Description

  

  



   Vorrichtung zur Anfertigung von stereoskopischen   Röntgenaufnahmen,    die   stereophotogrammetrische    Messungen an ZÏhnen und Kiefern erm¯glichen.



   Die   gewohnliche    R¯ntgenphotographie    gibt an und für sich keine Anhaltspunkte    iiber die   Tiefenlage    der Gegenstände. Einzig eine grosse Routine und die topographische   Ortskenntnis    setzen uns in den Stand, uns eine volumetrische Vorstellung davon zu rekonstruieren.



   Beim Betrachten einer Reihe voneinander leieh weit abstehender Gegenstände, die auf   einer sieh    von uns   entfernenden Geraden an-    geordnet sind, werden die   Konvergenzände-    rungen beim Übergang von einem Gegenstand zum andern um so kleiner, je entfern  @er diese GegenstÏnde    sein werden. Von einer    gewissen Entfernung an werden diese Ände-    rungen so klein, dass   unsere Wahrnehmung    des Reliefs gÏnzlich aufh¯rt. Es ist daher von Vorteil, die   Gegenstände mögliehst    nah von uns anzubringen, um den Wert dieser   Konvergenz-Änderungen    zu   erhohen.   



   Der Aufnahmewinkel, der in der Folge dem durchschnittlichen   Eonvergenzwinkel    entspricht, bestimmt die Entfernung, in wel  cher    wir die Gegenstände im   Stereoskop    zu sehen glauben. Die Wahl dies, es Winkels    steht uns vollkommen frei. vorausgesetzt, dass      15@ nicht  berschritten    werden ; wird haben uns f r einen Winkel von 12¯ entschieden, der fiir Augen in einem Abstand von 65 mm einer Entfernung von etwa 32 2 cm entspricht.



  Dies   ist die normalp Entfernung der Nah-       Akkomodation, wobe@ der Winkel von 12¯    ebenfalls einer normalen Konvergenz entspricht, die lange ohne Ermüdung beibehalten werden kann.



   In den beiliegenden Zeichnungen ist durch die Fig. 1-4 das an sich bekannte Verfahren veranschaulicht, welches der Verwen  dung    der erfindungsgemässen Vorrichtung zugrunde liegt. Fig.   1    stellt in a und b zwei Röntgenaufnahmen der GegenstÏnde A, B, C   und R dar,    wobei die Filme 1 und   2      gegen-    über den Gegenständen die gleiche   Lage ein-    nehmen. Da die Einfallwinkel der Lichtstrahlen auf die beiden Filme verschieden sind, ist die Verteilung der   Projektionspunkte    auf den Filmen   1    und   2 ebenàlls verschieden.   



   Zur   Erzielung einer grosseren Übersicht-    lichkeit der Zeiehnungen ist ein gröBerer Winkel als 12¯ gewÏhlt.



   Fig. 2 zeigt die Filme   1      und 2    in einein Stereoskop montiert, wobei sie gleichzeitig durch das linke und rechte Auge betrachtet werden.



   Der Blickwinkel der Augen ist beim Durchgang durch A'und A"gröBer als durch B'und B"und kleiner als durch R' und R". Diese Konvergenzabweichungen rufen den Eindruck des Reliefs hervor, und wir erblicken in   Av, Sv, Cv und Xv das    virtuelle Bild der in den   Raum ge-stellten    Gegenstände.



   Fig. 3 zeigt zwei Röntgenaufnahmen unter einem Winkel von   12 ,    unter Einhaltung der in der Zahnheilkunde  blichen VerhÏltnisse. 



   Die   Entfernung    L der   Robre    ist so   grogs    im Verhältnis zur Entfernung zwischen den Gegenständen und dem Film sowie auch zur Filmfläche Fl   selbst, daB die Gera, den       AA' BB'und AA"BB"als unter sich    parallel betrachtet werden d rfen. Dies um so mehr, als das   Strahlenausgangszentrum    nicht ein Punkt, sondern eine Fläche ist.



   Dieser   Parallelismus    ergibt in A A'A" und B B'B" Ïhnliche Dreiecke.



