Description
Die Erfindung bezieht sich auf Mund- und Zahnpflegemittel mit einer verstärkten antikariogenen Wirkung, die gleichfalls imstande sind, schon angegriffene Teile von Gebisselementen ganz oder teilweise wiederherzustellen.
In dieser Anmeldung werden unter Mund- und Zahnpflegemitteln die Produkte verstanden, die während einer verhältnismässig kurzen Zeit in dem Mund bleiben, dabei intensiv mit nahezu allen Flächen der Gebisselemente in Berührung gebracht werden und sodann entfernt werden.
Beispiele solcher Produkte sind Zahnpasten, prophylaktische Pasten, Zahnpoliermittel, Mundwässer, Applikationsflüssigkeiten und -gele, spezifische Kaugummis usw.
Es ist schon lange bekannt, dass die Zugabe von Fluorverbindungen zu solchen Mund- und Zahnpflegemitteln den Kariesfortschritt signifikant vermindern kann.
Beim Einbau der Fluorverbindungen trat jedoch das Problem auf, dass diese Stoffe, wie es sich zeigte, mit einer Anzahl oft angewendeter Basisrohstoffe kaum oder wenig verträglich waren.
Zumal bei der Anwendung in Zahnpaste stiess man auf viele Probleme. Zum Erhalten einer guten Verträglichkeit zwischen der Fluorverbindung einerseits und den übrigen Zutaten andererseits wurden neue Fluorverbindungen sowie neue Rohstoffe (namentlich Poliermittel) entwickelt.
Eine fundamentale Untersuchung nach der Verträglichkeit einiger Fluorverbindungen mit den gebräuchlichsten Ingredienzien für Zahnpaste wurde im Jahre 1962 von Ericsson durchgeführt.
Seine Untersuchung richtete sich namentlich auf eine damals ziemlich neue Fluorverbindung, und zwar das Natriummonofluorphosphat.
In diesem Stoff ist das Fluorion komplexgebunden und dadurch in bezug auf die übrigen Komponenten weniger reaktiv.
Weil das Natriummonofluorphosphat eine besonders gute antikariogene Wirkung besitzt, wird diese Fluorverbindung jetzt am meisten in Mund- und Zahnpflegemitteln angewendet. Andere Fluorverbindungen, und zwar NaF und SnF2 haben sich bei klinischen Versuchen auch als wirksam erwiesen, aber diese Fluorverbindungen konnten erst dann angewendet werden, nachdem man spezielle Rohstoffe entwickelt hatte, die mit diesen Fluorverbindungen kombiniert werden konnten. Dabei lassen sich namentlich die Poliermittel nennen, wie spezielle Kunststoff-Poliermittel für Mund-und Zahnpflegemittel, die NaF enthalten, und spezielle Polymere und Pyrophosphate für Mund- und Zahnpflegemittel, welche SnF2 enthalten.
Neben diesen drei am meisten angewendeten Fluorverbindungen sind viele andere Fluorverbindungen bekannt und brauchbar. Es können genannt werden: Fluoridverbindungen wie Aluminiumfluorid, Zäsiumfluorid, Cuprifluorid, Ferrifluorid, Magnesiumfluorid, Fluorphosphate wie Mono-fluorphosphate, Difluorphosphate und Polyfluorphosphate, z.B. Calciummonofluorphosphat, Natriumdifluorphosphat, Vanadiummonofluorphosphat, Lithiummonofluorpoly-phosphat.
Andere brauchbare anorganische Fluorverbindungen sind Hexafluorsilikate, Silicofluoridsäure und Zirkongerma-niumfluoride, während auch organische Fluorverbindungen wie Aminhydrofluoride, Imidazolhydrofluoride, quartäre Ammoniumfluoride, Aminofluorphosphate, Hydrofluoride von Aminosäurederivaten und fluoridierte Biguanidverbin-dungen in Betracht kommen.
Bei der wissenschaftlichen Untersuchung nach der Wirkung der Fluorverbindungen hat es sich gezeigt, dass verschiedene Reaktionsmechanismen eine Rolle spielen.
