Definitions
- the present invention relates to an embolic agent and a treatment system using the embolic agent.
- aortic diseases aortic aneurysm, aortic dissection, etc.
- artificial blood vessel replacement has been performed to replace the blood vessel in the affected area with an artificial blood vessel.
- this surgical method involves thoracotomy and abdominal opening, there is a problem that the burden (invasion) on the patient is large and hospitalization is prolonged.
- treatment using a stent graft stent graft interpolation
- stent graft interpolation has become widespread instead of this artificial blood vessel replacement surgery.
- the stent graft is stored in a thin catheter and advanced to the site with the aneurysm, and the stored stent graft is released and expanded from the catheter and placed in the site with the aneurysm or dissection (affected area). do.
- This stent-graft interpolation is a minimally invasive treatment and has the advantage of a good postoperative course for the patient.
- the above-mentioned stent graft interpolation may cause blood leakage (end leak).
- Endoleak Type II is a symptom that blood flows into the aneurysm from the inferior mesenteric artery or lumbar artery (branch blood vessel) due to a decrease in pressure inside the aneurysm, causing the aneurysm to expand. May cause.
- the biocompatible fluid composition is introduced into the blood vessel that causes endoleak as an embolic substance via a catheter and becomes the causative blood vessel.
- a method for forming an embolus is known (Japanese Patent Publication No. 2002-53853 (corresponding to US 6,303,100 B1)).
- the biocompatible fluid composition described in JP-A-2002-53853 contains a contrast medium so that the embolization process can be visually recognized (Col of paragraph "0015" (US 6,303,100 B1). . 4, Equivalent to lines 1-11)).
- the contrast medium interferes with the visibility, and the visibility of the contrast medium introduced separately for confirming the blood flow is impaired, and there is a problem that accurate image diagnosis is difficult or impossible. ..
- an object of the present invention is to provide a technique capable of ensuring good visibility when an embolic agent is introduced and reducing the visibility after the introduction.
- the present inventors conducted diligent research in order to solve the above problems. As a result, they have found that the above problems can be solved by appropriately controlling the swelling rate of the embolic agent and the visibility after swelling (CT value after swelling, which is an index of contrast), and completed the present invention.
- CT value after swelling which is an index of contrast
- the above object is an embolic agent having a hydrogel containing an ethylene-based unsaturated monomer, a cross-linking agent, a reaction product of a bifunctional macromer if necessary, and a visualization agent, and the swelling ratio of the embolic agent is high. It is achieved by an embolic agent that is 5 to 300 times, has a post-swelling CT value of the embolic agent of 50 to 300 HU, and is configured to be lower than the pre-swelling CT value of the embolic agent.
- FIG. 1 is a diagram illustrating a method of using an embolic agent according to an embodiment of the present invention.
- FIG. 1 is a diagram illustrating a method of using an embolic agent according to an embodiment of the present invention.
- FIG. 1 is a diagram illustrating a method of using an embolic agent according to an embodiment of the present invention.
- FIG. 1 is a diagram illustrating a method of using an embolic agent according to an embodiment of the present invention.
- FIG. 2 is a diagram illustrating a method of using an embolic agent according to another embodiment of the present invention.
- FIG. 2 is a diagram illustrating a method of using an embolic agent according to another embodiment of the present invention.
- FIG. 2 is a diagram illustrating a method of using an embolic agent according to another embodiment of the present invention.
- the first aspect of the present invention is an embolic agent having a reaction product of an ethylene-based unsaturated monomer and a cross-linking agent and a hydrogel containing a visualization agent, and the swelling ratio of the embolic agent is 5 to 300 times.
- the present invention relates to an embolic agent, wherein the post-swelling CT value of the embolic agent is 50 to 300 HU, and is configured to be lower than the pre-swelling CT value of the embolic agent. According to the embolic agent according to the first aspect of the present invention, good visibility can be ensured when the embolic agent is introduced, and the visibility can be lowered after the introduction.
- the second aspect of the present invention is an embolic agent having a hydrogel containing a bifunctional macromer, an ethylene-based unsaturated monomer, a cross-linking agent, and a visualization agent, and the swelling ratio of the embolic agent is 5.
