシ-2-プロパノール、ジメチルスルホキシド(DMSO)、ジメチルスルホン又はジメチルホルムアミド)の使用が記載されている。
 特許文献9には、ジメチルスルホキシド(DMSO)、少なくとも1つの二価金属塩および水を含む抽出試薬組成物であって、DMSO濃度が抽出試薬組成物の50容量%以上のものの使用が記載されている。
 また、このような前処理の多くは、薬物をその結合タンパク質から解離させた後、薬物をその結合性タンパク質から分離するために、タンパク質成分の沈殿操作(遠心分離)を必要とする。

 上述したような先行技術では、特殊な物質による試料の前処理および沈澱操作を要し、アッセイ全体として複雑になり易いことから、アッセイの簡便化が難しいという課題がある。
 また、揮発性の高い有機溶媒を用いる方法では、抽出の各段階やアッセイ中のインキュベーションの間に有機溶媒の蒸発が起こるため、薬物の濃度の測定値がみかけ上高い値を示し、精度に欠けるという課題もある。
 したがって、本発明の目的は、マクロライド系免疫抑制剤についての血液検査について、簡便および/または高精度に行い得る代替的な方法を提供することである。
 本発明者らは、鋭意検討した結果、マクロライド系免疫抑制剤についての血液検査を抗体で行う場合、酸またはアルカリ(塩基)による血液試料の前処理が有効であること、ならびにこのような前処理により上記課題を解決できることなどを見出し、本発明を完成するに至った。
 すなわち、本発明は、以下のとおりである。
〔1〕以下を含む、マクロライド系免疫抑制剤についての血液検査方法:
(a)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む血液試料を酸またはアルカリで処理すること;ならびに
(b)抗体を用いて、血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
〔2〕マクロライド系免疫抑制剤が、タクロリムス、シクロスポリンA、またはエベロリムスである、〔1〕の方法。
〔3〕血液試料がヒト血液試料である、〔1〕または〔2〕の方法。
〔4〕前記血液試料の処理が、酸による処理である、〔1〕~〔3〕のいずれかの方法。
〔5〕前記酸による処理が、前記血液試料と酸性緩衝液との混合により行われる、〔4〕の方法。
〔6〕酸性緩衝液のpHが1.0~5.0である、〔5〕の方法。
〔7〕免疫抑制剤に対する抗体が、IgG、IgM、またはそれらの抗体断片である、〔1〕~〔6〕のいずれかの方法。
〔8〕以下を含む方法である、〔1〕~〔7〕のいずれかの方法:
(a’)前記マクロライド系免疫抑制剤、または前記代謝物を含む血液試料を酸またはアルカリで処理して、酸性血液試料またはアルカリ性血液試料を調製すること;ならびに
(b’)抗体を用いて、前記酸性血液試料または前記アルカリ性血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
〔9〕以下を含む方法である、〔1〕~〔7〕のいずれかの方法:
(a’’)前記マクロライド系免疫抑制剤、または前記代謝物を含む血液試料を酸またはアルカリで処理して酸性血液試料またはアルカリ性血液試料を調製した後、中和を行い、中性血液試料を調製すること;ならびに
(b’’)抗体を用いて、前記中性血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
〔10〕前記中和が、カオトロピック変性剤を含む中和液を前記酸性血液試料またはアルカリ性血液試料と混合して行われる、〔9〕の方法。
〔11〕(1)酸またはアルカリ、および(2)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体を含む、マクロライド系免疫抑制剤についての血液検査試薬。
〔12〕酸が酸性緩衝液である、〔11〕の試薬。
〔13〕さらに(3)中和液を含む、〔11〕または〔12〕の試薬。
 本発明は、マクロライド系免疫抑制剤についての血液検査を良好に行うことができる。
 例えば、本発明は、簡便かつ高精度であり、しかも処理能力の高い検査を実現することができる。
 より具体的には、本発明によれば、マクロライド系免疫抑制剤およびその代謝物を含む血液試料の酸性化(例、血液試料および酸性緩衝液の混合)またはアルカリ性化(例、血液試料およびアルカリ性緩衝液の混合)という簡便な操作(前処理)により、酸性血液試料中またはアルカリ性血液試料中のマクロライド系免疫抑制剤およびその代謝物の濃度を抗体で首尾よく測定することができる。これにより、個人の技術習熟度に応じた測定値の変動を回避することができ、また、検査の処理能力(ハイスループット)を改善することができる。
 また、本発明によれば、有機溶媒の使用が必須でないため、有機溶媒の蒸発による濃縮を回避することができ、マクロライド系免疫抑制剤およびその代謝物の濃度を高精度に測定することができる。

 本発明は、以下を含む、マクロライド系免疫抑制剤についての血液検査方法を提供する:
(a)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む血液試料を酸またはアルカリで処理すること;ならびに
(b)抗体を用いて、血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
 本発明で用いられる血液試料は、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む動物血液試料である。このような血液試料は、マクロライド系免疫抑制剤が投与された動物から採取することができる。動物血液試料には、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する結合タンパク質が含まれており、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物、および結合タンパク質は、互いに結合して複合体を形成することができる。このような血液試料が由来する動物は、好ましくは哺乳動物(例、ヒト、サル、チンパンジー等の霊長類;マウス、ラット、ウサギ等の齧歯類;ならびにウシ、ブタ、ウマ、ヤギ、ヒツジ等の家畜および使役動物)であり、より好ましくは霊長類であり、特に好ましくはヒトである。また、血液試料としては、例えば、全血、血清、血漿、およびその他の血液画分が挙げられる。マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物、およびそれに対する結合タンパク質の大部分は所定の血液画分(例、赤血球画分)中に存在するため、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物の血中濃度の測定では、このような血液画分、またはこのような血液画分を含む血液試料(例、全血)を用いることができる。分画操作を省略して簡便に濃度を測定する観点から、血液試料として全血の使用が好ましい。血液試料はさらに、免疫の抑制が所望される疾患(例、自己免疫疾患、関節リウマチ、重症筋無力症、クローン病、ループス腎炎、活動期潰瘍性大腸炎、多発性筋炎、移植片宿主病(GVDH)、皮膚筋炎に合併する間質性肺炎)に罹患している哺乳動物、または臓器、組織もしくは細胞の移植(例、心移植、腎移植、骨髄移植、肺移植、肝移植、膵移植、小腸移植)を受けた哺乳動物から採取されるものであることが好ましい。このような哺乳動物から採取される血液試料は、予備処理(例、ろ過、加熱、溶血)に付されてもよい。
 マクロライド系免疫抑制剤は、マクロライド構造(12個以上の原子から構成される環状ラクトン構造)を含み、かつ免疫系の活動を低下させるか、または阻害する作用を有する化合物である。マクロライド系免疫抑制剤としては、タクロリムス(FK506)、シクロスポリンA、シロリムス(ラパマイシン)およびその誘導体(例、エベロリムス、テムシロリムス)、ゾタロリムス、バイオリムス、ノボリムス、ピメクロリムス、ミオリムス、マイオリムス、ならびにデフォロリムスが挙げられる。
