Abstract
本发明涉及一种治疗胃溃疡合并胃肠道出血的组合物,其活性成分的重量配比为:人参皂苷Re 8~25份、人参皂苷Rg3 10~30份、人参皂苷Rh1 5~20份。本发明具有明显改善胃黏膜损伤,促进凝血止血的作用,能够有效治疗和/或预防胃溃疡合并胃出血等胃肠道疾病。

技术领域
本发明涉及医药领域,特别是一种治疗胃溃疡合并胃肠道出血的组合物及其制备方法。
背景技术
近年来,随着人们生活水平的不断提高,人们的生活方式发生着很大的改变,伴随而来的是胃肠道疾病的发病率及患病率都呈明显上升趋势。胃溃疡是胃肠道系统疾病中的常见病、多发病,此病很容易合并出现胃出血,临床上主要表现为胃痛、胃胀、腹泻、便血、呕血等症状,在中医学上,属于“胃脘痛”、“痞满”等范畴。
在病因病机方面,胃溃疡、胃出血等胃肠道疾病的发生,多认为主要与脾胃虚弱、情志失调、饮食所伤、感受外邪等因素有关,其中脾胃虚弱是病理基础,病性多虚实夹杂。中医学认为:劳倦过度,饮食所伤,久病,素体不等均可损及脾阳,引起脾胃虚弱。
临床上,对脾胃虚寒引起的胃溃疡、胃出血等胃肠道疾病,西医主要采用抑酸、止痛、止泻等治疗方法,中医学则采用温胃散寒、散瘀止痛、温脾止泻的方法。目前中医学治疗该病的疗效已被证实,进一步采用中药材提取物进行研究,更能充分发挥中医药的优势。
近年来,人们对三七的提取工艺、化学成分和药理作用等各方面的研究越来越多。现代研究表明,三七的主要化学成分包括皂苷类、黄酮类、挥发油、糖类等,中医临床证实三七有散瘀止血,消肿定痛的作用;现代医学研究证实三七对人体心脑血管疾病有显著的治疗作用,同时具有降糖、抗高血压、抗肿瘤、抗癌、保肝护肾等作用。临床药理研究表明,三七水提醇沉对胃黏膜损伤有一定的保护作用;三七皂苷成分具有良好的止血功效,能通过提高血小板数量和聚集能力,缩短出血和凝血时间,达到止血作用。
CN1589808A“一种治疗消化性溃疡或肠炎的药物组合物及其制备工艺和用途”,其有效组分为:黄藤生物碱、三七总皂苷、白及多糖、白屈菜生物碱,其制备方法为:按每片(或者每粒、每丸)中本质上含有36-90mg的黄藤生物碱、30-75mg的三七总皂苷、30-75mg的白及多糖、0.5-2mg的白屈菜生物碱四种有效成,将这四种有效成份配制混合均匀后粉碎,过80~180目筛,再装入胶囊(普通药用软、硬胶囊)制成胶囊(软胶囊剂、硬胶囊剂),或进一步添加药用溶剂或赋形剂、防腐剂,采用普通制药工艺,制成丸剂或颗粒剂、片剂。该专利公开的用途为治疗消化性溃疡类病症及幽门螺旋杆菌引起的溃疡,十二指肠溃疡,急、慢性胃炎,菌痢、肠炎,腹泻病症或者进一步用于制备治疗消化性溃疡类药物,但并未提及该药物具有治疗胃出血或治疗胃溃疡合并胃出血的作用。

三七为五加科植物Parmx notogin seng(Burk.)F.H.Chen的干燥根和根茎。秋季花开前采挖,洗净,分开主根、支根及根茎,干燥。支根习称“筋条”,根茎习称“剪口”。【性味与归经】甘、微苦,温。归肝、胃经。【功能与主治】散瘀止血,消肿定痛。用于咯血,吐血,鯉血,便血,崩漏,外伤出血,胸腹刺痛,跌打肿痛。
发明人对本发明的三七中人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的提取方法及其疗效进行了大量系统性的实验研究,最后得到了本发明的提取技术方案,本发明采用独特提取、纯化工艺所得组合物,其特殊之处在于,所述组合物由配比如下的成分构成:人参皂苷Re:8~25份、人参皂苷Rg3:10~30份、人参皂苷Rh1:5~20份。
优选地,本发明所述的三七总皂苷,它是由配比如下的成分构成:人参皂苷Re:15~25份、人参皂苷Rg3:15~30份、人参皂苷Rh1:15~20份。
优选地,本发明所述的三七总皂苷,它是由配比如下的成分构成:人参皂苷Re:25份、人参皂苷Rg3:25份、人参皂苷Rh1:15份。
本发明还公开了一种制备具有治疗胃溃疡合并胃肠道出血作用的组合物的方法,具体步骤如下:
(1)将三七粉碎成40目粗粉,加药材重量20倍体积百分比为70-80%乙醇回流提取2~3次,每次1~3h,过滤,合并滤液,得三七醇提液;药渣再加入药材重量10-15倍水回流提取2次,每次1~2h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量乙醇溶解,通过D101大孔吸附树脂柱层析,先用1-4BV去离子水洗脱,弃去洗脱液;再用2-6BV 体积百分比为70-80%乙醇洗脱,洗脱速度为1~3BV/h,合并乙醇洗脱液,回收溶剂,干燥,得到三七总皂苷;
(2)三七总皂苷加水溶解后,上HPD-100树脂柱,用1~4BV去离子水洗脱,弃去洗脱液;用1~3BV 15%乙醇溶液洗脱,弃去洗脱液;依次用2~5BV 30%、2~5BV 50%、2~4BV75%乙醇洗脱,洗脱速度为2~5 BV/h,分别收集各段洗脱液,得人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的洗脱液,减压浓缩,干燥;
(3)将人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1按权利要求1所述重量份混合,即得。
优选地,制备方法为:
(1)将三七粉碎成40目粗粉,加药材重量20倍体积百分比为75%乙醇回流提取2次,每次3h,过滤,合并滤液,得三七醇提液;药渣再加入药材重量12倍水回流提取2次,每次1.5h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,80℃减压浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量70%乙醇溶解,通过D101大孔吸附树脂柱层析,先用2BV去离子水洗脱,弃去洗脱液;再用4BV 体积百分比为75%乙醇洗脱,洗脱速度为3 BV/h,合并乙醇洗脱液,回收溶剂,80℃干燥,得到三七总皂苷;

本发明还提供了含上述组合物的药物制剂,该制剂由上述组合物和药学上可接受的载体组成。
所述制剂为固体制剂或液体制剂,固体制剂为片剂、胶囊剂或颗粒剂;液体制剂为合剂或口服液。
所述药学上可接受的载体为乳糖、淀粉、糊精、甘露醇、蔗糖、羟丙甲基纤维素、交联羧甲基纤维素钠、低取代羟丙基纤维素、微晶纤维素、微粉硅胶、预胶化淀粉、山梨醇、硬脂酸镁、滑石粉、羟丙甲基纤维素、低取代羟丙基纤维素、苯甲酸钠、山梨酸钾、蔗糖或阿斯巴甜中、味精、蜂蜜、甜橙香精的一种或几种。
本发明所述三七总皂苷药物制剂的制备方法,该方法可以采用常规制剂工艺制备为各种常见剂型。
本发明还提供了上述组合物或制剂在制备治疗胃溃疡合并胃出血等胃肠道疾病时提高治疗效果的应用。
发明人在研究胃溃疡合并胃肠道出血的治疗过程中发现,五加科植物三七具有良好的保护胃黏膜、促进凝血止血的作用。三七提取物及三七总皂苷能显著减少大鼠的胃溃疡指数,增加大鼠的血小板计数,缩短大鼠的凝血酶原时间,且不良反应少。本发明研究证实三七、三七提取物及三七总皂苷具有明显改善胃肠道微循环,保护胃黏膜损伤的作用,可用于治疗脾胃虚寒引起的胃溃疡、胃肠道出血等胃肠道疾病,且具有疗效确切、安全,使用简便,不良反应少等特点。目前对其具体作用机制还不完全明了,但三七总皂苷对胃溃疡合并胃出血动物模型具有较好的胃黏膜保护作用及凝血止血作用,提示其具有良好的治疗胃溃疡和胃出血作用。
因此,申请人提三七活性成分提取物人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的组合物够用于治疗胃溃疡合并胃出血药物方面的新用途。
本发明提供的组合物用于胃溃疡合并胃出血药物方面的新用途具有以下优点:
1、本发明组合物各成分从五加科植物三七中提取而得,安全无毒,可长期服用。
2、实验结果显示,该组合物可显著减少胃溃疡合并胃出血动物模型的胃溃疡指数,显著缩短大鼠的出血时间和凝血时间。

下面通过实施例进一步说明本发明。应该理解的是,本发明的实施例是用于说明本发明而不是对本发明的限制。根据本发明的实质对本发明进行的简单改进都属于本发明要求保护的范围。除非另有说明,本发明中的乙醇量的百分数是体积百分数,v/v表示溶液的体积比。
实施例1 :本发明所述组合物的制备
