Die Friedreich's Ataxie (FRDA) ist eine autosomal-rezessiv vererbte, progressive neurodegenerative Erkrankung, die mit einer Inzidenz von rund 1:50.000 auftritt. Es handelt sich dabei um die am häufigsten vorkommende Ataxie beim Menschen und sie manifestiert sich klinisch initial als spinozerebelläre Ataxie, im weiteren Krankheitsverlauf tritt meist eine hypertrophe Kardiomyopathie hinzu, deren Komplikationen häufig für den frühen Tod der Patienten verantwortlich sind. 1996 wurde das verantwortliche Gen identifiziert, wobei man eine GAA Repeat-Expansion im betroffenen Gen für Frataxin fand (1). 95% der Patienten mit FRDA sind homozygot für eine große Expansion von GAA Repeats (60-1800 Repeats) welche innerhalb des ersten Introns auf dem X25 Gen lokalisiert ist und das Protein Frataxin codiert. Die Expansion verursacht eine starke Verminderung des Gehalts an Frataxin, ein aus 210-Aminosäuren aufgebautes Protein, welches in der mitochondrialen Matrix lokalisiert wurde. Die Schwere der Erkrankung ist direkt korreliert mit der Länge der Expansion. Eine kleine Gruppe der Patienten ist heterozygot für die GAA Repeat-Expansion bzw. für eine Punktmutation innerhalb des X25 Genes. Inzwischen konnte in mehreren Studien gezeigt werden, dass es sich bei Frataxin um ein mitochondrial lokalisiertes Protein handelt und dass ein Funktionsverlust bzw. eine zu geringe Expression sekundär zu einer mitochondrialen Eisenakkumulation und Depletierung des Cytosols führt. In Herzmuskelbiopsien von FRDA-Patienten konnte ein Defekt in Fe/S-Gruppen enthaltenden mitochondrialen Enzymkomplexen nachgewiesen werden. Betroffen sind Komplex I, II, und III der Atmungskette sowie das Enzym Aconitase des mitochondrial lokalisierten Citratzyklus (2,3).
Die Funktion des Proteins Frataxin, dessen Gendefekt als Ursache für die Entstehung der Friedreichs Ataxie gefunden wurde ist bislang noch nicht vollständig aufgeklärt, und mehrere Hypothesen werden diskutiert. Eine dieser Hypothesen erklärt die Akkumulation von Eisen in der mitochondrialen Matrix damit, dass Frataxin eine Rolle in der Regulation des Eisenstoffwechsels spielen könnte, möglicherweise im mitochondrialen Eisentransport. Die Eisenakkumulation könnte zur Produktion von Sauerstoffradikalen in den Mitochondrien führen, die wiederum sekundär zur Zerstörung von bekanntlich gegenüber Superoxidanionen sehr empfindlichen FeS-Gruppen in verschieden Enzymkomplexen führen (4,5,6). Eine zweite Hypothese erklärt den biochemischen Phänotyp damit, dass Frataxin einen direkten oder indirekten Einfluß auf die Bildung oder Detoxifikation von Radikalen in den Mitochondrien hat und es sekundär zu einer Störung der Eisenhomöostase mit einhergehender Zerstörung von FeS-Clustem in Enzymkomplexen kommt. Dies wird vor allem auch durch den klinisch nicht unterscheidbaren Phänotypus des hereditären Vitamin E-Mangels unterstützt und durch die Beobachtung, dass es bei Frataxin knock-out Mäusen anfänglich zu keiner Eisenakkumulation in Mitochondrien kommt, jedoch bereits in der frühen Embryonalphase der Tod eintritt (7)- .Eine dritte Hypothese geht davon aus, dass Frataxin mittelbar- oder unmittelbar an der Synthese von Eisen-Schwefel-Gruppen beteiligt sein könnte. Die mitochondriale Eisenakkumulation wäre dann eine Folge der gestörten Synthese dieser Gruppen. Diese Hypothese würde erklären, warum es praktisch ausschließlich zu einer Schädigung von Enzymkomplexen kommt, die FeS-Cluster enthalten, während andere mitochondriale Strukturen zumindest in der Initialphase nicht geschädigt werden (8). Phosphorylierung ist, was bei Uberexpression zum Anstieg des mitochondrialen Membranpotentials fuhrt und zu einer Erhöhung des zellulären ATP-Gehaltes (9).
