Description
La présente invention a pour objet une endopro-thèse intravasculaire cylindrique en matériau biocompatible, apte à un élargissement diamétral, destinée à être transportée et introduite à l'état rétréci de plus petit diamètre, dans une zone de sténose ou d'obstruction d'un vaisseau sanguin ou autre par le moyen d'un cathéter à fil de guidage, associée à un moyen de retenue temporaire à l'état rétréci autour du cathéter pendant son transport et son introduction dans la zone précitée.
Il existe différents types d'endoprothèses de ce genre qui ont pour caractère commun de se présenter sous la forme d'un manchon cylindrique dont la paroi, macroporeuse, forme un treillis à mailles dé-formables pour permettre sa rétraction et son extension diamétrales.
Dans l'un de ces types, l'endoprothèse est réalisée en acier inoxydable et se présente au départ sous forme rétreinte au plus petit diamètre. Elle est ensuite élargie, après introduction dans la zone à traiter du vaisseau, par le moyen d'un ballon d'an-gioplastie intercallé entre elle et le cathéter et mis sous pression d'un liquide, jusqu'au diamètre désiré correspondant à l'expansion maximale capable de ce ballon.
Dans un autre type, l'endoprothèse est réalisée en matériau thermo-expansible que l'on dilate par la chaleur après sa mise en piace.
Enfin dans un type de conception récente, l'endoprothèse est réalisée en acier inoxydable de bonnes qualités élastiques, formée de fils entrelacés en treillis, dont le diamètre est choisi légèrement supérieur à celui de la lumière normale du vaisseau à traiter, de façon à ce qu'elle puisse exercer une pression radiale résiduelle sur la paroi artérielle après son implantation.
Avant l'implantation et pendant son transport et son introduction dans la zone à traiter du vaisseau, l'endoprothèse ainsi constituée est rétrécie en diamètre par élongation et maintenue comprimée sur le cathéter par une membrane retirable. Une fois mise en place, la rétraction progressive de la membrane permet de déployer l'endoprothèse dans la lumière vasculaire.
De ces trois types d'endoprothèses connues, le second réalisé en matériau thermo-expansible présente les gros inconvénients d'être de manipulation et de mise en place difficiles et d'expansion peu contrôlable ni fiable.
Les deux autres types en acier inoxydable n'ont pas ces inconvénients mais en présentent cependant un autre dans Ge sens que leur expansion au diamètre maximal prévu se fait au prix d'un raccourcissement proportionnel de leur longueur. Ce raccourcissement, consécutif à la déformation géométrique des mailles du treillis, qui s'élargissent dans la direction transversale du cylindre et se raccourcissent dans celle de sa longueur, a pour effet de rendre l'implantation peu précise en regard de son emplacement souhaité.
En outre, la structure en treillis métallique fait que les extrémités du cylindre présentent des filaments libres qui sont traumatisants et peuvent induire une transformation fibreuse du tissu dans là lumière du vaisseau et la formation d'une cicatrice intraluminale à l'origine d'une probable resténose.
L'invention a pour but d'éviter les inconvénients précités, notamment de permettre une implantation aisée, très précise et nettement moins traumatisante.
A cet effet, l'endoprothèse intravasculaire selon l'invention est caractérisée en ce qu'elle est constituée par une feuille plane quadrangulaire roulée, destinée à être enroulée serrée sur elle-même en spirale à l'état rétréci de plus petit diamètre autour du cathéter et ensuite élargie à un plus grand diamètre par déroulement, après suppression du moyen de retenue temporaire à l'état rétréci.
De la sorte, le passage de la forme rétrécie de petit diamètre à la forme élargie de plus grand diamètre ne se fait plus ici au prix d'une diminution de la longueur de l'endoprothèse, du fait que ce changement dimensionnel ne s'obtient plus par déformation dimensionnelle de sa paroi mais par déroulement de celle-ci; et ceci fait que son implantation peut être réalisée de manière simple et extraordinairement plus précise dans la zone désirée,
il n'est plus indispensable d'utiliser une structure en treillis métallique, du fait que celle-ci n'est plus nécessaire pour obtenir l'accroissement et/ou la réduction du diamètre de l'endoprothèse, et ceci ouvre la possibilité d'emploi d'une feuille pleine pour des usages particuliers.