   Fig. 4 zeigt die Filme   1    und 2 der Fig. 1.   übereina.      ndergelegt    bzw. die Teilfig. la und 1b vereinigt. Jeder   Gegenstandspunkt    wird zweimal sichtbar : A in A'und   A",      B    in B' und B". Die Abstände A'A", B'B"-sogenannte ¸parallaktische AbstÏnde¯ - sind proportional der Entfernung zwischen den Gegenstandspunkten und dem Film. Diese Entfernungen, nachfolgend mit   AF,      BF, RF    usw. bezeichnet, sind eben die gesuchten Ent  fernungen.    Durch Aufstellung dieser Verhältnisse finden wir :
AF/A'A" = RF/R'R" oder AF = RF/R'R".A'A"    -L == oderBF==--.B'B"   
Bei.

   Untersuchung dieser zwei Formeln finden wir, da. ? die H¯hen AF und BF sich als Produkte aus dem konstanten Faktor     ---, und dem parallaktischen Abstand R'R" - - @@@@    von A oder B darstellen.



   Dieser Faktor kann im voraus berechnet werden, wenn im Apparat der Punkt R einem festen   Merkzeiger    entspricht, der z. B. in einer Entfernung von 60 mm vom Film angebracht ist, und auf dem Bild mit aufgenommen wird. Bei einer Entfernung von 60 mm und einem Winkel von   12  ist    der parallaktische Abstand dieses Merkzeigers etwa 12, 5 mm.



   RF/R'R" = 60/12,5 = 4,8
Um die Entfernung zwischen den   Gegen-    stÏnden und dem Film zu bestimmen, genügt es demnach, die gemessenen   parallaktischen    Abstände mit 4, 8 zu multiplizieren, so da? beispielsweise der Abstand AF bei einer Entfernung der Punkte A'A"von 4 mm,   4    X 4, 8 = 19, 2 mm beträgt.



   Es wurde bereits vorgeschlagen, zur Er   leichterung der röntgenstereophotogrammetri-    schen Messungen einen äussern Merkzeiger anzubringen. Die zur Be, rechnung der   Ent-      fernungen    vorgeschlagene Formel ist jedoch kompliziert und zeitraubend.



   Ferner wurde vorgeschlagen, einen innern   Merkzeige. r    an der Schleimhaut zu   befesti-    gen. Dies geschieht in der Weise, daB ein Bleikörnchen mittels eines   Blättchens    angeklemmt wird. Durch Messung des Tiefenunterschiedes   zwischen dem Merkzeiger    und dem gesuchten Gegenstand wird dessen Tiefe unter der Schleimhaut bestimmt.



   Die Aufnahme von   stereoskopischen    Rönt  genbildern,    die gleichzeitig eine gute optische Rekonstruktion und genaue   stereogramme-      lrische    Messungen ermöglichen sollen,   erfor-    dert naturgemaB äusserste Genauigkeit. Zur Erreichung dieses Ziels müssen folgende Bedingungen erfüllt werden :
1. Die Filme sollen flach bleiben, um Spannungen und Verformungen zu vermeiden.



   2. Der zweite Film soll im Mund genau dieselbe Lage einnehmen wie der erste.



   3. Der   Visierwinkel    soll   12"betragen.   



   4. Die Linie, die diesen Winkel in zwei gleiche Hälften schneidet, soll senkrecht zum Film verlaufen.



   5. Die zwei Anvisierungen sollen in der gleichen Ebene erfolgen, zwecks   Ermög-    lichung des   binokularen tibereinanderlegens.   



   6. Der Filmrand soll mit der   Visierebene    parallel verlaufen, zwecks Erleichterung des Einsetzens in das   Stereoskop.   



   Diese Bedingungen können nur bei Verwendung eines   Spezialapparates ermöglicht      werden, mitbels    welchem der zweite Film in derselben Lage wie der erste Film befestigt werden kann, wobei das Anvisieren von den Bewegungen des Patienten zwischen den beiden Belichtungen unabhängig sein soll. 



   Durch vorliegende Erfindung soll ein Apparat geschaffen werden, der erm¯glicht, diese technischen Bedingungen zu erfüllen.