(1)称取三七2kg,粉碎成40目粗粉,加入药材重量20倍75%乙醇回流提取2次,每次3h,过滤,合并滤液,得三七醇提液;药渣再加入药材重量12倍水回流提取2次,每次1.5h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,80℃减压浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量70%乙醇溶解,通过D101大孔吸附树脂柱层析,先用2BV去离子水洗脱,弃去洗脱液;再用4BV 75%乙醇洗脱,洗脱速度为3 BV/h,合并乙醇洗脱液,回收溶剂,80℃干燥,得到三七总皂苷;
(2)三七总皂苷加水溶解后,上HPD-100大孔树脂,用2BV去离子水洗脱,弃去洗脱液;用2BV 15%乙醇溶液洗脱,弃去洗脱液;依次用4BV 30%、4BV 50%、3BV 75%乙醇洗脱,洗脱速度为3 BV/h,分别收集各段洗脱液,得人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的洗脱液,减压浓缩,65℃干燥,即得人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1;
(3)将人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1按以下重量份混合: 25g、25g、15g混合,即得。
实施例2 :本发明所述组合物的制备
(1)称取三七2kg,粉碎成40目粗粉,加入药材重量15倍70%乙醇回流提取2次,每次1h,过滤,合并滤液,得三七醇提液;药渣再加入药材重量10倍水回流提取2次,每次1h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,80℃减压浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量70%乙醇溶解,通过D101大孔吸附树脂柱层析,先用1BV去离子水洗脱,弃去洗脱液;再用2BV 70%乙醇洗脱,洗脱速度为1 BV/h,合并乙醇洗脱液,回收溶剂,80℃干燥,得到三七总皂苷;
(2)三七总皂苷加水溶解后,上HPD-100大孔树脂,用1BV去离子水洗脱,弃去洗脱液;用1BV 15%乙醇溶液洗脱,弃去洗脱液;依次用2BV 30%、2BV 50%、2BV 75%乙醇洗脱,洗脱速度为2 BV/h,分别收集各段洗脱液,得人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的洗脱液,减压浓缩,65℃干燥,即得人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1;
(3)将人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1按以下重量份混合:8g、10g、5g混合,即得。
实施例3:本发明所述组合物的制备
(1)称取三七2kg,粉碎成40目粗粉,加入药材重量25倍70%乙醇回流提取3次,每次3h,过滤,合并滤液,得三七醇提液;药渣再加入药材重量15倍水回流提取2次,每次2h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,80℃减压浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量70%乙醇溶解,通过D101大孔吸附树脂柱层析,先用4BV去离子水洗脱,弃去洗脱液;再用3BV 70%乙醇洗脱,洗脱速度为1 BV/h,合并乙醇洗脱液,回收溶剂,80℃干燥,得到三七总皂苷;
(2)三七总皂苷加水溶解后,上HPD-100大孔树脂,用4BV去离子水洗脱,弃去洗脱液;用3BV 15%乙醇溶液洗脱,弃去洗脱液;依次用5BV 30%、5BV 50%、4BV 75%乙醇洗脱,洗脱速度为5 BV/h,分别收集各段洗脱液,得人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的洗脱液,减压浓缩,65℃干燥,即得人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1;

实施例4:本发明所述组合物的制备
取实施例1步骤(2)所得人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1,按以下重量份混合:15g、15g、15g混合,即得。
