Description

Die Erfindung betrifft ein Gelenkimplantat, bei dem zwei je in mindestens einen Knochen einsetzbare Verankerungsteile über mindestens einen Gelenkteil miteinander verbunden sind, wobei die Verbindung des Gelenkteils mit mindestens einem der Verankerungsteile als Steckverbindung ausgebildet ist.
Bei natürlichen Gelenken, insbesondere solchen, die — wie Ellenbogen- und Fingergelenke - scharniergelenkähnlich aufgebaut sind, kann die «momentane» physiologische Drehachse in gewissem Umfang relativ zu den das Gelenk verbindenden Knochen wandern und sich jeweils so einstellen, wie es für die Kraftwirkungen auf Muskeln und Bänder optimal ist. Bei bisherigen Implantaten der eingangs genannten Art (CH-PS 541 963) ist dagegen nach dem Einsetzen der Verankerungsteile in den Knochen die Drehachse starr fixiert. Jede Abweichung dieser Fixierung von der Lage der natürlichen Achse ergibt eine unnatürliche Belastung der Muskeln und Bänder; nicht physiologisch richtig verlaufende Kraftrichtungen belasten darüber hinaus die Verankerungen in den Knochen und führen mit der Zeit zu deren Lockerungen. Weiterhin kann diese bekannte Prothese etwaigen Zugbelastungen in ihrer Längsrichtung nicht nachgeben.
Um die «Einstell»-Möglichkeiten für die Drehachse eines Implantats zu erhöhen, wäre es denkbar, die Steckverbindung bzw. -Verbindungen zwischen den Verankerungsteilen und dem Gelenkteil mit grossem Spiel zu versehen. Eine derartige Konstruktion ergäbe jedoch erhebliche, ebenfalls unphysiologische Verdrehungsmöglichkeiten in Richtung der Längsachse der Steckverbindung sowie Verschiebungsmöglichkeiten im wesentlichen senkrecht zu dieser Achse, die unerwünschte Schwerbelastungen für die Bänder bewirkten.
Aufgabe der Erfindung ist es daher, eine physiologisch möglichst weitgehend richtige Einstellmöglichkeit für die Drehachse zu schaffen, wobei bei relativ kleinen Verdrehungsmöglichkeiten um die im wesentlichen in Richtung der Verankerung verlaufenden Längsachse und bei relativ geringem Spiel relativ grosse Kipp- oder Pendelbewegungen in der Steckverbindung des Implantats möglich sind. Diese Aufgabe wird erfindungsgemäss dadurch gelöst, dass die Steckverbindung zum einen aus einem im Querschnitt rotationssymmetriefreien, mit Spiel in eine Einstecköffnung einsteckbaren Zapfen besteht, dass zum anderen die Einstecköffnung in einem Teilbereich ihrer Länge einen an die Form des Zapfens angepassten engsten Querschnitt aufweist, und dass sich, beidseits an diesen Bereich anschliessend, dieser Querschnitt allseitig konisch nach aussen erweitert.
Querschnittsform des Zapfens und der Einstecköffnung schränken die Verdrehungsmöglichkeit der neuartigen Steckverbindung um ihre Längsachse sehr weitgehend ein; der Teilbereich mit dem engsten Querschnitt der Einstecköffnung stellt eine Führung für den auch in Längsrichtung nachgiebigen Zapfen dar, während die konischen Erweiterungen Kipp- oder Pendelbewegungen des Zapfens um diesen Teilbereich ermöglichen. Auf diese Weise sind eine Vielzahl von Einstellmöglichkeiten für die verschiedenen Teile des Implantats relativ zueinander gewährleistet, obwohl infolge der Führung des Zapfens in dem engsten Querschnitt der Einstecköffnung nur ein geringes Spiel für die Zapfenachse und damit für die Relativstellung des Gelenkteils zu den Verankerungsteilen bzw. des einen Verankerungsteils zum anderen, mit dem Gelenkteil drehbar verbundenen Verankerungsteil vorhanden ist.