     Die.    Erfindung besteht darin, dass ein   Trag-    organ einen FilmtrÏger, ein Merkzeiehen und   einen Strahlrichter    aufweist, und mit einem Arm gelenkig verbunden ist, welcher mit   'inemimMund    zu befestigenden Organ kuppelbar ist.



   In beiliegender Zeichnung ist ein Ausf hrungsbeispiel der   erfindungsgemässen Vor-    richtung veranschaulicht ; es zeigt :

   Der Sattel 10   (Fig.    5) wird mit plastischer Abdruckmasse oder mit Abdruckgips   ausgefüllt und    auf der dem zu photographierenden Zahn gegen berliegenden Kieferseite In dem Sattel 10 ist eine   Biichse    11 gelagert, die auf der Innenseite des Sattels   ''inen Vierkant 12 aufweist.    In die B chse   J l ist ein Vierkantbolzen    13 mit Bund so   e illgesehraubl-dass sieh    die Büehse 11 im Sattel drehen aber zur   Reinigtmg    nach Abschrauben des Bolzens aus dem Sattel entfernen lÏ?t.

   Solange der Gips in dem Sattel noch weich ist, kann der Bolzen 12, der in der B chse festgeschraubt ist, mit dieser gedreht werden; nach Hartwerden des Gipses verhindert der Vierkant 12 der   Büchse ein    Verdrehen des Bolzens. Der Verbindungsarm 14 (Fig. 7) ist an seinem vierkantigen Teil 15 mit Vierkantl¯chrn 16 versehen, die  ber den Bolzen 13 des Sattels 10 greifen. Die Lucher 16 sind entsprechend der   gewünsch-    ten Länge der Verbindung zwischen dem Tragb gel 19 und dem Sattel'10 angeordnet.



  Die Klemmh lse 17 hat eine Vierkantbasis und ist daher nicht drehbar. Sie wird durch die   Überwurfmutter    18 zusammengepresst. In die Elemmhülse greift eine an dem   Trag-    b gel 19 befestigte Kugel   20      (Fig. 12).    wodurch der Tragbügel 19 in jeder Lage mit dem Verbindungsarm 14 verbindbar ist. Der Tragbügel 19 dient als Träger des Filmträgers 21, des Merkzeigers 23 sowie des Strahl  richters      27.    An einem Ende des Tragb gels 19, das an dem Verbindungsarm   14    gelenkig befestigt ist, ist der Filmträger 21 aufgesteckt, während auf das andere Ende ein Schieber 22 aufgesteckt ist, der den Merkzeiger   23    trägt.

   An dem Schieber 22 ist, um den Zapfen 24 schwenkbar, ein winklig   ge-    bogener Arm 25 angelenkt, auf dessen   abge-      bogenem    Teil   26,    der einen quadratischen Querschnitt aufweist, der Strahlrichter 27 aufgesteckt ist. Der Strahlrichter 27 kann dadurch liegend oder aufreehtstehend auf dem Vierkant   @ 26 befestigt werden. Zweck-    mÏ?ig weist der   Strahlrichter    eine federnde Vierkantöse 28 auf. die sich beim   Auf-    stecken auf den Vierkant 26 festklemmt. Der   Strahlrichter 27 besteht    aus einer Platte mit zwei in einem Winkel von 12¯ zueinander verlaufenden Rändern 29 und 30.

   Der   Trag-    bügel 19 ist zweckmÏ?ig durch den Schlitz 31   aufgesehlitzt,    so dass der aufgesteckte Schieber 22 spiellos auf dem Bügel 19 sitzen kann. Der Bügel 19 ist an beiden Enden mit @ AnsÏtzen 32 und 33 versehen, damit der aufgesteckte Filmhalter und der Strahlrichter   gegen Versehieben    in   Aehsrichtung der Bii-    gelenden und gegen Verdrehen gesichert sind, so dass die Mittellinie des Strahlrichters in seiner liegenden Stellung auf die   Mité des    Films zeigt, dessen Flache senkrecht zur Winkelhalbierenden zwischen den beiden durch die Kanten   des Strahlriehters bestimm-    ten Aufnahmeriehtungen steht.