 マクロライド構造を有するマクロライド系免疫抑制剤の代謝物は、マクロライド系免疫抑制剤の種類に応じて異なるが、マクロライド系免疫抑制剤の種類に応じた結合タンパク質に対して結合できる限り特に限定されない。例えば、マクロライド系免疫抑制剤がタクロリムスである場合、マクロライド構造を有するタクロリムス代謝物としては、例えば、13-O-デスメチルタクロリムス(MI)、31-O-デスメチルタクロリムス(MII)、15-O-デスメチルタクロリムス(MIII)、12-O-ヒドロキシルタクロリムス(MIV)、15,31-O-ジデスメチルタクロリムス(MV)、13,31-O-ジデスメチルタクロリムス(MVI)、13,15-O-ジデスメチルタクロリムス(MVII)、MVIII タクロリムス、C22オキシムタクロリムス、C24スクシネートタクロリムス、C32スクシネートタクロリムスが挙げられること(Clin.Chem.,2014 Apr,60(4),621-30)、およびこのようなタクロリムス代謝物は、タクロリムス結合タンパク質と結合して複合体を形成できることが報告されている(Ther.Drug.Monit.,1999 Jun,21(3),274-80;Biochem.Biophys.Res.Commun.,1994 Jul,15,202(1),437-43;Drug Metabolism and Disposition,November 1993,21(6),971-977)。マクロライド系免疫抑制剤がシクロスポリンAである場合、マクロライド構造を有するシクロスポリンA代謝物としては、例えば、シクロスポリンAM1、シクロスポリンAM9、シクロスポリンAM4N、シクロスポリンAM1c、シクロスポリンAM19が挙げられること(Br.J.Clin.Pharmacol.,2003 Feb,55(2),203-11)、およびこのようなシクロスポリンA代謝物は、シクロスポリンA結合タンパク質と結合して複合体を形成できることが報告されている(Clin.Biochem.,1991 Feb,24(1),71-4;CLIN.CHEM.,37(3),403-410(1991))。マクロライド構造を有するシロリムス代謝物としては、例えば、16-O-デメチルシロリムス、24-ヒドロキシシロリムス、25-ヒドロキシシロリムス、46-ヒドロキシシロリムス、39-O-デメチルシロリムス、7-O-デメチルシロリムス、32-O-デメチルシロリムス、41-O-デメチルシロリムス、ヒドロキシシロリムス、セコ(seco)-シロリムス、11-ヒドロキシシロリムス、3,4-ジヒドロジオールシロリムス、5,6-ジヒドロジオールシロリムスが挙げられること(Ther.Drug.Monit.,2015 Jun,37(3),395-9;ラパリムス 1mg製造販売承認申請書添付資料 第2部(モジュール 2)2.4 非臨床試験の概括評価)、およびこのようなシロリムス代謝物は、シロリムス結合タンパク質と結合して複合体を形成できることが報告されている(Clin.Biochem.,1996 Aug,29(4),309-13)。マクロライド構造を有するエベロリムス代謝物としては、例えば、46-ヒドロキシエベロリムス、24-ヒドロキシエベロリムス、25-ヒドロキシエベロリムス、45-ヒドロキシエベロリムス、2-ヒドロキシエベロリムス、11-ヒドロキシエベロリムス、14-ヒドロキシエベロリムス、39-O-デスメチルエベロリムス、27-O-デスメチルエベロリムス、40-O-デスエチルヒドロキシエベロリムス(シロリムス)が挙げられる(Ther.Drug Monit.,2007 Dec,29(6),743-9)。マクロライド系免疫抑制剤、およびマクロライド構造を有するマクロライド系免疫抑制剤の代謝物は、個体差等の因子を考慮するべきであるが、ヒト等の動物の血液中において所定の比率で存在し得る。したがって、マクロライド系免疫抑制剤についての血液検査は、血液試料中の代謝物の濃度の測定によっても間接的に行うことができる。好ましくは、血液検査は、マクロライド系免疫抑制剤の濃度の測定により行うことができる。
 血液試料に含まれる、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する結合タンパク質としては、ヒト等の哺乳動物に対して内因性である種々のタンパク質が知られている。より具体的には、タクロリムス(FK506)、またはマクロライド構造を有するその代謝物に対する特異的結合タンパク質としては、例えば、FK506結合タンパク質(FKBP)(例、FKBP12、FKBP13、FKBP14、FKBP2、FKBP3)が挙げられる。シクロスポリンA、またはマクロライド構造を有するその代謝物に対する特異的結合タンパク質としては、例えば、シクロフィリンA、シクロフィリンBが挙げられる。シロリムス(ラパマイシン)およびその誘導体、またはマクロライド構造を有するそれらの代謝物に対する特異的結合タンパク質としては、例えば、FKBP12が挙げられる。このような特異的結合タンパク質は、赤血球画分に主に存在する。また、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する非特異的結合タンパク質としては、例えば、アルブミン、リポタンパク質、α1-酸性糖タンパク質が挙げられる。
 本発明の方法に用いられる酸としては、血液試料を酸性化できる任意の酸を利用することができる。このような酸としては、例えば、無機酸(例、リン酸、ホウ酸、炭酸、塩酸、硫酸、硝酸)、および有機酸(例、酢酸、クエン酸、グリシン、酒石酸、2-モルホリノエタンスルホン酸(MES)、ギ酸、シュウ酸、乳酸、マレイン酸、トリフルオロ酢酸、フタル酸)、ならびにそれらの2種以上(例、2種、3種、4種または5種)の混合物が挙げられる。
 また、本発明の方法に用いられるアルカリとしては、血液試料をアルカリ性化できる任意のアルカリを利用することができる。このようなアルカリとしては、例えば、無機塩基(例、水酸化ナトリウム、水酸化カリウム、水酸化マグネシウム、水酸化カルシウム等の金属(例、一価金属、二価金属)の水酸化物、炭酸水素ナトリウム、炭酸水素カリウム、炭酸水素カルシウム等の金属(例、一価金属、二価金属)の炭酸水素塩、炭酸ナトリウム、炭酸カリウム、炭酸カルシウム等の金属(例、一価金属、二価金属)の炭酸塩、リン酸ナトリウム、リン酸カリウム等の金属(例、一価金属)のリン酸塩、ホウ酸ナトリウム、ホウ酸カリウム等の金属(例、一価金属)のホウ酸塩、アンモニア)、有機塩基(例、トリメチルアミンおよびトリエチルアミン等のアミン、ピリジン等の含窒素複素環化合物)、ならびにそれらの2種以上(例、2種、3種、4種または5種)の混合物が挙げられる。
 本発明の方法において、血液試料の処理は、酸またはアルカリのいずれも使用できるが、血液試料中のマクロライド系免疫抑制剤の検出感度の観点から、血液試料の処理としては酸による処理を行うことがより好ましい。以下、酸またはアルカリによる処理について詳述する。
 マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む血液試料の酸またはアルカリによる処理としては、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物、およびそれに対する結合タンパク質を解離させることができる程度に血液試料の酸性度を増強できる(換言すれば、pHを低下できる)またはアルカリ性度を増強できる(換言すれば、pHを上昇できる)処理である限り特に限定されず、例えば、血液試料と酸性溶液(例、酸性緩衝液、緩衝能を有しない酸性溶液)またはアルカリ性溶液(例、アルカリ性緩衝液、緩衝能を有しないアルカリ性溶液)との混合(換言すれば、血液試料の希釈)、および血液試料中への酸(固体)またはアルカリ(固体)の溶解が挙げられるが、操作の簡便性等の観点から、血液試料と酸性溶液またはアルカリ性溶液との混合が好ましい。
