5The invention relates to a method for measuring optical properties of a filtrate flow, which is separated from a main flow of a flowing, loaded with microparticles in suspension medium via a particle-separating filter membrane of a filter device. 5
The invention also relates to a device for measuring optical properties of a Filtratflusses, with a filter device, which comprises a filtration chamber, a filtrate separated by this by a filter membrane filtrate, an inflow into the filtration chamber, an outflow therefrom, a Filtratabfluss from the filtrate and a optical io detector for a connected to the filtrate Filtratleitung has.
Leider wird die Wirksamkeit dieser Verfahren häufig durch technische Probleme, niedrige Selektivität und 25 geringe Leistungsfähigkeit begrenzt.DE 42 07 614 C1 describes a dynamic filter separation device which is intended for use in different areas, including in medicine for separating the plasma from blood. In this known filter device, a rotating disk induces a relative movement on the membrane surface in order to reduce clogging of the membrane pores. The known device is on the one hand because of the rotating disk and its drive quite expensive and expensive and on the other hand neither easy to clean and sterilize, nor appropriate for a disposable / disposable solution. In particular, the starting point of the invention is the detection of stains in blood plasma, for example by released hemoglobin. There are certain blood purification techniques, such as plasmapheresis and hemoperfusion / plasma perfusion, in which detoxification occurs by removing protein-bound or hydrophobic substances from the blood. Unfortunately, the effectiveness of these methods is often limited by technical problems, low selectivity, and low performance.
On the other hand, symptoms of patients suffering from liver failure or other hepatic dysfunctions can be improved by eliminating the toxins and other undesired substances that can not be eliminated with conventional dialysis treatments (hemodialysis, hemofiltration). In Fig. 1, a suitable for the purposes mentioned system is shown, which has become known from EP 0 776 223 B1. In this system or device, a primary extracorporeal circuit 3 is connected to a patient PAT, wherein an arterial line 1 via a blood pump 2 leads to a plasma filter 5, from which the return flow takes place via a venous line 4 to the patient PAT. The second extracorporeal circuit or secondary circuit 7 leads back from the filtrate side of the plasma filter 5 via a centrifugal or roller pump 8 and a device 6 for measuring optical properties of the filtrate flow in the circuit 7 back to the filtrate side of the plasma filter 5. The shown in Fig. 1 The device is state of the art, the invention being concerned with the device 6 explained in more detail below and an associated measuring method.
50Returning to FIG. 1, the relative movement of the fluids of the primary and secondary circuits produces a transmembrane pressure in the plasma filter 5, which results in a fluid exchange between the circuits. In the secondary circuit 7, microparticles 945 circulate, with the help of which toxins are specifically bound and thus removed from the blood. Such microparticles or microspheres have a diameter of less than 20 pm, in particular from 1 to 7 pm, which is comparable to the diameter of the blood cells. Particular characteristics of the microspheres are a large outer surface and short diffusion paths to inner pores, if any. 50
AT 008 501 U1To prevent direct contact between the blood cells and the particles, the whole blood is split with the plasma filter into a cell and plasma compartment, with the plasma in the secondary circuit circulating at high speed to maintain a high plasma transmembrane flux for efficient treatment. The flow rate in the 55 secondary circuit 7, which is typically 0.5 to 4 l / min, is also so high, so that no 3 AT 008 501 U1
Bildung von Ablagerungen erfolgen kann.Formation of deposits can take place.
In order to maximize patient safety, it is necessary to implement various security systems into a system as shown in FIG. If, for example, the transmembrane pressure in filter 5 is too high, the high shear forces between the filter membrane and blood cells can lead to destruction of the erythrocytes, and thus hemolysis. This causes a release of hemoglobin both in the primary circuit 3, and from here into the secondary circuit 7, which consists of plasma with microparticles in suspension. In conventional plasmapheresis and dialysis systems, the release of hemoglobin, which causes a red coloration of a clear liquid, is measured by means of a color sensor in transmitted light. Such color sensors are known and are very sensitive to a red coloration of the filtrate and can detect haemolysis so quickly.