   Die   Schwen-    kung des Armes   25    dient dazu, beim   Belich-    ten des Films den   Strahlrichter aus    dem Bereich der   Belichtungsstrahlen    zu bringen.



  Durch die Feder 34 wird der Arm   25    in beiden Stellungen festgehalten. An dem    Schieber 22 ist der sogenannte Beyelensche      Merkzeiger    23 im Abstand von 60 mm von der Filmebene   angeordnet. Dieser Merkzeiger    weist eine Spitze von etwa   1    mm Durchmesser auf, welche sich auf dem Film markiert und als Anhaltspunkt für die   stereo-      photogrammetrisch, e Auswertung    dient. Es ist eine Verstellbarkeit des   Strahlrichters    aus der Horizontalebene in die Vertikalebene vorgesehen, um auch zwei Aufnahmen in der Vertikalebene zu machen, wie in Fig. 12 strichpunktiert angedeutet, was durch die Vierkantöse 28 und den Vierkantdorn 26 des Armes 25 ermöglicht wird.



   Die Fil'mträger sind in   versehiedenen    Typen vorgesehen, um sich den anatomischen Bedingungen anpassen zu können.



   Fig. 8 zeigt beispielsweise einen Filmträger für den   Standardfilm.    Die Filmkassette wird in eine Nute 36 eingelassen, die an drei Seiten des Rahmens angeordnet ist. Je zwei   parallele Filmränder    sind in je einer Stellung des Strahlrichters parallel zur Ebene desselben.



   Fig. 10 und 11 zeigen den Filmträger für kleine Filme, dessen Rahmen 39 mittels des Halters 37 auf dem Bügel 19 verschiebbar sitzt, derart, daB der Rahmen 39, dank des Schlitzes 38, nach Gaumenhöhe oder Mundhöhe verstellt und mittels der Mutter 41 festgestellt werden kann, wobei stets je zwei parallele Filmränder mit der Ebene des in die horizontale oder vertikale Lage eingestellten Strahlrichters parallel bleiben. Soll der Rahmen auch schräg einstellbar sein, dann fÏllt die Führung 40 fort.



   Zur Einstellung des Strahls des Röntgenapparats ist auf den   Tonus. 42    des Strahlers 43 eine   Richtnadel    44 aufgesetzt, mittels welcher der Strahler 43 in die Richtung einer   gante    des   Strahlrichiers    27 eingestellt wird. Diese Richtnadel 44 ist an einer Haube 45 befestigt, welche über den Konus des Strahlers gestülpt wird und mit einem nicht näher gezeichneten entsprechenden Bindemittel, beispielsweise einem Gummiband, be  festigt ist.   



   Die Fig. 12 zeigt die Anwendung der Vorrichtung bei der Aufnahme von   Stereo-    Röntgenbildern im Mund. In dem   dargestell-    ten Beispiel ist der Sattel auf den Zähnen links unten durch   Aufgipsen befestigt, da    die Stelle, welche aufgenommen werden soll, sich rechts befindet. Zunächst wird die günstigste Lage f r die Befestigung des Sattels 10 gesucht und das zu beniitzende   Vierliant-    loch 16 des Verbindungsstückes 14 bestimmt.



  Hierauf wird die Schraubenmutter 18 zum Kugelgelenk des Verbindungsstückes 14 leicht angezogen, der Sattel 10 mit Gips aufgefüllt und an der   ausgesuchten Stelle    auf die Zähne bzw. den Kiefer aufgesetzt. Sodann wird das Verbindungsstück 14 mit dem Tragbügel 19 auf den   Vierkantbolzen    13 aufgesetzt, worauf je nach Bedarf gegenüber dem   Strahlrichter    ein Film für horizontale oder vertikale Aufnahme in den Filmträger eingesetzt wird, dessen Fläche durch die Lage des Filmträgers immer senkrecht zur Winkelhalbierenden zwischen den beiden, durch den   Strahlrichter    bestimmten Auf  nahmerichtungen    steht.

   Nun wird   abgewar-    tet, bis der Gips hart ist, während welcher   Zoit    sich der Patient nicht bewegen darf.



  Nach   Hartwerden    des Gipses wird die Lage des Films überprüft und hernach die Schraubenmutter 18 fest angezogen, worauf der Röntgenstrahler durch   Anvisieren    mit der VisiernadeI 44 in die richtige Lage gebracht wird.