Eine sichere Führung des Zapfens in der Einstecköffnung auch noch bei seinem teilweisen Herausgleiten unter Zugbelastung kann erreicht werden, wenn die axiale Länge des eingesteckten Zapfens den einsteckseitigen axialen Abstand des engsten Querschnitts der Einstecköffnung überragt. Weiterhin kann es vorteilhaft sein, wenn die Einstecköffnung in einem Zwischenstück angeordnet ist, das seinerseits im Passsitz in den Gelenkteil oder den Verankerungsteil eingesetzt ist, wobei zusätzlich das Zwischenstück aus einem Material bestehen kann, das elastischer ist als dasjenige für den Zapfen, den Gelenkteil und/oder die Verankerungsteile. Schliesslich ist es zweckmässig, die Eindringtiefe des Zapfens in die Einstecköffnung zu begrenzen.
Im folgenden wird die Erfindung anhand von Ausführungsbeispielen im Zusammenhang mit der Zeichnung näher erläutert:
Fig. 1 zeigt in einer Seitenansicht ein Ellenbogengelenk, bei dem die neuartige Steckverbindung auseinandergezogen dargestellt ist,
Fig. 2 gibt in grösserem Massstab einen Schnitt durch die Einstecköffnung und den eingesteckten Zapfen nach Fig. 1 wieder.

Fig. 4 schliesslich ist eine teilweise im Schnitt dargestellte Seitenansicht eines vergrössert dargestellten Fingergelenks, bei dem der Gelenkteil mit beiden Verankerungsteilen über die neuartige Steckverbindung verbunden ist.
Das Gelenkimplantat nach Fig. 1 für ein Ellenbogengelenk setzt sich zusammen aus einem ersten, im Oberarmknochen zu verankernden Verankerungsteil 1, dessen distales Ende als gabelartiges Scharnierteil ausgebildet ist, was nicht ausdrücklich gezeigt ist. In dieses Scharnierteil greift in bekannter Weise ein Gelenkteil 2 ein, das durch eine Drehachse 3 drehbar gehalten ist. Das Gelenkteil 2 besteht aus zwei mit ihren Längsachsen senkrecht aufeinanderstehenden, räumlich gegeneinander versetzten, zu einem Stück vereinigten Hohlkörpern. Der eine Hohlkörper nimmt die Drehachse 3 auf und hat daher eine entsprechende kreisförmige Bohrung 4, während der Hohlraum 6 des anderen Teilkörpers des Gelenkteils 2 im Schnitt die Form eines an den Schmalseiten abgerundeten Rechtecks hat. In ihn wird im Passsitz ein Zwischenstück 7 eingesetzt, dessen äussere Begrenzung, die der Form des Hohlraumes 6 entspricht, Fig. 3 zeigt. Ein flanschartiger Rand 5 verhindert ein zu tiefes Einsetzen des Zwischenstücks 7 in den Hohlraum 6 des Gelenkteils 2.
Im Zwischenstück 7 ist eine Einstecköffnung 8 für einen Zapfen 9 vorgesehen, der seinerseits auf dem proximalen Ende eines in der Speiche verankerbaren, distalen Verankerungsteils 10 sitzt. Wie aus Fig. 3 zu ersehen ist, hat der Zapfen 9 im Querschnitt im wesentlichen ebenfalls die Form eines abgerundeten Rechtecks, so dass er rotationssymmetriefrei, d. h. nach seiner Montage in der Öffnung 8 gegen unbeabsichtigte Drehung um seine Längsachse gesichert ist.
Vom Schaft des Verankerungsteils 10 ist der Zapfen 9 durch einen Flansch 11 getrennt, der zum einen die Eindringtiefe des Zapfens 9 in die Öffnung 8 durch eine Anschlagfläche 12 und zum anderen die Eindringtiefe des Verankerungsteils 10 in die Speiche durch eine Anschlagfläche 13 begrenzt.
