Abstract

BIOMARCADORES PARA IDENTIFICAR A EFICáCIA DE TEGASEROD EM PACIENTES COM CONSTIPAçãO CRÈNICA. A farmacogenética foi usada para avaliar o efeito dos polimorfismos em selecionar genes candidatos na resposta dos pacientes com constipação crónica ao tegaserod (Zelmac(r)/Zelnorm(r)). A análise identificou doze polimorfismos de nucleotídeo únicos (SNPs) em seis genes (HTR4, HTR3B, MLN, AQP3, SLC12A2, SCNN1A) que foram associados com pelo menos uma taxa de resposta de 60 % ao tegaserod e uma razão de probabilidade de não ocorrência de 5 ou mais (comparado ao placebo) depois de 4 semanas de tratamento. Os genes identificados exibem uma faixa ampla de funções diferentes, incluindo sinalização, secreção e motilidade de serotonina, todas as quais são importantes em manter a função normal do trato gastrointestinal. Assim, estes dados sugerem que a constipação crónica pode resultar de uma variedade de mecanismos fisiopatológicos relacionados às variações nos genes identificados acima, todas as quais respondem bem ao tratamento com tegaserod. Os pacientes sem estas variações não respondem ao tratamento significantemente mais do que eles respondem ao placebo, que poderia indicar que sua constipação crónica não é devido aos mecanismos fisiopatológicos mas preferivelmente aos fatores ambientais ou possivelmente psicológicos. Os pacientes com estas variações também são menos prováveis a responder ao placebo, novamente sugerindo que estas variações são associadas com uma fisiopatologia verdadeira.