Die klassische Erklärung für die kariostatische Wirkung von Fluoridverbindungen ist, dass Hydroxyapatit mit dem Fluoridion reagiert und dabei in fluoridiertem Hydroxyapatit umgesetzt wird.
Dieser Stoff ist in saurem Milieu weniger löslich als Hydroxyapatit. Dadurch ist der Zahn nach einer Fluorapplikation besser gegen die Säurestösse geschützt, die den Karies-prozess initiieren.
Neben diesem Wirkungsmechanismus sind auch andere vorgeschlagen, wie die Blockierung von Schraubdislokationen in dem Apatitkristall, «kink-site-poisening», usw.
Eine wichtige Theorie, die in den letzten Jahren wissen-schaflich weiter unterbaut wurde, geht von dem Einfluss des Fluoridions auf den Remineralisationsprozess aus.
Unter Remineralisation der Zahnglasur versteht man die Erscheinung, dass aus Stoffen, die in dem Speichel von Natur vorkommen, sich Hydroxyapatit bilden kann, und zwar an den Stellen, wo der Hydroxyapatit in einer vorhergehenden Phase verschwunden ist.
In dem Mund besteht ein natürliches Gleichgewicht zwischen einerseits dem Hydroxyapatit, der aus dem Zahn in Lösung geht, und andererseits Hydroxyapatit, der sich aus dem Speichel auf dem oder in den Zahn bildet. Dieses Gleichgewicht verschiebt sich fortwährend. Dieses wird u.a. durch die Ernährungsgewohnheiten und den physischen Zustand bestimmt. Liegt das Gleichgewicht derart, dass Hy-

droxyapatit in Lösung geht, dann spricht man von Déminéralisation. Es entsteht dann Karies. Liegt das Gleichgewicht derart, dass Hydroxyapatit gebildet wird, dann spricht man von Remineralisation. Durch Remineralisation kann man auf natürlichem Weg anfangenden Zahnzerfall wiederher- s stellen.
Es wurde festgestellt, dass Fluorverbindungen, auch in niedrigen Konzentrationen die Remineralisation fördern. Möglicherweise gibt dieser Wirkungsmechanismus sogar einen besseren Schutz gegen Zahnzerfall als die eher genannte 10 Herabsetzung der Säurelöslichkeit durch Fluorapatitbildung.
Das Mass, in dem die unterschiedlichen Fluorverbindungen den Remineralisationsprozess fördern, ist stark verschieden. Ebenso ist die Wirksamkeit des Einbaus von Fluorid bei Applikation unterschiedlicher Fluorverbindungen ver- 15 schieden. Es ist erwünscht, dass der optimale Effekt der bekannten Schutzmechanismen erhalten wird, und dies kann u.a. durch Anwendung einer Kombination von zwei oder mehreren Fluorverbindungen erreicht werden.

Aus unserer Untersuchung ist hervorgegangen, dass die Kombination eines Fluorids, wie z.B. Natriumfluorid, zusammen mit einem Monofluorphosphat, wie z. B. Natriummonofluorphosphat, die besten Resultate gibt.
Das Natriumfluorid/Natriummonofluorphosphat-Verhältnis von 3 : 1 (als F") wird besonders bevorzugt.
Zur Bestimmung des optimalen Fluoridverhältnisses wurde u.a. eine RES-Messung angewendet (Réduction Ena-mel Solubility). Ein gründlich gereinigter Rinderzahn wird dabei in einer Zahnpaste-Suspension (1:3 verdünnt) während 24 Stunden bei 37 °C behandelt. Nach Abspülen und Reinigen dieses Zahnes wird er 24 Stunden in einem Azetatpuffer gehalten (pH 5,0), wonach gemessen wird, wieviel (imol Calcium und wieviel p.mol Phosphat per cm2 Glasur in Lösung gegangen sind. Als Blindkontrolle wird ein mit Wasser behandelter Zahn angewendet. Für eine Paste, die 1500 ppm Fluor (berechnet als F) enthielt, wurden die nachstehenden Reduktionen gefunden:

Es ist bekannt, dass zyklische Phosphate, namentlich zyklische Trimetaphosphate, die kariostatische Wirkung von Monofluorphosphaten synergistisch verstärken.
