Descrizione
L'ibuprofene è notoriamente un prezioso farmaco antinfiammatorio, dotato di proprietà analgesiche e antipiretiche; il suo uso va rapidamente e largamente imponendosi e, a dosaggi appropriati, viene attualmente classificato fra ì farmaci di uso sicuro, non necessitanti, almeno sino a certi ben definiti dosaggi, di ricetta medica (farmaco da banco).
L'ibuprofene, o acido (±)-2-(p-isobutilfenil)-propionico (sinonimo: acido (±)-p-Ìsobutilidrotropico) ha la seguente formula di struttura:
E* stato descritto la T. Shiori e N. Kawai (J. Org. Chem. 43,2936 (1978)) e da J.T. Pinhey e B.A. Rowe (Tetrahedron Letters 2t, 965 (1980). L'indagine farmacologica condotta da Adams e al. risulta descritta negli Arch. Pharmacodyn. Ther. 178. 115 (1969). Una pubblicazione riassuntiva dei dati riguardanti l'ibuprofene è stata pubblicata nel 1975 (cfr. L. Cavallini e G. Lucchetti - Gazz. Med. It. 134. 7 (1975)). L'ibuprofene è rivendicato nel Brevetto inglese - BP 971 700, (1964) corrispondente a quelli americani - U.S. Patent 3 228 831 e 3 385 886 dei 1966 e 1968 assegnati alia ditta Boots Pure Drug. L'ibuprofene è un eccezionale antinfiammatorio non steroidale, a bassissima tossicità sistemica, e provvisto di interessanti proprietà analgesiche e antipiretiche. Questo farmaco si confronta favorevolmente con lo stesso acido acetilsalicilico sui quale ha il vantaggio di una minore tossicità topica, a livello del tratto gastrointestinale. Tuttavia, come con l'aspirina, ci si trova confrontati, nel caso di pazienti particolarmente sensibili, con fenomeni di tossicità a livello delia parete intestinale, specialmente quando il farmaco è somministrato in formulazioni solide, monolitiche (confetti e compresse).
L'impiego deli'ibuprofene per il trattamento degli stati infiammatori, ad esempio artrite reumatoide, rigonfiamento delle giunture, rigidità mattutina delie articolazioni, ma più ancora per quelli accompagnati da dolore e innalzamento della temperatura, richiederebbe di poter disporre di forme farmaceutiche liquide, facilmente deglutitali! anche da pazienti anziani che proprio i fenomeni artritici rendono particolarmente impacciati.
La realizzazione di una formulazione deli'ibuprofene in soluzione, non è oggi possibile per il pessimo gusto del prodotto e in particolare per il sapore amaro e per l'insopportabile bruciore in gola che compare al momento dell'assunzione e permane per un periodo prolungato.
Anche il tentativo di preparare compresse effervescenti palatabili non ha dato sino ad oggi risultati soddisfacenti. Nel brevetto europeo EP 0 228 164 A2 (8 luglio 1987), assegnato alla The Boots Go. di Nottingham (U.K.) si descrivono e rivendicano delle compresse effervescenti nelle quali l'ibuprofene è presente in forma indisciolta in sospensione, quest'ultima favorita da particolari eccipienti, quali il sodio lauriisolfato, il poliossietilensorbitano monolaurato e polimeri insolubili in acqua. II fatto che l'ibuprofene è presente in sospensione allo stato solido, indisciolto in acqua, impedisce l'eliminazione delle negative proprietà organolettiche sopracitate: il prodotto, in sospensione acquosa effervescente risulta, di fatto, totalmente non palatabile e di impiego pratico praticamente impossibile. Il farmaco antinfiammatorio secondo la presente invenzione, a base di ibuprofene elimina il sapore amaro, il bruciore in gola e la tossicità topica a livello dèlta parete intestinale nell'assunzione delle sue saluzioni acquose effervescenti. Esso è caratterizzato dalla parte caratterizzante della rivendicazione 1.
L'indagine sulle compresse e sui granulati effervescenti in generale, e su quelli che contengono come principio attivo l'ibuprofene in particolare, ha condotto secondo finvenzion, a risultati molto soddisfacenti ancorché imprevedibili. Infatti, operando con un opportuno rapporto fra acido citrico/bicarbonato di sodio e ibuprofene si sono potuti preparare, anche grazie all'impiego di opportune attrezzature, granulati e compresse effervescenti che posti in acqua - 100/200 mi - sviluppano anidride carbonica e forniscono una soluzione limpida. Le proporzioni di questi componenti sono date nella rivendicazione 1,
In particolare 200 mg di ibuprofene, 2100 mg di sodio bicarbonato e 500 mg di acido citrico, opportunamente mescolati e granulati a secco in un'adatta apparecchiatura (letto fluido), eventualmente compressi sino a formare compresse da 4,5 grammi, si sciolgono rapidamente e completamente in 100/200 mi di acqua a temperatura ambiente. Che si tratti di una vera e propria soluzione, ovviamente ottenuta attraverso la salificazione deli'ibuprofene, può essere facilmente verificato filtrando la soluzione su carta da filtro e misurando il contenuto di ibuprofene (sale sodico) nella fase acquosa, contenuto che risulta superiore al 98% del teorico.