Mais il est cependant possible, lorsqu'une structure macroporeuse est recherchée, d'ajourer la feuille plane qui constitue l'endoprothèse de trous arrangés par exemple pour lui donner l'aspect d'un treillis à mailles régulières, indéformables.
Dans ces deux cas, pleine ou perforée, la surface dépourvue d'aspérités de la paroi de l'endoprothèse, ainsi que l'absence de filaments métalliques libres rendue possible à ses deux extrémités, font qu'elle peut être réalisée de manière largement atraumatique. En outre, la pianéité intérieure de l'endoprothèse permet un bon écoulement du fluide.
Ces effets répondent bien, de manière simple et fiable, au but recherché.
Des développements avantageux, rendus possibles par ce concept de base de l'invention, res-sortiront clairement de la description qui suit, notamment en ce qui concerne la possibilité d'emploi de matériaux non trombogènes et dégradables, à la place de l'acier inoxydable.
Le dessin annexé représente, à titre d'exemples, une forme d'exécution de l'objet de l'invention ainsi qu'une variante de sa structure.
La fig. 1 est une double vue partielle schématique d'ensemble de la forme d'exécution, à gauche dans son état rétréci, à droite dans son état élargi.
La fig. 2 en est une section droite dans son état rétréci.

L'endoprothèse représentée par les fig. 1 à 4 est constituée par une feuille plane quadranguiaire droite, en matériau malléable biocompatible, roulée à recouvrement et en forme de cylindre 1, selon fig. 1.
Cette feuille roulée 1 est ajourée par des trous arrangés de manière à lui donner l'aspect d'un treillis à mailles carrées 2 indéformables dont deux côtés parallèles 3 sont orientés dans la direction de là longueur du cylindre formé, selon une structure macroporeuse dont un angle est détaillé sur la fig. 4.
Le bord recouvert 4 de la feuille roulée 1 comporte des languettes de retenue 5, légèrement relevées en saillie et orientées dans la direction transversale du cylindre formé afin de pouvoir s'engager dans les mailles 2 de la partie du treillis qui le recouvre.
Ces languettes 5 sont ici décalées deux par deux à l'intérieur des mailles 2 afin de faciliter leur engagement dans celles-ci.
Dans la partie gauche de la fig. 1 et selon sa section droite représentée fig. 2, une partie de l'endoprothèse 1 est montrée à l'état rétréci de plus petit diamètre (d), enroulée serrée sur elle-même en spirale sur un ballon d'angioplastie 6 dégonflé, monté autour de l'extrémité introductrice d'un cathéter 7, ces deux derniers éléments, ballon et cathéter, étant d'usage habituel pour l'implantation d'une en-doprothèse.
Un moyen de retenue temporaire de l'endoprothèse à l'état rétréci est ici constitué par un fil 8 lacé dans les mailles des deux dernières couches de la spirale formée par l'enroulement de la feuille 1 sur elle-même, près du bord extérieur 9 de recouvrement, ce fil 8 étant prévu enlevable par traction depuis l'extérieur.
Dans là partie droite de la fig. 1 et selon sa section droite représentée fig. 2, l'autre partie de l'endoprothèse 1 est montrée à l'état élargi de plus grand diamètre (D) par son déroulement obtenu par le moyen de la mise sous pression du ballon d'angioplastie 6, après enlèvement par traction du fil 8 de retenue, ce ballon 6 et le cathéter 7 n'étant plus représentés sur cette partie de la fig. 1 pour ménager la clarté du dessin.
Pendant le gonflement du ballon d'angioplastie 6, les languettes de retenue 5, qui se trouvent orientées dans le sens opposé au déroulement de la feuille 1, glissent de ce fait de maille en maille, sans accrocher celles-ci.