 処理される血液試料の容量は、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を本発明の方法により測定できる限り特に限定されないが、例えば0.1~500μLであり、好ましくは0.5~400μLであり、より好ましくは1.0~300μLであり、さらにより好ましくは2.0~200μLである。血液試料と混合されるべき酸性溶液またはアルカリ性溶液の容量は、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物、およびそれに対する結合タンパク質を解離させることができる程度に血液試料の酸性度またはアルカリ性度を増強できる限り特に限定されないが、例えば0.5~5,000μLであり、好ましくは1.0~3,000μLであり、より好ましくは2.0~1,000μLであり、さらにより好ましくは5.0~500μLである。血液試料と酸性溶液またはアルカリ性溶液との混合比率(血液試料/酸性溶液または血液試料/アルカリ性溶液)は、血液試料の酸性度またはアルカリ性度を増強できる限り特に限定されないが、例えば1/100~100であり、好ましくは1/50~50であり、より好ましくは1/30~30であり、さらにより好ましくは1/20~20である。血液試料は上述したような動物由来試料であり、中性の性質を示す。このような血液試料を酸により処理すると、それに含まれるマクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を、その結合タンパク質から解離させることができる。したがって、このような処理により、結合タンパク質から解離している状態にある、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む酸性血液試料またはアルカリ性血液試料を調製することができる。マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を結合タンパク質から十分に解離させるためには、混合または溶解後、所定の温度条件下で、十分な解離のための時間を設けること(例、放置)が好ましい。一方で、酸による成分(例、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物)の加水分解を極力回避するためには、温度は高温ではなく、時間は長期でないことが好ましい。このような温度は、例えば5~45℃であり、好ましくは15~40℃である。このような時間としては、例えば10秒~60分であり、好ましくは20秒~30分であり、より好ましくは30秒~15分である。
 血液試料の処理を酸性溶液で行う場合、好ましくは、処理は、酸性血液試料の簡便かつ迅速な調製、および酸性血液試料に対する一定の酸性度の付与等の観点から、血液試料と酸性緩衝液との混合である。酸性緩衝液としては、酸性領域において緩衝能を示すことができる任意の酸を含む緩衝液を用いることができる。このような酸としては、例えば、無機酸(例、リン酸、ホウ酸、炭酸)、および有機酸(例、酢酸、クエン酸、グリシン、酒石酸、2-モルホリノエタンスルホン酸(MES)、トリフルオロ酢酸、フタル酸)、ならびにそれらの2種以上(例、2種、3種、4種または5種)の混合物が挙げられる。
 酸性緩衝液等の酸性溶液のpHは、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物、およびその結合タンパク質の種類および濃度等の因子によって変動し得るが、例えば、1.0~5.0である。pH1.0~5.0の酸性溶液としては、このようなpHの酸性領域において緩衝能を示すことができる緩衝液が好ましい。このような緩衝液としては、例えば、リン酸緩衝液、酒石酸緩衝液、クエン酸緩衝液、酢酸緩衝液、トリフルオロ酢酸緩衝液、フタル酸緩衝液、グリシン緩衝液、炭酸-重炭酸緩衝液、2-モルホリノエタンスルホン酸(MES)緩衝液が挙げられる。上述した因子の影響を極力排除して汎用性を向上させる等の観点から、pHは、好ましくは1.5以上、より好ましくは2.0以上、さらにより好ましくは2.5以上であってもよい。pHはまた、好ましくは4.8以下、より好ましくは4.5以下であってもよい。より具体的には、pHは、好ましくは1.5~4.8、より好ましくは2.0~4.8、さらにより好ましくは2.5~4.8、特に好ましくは2.5~4.5であってもよい。pHの測定は、当該分野における公知の方法を用いて行うことができる。好ましくは、pHは、ガラス電極を有するpH計を用いて25℃で測定された値を採用することができる。
 血液試料の処理をアルカリ性溶液で行う場合、好ましくは、処理は、アルカリ性血液試料の簡便かつ迅速な調製、およびアルカリ性血液試料に対する一定のアルカリ性度の付与等の観点から、血液試料とアルカリ性緩衝液との混合である。アルカリ性緩衝液としては、塩基性領域において緩衝能を示すことができる任意の塩基を含む緩衝液を用いることができる。このような塩基は、上述したものと同様である。
 アルカリ性緩衝液等のアルカリ性溶液のpHは、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物、およびその結合タンパク質の種類および濃度等の因子によって変動し得るが、例えば、8.0~13.0である。pH8.0~13.0のアルカリ性溶液としては、このようなpHの塩基性領域において緩衝能を示すことができる緩衝液が好ましい。このような緩衝液としては、例えば、リン酸緩衝液、炭酸―重炭酸緩衝液、ホウ酸緩衝液、MES緩衝液、N-(2-アセトアミド)イミノ二酢酸(ADA)緩衝液、ピペラジン-1,4-ビス(2-エタンスルホン酸)(PIPES)緩衝液、N-(2-アセトアミド)-2-アミノエタンスルホン酸(ACES)緩衝液、コラミン塩酸緩衝液、N,N-ビス(2-ヒドロキシエチル)-2-アミノエタンスルホン酸(BES)緩衝液、N-トリス(ヒドロキシメチル)メチル-2-アミノエタンスルホン酸(TES)緩衝液、2-[4-(2-ヒドロキシエチル)-1-ピペラジニル]エタンスルホン酸(HEPES)緩衝液、アセトアミドグリシン緩衝液、トリシン緩衝液、グリシンアミド緩衝液、ビシン緩衝液、トリス緩衝液が挙げられる。上述した因子の影響を極力排除して汎用性を向上させる等の観点から、pHは、好ましくは9.0以上であってもよい。pHはまた、好ましくは12.0以下、特に好ましくは11.0以下であってもよい。より具体的には、pHは、好ましくはpH9.0~12.0、特に好ましくはpH9.0~11.0であってもよい。pHの測定は、上述のとおりである。
 酸性緩衝液等の酸性溶液またはアルカリ性緩衝液等のアルカリ性溶液は、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体を含んでいても、含んでいなくてもよい。酸性溶液またはアルカリ性溶液がこのような抗体を含んでいる場合、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物の濃度の測定は、当該抗体を利用して行うことができる。酸性溶液またはアルカリ性溶液はまた、他の物質を含んでいてもよい。このような他の物質としては、例えば、水と混和可能な有機溶媒(例、アルコール、エーテル、エステル)、変性剤(例、カオトロピック剤)、界面活性剤(例、カチオン性界面活性剤、アニオン性界面活性剤、両イオン性界面活性剤、非イオン性界面活性剤、胆汁酸塩界面活性剤)、溶血剤、競合剤(例、類似体)等の物質が挙げられる(例、上述した特許文献1~9)。
 血液試料の酸またはアルカリによる処理により調製される酸性血液試料またはアルカリ性血液試料のpHは、酸性緩衝液等の酸性溶液またはアルカリ性緩衝液等のアルカリ性溶液について上述したpHと同様である。したがって、このような酸性血液試料またはアルカリ性血液試料のpHの例および好ましい例については、上述した酸性溶液またはアルカリ性溶液のものを参照することができる。
 本発明で用いられる抗体は、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体である。このような抗体は、ポリクローナル抗体またはモノクローナル抗体のいずれであってもよい。抗体は、免疫グロブリン(例、IgG、IgM、IgA、IgD、IgE、IgY)のいずれのアイソタイプであってもよい。抗体はまた、全長抗体であってもよい。全長抗体とは、可変領域および定常領域を各々含む重鎖および軽鎖を含む抗体(例、2つのFab部分およびFc部分を含む抗体)をいう。抗体はまた、このような全長抗体に由来する抗体断片であってもよい。抗体断片は、全長抗体の一部であり、例えば、定常領域欠失抗体(例、F(ab’)、Fab’、Fab、Fv)が挙げられる。抗体はまた、単鎖抗体等の改変抗体であってもよい。好ましくは、抗体は、IgG、IgM、またはそれらの抗体断片であってもよい。本発明では、1種の抗体のみならず、2種以上(例、2種、3種)の抗体を用いることができる。