Seit Entdeckung der genetischen Grundlage der Friedreich's Ataxie wird versucht, ein passendes Therapiekonzept zu entwickeln, um die Symptome der Krankheit abzuschwächen bzw. ein weiteres Fortschreiten zu verhindern. Dabei werden zur Zeit drei Konzepte verfolgt: a) Basierend auf der ersten Hypothese zur Funktion von Frataxin wird nach geeigneten Eisenchelatoren gesucht, um die mitochondriale Eisenüberladung zu verhindern bzw. das Eisen zu binden und exportieren. Das Problem dabei ist dass FRDA-Patienten keine allgemeine Eisenüberladung haben und die zur Zeit am Markt erhältlichen Chelatoren (Desferrioxamin und LI) nicht gezielt mitochondriales Eisen chelatieren können, und es nicht klar ist, ob sie in die Mitochondrien gelangen. Es würde vielmehr zu einer allgemeinen Eisendepletierung kommen, und in der Folge zu vermehrter Transferrinrezeptor-Expression und damit verstärkter Aufnahme von transferrin-gebundenem Eisen und neuerlichem Einbau in die Mitochondrien. Es müssten also effektive Chelatoren für mitochondriales Eisen zu Anwendung kommen, die allerdings die empfindlichen FeS-Cluster mitochondrialer Enzyme nicht angreifen dürfen. Die Entwicklung entsprechender Chelatoren wird zur Zeit von mehreren Gruppen in England und Australien verfolgt, b) In einem anderen Therapieansatz wird versucht, Substanzen zu finden, die den oxidativen Streß, d.h. je nach Hypothese über die Funktion von Frataxin die primäre oder sekundäre Radikalbildung abfangen können. Bereits in klinischer Studie ist die Substanz Idebenon (10) für die Cardiomyopalhie der Patienten erfolgsversprechend angewendet worden, nicht jedoch für die Ataxie. Es handelt sich dabei um ein Derivat von Coenzym Q]0, das endogen in der Atmungskette voikommt und erst direkt in den Mitochondrien reduziert wird und dann seine antioxidative Wirkung entfaltet. Effekte von Coenzym Q10 und dessen Derivaten auf die Frataxin-Expression wurden bislang nicht publiziert, c) Denkbar wäre auch ein Gentherapeutischer Ansatz dessen Realisierung aber noch nicht absehbar ist.
Unsere Erfindung basiert auf einem völlig neuen Ansatz zur Therapie von Friedreich's Ataxia und/oder zur Behandlung und Prävention von seinen Folgeerkrankungen die auf eine verminderte Expression von Frataxin zurückzuführen sind. In einer Arbeit wurden lymphoblastoiden Zellinien von Friedreich' s Ataxia Patienten (mit GAA Repeat-Expansion) mit 8mM 3-Nitroproprionsäure während 24h inkubiert wodurch ein Stimulierung der Frataxin-Expression um das 6.5-fache erzielt werden konnte, verglichen zu unbehandelten Zellen dieser Patienten (11). Weiters wurde in dieser Arbeit gezeigt dass der Frataxin-Gehalt der Patientenzellen durch Stimulation mit 3-Nitroproprionsäure auf den Frataxin Gehalt von nicht stimulierten Kontrollzellen (gewonnen von gesunden Personen) angehoben werden kann. Die Aufgabe war daher ein geeignetes Medikament zu finden, dass die Frataxinexpression stimuliert welches dann in der Folge bei den Patienten therapeutisch eingesetzt werden kann.