Après le dégonflement du ballon d'angioplastie 6, la pression du vaisseau autour de l'endoprothèse a tendance à réenrouler la feuille 1 qui la constitue. C'est alors que les languettes 5, qui se trouvent orientées dans le sens de ce réenroulement s'engagent dans les premières mailles du treillis rencontrées et viennent buter et s'accrocher contre les bords de celles-ci, comme cela est rendu bien visible sur ces deux fig. 1 et 3.
L'endoprothèse est ainsi maintenue fermement et de manière propre et atraumatique à l'état élargi dans le vaisseau.
Bien entendu, la largeur enroulable de la feuille 1 et la largeur des mailles du treillis sont déterminées en fonction du diamètre maximal capable du ballon d'angioplastie 6 et, de préférence, pour qu'au gonflement maximal de celui-ci il y ait encore le bord 4 présentant les languettes de retenue 5 recouvert par au moins la dernière maille du bord opposé extérieur 9.
Le concept en feuille roulée de l'endoprothèse telle que décrite présente les avantages dé pouvoir être déroulée facilement et enroulée sur un ballon d'angioplastie 6 sans nécessiter de modifications de celui-ci.
La forme carrée des mailles 2 du treillis permet d'obtenir une grande stabilité radiale du cylindre, et ceci est intéressant pour le traitement des vaisseaux rectilignes. Mais cette forme carrée n'est pas limitative.
Ainsi par exemple, dans le but de faciliter l'introduction de l'endoprothèse dans des vaisseaux blessés, une flexibilité relativement haute peut être obtenue par des mailles en forme de losanges, telles que les mailles 10 de la variante représentée fig. 5, dont le petit axe (a) est orienté dans la direction longitudinale du cylindre.
Dans ce cas, les languettes de retenue destinées à s'opposer au réenroulement de l'endoprothèse sous l'effet de la pression du vaisseau, après dégonflement du ballon d'angioplastie 6, peuvent être constituées par des prolongements relevés 11 des angles aigus des mailles 10 du bord recouvert 4 du treillis, ces languettes ainsi conformées venant s'accrocher dans les angles intérieurs correspondants des mailles qui les recouvrent.
En fait, toutes formes géométriques peuvent être données aux trous destinés à rendre macroporeuse la feuille roulée 1, selon l'effet particulier recherché et selon le mode d'application de l'endoprothèse, et ceci sans nécessairement lui conférer l'aspect d'un treillis, le principe et l'avantage de l'élargissement par déroulement qui permet d'éviter tout raccourcissement de l'endoprothèse étant préservés dans toutes les formes de ces trous.
Le dessin fait bien ressortir l'effet avantageux déjà signalé, par rapport à l'état de la technique actuelle, par l'absence de filaments libres traumatisants aux extrémités du cylindre de l'endoprothèse, celles-ci ne présentant que des éléments géométriques fermés, carrés 2 et losanges 10, cet effet étant conservé pour toutes autres formes.
Le système de retenue de l'endoprothèse dans son état élargi de grand diamètre (D) constitué par l'accrochage des languettes 5 et 11, et qui est auto-verrouillable, élimine la nécessité de devoir utiliser un matériau de bonnes caractéristiques d'élasticité. Et ceci permet d'envisager avantageusement l'utilisation de matériaux malléables non trombogènes et éventuellement résorbables.
Ce système de retenue peut également être utilisé avec une feuille roulée pleine, en pratiquant seulement quelques trous d'engagement des languettes de retenue dans le bord de la feuille qui les recouvre.
Il est possible aussi, dans un concept sans recouvrement des deux bords du cylindre, de prévoir des éléments d'agrippage sur ces deux bords destinés à s'implanter dans la paroi même du vaisseau, sous la pression de celle-ci.

Un autre avantagé du concept de la feuille roulée, qui est à la base de l'invention, réside dans le fait qu'il autorise l'emploi de toutes sortes de matériaux tels que les métaux, les matières synthétiques et les céramiques, pourvu qu'ils soient biocompatibles.
Le fait de pouvoir utiliser les matières synthétiques offre une meilleure possibilité d'enrobage par des produits ou des matériaux empêchant la prolifération cellulaire à l'intérieur de l'endoprothèse, par un choix plus étendu dans la recherche des compatibilités.