Zum Erhalten einer optimalen Wirkung des Monofluor-phosphates in den Mund- und Zahnpflegemitteln ist es daher erwünscht, eine geringe Menge an zyklischem Phosphat zuzugeben. Vorzugsweise wird dazu Natriumtrimetaphosphat angewendet, das, in Kombination mit Natriummonofluorphosphat in einer Zahnpaste, klinisch untersucht wurde, wobei sehr hohe Kariesreduktionen gefunden wurden.
Die üblichsten Konzentrationen liegen zwischen 0,1 und 5%, und vorzugsweise wird zwischen 0,25 und 2% zyklisches Phosphat angewendet.
Die Gesamtmenge an Fluorverbindungen, die man den Mund- und Zahnpflegemitteln gemäss der Erfindung zugeben kann, hängt namentlich von dem betreffenden Produkt ab, in dem sie angewendet werden. Als Kriterium für die maximale Fluoridkonzentration, die angewendet werden kann, gilt dabei die mittlere Menge an Produkt, die pro Behandlung angewendet wird, sowie die Anwendungsfrequenz. Die Fluoridkonzentration soll derart gewählt werden, dass die toxische Dosis nicht überschritten wird.
In Zahnpasten beträgt die Gesamtmenge an Fluorverbindungen meistens die Menge, die mit 1000 bis 1500 ppm F~ übereinstimmt. In prophylaktischen Pasten liegt diese Konzentration etwa 5mal höher, während Applikationsflüssigkeiten und -gele meistens 10 000 bis 15 000 ppm F~ enthalten.
Bei Fluoridspülflüssigkeiten, die zum täglichen Gebrauch bestimmt sind, wird die Gesamtmenge an Fluorid wieder niedriger sein als bei Zahnpaste. Eine brauchbare Konzentration bei solchen Flüssigkeiten beträgt 250 à 500 ppm F-.
Insofern dies jetzt beurteilt werden kann, scheinen die Konzentrationen an Fluorid, die verabreicht werden, etwas zu hoch zu sein, um eine optimale Remineralisation zu erhalten. Es ist jedoch wichtig, dass eine solche Menge an Fluorid angeboten wird, dass sich auf dem Zahn ein Puffervorrat bildet, meistens in Form von Calciumfluorid - der imstande ist, das Fluor wieder allmählich dem Speichel abzugeben, wodurch längere Zeit die erforderliche, ziemlich niedrige Fluoridkonzentration in dem Speichel vorhanden ist, um eine op-35 timale Remineralisation bewirken zu können.
Es ist von wesentlicher Bedeutung für remineralisierend wirkende Mund- und Zahnpflegemittel, dass sie imstande sind, neben Fluoridionen sowie Calciumionen wie Phosphationen abzugeben.
In den erfindungsgemässen Präparaten werden zweierlei Arten Phosphate angewendet. Zunächst sind cyclische Phosphate vorhanden, welche die Wirkung der Fluorverbindung synergistisch verstärken, weiter sind lineare Phosphate zugegeben, um den Remineralisationsprozess zu verstärken.
Geeignete lineare Phosphate sind Phosphorsäure, Na-triumorthophosphat, Natriumtripolyphosphat, Natriumte-trapolyphosphat, Natriumpyrophosphat, das lineare Na-triumhexametaphosphat u.dgl.
Phosphate mit einer Wasserlöslichkeit von > 5% sind besonders brauchbar. Die linearen Phosphate werden meistens, jedoch nicht notwendigerweise, in einer Konzentration unterhalb 5% angewendet. Vorzugsweise in einem stöchio-metrischen Verhältnis zu Calcium, wie dieses in Hydroxyapatit vorkommt, d.h. 1,66 Ca: 1 P.
Auch die Calciumionen sind für den Remineralisationsprozess wesentlich. Sogar mehr als die Phosphationen, weil bei dem Demineralisationsprozess das Calcium als erstes aus dem Hydroxyapatit in Lösung geht.