 抗体としては、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物のみを認識する抗体を用いることができる。あるいは、2種以上の抗体が本発明で用いられる場合、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物のみを認識する抗体に加えて、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物およびそれに対する抗体の複合体を認識する抗体を、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体として用いることができる(例、国際公開第2004/11644号;国際公開第2013/042426号;Nucleic Acids Res.(Feb 2011) vol.39,no.3,e14)。
 マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体は、当該分野において公知である任意の方法を用いて作製することができる。例えば、抗体は、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を抗原として用いて作製することができる。また、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体が市販されているので、このような市販品を使用することもできる。
 本発明では、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上(例、1種、2種)の抗体を用いることができる。本発明では、抗体に対する2次抗体を用いることもできる。
 抗体は、固相に固定されていてもよい。本明細書において、固相に固定された抗体を、単に固相抗体ということがある。固相としては、例えば、液相を収容または搭載可能な固相(例、ウェルプレート、マイクロ流路、ガラスキャピラリー、ナノピラー、モノリスカラム、チューブ等の容器;およびプレート、メンブレン、ろ紙等の支持体)、ならびに液相中に懸濁または分散可能な固相(例、粒子等の固相担体)が挙げられる。固相の材料としては、例えば、ガラス、プラスチック、金属、及びカーボンが挙げられる。固相の材料としてはまた、非磁性材料、又は磁性材料を用いることができるが、固相担体については、操作の簡便性等の観点から、磁性材料が好ましい。抗体を固相に固定する方法としては、当該分野において公知である任意の方法を利用することができる。このような方法としては、例えば、物理的吸着法、共有結合法、親和性物質(例、ビオチン、ストレプトアビジン)を利用する方法、およびイオン結合法が挙げられる。
 抗体は、標識物質で標識されていてもよい。本明細書において、標識物質で標識された抗体を、単に標識抗体ということがある。標識物質としては、例えば、酵素(例、ペルオキシダーゼ、アルカリホスファターゼ、ルシフェラーゼ、βガラクトシダーゼ)、親和性物質(例、ストレプトアビジン、ビオチン)、蛍光物質またはタンパク質(例、フルオレセイン、フルオレセインイソチオシアネート、ローダミン、緑色蛍光タンパク質、赤色蛍光タンパク質)、発光又は吸光物質(例、ルシフェリン、エクオリン、アクリジニウム)、放射性物質(例、H、14C、32P、35S、125I)が挙げられる。
 抗体によるマクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物の濃度の測定は、イムノアッセイにより行うことができる。このようなイムノアッセイとしては、例えば、酵素免疫測定法(EIA)〔例、化学発光EIA(CLEIA)、酵素吸着EIA(ELISA)〕、蛍光免疫測定法、化学発光免疫測定法、電気化学発光免疫測定法、凝集法、免疫染色、フローメトリー法、バイオレイヤー干渉法、In Situ PLA法、化学増幅型ルミネッセンス・プロキシミティ・ホモジニアス・アッセイ、ラインブロット法、ウエスタンブロット法が挙げられる。
 本発明は、以下を含む方法により行われる。
(a)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む血液試料を酸またはアルカリで処理すること;ならびに
(b)抗体を用いて、血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
 工程(a)および(b)は、並行して、または別々に行われてもよい。例えば、酸またはアルカリによる血液試料の処理において、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体を含む酸性緩衝液もしくはアルカリ性緩衝液を用いる場合、酸またはアルカリによる血液試料の処理および抗体による濃度の測定は、並行して行うことができる。この場合、抗体による濃度の測定において、酸性緩衝液もしくはアルカリ性緩衝液に含まれるマクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する第1抗体(例、固相抗体)と異なる、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する第2抗体(例、標識抗体)がさらに用いられてもよい。一方、例えば、酸またはアルカリによる血液試料の処理において、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体を含む酸性緩衝液もしくはアルカリ性緩衝液を用いない場合、酸またはアルカリによる血液試料の処理および抗体による濃度の測定は、別々に行うことができる。この場合、工程(a)および(b)は、連続的または非連続的に行うことができる。工程(a)および(b)が非連続的に行われる場合、酸で処理された血液試料は、さらなる処理に付されてもよい。このような処理としては、例えば、上述したような予備処理と同様の処理(例、ろ過、加熱、溶血)、pHの変更、所定の化学物質による処理が挙げられる。
 特定の実施形態では、本発明は、以下を含む方法により行われてもよい:
(a’)前記マクロライド系免疫抑制剤、または前記代謝物を含む血液試料を酸またはアルカリで処理して、酸性血液試料またはアルカリ性血液試料を調製すること;ならびに
(b’)抗体を用いて、前記酸性血液試料または前記アルカリ性血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
 工程(a’)および(b’)は、並行して、または別々に行われてもよい。また、工程(a’)および(b’)は、連続的または非連続的に行うことができる。例えば、工程(a’)および(b’)が連続して行われる場合、工程(a’)で調製された酸性血液試料またはアルカリ性血液試料をそのまま、工程(b’)に付すことができる。工程(a’)および(b’)が非連続的に行われる場合、酸性血液試料またはアルカリ性血液試料は、さらなる処理に付されてもよい。このような処理は、例えば、工程(a)および(b)に関する説明において上述したようなものと同様である。
 本発明は、酸性条件下またはアルカリ性条件下の血液試料において、免疫抑制剤として用いられるマクロライド系物質およびその結合タンパク質の結合を解離させつつ、解離したマクロライド系物質の濃度を抗体で測定することができ、しかも測定において定量性に優れるという知見に基づく。このようなマクロライド系物質は、特定の環状構造(比較的リジッドな構造であり、フレキシビリティーに欠ける)を有し、かつ比較的高い疎水度を有する高分子量(分子量800から1300程度)の化合物である。本説明により本発明の範囲の限定を意図するものではないが、酸性条件またはアルカリ性条件という相対的に過酷な条件下で行われるにもかかわらず本発明が定量性に優れる一因は、このような特徴を有するマクロライド系物質の使用に起因する可能性がある。
 他の実施形態では、本発明は、以下を含む方法により行われてもよい:
(a’’)前記マクロライド系免疫抑制剤、または前記代謝物を含む血液試料を酸またはアルカリで処理して酸性血液試料またはアルカリ性血液試料を調製した後、中和を行い、中性血液試料を調製すること;ならびに
(b’’)抗体を用いて、前記中性血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