Dans la cas d'un matériau de très bonnes caractéristiques d'élasticité II est possible par exemple de prévoir le formage de ta feuille cylindrique 1 de telle sorte que son élargissement par déroulement soit obtenu par son élasticité propre, c'est-à-dire en fait que ce formage lui donne au repos l'aspect d'un cylindre ouvert. A l'implantation, l'élasticité du matériau s'oppose ainsi à la pression du vaisseau, jusqu'à équilibre de ces deux contraintes, et tout autre moyen de retenue tel que les languettes 5 ou 11 n'est plus nécessaire.
Enfin, d'autres moyens de retenue temporaire de l'endoprothèse à l'état rétréci autour du cathéter peuvent être appliqués, tels que par exemple une enveloppe ou des points de collage dégradables ou bien une combinaison de pointe de collage et du fil 8, la traction de ce dernier faisant lâcher les premiers.

Claims (8)

Revendications
1. Endoprothèse intravasculaire cylindrique en matériau biocompatible, apte à un élargissement diamétral, destinée à être transportée et introduite à l'état rétréci de plus petit diamètre dans une zone de sténose ou d'obstruction d'un vaisseau sanguin ou autre par le moyen d'un cathéter à fil de guidage, associée à un moyen de retenue temporaire à l'état rétréci autour du cathéter pendant son transport et son introduction dans la zone précitée, caractérisée en ce qu'elle est constituée par une feuille plane quadrangulaîre roulée (1), destinée à être enroulée serrée sur elle-même en spirale à l'état rétréci de plus petit diamètre (d) autour du cathéter (7) et ensuite élargie à un plus grand diamètre (D) par déroulement, après suppression du moyen de retenue temporaire (8) à l'état rétrici.
2. Endoprothèse selon la revendication 1, caractérisée en ce qu'elle est en matériau élastique et en ce que son élargissement par déroulement est obtenu par son élasticité propre.
3. Endoprothèse selon la revendication 1, caractérisée en ce qu'elle est en matériau malléable et en ce que son élargissement par déroulement est obtenu par gonflement d'un ballon intérieur d'angioplastie (6} monté sur et autour du cathéter, et en ce qu'elle comporte des moyens de retenue à l'état élargi (5,11) destinés à l'empêcher de se réenrouler sur elle-même sous l'effet de la compression du vaisseau dans lequel elle a été introduite, après dégonflement du ballon d'angioplastie.
4. Endoprothèse selon la revendication 1, caractérisée en ce que la feuille plane roulée qui la constitue est ajourée par des trous arrangés de manière
à lui donner l'aspect d'un treillis à mailles régulières (2,10) indéformables.
5. Endoprothèse selon les revendications 3 et 4, caractérisée en ce qu'elle comporte des moyens de retenue par accrochage à l'état élargi constitués par un nombre choisi de languettes relevées (5,11) disposées en saillies le long du bord longitudinal recouvert (4) du cylindre à l'état élargi, orientées dans la direction transversale de celui-ci, et destinées à s'engager ainsi automatiquement dans les premières mailles rencontrées du treillis qui les recouvre dès le dégonflement du ballon d'angioplastie.
6. Endoprothèse selon la revendication 4, caractérisée en ce que les mailles du treillis formé par la feuille roulée qui la constitue sont en forme de losange (10) dont le petit axe (a) est orienté dans la direction de sa longueur.
7. Endoprothèse selon la revendication 4, caractérisée en ce que les mailles du treillis formé par la feuille roulée qui la constitue sont en forme de carré (2) dont deux côtés parallèles (3) sont orientés dans la direction de sa longueur.
8. Endoprothèse selon les revendications 1 et 4, caractérisée en ce que le moyen de retenue temporaire à l'état rétréci autour du cathéter est constitué par un fil (8) lacé dans les mailles d'au moins les deux dernières couches de la spirale formée par enroulement de la feuille (1) sur elle-même, ce fil étant prévu enlevable depuis l'extérieur.