   Fig. 12 stellt den Apparat dar während des Anvisierens, wobei auf den bonus 42 des Röntgenstrahlers 43 eine Visiernadel 44 aufgesetzt ist. Die genaue Einstellung des Strahlers durch Anvisierung hat eine sehr gro?e Bedeutung. Mit den gebrÏuchlichen   Röntgenapparaten,    die einen breiten abgerundeten Konus aufweisen, lässt sich ohne Visiernadel nur eine annähernde Einstellung erzielen.



   Diese Visiernadel kann aus einer me  tallenen    vulkanisierten Stange bestehen, die auf einer Haube 45 befestigt ist und mit einem Gummiband an dem Konus 42 befestigt werden. Diese Stange muss   jedoeh    wÏhrend der Belichtung entfernt und bei jerlem   Einvisieren    wieder neu   eingesetztwer-    den. Es ist daher zweckmÏ?iger, in die Haubenspitze eine   Zelluloidstricknadel    einzusetzen, die dann ständig eingesetzt bleiben   kann. weil diese    Nadel den Röntgenstrahlen   keinen. Widerstand entgegensetzt.   



   Während der Belichtung wird, wie in Fig. 12 strichpunktiert gezeichnet, der Strahlrichter 27 nach oben geklappt und die metallische Visierstange des   Rontgenappa-      rates entfernt.    Der   Beyelen, sehe blerkzeiger    wird bei einer Entfernung von 60 mm und einem Winkel des Strahlriehters von   12     auf den Film etwa 6 mm rechts oder links von der Mittellinie projiziert.



   Zum Auswechseln des Films wird durch Abziehen des Verbindungsarmes 14 vom   Vierkantbolzen    13 ein Teil der Vorrichtung entfernt, während der andere Teil im Mund verbleibt. Nach Einsetzen eines neuen Films in den Rahmen kann der abgehobene Teil   der Vorrichtung durch Aufstecken    auf den    Vierkantbolzen    13 wieder genau in die vor  heriae    Lage gebracht werden. Für die zweite   Röntgenaufnahme    wird der Röntgenstrahler   43    nach der   a. ndern    Kante des   Strahlrichters    27 eingestellt, so dass die beiden Aufnahmen in einem Winkel von 12  zueinander   erfol-      gon.   



   Es empfiehlt sich, stets vier Aufnahmen zu machen, wovon zwei in der Horizontalebene und zwei in der Vertikalebene. Zum Auseinanderhalten dieser Aufnahmen k¯nnen im Handel erhÏltliche, kleine metallische Buchstaben vor der Belichtung am   Filmrand    werden, was die nachherige Mon   tage zur stereophotogrammetrischen Auswer-      tungderAufnahmeerleichtertund    Verwechslungen verhindert.

   The usual X-ray photography does not give any indications of the depth of the objects. Only a big routine and the topographical local knowledge enable us to reconstruct a volumetric idea of it.



   When looking at a series of objects that are far apart from one another and are arranged on a straight line that is far away from us, the changes in convergence on the transition from one object to another become smaller, the further these objects are. From a certain distance on, these changes become so small that our perception of the relief ceases entirely. It is therefore an advantage to place the objects as close as possible to us in order to increase the value of these changes in convergence.



   The angle of view, which then corresponds to the average eonvergence angle, determines the distance at which we believe we can see the objects in the stereoscope. The choice of this angle is entirely up to us. provided that 15 @ are not exceeded; We have decided on an angle of 12¯, which corresponds to a distance of about 32 2 cm for eyes at a distance of 65 mm.



  This is the normalp distance of near accommodation, where @ the angle of 12¯ also corresponds to a normal convergence that can be maintained for a long time without fatigue.



   In the accompanying drawings, the method known per se is illustrated by FIGS. 1-4, which is based on the use of the device according to the invention. In a and b, FIG. 1 shows two X-ray recordings of the objects A, B, C and R, with the films 1 and 2 occupying the same position with respect to the objects. Since the angles of incidence of the light rays on the two films are different, the distribution of the projection points on films 1 and 2 is also different.