In a device according to FIG. 1, a direct detection of hemolysis 15 is generally not possible because of the high optical density of the microparticle suspension, wherein it should be noted for illustration that the microparticles can also be activated carbon particles, which makes the problem of optical recognition clear.
25It is an object of the invention to provide a method and apparatus for measuring optical properties of a filtrate flow, in particular particle-laden blood plasma, more generally, a filtrate of blood which, with a cheap and simple construction, provides reliable results meeting the safety requirements in the medical field be fair, in particular, even in optically dense main flow, namely a particle suspension measurement should be possible. 25
30This object is achieved with a method of the type mentioned, in which according to the invention, the main flow is introduced into the filtration chamber so that it generates a constant flow on the membrane surface and counteracts a deposition of microparticles on this solely due to its own flow. 30
Thanks to the invention, a long-lasting function without clogging of the filter membrane and thus the use is made possible in a simple way, especially in blood purification systems.
In an advantageous embodiment, it is provided that the main flow is introduced substantially tangentially into the filtration chamber, so that the flow on the membrane surface can be generated in a simple manner.
40In order to avoid loss of filtrate, for example to blood plasma, it is advantageous if the filtrate flow is fed back to the main flow downstream of the filter device. 40
For reasons of safety, it is particularly advisable if the pressure in the filtrate flow downstream of the filter device is monitored by means of a pressure sensor, the filtrate flow being throttled downstream of the pressure sensor. In practice, an embodiment has proven to be useful in which the filtrate flow is temporarily throttled and thereby monitored by the pressure sensor for a pressure increase.
The method is particularly suitable for an extracorporeal blood purification device, in which the main flow is a blood plasma flow.
55 4 AT 008 501 U1The object is also achieved with a device of the type mentioned, in which opens according to the invention, the inflow to produce a vortex flow substantially tangentially into the filtration chamber. 55 4 AT 008 501 U1
For uniform turbulence can also be provided that several tributaries for the main flow opens substantially tangentially into the filtration chamber.
In use and in production is useful a variant in which the filtration chamber 5 and the filtrate chamber are cylindrical and separated by a substantially flat membrane.
15Furthermore, this is advantageous in terms of a simple generation of the desired flow, if inflow and outflow are substantially tangent to the filtration chamber and direction whose io height offset. Here, it may be advantageous to avoid inhomogeneities, when the outflow is guided substantially normal to the membrane and center of the filtration chamber. It can be provided for further improvement that in the axis of the filtration chamber a flow-directing, preferably frusto-conical insert is arranged. 15
20If a pressure sensor is arranged on the filtrate line and a throttle downstream of the pressure sensor, wherein in particular the throttle can be actuated temporarily and the pressure in the filtrate line can be monitored by means of the pressure sensor, the safety of the device can easily be increased with regard to an unforeseen failure become. 20
To avoid filtrate losses, it is advisable, if the Filtratleitung opens downstream of the filter device in the drain.
The device is particularly advantageous if the filter device is arranged in and out of a line for the plasma flow of an extracorporeal blood purification device.
30 In dieser zeigen:The invention together with further advantages is explained in more detail below with reference to illustrative embodiments, which are illustrated in the drawing. 30 In this show:

Fig. 8 eine Draufsicht auf Fig. 7.8 is a plan view of Fig. 7.
Referring now to Fig. 2, there is a schematic representation of the formation of a device for measuring the optical properties of a Filtratflusses according to the invention. The device has a filter device 110 which has a filtration chamber 11, a filtrate chamber 12 separated by a filter membrane 13, an inlet 100 into the filtration chamber, a drain 19 from this filtration chamber, a filtrate drain 16 from the filtrate chamber and an optical detector 17 for a Has filtrate 160 connected to the filtrate.