 すなわち、本発明の方法において、血液試料を酸またはアルカリで処理した後、中和処理を行ってもよい。中和処理を行うことで、後段の抗体を用いてマクロライド系免疫抑制剤等の濃度を測定する工程(工程(b’’)で使用する抗体について、酸またはアルカリに対する耐性の低い抗体を選択することも可能となる。
 中和処理に使用する中和液としては、酸またはアルカリによる処理で生成された酸性血液試料またはアルカリ性血液試料のpHを6.5~7.5程度にできる組成であれば、特に限定されるものではない。酸性血液試料に対する中和液は、例えば、上述のアルカリとして示した物質を含み、pHは、pH8.0~13.0のものを好適に使用できる。アルカリ性血液試料に対する中和液は、例えば、上述の酸として示した物質を含み、pHは、pH1.0~6.5のものを好適に使用できる。
 また、中和液は、アルカリ性物質または酸性物質の他に、カオトロピック変性剤を含んでいてもよい。カオトロピック変性剤としては、尿素、チオ尿素、グアニジン塩酸塩、グアニジンチオシアン酸塩、サリチル酸ナトリウム、チオシアン酸ナトリウム、過塩素ナトリウム、アセトアミドおよびホルムアミド等が挙げられる。カオトロピック変性剤の終濃度は、0.5M~8.0Mとすることが好ましい。
 血液試料を酸またはアルカリで処理した後、中和を行うと、酸またはアルカリへの耐性の低い抗体の活性が低下するリスクを低減できる一方で、解離させたマクロライド系免疫抑制剤と結合タンパク質との再結合により感度が低下するリスクが懸念される。中和液にカオトロピック変性剤を添加することにより、前記再結合の影響を低減することができる。なお、カオトロピック変性剤は、前述のように、中和液ではなく、酸性溶液またはアルカリ性溶液に添加してもよい。あるいは、中和液と酸性溶液もしくはアルカリ性溶液の両方に添加してもよい。
 本発明はまた、(1)酸またはアルカリ、および(2)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体を含む、マクロライド系免疫抑制剤についての血液検査試薬を提供する。
 酸またはアルカリは、上述したとおりである。酸またはアルカリは、固体であっても液体であってもよいが、好ましくは液体である。したがって、本発明の試薬は、(1’)酸性溶液またはアルカリ性溶液、および(2’)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上(例、1種、2種)の抗体を含むことが好ましい。酸性溶液またはアルカリ性溶液およびそのpHは、上述したとおりである。
 酸性溶液またはアルカリ性溶液は、酸性緩衝液またはアルカリ性緩衝液であっても、緩衝能を有しない酸性溶液またはアルカリ性溶液であってもよいが、好ましくは酸性緩衝液またはアルカリ性緩衝液である。したがって、本発明の試薬は、(1’’)酸性緩衝液またはアルカリ性緩衝液、および(2’’)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体を含むことが好ましい。酸性緩衝液またはアルカリ性緩衝液およびそれらのpHは、上述したとおりである。
 酸性緩衝液等の酸性溶液またはアルカリ性緩衝液等のアルカリ性溶液は、液体または凍結物の形態において提供することができる。酸性緩衝液またはアルカリ性緩衝液は、上述したような容器、またはボトル等の大型の容器に収納されていてもよい。マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体は、緩衝液等の液体中に溶解した形態において、またはその凍結物の形態において、もしくは抗体の凍結乾燥物の形態において提供することができる。マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体は、遊離した形態(例、溶液、凍結物)において、または上述したような固相に固定された形態において提供することができる。
 一実施形態では、本発明の試薬は、(1’’)酸性緩衝液またはアルカリ性緩衝液、および(2’’)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体が互いに一体となった組成物である、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体を含む酸性緩衝液またはアルカリ性緩衝液であってもよい。このような酸性緩衝液またはアルカリ性緩衝液を用いる場合、酸またはアルカリによるマクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物のその結合タンパク質からの解離、および解離したマクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物の濃度の抗体による測定を、並行して簡便に行うことができる。このような場合、本発明の試薬は、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体を含む酸性緩衝液またはアルカリ性緩衝液に加えて、当該抗体とは異なるマクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体および/または抗体に対する2次抗体を含んでいてもよい。このような試薬は、2種以上の抗体を用いるイムノアッセイにおいて好適に用いることができる。
 別の実施形態では、本発明の試薬は、(1’’)酸性緩衝液またはアルカリ性緩衝液、および(2’’)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体を、隔離した形態で含むキットであってもよい。このような形態であれば、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体および酸性緩衝液またはアルカリ性緩衝液が隔離されているので、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体の性能を長期にわたり維持することができる。このような場合、本発明の試薬は、保存性に優れる。
 さらに別の実施形態では、本発明の試薬は、(1)酸またはアルカリ、および(2)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体、に加えて、(3)中和液を含む。中和液およびそのpHは、上述したとおりである。
 本発明の試薬はまた、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物(標品)を含んでいてもよい。マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物は、例えば、ポジティブコントロールとして、および/または定量のための検量線の作成に用いることができる。
 本発明の試薬はさらに、マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する抗体を用いるイムノアッセイに必要な構成要素を含んでいてもよい。このような構成要素としては、上述したような標識物質および酵素の基質、ならびに希釈液、2次抗体、および抗体の安定化剤、哺乳動物から試料を採取し得る器具(例、注射器、生検針)が挙げられる。
 以下、本発明を実施例により詳細に説明するが、本発明は、これらの実施例に限定されるものではない。
実施例1:酸処理による全血検体中のタクロリムス検出ELISA
 実施例1で用いた材料は、以下のとおりである。
 96穴イムノプレート:Nunc Maxisorp C96イムノプレート(Thermo,Cat#430341)
 抗タクロリムス固相抗体1(マウスIgG):Tac5-26-8(自社抗体)
 抗タクロリムス固相抗体2(マウスIgM):FK1(HyTest,カタログ番号:4FK42)
 抗タクロリムス固相抗体3(マウスIgG):FK2(Biorbyt,カタログ番号:orb79532)
 洗浄液:0.5容量% Tween20/PBS
 ブロッキング溶液:1重量% BSA/PBS