   An angle greater than 12¯ has been chosen to provide a better overview of the drawings.



   Figure 2 shows films 1 and 2 mounted in a stereoscope while being viewed through the left and right eyes simultaneously.



   When passing through A 'and A, the viewing angle of the eyes is "larger than through B' and B" and smaller than through R 'and R ". These deviations in convergence give the impression of relief, and we see Av, Sv, Cv and Xv the virtual image of the objects placed in the room.



   Fig. 3 shows two X-ray recordings at an angle of 12, while observing the ratios customary in dentistry.



   The distance L of the robre is so great in relation to the distance between the objects and the film as well as to the film surface F1 itself that the devices, the AA 'BB' and AA "BB" can be regarded as parallel to each other. All the more so as the beam exit center is not a point but a surface.



   This parallelism results in similar triangles in A A'A "and B B'B".

   It has already been proposed to attach an external marker to facilitate the x-ray stereophotogrammetric measurements. The formula proposed for calculating the distances is, however, complicated and time-consuming.

   The recording of stereoscopic x-ray images, which are intended to enable good optical reconstruction and precise stereogrammetric measurements at the same time, naturally requires extreme accuracy. To achieve this goal, the following conditions must be met:
1. The films should remain flat to avoid tension and deformation.



   2. The second film should occupy exactly the same position in the mouth as the first.



   3. The sight angle should be 12 ".



   4. The line that cuts this angle in two equal halves should be perpendicular to the film.



   5. The two sights should take place in the same plane, in order to enable binocular overlapping.



   6. The edge of the film should run parallel to the plane of the sight to make it easier to insert into the stereoscope.



   These conditions can only be made possible with the use of a special apparatus by means of which the second film can be attached in the same position as the first film, the sighting being independent of the patient's movements between the two exposures.



   The present invention is intended to create an apparatus which makes it possible to meet these technical conditions.



     The. The invention consists in that a support element has a film carrier, a marker and a beam director, and is connected in an articulated manner to an arm which can be coupled to the organ to be fastened in the mouth.

   The saddle 10 (Fig. 5) is filled with plastic impression material or with plaster of paris and on the jaw side opposite the tooth to be photographed.In the saddle 10, a cage 11 is mounted, which has a square 12 on the inside of the saddle. The connecting arm 14 (FIG. 7) is provided on its square part 15 with square bars 16 which grip over the bolt 13 of the saddle 10. The lugs 16 are arranged according to the desired length of the connection between the support bracket 19 and the saddle 10.



  The clamping sleeve 17 has a square base and therefore cannot be rotated. It is pressed together by the union nut 18. A ball 20 fastened to the support bracket 19 engages in the clamping sleeve (FIG. 12). whereby the support bracket 19 can be connected to the connecting arm 14 in any position. The support bracket 19 serves as a carrier for the film carrier 21, the marker pointer 23 and the beam judge 27.At one end of the support bracket 19, which is hinged to the connecting arm 14, the film carrier 21 is attached, while a slide 22 is attached to the other end is attached, which carries the pointer 23.

   An angled bent arm 25 is articulated on the slide 22, pivotable about the pin 24, and the jet straightener 27 is pushed onto the bent part 26 of this arm, which has a square cross section.    The pivoting of the arm 25 serves to move the beam director out of the area of the exposure beams when the film is exposed.



  The arm 25 is held in both positions by the spring 34. The so-called Beyele's marker pointer 23 is arranged on the slide 22 at a distance of 60 mm from the plane of the film. This marker has a tip about 1 mm in diameter, which is marked on the film and serves as a reference point for the stereophotogrammetric evaluation. An adjustability of the beam director from the horizontal plane into the vertical plane is provided in order to also make two recordings in the vertical plane, as indicated in phantom in FIG. 12, which is made possible by the square eyelet 28 and the square mandrel 26 of the arm 25.



   The film carriers are provided in different types in order to be able to adapt to the anatomical conditions.



   For example, Fig. 8 shows a film carrier for the standard film. The film cassette is inserted into a groove 36 which is arranged on three sides of the frame. In each case two parallel film edges are in one position of the beam director parallel to the plane of the same.
