Definitions
- SUBSTANCE group of inventions relates to medicine, namely, to surgical operations under the control of X-ray computed tomography (XCT) and means for positioning and orienting surgical instruments, for example, in radiation diagnostics.
- XCT X-ray computed tomography
- the problem is a change in the spatial arrangement of a medical instrument inserted into the human body due to the movement of the human body, organs and tissues during a surgical operation (due to respiratory excursions of various depths, contraction of skeletal muscles, changes in posture, peristalsis of internal organs). Changes in the spatial position of the instrument reduce the accuracy of the operation, require additional control, increase the radiation exposure and operation time, increase the risk of complications, worsen the results of surgical operations, and require additional equipment for control.
- X-ray real-time control tools that are not standard for a CT scanner (X-ray television fluoroscopic devices) have a lower resolution and worse visualization, which also does not allow you to see the picture in the operation area in detail, as in a CT scan. It is required to pair the data output by additional control tools to the user interfaces, which requires their rapid analysis and interpretation by a person (within a few seconds) for making a decision and subsequent correction of the position of the instrument and the human body.
- This pairing at the present stage still needs to develop algorithms for the analysis of medical images, improve the hardware and element base of the medical equipment used, in the high computing power of workstations, in the use of artificial intelligence to help a person with quick pairing, analysis, interpretation of data received during the operation, which is an unresolved problem at the present stage of development of medicine and technology.
- the so-called real-time control is conditional and means only control with a higher scan rate and a higher frame rate of the resulting medical imaging images.
- Real-time controls based on other physical principles have all the same disadvantages that are characteristic of X-rays. Their use is limited to even more heterogeneous data entering the user interface. At the present stage, there is no possibility of fast and direct interaction when analyzing data coming from such tools, due to different manufacturers, property rights, interaction protocols, etc.
- Some monitoring tools do not provide information about the pathological focus, internal organs, reflecting only the spatial relationship between their sensors (electromagnetic monitoring tools) or the position of the instrument on the surface of the human body (optical navigation tools).
- Known passive means including stereotaxic control devices, perform the functions of marking and holding a medical instrument in the required spatial position, but are unable to control displacement during respiratory and other excursions of the human body.
- a group of inventions is known for patient-specific positioning and orientation of surgical instruments when placing passages, in which the method for positioning medical instruments for invasive surgery contains the steps at which the position of the passage for the medical instrument is determined, data of a particular patient is collected, and the optimal position for placement of the orienting device is selected in the form stereotaxic holder of a medical instrument, including a trajectory indicator mounted in arcuate brackets fixed in an annular base with the possibility of rotation in two mutually perpendicular to vertical planes (RU 2 662 872 C2, IPC A61V 34/20 (2016.01), A61V 90/11 (2016.01)).
- Known x-ray contrast mesh for marking the surgical field which contains a substrate of radiolucent material, x-ray contrast threads located on the substrate, forming a rectangular grid with rectangular cells, the side size of which is from 20 to 50 mm.
- An adhesive base was applied along the perimeter of the substrate on one side to fix it on the patient's skin (RU 152847 U1, Published 06/20/2015 Bull. No. 17).
- a stereotaxic holder of a medical instrument including a trajectory indicator mounted in arcuate brackets fixed in an annular base with the possibility of rotation in two mutually perpendicular vertical planes (description of the patent RU 2569720, IPC A61B 19/00 (2006.01), published: 27.11.2015) .
- the objective of the invention is the development of control and navigation tools for performing operations under the control of CT in medicine, radiology, surgery, medical equipment.
- EFFECT improved positioning accuracy of a medical instrument under conditions of various movements of the human body during surgery by visual and X-ray CT monitoring of the displacement of tissues, organs and parts of the human body during surgery, the possibility of accurately measuring the displacement and timely correction of respiratory movements during the operation.
- the optimal position is selected for placing the orienting device in the form of a stereotaxic holder of a medical instrument, including a trajectory indicator mounted in arc-shaped brackets fixed in an annular base with the possibility of rotation in two mutually perpendicular vertical planes, which at the stage of data collection determine the amount of displacement of the patient's body surface during breathing and the corresponding amplitude of respiratory movements, placing on the table under the patient’s body is an X-ray contrast linear marker in the axial plane of the operation object, and on the surface of the patient’s body in the area of the intended operation, an X-ray contrast coordinate grid is placed in the form of a rectangular frame with transverse bridges forming internal rectangular cells, and two rows of external rectangular cells located on the transverse sides frames, while placing the grid so that the jumpers of the inner cells are oriented
- X-ray contrast coordinate grid for marking the surgical field in the form of a rectangular frame with transverse jumpers forming internal rectangular cells
- a number of external rectangular cells are placed on each of the transverse sides of the frame.
- the stereotaxic holder of a medical instrument including a trajectory indicator mounted in arc-shaped brackets fixed in an annular base with the possibility of rotation in two mutually perpendicular vertical planes, is additionally provided with four pairs of X-ray contrast marks in the form of parallel protrusions, placed on the base on both sides of the bracket mounts.
- Figure 1 shows schematically the stages of choosing a location for positioning a medical instrument for invasive surgery; figure 6 - x-ray contrast grid (RCS); in fig. 7 - stereotaxic holder; Fig.8 - a pair of X-ray contrast marks stereotaxic holder.