Demineralisierte Glasur, sowie leicht kariöse Glasur enthält daher meistens gegenüber einem herabgesetzten Gehalt an Phosphat einen viel stärker herabgesetzten Gehalt an Calciumionen.
Die Verabreichung von Calciumionen durch Mund- und Zahnpflegemittel in einer wirksamen Form ist jedoch nicht einfach. Calciumionen sind ziemlich reaktiv mit den üblichen Ingredienzien für diese Produkte und dadurch, dass sie mit diesen Ingredienzien eine Verbindung bilden, können die Calciumionen ihre Wirksamkeit nahezu völlig verlieren.

So ist es nicht ohne weiteres möglich, Calcium neben Phosphationen den Mund- und Zahnpflegemitteln zuzugeben, weil beide miteinander reagieren unter Bildung des kaum wirksamen Calciumphosphates. Ebenso ist es nicht ohne weiteres möglich, Calciumionen mit Fluoridionen zu kombinieren, weil dann das nahezu unwirksame Calciumfluorid niedergeschlagen wird. Das Problem, Calciumionen in einer wirksamen Form den Mund- und Zahnpflegemitteln zuzugeben, wird noch grösser, wenn Natriumtrimetaphos-phat vorhanden ist, weil diese Verbindung ein sehr guter Calciumbinder ist.
Ein Überschuss einer calciumhaltigen Verbindimg ist also nicht gut möglich, weil dann das Fluorid und das Phosphat inaktiviert werden, aber eine niedrige Konzentration eines Calciumdonors ist auch problematisch, weil dann all das Calcium von dem Trimetaphosphat weggefangen wird. Auch andere Ingredienzien können das Calcium inaktivieren, aber obige sind genannt worden, weil dabei der nachteilige Effekt doppelt ist.
Das Calcium macht die kariostatischen Phosphat- und Fluorid-Ionen unwirksam und umgekehrt machen diese Stoffe die remineralisierend wirkenden Calciumionen unwirksam.
Überraschenderweise wurde nun gefunden, dass spezifische Calciumkomplexe imstande sind, das Calcium in einer aktiven Form in Mund- und Zahnpflegemitteln zu behalten, ohne dass dabei andere aktive Stoffe inaktiviert werden. Die Aufgabe der Erfindung ist, Mund- und Zahnpflegemittel mit optimalen Remineralisationseigenschaften herzustellen, die sowohl Calciumionen wie Phosphationen und Fluorverbindungen in aktiver Form enthalten.
Gefunden wurde, dass Calciumsalze von organischen Säuren, wie die Calciumsalze von Zitronensäure, Adipinsäure und Weinsäure den gestellten Forderungen entsprechen. Es hat sich gezeigt, dass als Calciumdonor auch mit Calcium angereicherte Minerale, wie Calciumzeolith und Calcium-apophyllit brauchbar sind. Die Calciumverbindungen werden vorzugsweise in einer Menge von 0,05-5 Gew.% angewendet.
Weiter wurde gefunden, dass das pH der Mund- und Zahnpflegemittel den Remineralisationsprozess beeinflusst. Ein optimaler Austausch ist möglich in schwachsaurem Milieu. Die besten Resultate werden bei einem pH von 6 bis 7 gefunden.
Die Wirksamkeit der verschiedenen Mund- und Zahnpflegemittel wurde mit Hilfe von Grenzwinkelmessungen an der Zahnoberfläche bestimmt. Mit dieser Methode wird das Mass der Oberflächenremineralisation zahlenmässig festgesetzt.
Die Grenzwinkelmessung umfasst die Bestimmung des Grenzwinkels eines Wassertropfens, der auf einem gut gereinigten, egal flach geschliffenen Zahnglasurteil angebracht wird. Die erste Messung wird vor dem Anfang des Versuchs durchgeführt. Eine zweite Bestimmung erfolgt, nachdem der Zahn 10 Minuten mit einer 0,1 M Milchsäure/Hydroxy-äthyl-cellulose-Lösung demineralisiert ist. Durch diese Déminéralisation vermindert der Grenzwinkel durchschnittlich mit 30 bis 35°. Sodann wird der Zahn während 30 Minuten mit einer 30%igen Suspension von Zahnpaste, oder aber eine Lösung eines der anderen Mund- und Zahnpflegemittel, in Berührung gebracht, wonach wiederum der Grenzwinkel gemessen wird.