- the present invention relates to an embolic agent, which is ⁇ 300 times, has a post-swelling CT value of the embolic agent of 50 to 300 HU, and is configured to be lower than the pre-swelling CT value of the embolic agent. According to the embolic agent according to the second aspect of the present invention, good visibility can be ensured when the embolic agent is introduced, and the visibility can be lowered after the introduction.
- the first and second aspects of the present invention have a hydrogel containing an ethylene-based unsaturated monomer, a cross-linking agent, a reaction product of a bifunctional macromer, if necessary, and a visualization agent (preferably a hydrogel).
- An embolic agent (composed of a filament), the embolic agent has a swelling ratio of 5 to 300 times, the post-swelling CT value of the embolic agent is 50 to 300 HU, and the pre-swelling CT of the embolic agent.
- embolic agents which are configured to be lower than the value.
- the embolic agent of the present invention can be visually recognized well when the embolic agent is introduced, and the visibility is reduced after the embolic agent is introduced into a predetermined site. Therefore, for example, when an embolic agent is introduced into an aneurysm, the position of the embolic agent can be satisfactorily confirmed until the embolic agent is placed at a predetermined site. On the other hand, after the embolic agent is placed at a predetermined site, its visibility is reduced.
- the embolic agent of the present invention has a high swelling ratio of 5 times or more.
- the content (density) of the visualization agent after swelling per unit volume is significantly lower than the content (density) of the visualization agent before swelling per unit volume. Since the embolic agent does not swell until the embolic agent is placed in a predetermined site, the content of the visualization agent in the embolic agent per unit volume is sufficiently high, so that the position of the embolic agent is appropriate such as X-ray. It can be confirmed well by means. On the other hand, after the embolic agent is placed at a predetermined site, the embolic agent comes into contact with a body fluid (for example, blood) and swells greatly.
- a body fluid for example, blood
- the content (density) of the visualization agent per unit volume is greatly reduced, and the visibility by an appropriate means such as X-ray is greatly reduced. Therefore, the embolic agent causes little or no confirmation of the influx of contrast medium into the aneurysm through the blood vessel that causes the endoleak (eg, inferior mesenteric artery, lumbar artery, etc.). Therefore, by using the embolic agent of the present invention, the presence or absence of endoleak, particularly endoleak type II, can be effectively confirmed.
- endoleak eg, inferior mesenteric artery, lumbar artery, etc.
- the embolic agent of the present invention is placed in the catheter lumen, and is placed in a desired site (for example, an aneurysm) from the catheter lumen by a pusher. Therefore, the third aspect of the present invention includes an embolic agent according to the first or second aspect, a catheter having a lumen and the lumen filled with the embolic agent, and the embolic agent.
- the present invention relates to an embolic agent kit having a pusher for pushing out from the lumen of the catheter.
- a fourth aspect of the present invention relates to a therapeutic system having an embolic agent according to the first or second aspect or an embolic agent kit and graft (particularly a stent graft) according to the third aspect.
- X to Y indicating a range includes X and Y and means “X or more and Y or less”.
- X and / or Y means to include at least one of X and Y, and includes "X alone", “Y alone” and “combination of X and Y”. Unless otherwise specified, the operation and physical properties are measured under the conditions of room temperature (20 to 25 ° C.) / relative humidity of 40 to 60% RH.
- the term “(meth) acrylic” includes both acrylic and methacryl.
- the term “(meth) acrylic acid” includes both acrylic acid and methacrylic acid.
- the term “(meth) acryloyl” includes both acryloyl and methacryloyl.
- the term “(meth) acryloyl group” includes both acryloyl and methacryloyl groups.
- the embolic agent of the present invention has a hydrogel containing an ethylene-based unsaturated monomer, a cross-linking agent, a reaction product of a bifunctional macromer if necessary, and a visualization agent, and has a swelling ratio of 5 to 300 times.
- the embolic agent has a post-swelling CT value of 50 to 300 HU, and is configured to be lower than the pre-swelling CT value.
- the embolic agent is composed of a hydrogel filament containing an ethylene unsaturated monomer, a cross-linking agent, a reaction product of a bifunctional macromer, if necessary, and a visualization agent. That is, in one embodiment of the present invention, the embolic agent is composed of only the above-mentioned specific hydrogel filament and does not include other members, for example, a support member (metal support member). In particular, since the embolic agent of the present invention does not contain a metal support member, for example, when the embolic agent is observed by an appropriate means such as X-ray, metal-derived halation (artifact, flare or reflection) does not occur. , The position of the embolic agent can be confirmed more clearly and accurately.
