Abstract

Une utilisation du pertuzumab est décrite en combinaison avec le trastuzumab et le docétaxel dans le traitement du cancer du sein métastatique HER2 positif sans accroître la toxicité cardiaque. La combinaison est particulièrement utile chez les patients atteints du cancer du sein métastatique HER2 positif présentant une fraction déjection du ventricule gauche (LVEF) de plus de 50 % avant le traitement qui nont pas reçu de chimiothérapie ou de thérapie biologique préalable pour leur maladie métastatique. La toxicité cardiaque est un dysfonctionnement systolique du ventricule gauche (LVSD) et lincidence du LVSD après au moins 15 cycles de pertuzumab, trastuzumab et docétaxel est moindre que lincidence du LVSD chez les patients atteints du cancer du sein métastatique HER2 positif traités seulement au moyen du trastuzumab et du docétaxel. Le traitement comprend larrêt de ladministration du pertuzumab et du trastuzumab pendant au moins trois semaines si le patient enregistre une chute de LVEF à moins de 40 % ou une LVEF de 40 % à 45 % ayant une diminution absolue de 10 % ou plus sous la valeur avant le traitement, et la reprise de ladministration du pertuzumab et du trastuzumab si la LVEF remonte à plus de 45 % ou se situe de 40 % à 45 % avec une diminution absolue de moins de 10 % sous la valeur avant le traitement.