 検体:Lyphocheck(登録商標) Whole Blood Immunosuppressant Control(BIO-RAD社) Levels 1,2,3,4および5。本検体では、タクロリムス、シクロスポリンA、およびエベロリムスが全血試料中にスパイクされており、それぞれ種々の測定システムによる測定値が開示されている。本願実施例で採用したHPLC-MSによるタクロリムスの測定値は、4.75ng/mL(Level 1)、8.44ng/mL(Level 3)、15.8ng/mL(Level 4)、23.6ng/mL(Level 5)である。本願実施例で採用したHPLC-MSによるシクロスポリンAの測定値は、87ng/mL(Level 1)、152ng/mL(Level 2)、354ng/mL(Level 3)、755ng/mL(Level 4)、1211ng/mL(Level 5)である。以下の実施例では、検体の濃度および数を、アッセイの目的に応じて適宜変更して用いた。
 抗タクロリムス抗体含有培養上清(検出抗体):抗タクロリムスニワトリIgM含有CHO培養上清(自社抗体)
 培養上清希釈液:1重量% BSA/PBS
 ALP標識抗体:アルカリホスファターゼ標識マウス抗ニワトリIgM mAb(自社抗体)
 標識抗体希釈液:1重量% BSA/TBS
 発光基質液:ルミパルス用発光基質(AMPPD)
 実施例1において、以下の酸性緩衝液を用いた。
 リン酸緩衝液(pH1.0~pH3.0)は、リン酸(HPO)、およびリン酸二水素ナトリウム・二水和物(NaHPO・2HO)水溶液を混合して調製した。
 クエン酸緩衝液(pH3.5)は、クエン酸(C)水溶液、およびクエン酸ナトリウム(Na)水溶液を混合して調製した。
 酢酸緩衝液(pH4.0~pH5.5)は、Biacore用の市販品を使用した。
 リン酸緩衝液(pH6.0~pH8.0)は、リン酸二水素ナトリウム・二水和物(NaHPO・2HO)、およびリン酸水素二ナトリウム・12水和物(NaHPO・12HO)水溶液を混合して調製した。
 ホウ酸緩衝液(pH8.5)は、Biacore用の市販品を使用した。
 すなわち、実施例1で用いた酸性緩衝液のpHおよび種類は、以下のとおりである。

 実施例1は、以下の手順で行った。
(1)抗タクロリムス固相抗体1、抗タクロリムス固相抗体2および抗タクロリムス固相抗体3をそれぞれ3μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5)酸性緩衝液で検体を10倍希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)培養上清希釈液で抗タクロリムス抗体含有培養上清を0.2μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)標識抗体希釈液でALP標識抗体を0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(10)ウェルを洗浄液で3回洗浄した。
(11)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
 その結果、3種類の抗体を用いた実験で共通して、酸性のpH領域において、タクロリムスの濃度に応じた発光カウントが認められた(図1-1~1-3)。このことは、酸性のpH領域において、タクロリムスをその特異的結合タンパク質(FKBP)および非特異的結合タンパク質(アルブミン等)から解離させつつ、解離したタクロリムスを抗体で検出できることを示す。
 以上より、タクロリムスおよびその特異的結合タンパク質を含む血液試料中のタクロリムスを抗体で検出する場合、酸による血液試料の前処理が有効であることが示された。
実施例2:酸処理による全血検体中のタクロリムスの定量ELISA
 実施例2で用いた材料は、以下のとおりである。
 酸性緩衝液:100mM リン酸緩衝液/pH2.0
 検量線物質:タクロリムス(1mg/mL in DMSO)
 検量線希釈液:0.5% Tween20/PBS
 実施例2で用いた他の材料は、実施例1と同様である。
 実施例2は、以下の手順で行った。
(1)抗タクロリムス固相抗体1を3μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5A)リン酸緩衝液(pH2.0)で検体を10倍希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(5B)検量線物質であるタクロリムス(1mg/mL in DMSO)を検量線希釈液で希釈し40,13,4.4,1.5,0.49,0.16,0.050,0ng/mLの希釈系列を調製して1ウェル当たり50μL添加し、37℃10分間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)培養上清希釈液で抗タクロリムス抗体含有培養上清を0.2μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)標識抗体希釈液でALP標識抗体を0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(10)ウェルを洗浄液で3回洗浄した。
(11)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
(12)検量線の線形近似式を用いて、検体測定時のカウントをもとにタクロリムス測定値を算出した。
 その結果、タクロリムスおよびその特異的結合タンパク質および非特異的結合タンパク質を含む血液試料を酸により前処理した場合におけるタクロリムス濃度の測定値は、タクロリムスのスパイク濃度と非常に高い相関性を示した(図2)。
 以上より、タクロリムスおよびその特異的結合タンパク質および非特異的結合タンパク質を含む血液試料を酸により前処理した場合、当該血液試料中のタクロリムスの定量性に優れることが示された。
実施例3:酸処理による全血検体中のシクロスポリンAの検出ELISA
 実施例3で用いた材料は、以下のとおりである。
 抗シクロスポリン固相抗体1(マウスIgG):CS5-12-27(自社抗体)
 酸性緩衝液:10mM 酢酸緩衝液/pH4.0
 抗シクロスポリン抗体含有培養上清:抗シクロスポリンニワトリIgM含有CHO培養上清(自社抗体)
 実施例3で用いた他の材料は、実施例1、2と同様である。
 実施例3は、以下の手順で行った。
(1)抗シクロスポリン固相抗体1を2.5μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5)酸性緩衝液で検体を10倍希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)培養上清希釈液で抗シクロスポリン抗体含有培養上清を0.2μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)標識抗体希釈液でALP標識抗体を0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(10)ウェルを洗浄液で3回洗浄した。
(11)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
 その結果、抗体を用いた実験について、酸性のpH領域において、高い発光カウントが認められた(図3)。このことは、酸性のpH領域において、シクロスポリンAをその特異的結合タンパク質(シクロフィリン)および非特異的結合タンパク質(アルブミン等)から解離させつつ、解離したシクロスポリンAを抗体で検出できることを示す。
 以上より、シクロスポリンAおよびその特異的結合タンパク質を含む血液試料中のシクロスポリンAの検出では、酸による血液試料の前処理が有効であることが示された。
実施例4:酸処理による全血検体中のシクロスポリンAの定量ELISA
 実施例4で用いた材料は、以下のとおりである。
 酸性緩衝液:10mM 酢酸緩衝液/pH4.0
 検量線物質:シクロスポリンA(1mg/mL in DMSO)
 検量線希釈液:0.5% Tween20/PBS
 実施例4で用いた他の材料は、実施例1~3と同様である。
 実施例4は、以下の手順で行った。