- the hydrogel according to the present invention may have any form.
- filament (fiber) shape, particle shape, sheet shape, strip shape and the like can be mentioned.
- the embolic agent can be made into particles by appropriately cutting the hydrogel filament as described above.
- a filled catheter described later can be obtained.
- a sheet-shaped or strip-shaped hydrogel can be obtained.
- filaments (fibers) are formed from the viewpoints of ease of placement in a predetermined site (for example, aneurysm), swelling after placement in a predetermined site (for example, aneurysm), and reduction of the risk of distal embolus. preferable. That is, in the preferred embodiment of the present invention, the hydrogel is in the form of filaments (fibers).

- “the embolic agent is substantially composed of hydrogel” means that the content of the hydrogel in the embolic agent is 95% by weight or more, preferably 98% by weight or more (upper limit:). 100% by weight).
- the hydrogel according to the present invention swells on contact with an aqueous liquid under physiological conditions.
- physiological condition means a condition having at least one environmental property in or on the surface of a mammal (eg, human). Such properties include an isotonic environment, a pH buffering environment, an aqueous environment, a near-neutral (about 7) pH, temperature, or a combination thereof.
- aqueous liquid includes, for example, isotonic fluid, water; body fluid of a mammal (for example, human) such as blood, cerebrospinal fluid, plasma, serum, glass body fluid, and urine.
- the embolic agent of the present invention has a swelling ratio of 5 to 300 times.
- the swelling ratio is less than 5 times, the density of the visualization agent in the embolic agent does not become sufficiently low after swelling, and the visibility by an appropriate means such as X-ray after the embolic agent is placed in the aneurysm is improved. It cannot be lowered sufficiently.
- the degree of swelling of the embolic agent is low, and it is necessary to insert the embolic agent (for example, hydrogel filament) many times to fill the space between the aneurysm and the stent graft, which takes time and effort.
- the swelling ratio of the embolic agent is preferably 50 to 250 times from the viewpoint of further reducing the visibility by an appropriate means such as X-ray after placing the embolic agent in the aneurysm and reducing the strength of the embolic agent. , 80 to 200 times is more preferable, 100 to 200 times is even more preferable, and 100 to 180 times is particularly preferable.
- the "swelling ratio" is a value measured by the following method.
- the embolic agent of the present invention shows a CT value after swelling of 50 to 300 HU.
- the CT value after swelling is less than 50 HU, it becomes difficult to distinguish it from blood or blood vessel wall when swelling after indwelling the embolic agent.
- the CT value of the blood vessel depicted with the contrast medium is generally 200 to 300 HU. Therefore, when the CT value after swelling exceeds 300 HU, it is difficult to distinguish between the contrast-enhanced blood vessel and the embolic agent, and the aneurysm is mediated by a blood vessel (for example, inferior mesenteric artery, lumbar artery, etc.) that causes endoleak.
- a blood vessel for example, inferior mesenteric artery, lumbar artery, etc.
- the CT value after swelling of the embolic agent is more preferably 55 to 200 HU, more preferably 65 to 180 HU. , 80 HU or more and less than 135 HU is particularly preferable. That is, in a preferred embodiment of the present invention, the embolic agent has a swelling ratio of 100 to 200 times and a post-swelling CT value of 65 to 180 HU.
- the "post-swelling CT value” is an index of visibility (contrast-enhancedness) under X-ray fluoroscopy after swelling, and is an image density value (unit: HU (Hounsfield Unit)) measured by the following method. ), And the CT values of water and air are represented by relative values of 0 HU and -1000 HU, respectively.
- An embolic agent eg, hydrogel filament 8 cm is dried under reduced pressure for a sufficient time (eg, 48 hours) to obtain a dehydrated sample.
- the dehydrated sample is immersed in a sufficient amount (for example, 1000 mL or more) of PBS (pH 7.4) for 24 hours or more to swell the dehydrated sample to obtain a swollen sample.
- a 50 mL centrifuge tube containing an embolic agent (hydrogel filament) is laid on the bed of a medical whole-body X-ray CT diagnostic device (TSX-021B, manufactured by Canon Medical Systems Corporation), and the embolic agent is applied.