实施例5:本发明所述组合物的制备
取实施例2步骤(2)所得人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1,按以下重量份混合:25g、30g、20g混合,即得。
实施例6:本发明颗粒剂
取实施例1所得组合物,加入辅料淀粉、蔗糖、乳糖混合均匀,用70%乙醇制软材,过60目筛制粒,干燥,整粒,分装,制成颗粒剂,即得。
实施例7:本发明片剂
取实施例2所得组合物,加入辅料微晶纤维素、羟丙基纤维素、羧甲基淀粉钠混合均匀,加入润滑剂滑石粉、硬脂酸镁混合均匀,压片,即得。
实施例8:本发明胶囊剂
取实施例3所得组合物,干燥成干膏粉,加入淀粉乳糖等胶囊剂常用辅料混合均匀后,按常规生产方法制备,粉碎、过筛,填充、套合,即得。
对比例1:
称取三七2kg,粉碎成40目粗粉,加入20倍75%乙醇回流提取2次,每次3h,过滤,合并滤液,得三七醇提液;药渣再加入12倍水回流提取2次,每次1.5h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,80℃减压浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量70%乙醇溶解,通过D101大孔吸附树脂柱层析,先用2BV去离子水洗脱,弃去洗脱液;再用4BV 75%乙醇洗脱,洗脱速度为3 BV/h,合并乙醇洗脱液,回收溶剂,80℃干燥,得到三七总皂苷,加入淀粉,制粒,再装入胶囊,制成胶囊剂。
药效学试验:
1实验材料
1.1实验动物
SPF级SD大鼠,雄性,180~220克,每组10只。购买自广西医科大学实验动物中心,许可证号SCXK(湘)2015-0003。
1.2受试样品与试剂
实施例1、对比例1药物;阿司匹林(长白山制药股份有限公司,批号:140101);水合氯醛(成都市科龙化工试剂厂,批号:2013092101);甲醛(天津博迪化工股份有限公司,批号:20140401)。
1.3实验仪器
带标尺的解剖镜、EPOCH BIOTEK全波长酶标仪(美国伯腾仪器有限公司)。
2实验方法
2.1胃溃疡合并胃出血大鼠动物模型造模
随机取10只作为空白对照组,其余大鼠禁食不禁水18小时后,用乙醚麻醉,固定于木板上,将其浸入23℃的恒温水槽中,水没于剑突部30min,同时灌胃剂量为6mg/kg的阿司匹林,给药量为10ml/kg,每天灌胃2次,连续灌胃7天,制备胃溃疡合并胃出血大鼠模型。
2.2动物分组与给药
造模后按体重随机分为模型组、阳性组和实施例1组。阳性组给予剂量为4mg/kg的奥美拉唑,各给药给予相应的实施例药物,给药量为20ml/kg,每天给药1次,连续30天,空白组和模型组灌胃等体积的蒸馏水。
2.3标本采集与检测
末次给药后禁食不禁水12小时后,自大鼠尾静脉采血,测定血小板数量 (/L)及凝血酶原时间(PT)。然后用水合氯醛麻醉大鼠,剖开腹腔,结扎幽门和贲门,取出整个胃,胃内灌注5mL的10%甲醛溶液,浸泡10分钟后沿胃大弯剪开,洗净内容物,将整个胃展开平铺于玻璃板上,用纸吸干溃疡内的水分,观察胃溃疡的发生情况,并对溃疡程度进行评价,对溃疡程度的评定方法为:将溃疡面积和数量结合划分溃疡指数,0:无溃疡;1:1~3个小溃疡(3mm以下);2:3个以上小溃疡或1个大溃疡;3:1个大溃疡和几个小溃疡;4:几个大溃疡;5:穿孔性溃疡;计算溃疡抑制百分率:溃疡抑制百分率=(模型对照组溃疡指数-给药组溃疡指数)/模型对照组溃疡指数/100%。
3 统计学处理
实验数据采用SPSS进行统计学处理,计量数据以±s表示,组间比较采用两样本均数的t检验,多组间样本均数比较采用方差分析;以P<0.05认为有显著性差异。
4 实验结果
4.1 对大鼠胃溃疡指数的影响,实验结果见表1

结论:实验结果显示,模型组大鼠血小板计数明显少于空白对照组,而胃溃疡指数和凝血时间多于正常对照组。各给药组血小板计数明显高于模型组,胃溃疡指数和凝血时间均低于模型组,与对比例比较,各实施例组血小板计数明显高于对比例组,胃溃疡指数和凝血时间均低于对比例组,以上组间指标比较差异均有显著性差异(P<0.05或P<0.01)。说明本发明组合物具有良好的保护胃黏膜,促进凝血止血的作用,能够有效治疗胃溃疡合并胃出血等胃肠道疾病,且治疗效果明显优于现有技术。