(1)抗シクロスポリン固相抗体1を2.5μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5A)酢酸緩衝液(pH4.0)で検体を10倍希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(5B)シクロスポリンA(1mg/mL in DMSO)を検量線希釈液で希釈し200,100,50.0,25.0,12.5,6.25,3.13,0ng/mLの希釈系列を調製して1ウェル当たり50μL添加し、37℃10分間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)培養上清希釈液で抗シクロスポリン抗体含有培養上清を0.3μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)標識抗体希釈液でALP標識抗体を0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(10)ウェルを洗浄液で3回洗浄した。
(11)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
(12)検量線の線形近似式を用いて、検体測定時のカウントをもとにシクロスポリンA測定値を算出した。
 その結果、シクロスポリンAおよびその特異的結合タンパク質および非特異的結合タンパク質を含む血液試料を酸により前処理した場合におけるシクロスポリンA濃度の測定値は、シクロスポリンAのスパイク濃度と非常に高い相関性を示した(図4)。
 以上より、シクロスポリンAおよびその特異的結合タンパク質および非特異的結合タンパク質を含む血液試料を酸により前処理した場合、当該血液試料中のシクロスポリンAの定量性に優れることが示された。
実施例5:酸性溶液中での測定における固相抗体の特異性への影響の確認
 実施例5で用いた材料は、以下のとおりである。
 界面活性剤入り酸性緩衝液:0.5% Tween20/10mM 酢酸緩衝液/pH4.0
 界面活性剤入り中性緩衝液:0.5% Tween20/PBS/pH6.8

 実施例5は、以下の手順で行った。
(A)ビオチン標識タクロリムスとタクロリムス類似化合物混合液の調製
(1)ビオチン標識タクロリムスを界面活性剤入り酸性緩衝液もしくは界面活性剤入り中性緩衝液で10ng/mLになるように希釈した。
(2)(1)で調製したビオチン標識タクロリムス入りの溶液に対して、タクロリムスもしくはタクロリムス類似化合物(1mg/mL in DMSO)を最終濃度で100,000ng/mLから10倍希釈ずつになるように添加して希釈系列を作製した。
(B)阻害アッセイ
(1)抗タクロリムス固相抗体1を3μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5)(A)で調製したビオチン標識タクロリムスとタクロリムス類似化合物の混合液を1ウェル当たり50μL添加し、37℃で1時間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)希釈液でALP標識ストレプトアビジンを0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
 その結果、抗タクロリムス固相抗体およびビオチン標識タクロリムス、ならびにそれらの結合の阻害剤である各種タクロリムス類似化合物を用いた結合阻害実験において、酸性溶液中での結合阻害様式は、中性溶液中のものと同様であった(図5-1および5-2)。このことは、酸性条件が抗体の特異性に影響しないことを示す。
 以上より、抗体の特異性が、酸性溶液中で維持されていることが示された。
実施例6:酸処理における特異的結合タンパク質からのマクロライド系免疫抑制剤の解離の確認
 実施例6で用いた材料は、以下のとおりである。
 FKBP12:リコンビナントFKBP12(Sino Biological Inc,カタログ番号:10268-H08E)
 タクロリムス:タクロリムス(1mg/mL in DMSO)
 タクロリムス・FKBP希釈液1:0.5% Tween20/PBS
 タクロリムス・FKBP希釈液2:0.5% Tween20/200mM リン酸緩衝液/pH2.0
 実施例6で用いた他の材料は、実施例1~5と同様である。
 実施例6は、以下の手順で行った。
(A)タクロリムスとFKBP12の結合
(1)タクロリムス・FKBP希釈液1でタクロリムスを希釈し、240ng/mL,80ng/mL,26.6ng/mL,8.89ng/mL,1.48ng/mL,0.49ng/mL,0.165ng/mL,0の希釈系列を作製した。
(2)タクロリムス・FKBP希釈液1でFKBP12を100μg/mLに希釈した。
(3)希釈したタクロリムスと希釈したFKBP12を等量ずつ混合した(例:最終濃度:タクロリムス120μg/mL+FKBP12 50μg/mL、タクロリムス 40μg/mL+FKBP12 50μg/mL)。
(4)タクロリムスとFKBP12を溶液中において室温で1時間反応させた。
(B)結合させたタクロリムスとFKBP12の解離実験
(1)抗タクロリムス固相抗体1を3μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5)(A)で調製したタクロリムスとFKBP12の混合液をタクロリムス・FKBP希釈液1またはタクロリムス・FKBP希釈液2で10倍希釈した後に1ウェル当たり50μL添加し、37℃で1時間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)培養上清希釈液で抗タクロリムス抗体含有培養上清を0.2μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)標識抗体希釈液でALP標識抗体を0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(10)ウェルを洗浄液で3回洗浄した。
(11)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
 その結果、タクロリムスとFKBP12を混合し、次いで混合液を中性溶液で希釈することにより中性条件下で抗原抗体反応を実施すると、タクロリムス濃度依存的なシグナルは認められなかった(図6における0.5% Tween20/PBSの条件)。一方、タクロリムスとFKBP12を混合し、次いで混合液を酸性溶液で希釈することにより酸性溶液中で抗原抗体反応を実施すると、タクロリムス濃度依存的なシグナルが認められた(図6における0.5% Tween20/200mM PB/pH2.0の条件)。このことは、酸性条件下では、タクロリムスおよびFKBP12の結合を解離させつつ、解離したタクロリムスの濃度を抗体で測定することができることを示す。
 本実施例の結果を実施例1~5の結果と併せて考慮すると、マクロライド系物質(マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物)およびその特異的結合タンパク質を含む血液試料中の免疫抑制剤の濃度を抗体により測定する場合、酸による血液試料の前処理が有効であることが示された。
実施例7:アルカリ処理による全血検体中のタクロリムス検出ELISA
 実施例7で用いた96穴イムノプレート、抗タクロリムス固相抗体1、洗浄液、ブロッキング溶液等の材料は、実施例1~6で用いた材料と同様である。
 実施例7は、以下の手順で行った。
(1)抗タクロリムス固相抗体1を3μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5)タクロリムス濃度既知の4検体について、中性緩衝液(50mM リン酸緩衝液、pH7.0)もしくは塩基性緩衝液(50mM MES、pH10.0)で10倍希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)抗タクロリムスIgM抗体希釈液で抗タクロリムスIgM抗体を0.5μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)標識抗体希釈液でアルカリホスファターゼ標識抗ニワトリIgM抗体を0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(10)ウェルを洗浄液で3回洗浄した。
(11)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
 各条件における発光カウントの結果を図7に示す。全血検体をアルカリ性の緩衝液と混合して処理した場合、中性の緩衝液と混合した場合と比して、得られる発光カウントがタクロリムス濃度に依存して高く検出された。