Zur Ermittlung des Effektes, der nach der Behandlung verbleibt, wird der Zahn sodann in eine künstliche Speichellösung gelegt, wobei zu bestimmten Zeiten über die Grenzwinkelmessung der Fortschritt der Oberflächenremineralisation bestimmt wird.
Ausgehend von einer Paste, wie in Beispiel I erwähnt,
werden die nachstehenden Pasten zusammengesetzt.
A. Rezeptur nach Beispiel I, jedoch ohne aktive Komponenten
B. Wie Paste A mit Natriummonofluorphosphat.
C. Wie Paste A mit Natriummonofluorphosphat und Na-triumfluorid.
D. Wie C mit Natriumphosphat und Phosphorsäure.
E. Paste nach C mit Natriumtrimetaphosphat.
F. Zahnpaste nach C mit Natriumphosphat/Phosphorsäure und Calciumzitrat.
G. Zahnpaste nach C mit Natriummetaphosphat und Calciumzitrat.
H. Zahnpaste nach C mit Phosphat/Phosphorsäure und Natriumtrimetaphosphat und Calciumzitrat.
Resultate
1 : Änderung des Grenzwinkels nach Behandlung mit der zu untersuchenden Zahnpaste während 30 Minuten.
2: Änderung des Grenzwinkels nach Lagerung in einer künstlichen Speichellösung während 1 Stunde.
3: Die Änderung in dem Grenzwinkel, gemessen ab der Déminéralisation bis zur Behandlung mit künstlichem Speichel.

1. Mund- und Zahnpflegemittel mit antikariogener Wirkung, dadurch gekennzeichnet, dass es mindestens zwei Fluorverbindungen, ein cyclisches Phosphat, ein lösliches Salz, das der Zahnglasur Phosphationen abgeben kann und einen Stoff, welcher der Zahnglasur Calciumionen abgeben kann, enthält.
2. Mund- und Zahnpflegemittel nach Patentanspruch 1, dadurch gekennzeichnet, dass als Fluorverbindungen ein Fluorid und ein Monofluorphosphat gewählt werden.
3: 1 beträgt.
3. Mund- und Zahnpflegemittel nach Patentanspruch 2, dadurch gekennzeichnet, dass als Fluoridverbindungen Na-triumfluorid und als Monofluorphosphat Natriumphosphat gewählt werden.
4. Mund- und Zahnpflegemittel nach Patentanspruch 2, dadurch gekennzeichnet, dass als Phosphatverbindungen ein cyclisches Metaphosphat und ein lineares Phosphat gewählt werden.
5. Mund- und Zahnpflegemittel nach Patentanspruch 1, dadurch gekennzeichnet, dass dieses als Calciumionen abspaltende Verbindung ein Calciumsalz einer organischen Säure enthält.
6. Mund- und Zahnpflegemittel nach Patentanspruch 3, dadurch gekennzeichnet,' dass das Verhältnis von Natrium-fluorid zu Natriummonofluorphosphat, berechnet als F, zwischen 1:1 und 4:1 hegt.
7. Mund- und Zahnpflegemittel nach Patentanspruch 6, dadurch gekennzeichnet, dass das Verhältnis von Natrium-fluorid zu Natriummonofluorphosphat, berechnet als F,
8. Mund- und Zahnpflegemittel nach Patentanspruch 4, dadurch gekennzeichnet, dass als cyclisches Phosphat das Natriumtrimetaphosphat und als lineares Phosphat ein Or-thophosphat und/oder ein Tripolyphosphat gewählt wird.
9. Mund- und Zahnpflegemittel nach Patentanspruch 5, dadurch gekennzeichnet, dass als Calciumsalz einer organischen Säure Calciumcitrat, Calciumadipat oder Calciumtar-trat, oder aber eine Mischung von zwei oder mehreren dieser Stoffe gewählt wird.