Verankerungsteile 1 und 10 und Gelenkteil 2 sind beispielsweise aus einem für die Implantat-Technik gebräuchlichen, metallischen Werkstoff gefertigt, während in Verbindung mit diesen Metallteilen das Zwischenstück 7 mit Vorteil aus - ebenfalls in der Implantat-Technik viel verwendetem — hochmolekularem Polyäthylen oder einem anderen geeigneten Kunststoff besteht; es ist jedoch auch möglich, das Implantat ganz oder teilweise aus Biokeramik zu fertigen. Besteht das ganze Implantat aus einem Werkstoff, so ist es weiterhin möglich, die Einstecköffnung 8 direkt in den Gelenkteil 2 hineinzuarbeiten.
Die Form der Einstecköffnung 8 zeigt Fig. 2. In einem Bereich a, der in seiner Längsausdehnung höchstens V4 der Länge des Zapfens 9 erreichen soll, ist die Form der Öffnung 8 erfindungsgemäss eng, aber mit Spiel an die Form des Zapfens 9 angepasst. An diesen Teilbereich a, der bei Beispiel 1

etwa in der Mitte der Einstecköffnung 8 angeordnet ist, schliessen sich, nach beiden Seiten sich allseitig konisch erweiternde Bereiche b und c an, so dass der im Bereich a bei einer Bewegung geführte Zapfen 9 Kipp- und Pendelbewegungen um diesen Bereich als «Drehpunkt» ausführen kann. Dabei ist es unter Umständen zweckmässig, wenn dieser «Drehpunkt» bei den verschiedenen Gelenken relativ zu der Drehachse des Gelenkes eine unterschiedliche Lage hat, was sich in gewissem Umfang durch unterschiedliche Abstände des Bereiches a vom einsteckseitigen Ende der Eintrittsöffnung erreichen lässt.
Der Abstand b des Bereichs a vom einsteckseitigen Ende der Öffnung 8 und die Länge des Zapfens 9 sind so aufeinander abgestimmt, dass bei vollständig eingestecktem Zapfen 9 der Bereich a höchstens etwa im mittleren Bereich der Zapfenhöhe liegt; damit wird erreicht, dass ein durch Zugbelastung teilweise aus der Öffnung 8 herausgleitender Zapfen 9 im Bereich a noch in ausreichendem Masse geführt ist.
Bei dem stark vergrösserten Fingergelenk nach Fig. 4 sitzen gleich ausgebildete Verankerungsteile 21 und 21' in je einem nur schematisch angedeuteten Fingerknochen 20 und 20'. In beide Teile 21 und 21' ist je über die neuartige Steckverbindung lösbar ein Gelenkteil eingesteckt, dessen beide Teile 22 und 22' über eine Drehachse 23 drehbar miteinander verbunden sind. Zur Verankerung in den Teilen 21 und 21'
weisen beide Teilstücke 22 und 22' daher je einen Zapfen 29 und 29' auf, der in eine Einstecköffnung 28 bzw. 28' von Zwischenkörpern 27 bzw. 27' eingreift. Die Körper 27 und 27' sind wiederum in Hohlräume 26 und 26' eingepasst, die in den Verankerungsteilen 21 und 21' vorgesehen sind.
Flanschartige Ränder 25 und 25' der Zwischenstücke 27 und 27' und Anschlagflächen 32 und 32' in den Gelenkteilstücken 22 und 22' verhindern ein zu tiefes Einsetzen der Zwischenstücke 27 und 27' in die Hohlräume 26 und 26' bzw. begrenzen die Eindringtiefe der Zapfen 29 und 29' in den Einstecköffnungen 28 und 28'.