 以上より、タクロリムスを含む血液試料をアルカリにより前処理した場合、当該血液試料中のタクロリムスを高感度で測定できることが示された。
実施例8:酸処理および中和処理による全血検体中のタクロリムス検出ELISA
 実施例8で用いた材料は、以下のとおりである。
 酸性緩衝液:50mM リン酸緩衝液/pH2.0
 中性緩衝液:200mM リン酸緩衝液/pH7.0
 中和液1:8M 尿素/200mM リン酸緩衝液/pH8.0
 中和液2:4M グアニジン塩酸塩/200mM リン酸緩衝液/pH8.0
 実施例8で用いた他の材料は、実施例1~7と同様である。
 実施例8は、以下の手順で行った。
(1)抗タクロリムス固相抗体1を3μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5)検体6μLに対し、酸性緩衝液24μLを添加して反応させたのち、中和液1または中和液2を30μL添加して前処理済検体を中和した。比較のため、中性緩衝液で検体を5倍希釈した後、8M尿素または4Mグアニジン塩酸塩を含む中性緩衝液(pH7.0)で2倍に希釈したもの、検体を中性緩衝液で10倍に希釈したものも用意した。
(6)処理済みのサンプルを1ウェル当たり50μL添加し、37℃で10分間反応させた。
(7)ウェルを洗浄液で3回洗浄した。
(8)抗タクロリムスIgM抗体希釈液で抗タクロリムスIgM抗体を0.5μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(9)ウェルを洗浄液で3回洗浄した。
(10)標識抗体希釈液でアルカリホスファターゼ標識抗ニワトリIgM抗体を0.1μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(11)ウェルを洗浄液で3回洗浄した。
(12)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
 各条件における発光カウントの結果を図8に示す。検体を酸処理後、カオトロピック変性剤(特にグアニジン塩酸塩)を含有する中和液で中和した場合、中性緩衝液で希釈のみを行った場合と比して、カウントが顕著に上昇することが示された。一方で、酸による処理を行わず、カオトロピック変性剤のみを添加した場合も、カウント上昇が見られた。
実施例9:酸処理および中和処理による全血検体中のエベロリムス検出ELISA
 実施例9で使用された材料は以下の通りである。
 コンジュゲート固相:エベロリムス-BSAコンジュゲート(自社抗体)
 抗エベロリムス抗体(ウサギ ポリクローナル抗体):EVER抗体液1(エベロリムスキット ナノピア TDM エベロリムス(積水メディカル社製))
 洗浄液:ルミパルス洗浄液
 検体:ClinCal(登録商標)Whole Blood Calibrator Set Lyophilised (RECIPE)。本願実施例で採用したHPLC-MSによるエベロリムスの測定値は、0ng/mL(Level 0)、1.45ng/mL(Level 1)、2.86ng/mL(Level 2)、5.27ng/mL(Level 3)、11.7ng/mL(Level 4)、23.2ng/mL(Level 5)、48.0ng/mL(Level 6)である。 
 ALP標識抗体:アルカリホスファターゼ標識ブタ抗ウサギIgポリクローナル抗体(DAKO,カタログ番号:D0306)
 抗体希釈液:0.5容量% Tween20/PBS
 実施例9で用いた他の材料は、実施例1~8と同様である。
 実施例9は、以下の手順で行った。
(1)エベロリムス-BSAコンジュゲートを5μg/mLになるようにPBSで希釈し、96穴イムノプレートに1ウェル当たり50μL添加し、37℃で1時間反応させた。
(2)ウェルを洗浄液で3回洗浄した。
(3)ブロッキング溶液を1ウェル当たり150μL添加し、4℃で一晩反応させた。
(4)ウェルを洗浄液で3回洗浄した。
(5)6μLの検体に48μLの酸性緩衝液を反応させ、18μLの中和液2または酸性緩衝液を添加し、前処理液を調製した。この前処理液と5倍希釈した抗体溶液を体積比1:1で混合し、エベロリムス-BSAコンジュゲートを固相化した96穴イムノプレートに1ウェル当たり50μL添加し、37℃で10分間反応させた。
(6)ウェルを洗浄液で3回洗浄した。
(7)抗体希釈液でALP標識抗体を0.2μg/mLになるように希釈して1ウェル当たり50μL添加し、37℃で10分間反応させた。
(8)ウェルを洗浄液で3回洗浄した。
(9)発光基質を1ウェル当たり50μL添加し、室温で5分間反応させた後、発光を検出した。
 各条件における発光カウントの結果を図9に示す。全血検体を酸で処理した後、中和を行わなかった検体については、酸により抗エベロリムス抗体が失活したためか、発光カウントが得られなかった(データ示さず)。全血検体をpH2.0の酸性緩衝液処理後、クアニジン含有の中和液で中和した前処理液は、酸性緩衝液に代えて中性緩衝液を加えた検体よりも高感度でエベロリムスを検出できた。このことは、酸性緩衝液は効率よく全血検体からエベロリムスを抽出することを示す。
 以上より、エベロリムスおよびその特異的結合タンパク質を含む血液試料中のエベロリムスを抗体で検出する場合、酸による血液試料の前処理が有効であることが示された。また、酸に対する耐性の低い抗体を使用する場合においても、検体の酸処理の後、抗原抗体反応の前に中和工程を行うことで、効率的なエベロリムス検出が可能であることが示された。
 本発明は、例えば、マクロライド系免疫抑制剤のTDM(Therapeutic Drug Monitoring)に有用である。

Claims (13)

  1.  以下を含む、マクロライド系免疫抑制剤についての血液検査方法:
    (a)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物を含む血液試料を酸またはアルカリで処理すること;ならびに
    (b)抗体を用いて、血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
  2.  マクロライド系免疫抑制剤が、タクロリムス、シクロスポリンA、またはエベロリムスである、請求項1記載の方法。
  3.  血液試料がヒト血液試料である、請求項1または2記載の方法。
  4.  前記血液試料の処理が、酸による処理である、請求項1~3のいずれか一項記載の方法。
  5.  前記酸による処理が、前記血液試料と酸性緩衝液との混合により行われる、請求項4記載の方法。
  6.  酸性緩衝液のpHが1.0~5.0である、請求項5記載の方法。
  7.  免疫抑制剤に対する抗体が、IgG、IgM、またはそれらの抗体断片である、請求項1~6のいずれか一項記載の方法。
  8.  以下を含む方法である、請求項1~7のいずれか一項記載の方法:
    (a’)前記マクロライド系免疫抑制剤、または前記代謝物を含む血液試料を酸またはアルカリで処理して、酸性血液試料またはアルカリ性血液試料を調製すること;ならびに
    (b’)抗体を用いて、前記酸性血液試料または前記アルカリ性血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
  9.  以下を含む方法である、請求項1~7のいずれか一項記載の方法:
    (a’’)前記マクロライド系免疫抑制剤、または前記代謝物を含む血液試料を酸またはアルカリで処理して酸性血液試料またはアルカリ性血液試料を調製した後、中和を行い、中性血液試料を調製すること;ならびに
    (b’’)抗体を用いて、前記中性血液試料中の前記マクロライド系免疫抑制剤、または前記代謝物の濃度を測定すること。
  10.  前記中和が、カオトロピック変性剤を含む中和液を前記酸性血液試料またはアルカリ性血液試料と混合して行われる、請求項9記載の方法。
  11.  (1)酸またはアルカリ、および(2)マクロライド系免疫抑制剤、またはマクロライド構造を有するその代謝物に対する1種以上の抗体を含む、マクロライド系免疫抑制剤についての血液検査試薬。
  12.  酸が酸性緩衝液である、請求項11記載の試薬。
  13.  さらに(3)中和液を含む、請求項11または12記載の試薬。