- RCS x-ray contrast grid
- Marking tool RCS for marking the surgical field (figure 6) is a rectangular frame with transverse parallel jumpers forming the inner rectangular cells. On each of the transverse sides of the frame there is a number of external rectangular cells, the walls of which are perpendicular to the transverse bridges and form external protrusions.
- the elasticity and rigidity of the structural elements of the RCS marking tool can be different.
- the distance between the outer ledges, between the inner jumpers can be different.
- the distance between the protrusions on opposite sides of the frame can be different.
- RKS can be calibrated with different divisions.
- Stereotactic holder (figure 7) has four pairs of x-ray contrast marks in the form of parallel protrusions (figure 8) placed on the base on both sides of the bracket mounts in the area of attachment of the upper and lower segments to the base.
- the protrusions are designed to be combined with the corresponding structural elements of the RCS and RLM.
- the protrusions can be located on the outer and (or) on the inner the circumference of the base of the stereotaxic holder may have different contrast under the conditions of fluoroscopy, X-ray computed tomography, magnetic resonance imaging.
- the stereotaxic holder can be equipped with means of fixation to the underlying surface (adhesive base, bandage, plaster, sewing holes, etc.)
- the RLM marking tool can have graduations with different scale intervals.
- the method for positioning a medical instrument for invasive surgery includes the following steps.
- an X-ray contrast linear marker (RLM) is placed in the projection of the area of the proposed intervention.
- the correspondence of the position of the RLM marking tool to the axial plane is controlled by the centering laser of the CT scanner in the axial plane (Fig. 1).
- the RLM marking tool may have graduations.
- the number of the required section is determined, in which the object of the operation is located and the place of insertion of the instrument on the surface of the human body.
- the number of the slice is identified, in which the nearest (in relation to the number of the slice of the operation object and the place of insertion of the medical instrument into the human body) the RLM marking tool is located. In the case of placing a single RLM markup, the slice corresponding to it is selected.
- the CT scanner table together with the patient located on it, is placed in a position in which the RLM coincides with the centering laser of the CT scanner (Fig. 1.). 5th.
- the RCS On the surface of the patient's body in the area of the proposed operation, the RCS is located, so that the internal transverse bridges are oriented in the sagittal plane, the external projections in the axial one.
- the slice of the selected (or the only) RLM marking tool together with the axial line of the centering laser, must be located inside the array of slices that include the RCS.
- RKS covers RLM (Fig. 2).
- the patient is asked to inhale and exhale several times, and the displacement of the outer protrusions of the RCM marking tool is assessed in relation to the line created by the laser illumination of the diagnostic equipment and in relation to the RLM marking tool located under the patient's body on the table of the diagnostic equipment.
- the displacement is recorded by visual inspection with the naked eye of a person.
- the displacements can be estimated approximately by counting the outer protrusions of the marking tool RKS located above or below in relation to the aligned with each other marking tool RLM and the axial line of the centering laser.
- the displacement can be estimated more precisely in mm using appropriate measuring instruments with scale divisions in mm (Fig. 2, Fig. 3, Fig. 6 and Fig. 7).
- the first intraoperative CT examination is performed in the volume limited by the operation area with the obligatory inclusion of the RLM, with taking into account their displacement amplitude.
- the value of the displacement of the patient's body surface during breathing is estimated by the displacement of the jumpers of the outer cells in relation to the RLM linear marker on the table and in relation to the line created by the laser illumination of the diagnostic equipment; and sagittal planes, marking the place of insertion of the medical instrument, after which a stereotaxic holder is installed on the latter,
- the data obtained as a result of the 7th stage are subjected to multiplanar reconstruction by means of a CT scanner.
- An axial slice containing the insertion site of the instrument is selected.
- the position of this slice can be obtained by moving the table to the desired slice number, followed by illumination by the axial line of the centering laser, or by counting in millimeters from the upper or lower edges of the frame of the PKC marking tool.
- a marking line is applied on the skin with a marker in the axial plane, passing through the selected axial slice.
- a sagittal slice containing the insertion site of the instrument is selected.
- the position of this cut can be obtained by reading the nearest internal bridge of the RCM marking tool or by reading in millimeters from the left or right edges of the RCC marking tool frame.
- a marking line is applied on the skin with a marker in the sagittal plane, passing through the selected sagittal section.
- the RCS can be removed from the surface of the human body, according to indications, it can be left in the position as at the 5th stage.
- a stereotaxic holder is installed at the marked insertion site of the instrument on the skin, so that the protrusions of the base of the stereotaxic holder coincide with the axial and sagittal marking lines.
- the stereotaxic holder is fixed on the patient's skin, adjusted to the desired angle.
- the introduction of a medical instrument into the human body is carried out with the control of the introduction by step-by-step actions at the 13th stage or simultaneously with additional control by means of real-time control.
- the displacement of the protrusions of the base of the stereotaxic holder and the amplitude of respiratory movements are controlled as at the 13th stage. If the CT scanner table together with the patient was extended in any direction (for example, for more space and freedom of action for the operator) and the projection of the axial line of the centering laser does not coincide with the projection of the RLM marking tool, then the displacement of the protrusions of the base of the stereotaxic holder is visually assessed by in relation to the current position of the axial line of the centering laser (Fig. 4 and Fig. 5).
- a control CT scan is performed, after which the medical instrument and stereotaxic holder are removed, and the operation is completed.