25 30In connection with FIG. 1 it can be seen that, for example, plasma with microparticles suspended therein flows through the infiltration chamber 100 through the filtration chamber 11 and leaves the filtration chamber via the outflow 19 again. As will also be apparent from FIG. 5, in the exemplary embodiment shown, the filtration chamber 11 and the filtrate chamber 12 are cylindrical and separated by the substantially flat membrane 13. The medium laden with microparticles in suspension is allowed to flow tangentially into the filtration chamber 11 via the inflow 100, whereby a swirling flow is generated which leaves the chamber 11 again via the outflow 19, which is likewise arranged tangentially. A frusto-conical insert 20 directs the flow and favors the rotational movement thereof. The constant flow on the membrane surface, which is generated solely due to the internal flow of the inflow or main flow 10, counteracts deposition of microparticles on one side of the membrane, without the need for other extensive Eino io handles, as according to the aforementioned state of Technique example, by the rotating disk is the case. An optical detector 17 is arranged on the filtrate line 160 for the filtrate flow, which can be set up, for example, for the detection of free hemoglobin and which can emit a warning and / or stop signal when a corresponding coloration occurs in order to protect the patient. In general, of course any desired 15 optical properties of the filtrate 14 can be measured, such as its optical density, fluorescence, etc. By suitable choice of the lumen of inflow 10 or outflow 19, preferably in the diameter range of 3 to 4 mm, or by using valves, the pressure difference between filtration chamber 11 and filtrate chamber 12 can be determined. Due to the pressure gradient that occurs, part of the particle suspension is filtered via the membrane 13 in the direction of the filtrate chamber 12. As a filter membrane is preferably used for the purposes envisaged here, a cellulose acetate membrane with a pore size of 3 to 8 pm. A continuous filtration through the membrane 13 is possible here, since the membrane pores are constantly washed free in the manner described. 25 30
35 dP _ _1_dQ dt ~KV0 dt (1)The filtrate flow through the membrane 13 is very low due to the high viscosity and high density of the particle suspension and is 0.1 to 4 ml / min. However, monitoring of the filtrate flow is necessary in most cases to confirm the function of the device. Such Filtratflussüberwachung can be done by periodically throttling the Filtratkreises with a throttle 15, for example, a valve or a hose clamp. Such disconnection leads to an increase in the filtrate in the presence of filtrate. In order to measure this, a pressure sensor 18 is arranged upstream of the throttle 15 on the filtrate line 160. From (1) it can be seen that the change in the filtrate pressure dP / dt is proportional to the material elasticity K of the filtrate cycle, the filtrate volume V0 between the filter membrane 13 and the restrictor 15 and the filtrate flow dQ / dt. 35 dP _ _1_dQ dt ~ KV0 dt (1)
The measurement data of the pressure sensor 18 can be processed, for example, with the aid of a microcontroller and compared with a set of stored pressure profiles. If the conformity of the measuring profile with the stored data lies outside a predefined tolerance, which is shown here by dashed lines as 22, a deviation of the filtration properties of the filter and a change in the filtrate flow must be concluded.
In addition, the filtrate flow can be determined via a simple threshold detection on the pressure profile. After closing the throttle 15, the measured value of the pressure sensor 18 must have exceeded a specific threshold value 23 within a predefined time, so that the function of the filter is faultless and a sufficient filtrate flow is present.
Fig. 4 zeigt gemessene, typische Verläufe des Membrandrucks 24 und des Filtratflusses 25 bei Verwendung einer 3 pm-Zellulose-Acetat-Membran und einer 25%igen VA/-Partikelsuspension bei einem Partikeldurchmesser < 25 pm in Humanplasma, wobei der Suspensionsfluss 55 600 ml/min betrug.FIG. 4 shows measured typical runs of the membrane pressure 24 and the filtrate flow 25 using a 3 μm cellulose acetate membrane and a 25% VA / particle suspension with a particle diameter < 25 pm in human plasma, the suspension flow being 55 600 ml / min.