In Abwandlung zum ersten Ausführungsbeispiel sind die Bereiche a der engsten Querschnitte der Einstecköffnungen 28 und 28' nahe den einsteckseitigen Enden dieser Öffnungen angeordnet, wodurch der Bereich b zu einer trompetenartigen, relativ stark gekrümmten Erweiterung entartet, während der Bereich c etwa die in Fig. 2 vergrössert gezeigte Form beibehält. Mit der abgewandelten Ausführungsform der Erfindung lässt sich die Baulänge der Steckverbindung wirksam verkürzen; es ergibt sich aber herstellungsmässig ein etwas höherer Aufwand.
Die für das beschriebene Fingergelenk verwendeten Materialien entsprechen denjenigen, die im Zusammenhang mit dem ersten Beispiel erwähnt worden sind.
Die Formgebung der Einstecköffnung 8, 28 bzw. 28' kann in beiden Fällen beispielsweise durch eine materialabhebende Bearbeitung mit Hilfe eines Fräsers aus dem vollen Material heraus erfolgen.

1. Gelenkimplantat, bei dem zwei je in mindestens einen Knochen einsetzbare Verankerungsteile über mindestens einen Gelenkteil miteinander verbunden sind, wobei die Verbindung des Gelenkteils mit mindestens einem der Verankerungsteile als Steckverbindung ausgebildet ist, dadurch gekennzeichnet, dass die Steckverbindung zum einen aus einem im Querschnitt rotationssymmetriefreien. mit Spiel in eine Einstecköffnung einsteckbaren Zapfen (9, 29, 29') besteht, dass zum anderen die Einstecköffnung (8, 28, 28') in einem Teilbereich (a) ihrer Länge einen an die Form des Zapfens (9, 29, 29') angepassten engsten Querschnitt aufweist und dass sich, beidseits an diesen Bereich (a) anschliessend, dieser Querschnitt allseitig konisch nach aussen erweitert.
2. Gelenkimplantat nach Anspruch 1, dadurch gekennzeichnet, dass die axiale Länge (d) des eingesteckten Zapfens (9, 29, 29') den einsteckseitigen axialen Abstand (b) des engsten Querschnitts (a) der Einstecköffnung (8, 28, 28') überragt.

3. Gelenkimplantat nach Anspruch 1, dadurch gekennzeichnet, dass die Einstecköffnung (8, 28, 28') in einem Zwischenstück (7, 27, 27') angeordnet ist, das seinerseits im Passsitz in den Gelenkteil (2) oder den Verankerungsteil (21,21') eingesetzt ist.
4. Gelenkimplantat nach Anspruch 1 oder 3, dadurch gekennzeichnet, dass die Eindringtiefe des Zapfens (9, 29, 29') in die Einstecköffnung (8, 28, 28') begrenzt ist.
5. Gelenkimplantat nach Anspruch 3, dadurch gekennzeichnet, dass das Zwischenstück (7, 27, 27') aus einem Material besteht, das elastischer ist als dasjenige für den Zapfen (9, 29, 29'), den Gelenkteil (2, 22, 22') und/oder die Verankerungsteile (1, 10, 21, 21').
6. Gelenkimplantat nach Anspruch 1, dadurch gekennzeichnet, dass die Gelenkverbindung scharniergelenkartig ausgebildet ist.
7. Gelenkimplantat nach Anspruch 1, dadurch gekennzeichnet, dass der engste Querschnitt (a) mindestens nahezu in der Mitte der axialen Länge der Einstecköffnung (8, 28, 28') angeordnet ist und dass sich diese nach beiden Seiten symmetrisch erweitert.
8. Gelenkimplantat nach Anspruch 1, dadurch gekennzeichnet, dass der engste Querschnitt (a) in der Einstecköffnung (8, 28, 28') nahe dem einsteckseitigen Ende angeordnet ist und sich die Einstecköffnung (8, 28, 28') nach beiden Seiten verschieden stark erweitert.
9. Gelenkimplantat nach Anspruch 1, dadurch gekennzeichnet, dass die axiale Länge des engsten Querschnitts (a) =s '/4 der Zapfenlänge (d) ist.