condì - risulta di gusto del tutto tollerabile: in particolare non è amara e non provoca irritazione in gola. Ovviamente il granulare e le compresse possono esser ottenute anche Impiegando il sale sodico deli'ibuprofene. In questo caso 221,3 mg di ibuprofene sale sodico vengono opportunamene mescolati e granulati con 2035 mg dì sodio bicarbonato e 540 mg di acido citrico. Il dosaggio di 200 mg è quello di elezione per l'impiego del farmaco quale prodotto da banco. E' comunque possibile comprimere quantitativi maggiori e multipli del dosaggio di base al fine di ottenere formulazione con un contenuto fino a 800 mg di ibuprofene.
Ovviamente è possibile sciogliere due o più compresse nello stesso volume di acqua dì 100/200 mi, ottenendo una soluzione dotata di buona palatabilità.
L'aromatizzazione dei granulati, risp. delle compresse, permette di ottenere un prodotto dì eccezionale qualità. Gli aromi particolarmente efficienti sono la menta 3813 l'anice 3812, ma questo non costituisce una limitazione, altre essenze essendo egualmente efficaci. Infine, ma non per ultimo, con la somministrazione del princìpio attivo in soluzione si ottiene un'azione farmacodinamica più pronta e certo di più rapida insorgenza rispetto a quella ottenibile con le compresse (monoliti) tradizionali. Questo aspetto riveste particolare rilevanza nell'impiego deli'ibuprofene quale antidolorifico.
Teoricamente la tollerabilità locale, di per sè già buona, risulta ulteriomente incrementata impiegando soluzioni diluite, incapaci di produrre alte concentrazioni topiche del principio attivo.

Claims (6)
1. Farmaco antinfiammatorio a base di ibuprofene con eliminazione del sapore amaro, del bruciore in gola e della tossicità topica a livello della parete intestinale nell'assunzione delle sue soluzioni acquose effervescenti, caratterizzato dalie seguenti proporzioni fra i principali componenti: ibuprofene da mg 200 a mg 800; rispettivamente sale sodico di ibuprofene da mg 221,3 a mg 885,2; bicarbonato di sodio da gr. 2,100 a gr. 8,402; acido citrico dagr. 0,450 a gr. 1,800.
2. Farmaco secondo la rivendicazione 1 completamente e rapidamente solubile in un minimo quantitativo d'acqua, realizzato in compresse effervescenti, caratterizzato da ciò che ciascuna compressa contiene: ibuprofene da 200 a 800 mg bicarbonato di sodio da gr. 2,100 a gr. 8,402, acido citrico da gr. 0,450 a gr. 1,800 e acqua 100-200 mi.

Farmaco secondo la rivendicazione 1, completamente e rapidamente solubile in un mìnimo quantitativo d'acqua, realizzato in compresse effervescenti, caratterizzato da ciò che ciascuna compressa contiene: sale sodico di ibuprofene da 221,3 mg a 885,2 mg bicarbonato sodico da gr2,100 a gr. 8,402, acido citrico da gr. 0,450 a gr.1,800 e acqua 100-200 mi.
4. Farmaco secondo la rivendicazioni 1, 2, 3 in soluzione perfettamente palatabile, aromatizzata e dolcificata.

Farmaco secondo la rivendicazione 1, completamente e rapidamente solubile in un minimo quantitativo d'acqua, ralizzato in granulato effervescente, caratterizzato da ciò che il granulato contiene: ibuprofene da 200 a 800 mg, bicarbonato di sodio da gr. 2,100 a gr. 8,402, acido citrico da gr. 0,450 a gr. 1,800 scioglibili in 100/200 mi di acqua, con sviluppo di gas carbonico.