Die Erfindung betrifft den therapeutischer Einsatz von humanem Erythropoietin, rekombinantem humanem Erythropoietin, Derivaten von Erythropoietin inklusive aller Polypeptidvarianten von Erythropoietin zur Herstellung eines Medikaments zur Erhöhung der Frataxin-Expression. Die Verabreichung kann als Injektions- bzw. Infusionslösung, als Lyophylisat intravenös, intramuskulär, intracranial bzw intranasal als Nasenspray erfolgen. Die Erfindung ist im weiteren eine neue medizinische Anwendungen eines ·· ·· ···· ·< * · · · · • · >··· ; · · · • · · · · *· ···· ··· ·· ··· ·· ·· ····
Medikamentes, welches humanes Erythropoietin, rekombinantes humanes Erythropoietin oder Derivate von Erythropoietin inklusive aller Polypeptidvarianten von Erythropoietin enthält, um die Expression des Proteins Frataxin zu erhöhen. Als Derivate von Erythropoietin kommen jene in Frage die sich durch Veränderungen im Zuckergehalt (zum Beispiel Epoietin Alfa, Epoietin Beta, Aranesp, Asialoerythropoietin), durch Austausch von mindestens zwei Aminosäuren wie zum Beispiel dem Austausch von Lysin gegen Homocitrullin (zum Beispiel CEPO) von humanen Erythropoietin unterscheiden.
Die vorliegende Erfindung betrifft die Verwendung eines Medikaments in welchem humanes Erythropoietin, rekombinantes Erythropoietin oder Derivate von Erythropoietin inklusive aller Polypeptidvarianten und ein geeigneter Träger enthalten ist in einer Dosierung von 100-40.000 Units pro Woche für die Behandlung von Friedreich's Ataxie und /oder zur Behandlung und Prävention ihrer Folgeerkrankungen die mit einer verminderten Frataxin-Expression einhergehen.
Diese Lösung wird damit begründet, dass wir durch eine Behandlung verschiedener Zellkulturen und Zelltypen mit rhuEpo eine Erhöhung des zellulären Frataxingehaltes erzielen konnten. Diese Erhöhungen wurden immer mittels Western Blot detektiert.
Beispiel 1:
Die Expression von Frataxin in humanen primären Herzmuskelzellen (Kardiomyozyten) konnte durch die Inkubation mit rekombinantem humanem Erythropoietin (EPO) (konzentrationsabhängig von 0-9,9U/ml) über einen Zeitraum von 48h um ein bis zu 2.5-faches der Kontrolle (ohne Erythropoietin) erhöht werden. Die Zellen wurden nach der Inkubation in Zelllysierungspuffer (von Promega) lysiiert und die Proteine des Zelllysats (30pg Protein/Bahn) mittels SDS-PAGE aufgetrennt, auf Nitrozellulose geblottet und die Frataxin-Expression mittels Immunblot unter Verwendung eines ersten Antikörpers gegen humanes Frataxin (hergestellt im Kaninchen) und einem zweiten mit Meerettich-Peroxidase gekoppelten Antikörper gegen Kaninchen (hergestellt in der Ziege, von DAKO) nachgewiesen. Der Blot wurde mittels SuperSignal (von Pierce) entwickelt und das Signal in einem Chemilmager von Alpha Innotech aufgenommen (Abbildung! 1).