- TSX-021B medical whole-body X-ray CT diagnostic device
- the CT data is read into analysis software (OsiriX MD, version 6.0, 64 bit, Pixmeo, SARL), and the central slice in the long axis direction of the 50 mL centrifuge tube is displayed.
- a circular region of interest (ROI) is set on the cross section of the embolic agent. After the ROI setting is completed, the CT value of each ROI is recorded, and this is used as the CT value after swelling.
- ROI circular region of interest
- the embolic agent of the present invention preferably shows a pre-swelling CT value of 100 HU or more.
- the pre-swelling CT value is more preferably 200 HU or more, and particularly preferably more than 300 HU.
- the visibility becomes the same as that of the contrast medium at the time of angiography by X-ray, so that the operator can more easily see the embolic agent of the present invention under fluoroscopy.
- the upper limit of the CT value before swelling is preferably about 5000 HU from the viewpoint of ease of control of the CT value after swelling.
- the "pre-swelling CT value” is an index of visibility (contrast-enhancedness) under X-ray fluoroscopy before swelling, and is an image density value (unit: HU (Hounsfield Unit)) measured by the following method. ), And the CT values of water and air are represented by relative values of 0 HU and -1000 HU, respectively.

It can be appropriately selected depending on the thickness of bifurcated blood vessels such as arteries.
- the diameter of the hydrogel filament before swelling (dry state) is about 0.1 to 5 mm, preferably about 0.2 to 3 mm.
- the length of the hydrogel filament before swelling (dry state) is about 0.5 to 200 cm, preferably about 10 to 60 cm.
- the term "before swelling (dry state)" means a state after the embolic agent (for example, a hydrogel filament has been dried under reduced pressure for a sufficient time (for example, 24 hours).
- the reactant constituting the hydrogel filament is a reactant of an ethylene-based unsaturated monomer, a cross-linking agent, and, if necessary, a bifunctional macromer. That is, the reactant constituting the hydrogel filament is a reaction product of an ethylene-based unsaturated monomer and a cross-linking agent (first aspect) or a reaction product of a bifunctional macromer, an ethylene-based unsaturated monomer and a cross-linking agent. (Second aspect).

Therefore, the embolic agent can be sufficiently visually recognized by an appropriate means such as X-rays until the embolic agent is placed in a predetermined site, but after the embolic agent is indwelled and swollen, the embolic agent is used by an appropriate means such as X-rays. Visibility is reduced.

- the embolic agents 2 to 5, 7 to 18 and 21 of the present invention have a swelling ratio in the range of 5 to 300 times and a post-swelling CT value in the range of 50 to 300 HU. Moreover, it is significantly lower than the CT value before swelling. For this reason, when the embolic agent is inserted into the aneurysm, it comes into contact with body fluid such as blood and swells sufficiently to reduce its visibility, and the embolic agent indwelled in the aneurysm is placed in the aneurysm after the embolic agent is placed. It can be seen that it is possible to distinguish between the contrast medium flowing into the aneurysm and living tissues such as blood and blood vessel walls.
- the embolic agents 1, 19 and 20 of the comparative examples have a CT value (CT value after swelling) exceeding 300 HU even after swelling. It is presumed that it is difficult to distinguish it from the contrast medium that flows into the aneurysm after indwelling. Further, since the embolic agents 6 and 22 of the comparative example have a CT value of less than 50 HU after swelling, the embolic agent placed in the aneurysm and swollen can be distinguished from living tissues such as blood and blood vessel wall. It is presumed to be difficult.
- the pre-swelling CT values of the embolic agents 6 and 22 of the comparative example are negative, the visibility before the swelling is remarkably low, and it is presumed that it is difficult to confirm the position of the embolic agent at the time of insertion. Will be done.
- the embolic agent of the present invention since the embolic agent of the present invention has a sufficiently high pre-swelling CT value, it exhibits sufficient visibility before being inserted into the aneurysm (contact with body fluid such as blood), and the embolic agent at the time of insertion The position can be easily confirmed.
- the embolic agent of the present invention can easily confirm the position of the embolic agent at the time of insertion and clearly confirm the presence of the contrast agent after placement.

Abstract