虽然,上文中已经用一般性说明、具体实施方式及试验,对本发明作了详尽的描述,但在本发明基础上,可以对之作一些修改或改进,这对本领域技术人员而言是显而易见的。因此,在不偏离本发明精神的基础上所做的这些修改或改进,均属于本发明要求保护的范围。
Claims (8)
1.一种治疗胃溃疡合并胃肠道出血的组合物,其特征在于,所述组合物由如下重量份的成分组成:人参皂苷Re:8~25份、人参皂苷Rg3:10~30份、人参皂苷Rh1:5~20份。
2.根据权利要求1所述的治疗胃溃疡合并胃肠道出血的组合物,其特征在于,所述组合物由如下重量份的成分组成:人参皂苷Re:15~25份、人参皂苷Rg3:15~30份、人参皂苷Rh1:15~20份。
3.根据权利要求2所述的治疗胃溃疡合并胃肠道出血的组合物,其特征在于,所述组合物由重量份配比如下的成分构成:人参皂苷Re:25份、人参皂苷Rg3:25份、人参皂苷Rh1:15份。
4.根据权利要求2所述的治疗胃溃疡合并胃肠道出血的组合物,其特征在于制备方法包括如下步骤:(1)将三七粉碎成粗粉,加体积百分比为70-80%乙醇回流提取2~3次,每次1~3h,过滤,合并滤液,得三七醇提液;药渣再加入药材重量10-15倍水回流提取2次,每次1~2h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量乙醇溶解,通过D101大孔吸附树脂柱层析,先用1-4BV去离子水洗脱,弃去洗脱液;再用2-6BV 体积百分比为70-80%乙醇洗脱,洗脱速度为1~3 BV/h,合并乙醇洗脱液,回收溶剂,干燥,得到三七总皂苷;
(2)三七总皂苷加水溶解后,上HPD-100树脂柱,用1~4BV去离子水洗脱,弃去洗脱液;
用1~3BV 15%乙醇溶液洗脱,弃去洗脱液;依次用2~5BV 30%、2~5BV 50%、2~4BV75%
乙醇洗脱,洗脱速度为2~5 BV/h,分别收集各段洗脱液,得人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的洗脱液,减压浓缩,干燥;
(3)将人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1按权利要求2所述重量份混合,即得。
5.根据权利要求4所述的治疗胃溃疡合并胃肠道出血的组合物,其特征在于制备方法包括如下步骤:(1)将三七粉碎成40目粗粉,加药材重量20倍体积百分比为75%乙醇回流提取2次,每次3h,过滤,合并滤液,得三七醇提液;药渣再加入药材重量12倍水回流提取2次,每次1.5h,过滤,合并滤液,得三七水提液;合并三七醇提液和水提液,80℃减压浓缩,干燥,得三七干浸膏;三七干浸膏,加入少量70%乙醇溶解,通过D101大孔吸附树脂柱层析,先用2BV去离子水洗脱,弃去洗脱液;再用4BV 体积百分比为75%乙醇洗脱,洗脱速度为3 BV/h,合并乙醇洗脱液,回收溶剂,80℃干燥,得到三七总皂苷;
(2)三七总皂苷加水溶解后,上HPD-100树脂柱,用2BV去离子水洗脱,弃去洗脱液;用2BV 15%乙醇溶液洗脱,弃去洗脱液;依次用4BV 30%、4BV 50%、3BV 75%乙醇洗脱,洗脱速度为3 BV/h,分别收集各段洗脱液,得人参皂苷Re、人参皂苷Rg3、人参皂苷Rh1的洗脱液,减压浓缩,65℃干燥;
(3)将人参皂苷Re,人参皂苷Rg3、人参皂苷Rh1按权利要求2所述重量份混合,即得。
6.一种治疗胃溃疡合并胃肠道出血的制剂,由权利要求1至3任一项所述的治疗胃溃疡合并胃肠道出血的组合物与医药学上接受的载体配合制备而成。
7.根据权利要求6所述的治疗胃溃疡合并胃肠道出血的制剂,其特征在于,所述医药学上接受的载体包括:乳糖、淀粉、糊精、甘露醇、蔗糖、交联羧甲基纤维素钠、微晶纤维素、微粉硅胶、预胶化淀粉、山梨醇、硬脂酸镁、滑石粉、羟丙甲基纤维素、低取代羟丙基纤维素、苯甲酸钠、山梨酸钾或阿斯巴甜、蜂蜜、甜橙香精的任一种或一种以上的混合。
8.根据权利要求6所述的治疗胃溃疡合并胃肠道出血的制剂,其特征在于,所述制剂包括胶囊剂、片剂、颗粒剂、合剂、口服液。