Beispiel 2: (
Die Expression von Frataxin in Mäusembryocarzinoma P19-Zellen, die durch Retinolsäure zu Neuronen differenziert wurden konnte durch die Inkubation mit rekombinantem humanem Erythropoietin (EPO)(konzentrationsabhängig von 0-9,9U/ml) über einen Zeitraum von 48h um ein bis zu 2.5-faches der Kontrolle (ohne Erythropoietin) erhöht werden. Die Expression von Frataxin in Mausembryocarzinoma P19-cellen, die durch Retinolsäure zu Neuronen differenziert wurden konnte durch die Inkubation mit rekombinantem humanem Erythropoietin (EPO)(konzentrationsabhängig von 0-9,9U/ml) über einen Zeitraum von lh und anschließendem wegwaschen der Inkubationslösung und anschließender Weiterinkubation der Zellen für 48h ohne Erythropoietin konnte ein Anstieg der Frataxin-Expression um das 2-fache im Vergleich zur Kontrolle (ohne Vorinkubation mit Erythropoietin) erhöht werden. Die Zellen wurden nach der Inkubation in Zelllysierungspuffer (von Promega) lysiiert und die Proteine des Zelllysats (50pg Protein/Bahn) mittels SDS-PAGE aufgetrennt, auf Nitrozellulose geblottet und die Frataxin-Expression mittels Immunblot unter Verwendung eines ersten Antikörpers gegen humanes Frataxin, der auch mit Maus-Frataxin reagiert (hergestellt im Kaninchen) und einem zweiten mit Meerettich-Peroxidase gekoppelten Antikörper gegen Kaninchen (hergestellt in der Ziege, von DAKO) nachgewiesen. Der Blot wurde mittels SuperSignal (von Pierce) entwickelt und das Signal in einem Chemihnager von Alpha Innotech aufgenommen (Abbildung 3).
In lymphoblastoiden Zellinien von Friedreich' s Ataxia Patienten (mit GAA Repeat-Expansion) konnte durch Inkubation mit 8mM 3-Nitroproprionsäure während 24h die Frataxin-Expression um das 6.5-fache erhöht werden verglichen zu unbehandelten Zellen dieser Patienten (11). Weiters wurde gezeigt dass der ?
Frataxin-Gehalt durch geeignete Stimulation der Zellen auf den Frataxin Gehalt von nicht stimulierten Kontrollzellen (gewonnen von gesunden Personen) angehoben werden kann

Claims (10)
- Patentansprüche: 1. ) Verwendung von therapeutisch effektiven Menge von humanem Erythropoietin, rekombinantem humanem Erythropoietin, Derivaten von Erythropoietin inklusive aller Polypeptidvarianten von Erythropoietin zur Herstellung eines Medikaments zur Behandlung von Friedreich's Ataxie und zur Behandlung und Prävention der als Folgeerkrankungen der Friedreich's Ataxie ausgewiesenen Krankheiten.
- 2. ) Verwendung gemäß Anspruch 1 dadurch gekennzeichnet, dass die Friedreich's Ataxie mittels Genanalyse und/oder ELISA und/oder realtime-PCR diagnostiziert wurde und dass die Frataxin-Expression durch eine GAA-Repeat-Expansion oder Mutationen auf einem oder auf beiden Allelen im Frataxin-Gen vermindert ist.
- 3. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet, dass die Folgekrankheit eine Herzerkrankung von Patienten mit verringerter Frataxin-Expression ist.
- 4. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet dass die Folgekrankheit Diabetes (inklusive aller Formen des Diabetes wie zum Beispiel Diabetes Typ 2, Insulinunabhängiger Diabetes, latenter Diabetes und präklinischer und manifester Diabetes und Glukoseintoleranz) von Patienten mit verringerter Frataxin-Expression ist.
- 5. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet dass die Folgekrankheit eine neurodegenerativen Erkrankung (Ataxie) von Patienten mit verringerter Frataxin-Expression ist.
- 6. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet dass die Folgekrankheit eine Knochendeformationen (bevorzugt Skoliose und Hohlfuß (Pes cavus)) ) von Patienten mit verringerter Frataxin-Expression ist.
- 7. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet, dass die Folgekrankheit ein Nystagmus von Patienten mit verringerter Frataxin-Expression ist.
- 8. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet . dass die Folgekrankheit eine Schwerhörigkeit von Patienten mit verringerter Frataxin-Expression ist.
- 9. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet, dass die Folgekrankheit eine Augenerkrankung (bevorzugt eine Optikusatrophie (Makuladegeneration^jvon Patienten mit verringerter Frataxin-Expression ist.
- 10. ) Verwendung eines Präparates gemäß Anspruch 1 dadurch gekennzeichnet dass die Folgekrankheit eine Krebserkrankung von Patienten mit verringerter Frataxin-Expression ist.
