Abstract
本发明公开了一种中药组合物在制备预防、治疗肥胖的药物中的应用,所述组合物包括如下原料:黄连,乳香,没药,血竭,法半夏,全瓜蒌,绞股蓝,茯苓,穿山龙。本发明的中药组合物针对痰瘀互结型肥胖发展过程中脂肪在体内生成和堆积的过程和病理机制,有针对性的进行化痰活血,降浊通络,达到改善肥胖和减肥的目的,对预防、治疗痰瘀互结型肥胖的疗效显著,见效快、毒副作用小、是一种安全、高效、稳定、制备工艺简单的预防和治疗肥胖药物,适于工业化生产,易于推广。本发明为预防和治疗痰瘀互结型肥胖及其并发症提供了一种新的药物来源。

本发明涉及医药领域,涉及一种预防、治疗肥胖的药物或保健食品,特别涉及一种中药组合物在预防、缓解、治疗肥胖药物或保健食品中的应用。
背景技术
肥胖是一种复杂的多因素疾病。在过去30年中,全世界肥胖症的患病率在成年人中增加了27.5%,在儿童中增长了47.1%,以致目前将近三分之一的世界人口归为超重或肥胖症。肥胖的发生,与遗传、生活方式、饮食习惯等密切相关,也受到城市发展、消费方式、社会经济和文化等因素的影响。肥胖会导致许多与代谢相关的疾病如中风、高血压、糖尿病等,并且是导致多种疾病合并症产生的风险因子。肥胖不但会带来身体的危害,也会增加患者的医疗费用,对社会造成经济负担。肥胖的发生率在所有年龄段和性别中均增加,而与地理位置,种族或社会经济地位无关。肥胖的患病率通常在老年人和妇女中较高。尽管超重和肥胖的绝对患病率差异很大,但各个地区和国家的趋势相似。对于某些发达国家,肥胖的患病率在过去几年中似乎已经趋于平稳。在流行病学研究中,体重指数(BMI)通常用于定义超重和肥胖。但是,对于任何给定的BMI值,BMI的敏感性都很低,并且体内脂肪百分比之间存在很大的个体差异,部分原因是年龄,性别和种族。例如,对于相同的BMI,亚洲人的身体脂肪百分比比白种人高。更高的心脏代谢风险还与内脏脂肪组织和异位贮藏库(例如肌肉和肝脏)中多余脂肪的定位有关。仅依靠BMI来评估其患病率可能会阻碍将来针对肥胖症预防和控制的干预措施。肥胖症的实际情况可能比大型流行病学研究表明的更为普遍,更急切地需要被关注。
目前肥胖的现代医学治疗主要包括生活方式干预和医疗手段的介入,医疗手段的介入包括药物治疗、手术治疗和肠道菌群干预等。生活方式干预主要包括禁食疗法、运动疗法、行为疗法等。药物治疗主要包括靶向神经系统药如芬特明等、靶向脂肪组织药如瘦素及其类似物等、靶向消化系统药如奥利司他等、靶向肾脏药如钠-葡萄糖协同转运蛋白2抑制剂等、靶向免疫系统药等。手术干预包括胃旁路术、垂直袖状胃切除术等。肠道菌群干预则包括口服肠道细菌Akkermansia muciniphila、双歧杆菌等。生活方式干预对患者意志力要求较高,适用范围有限,减肥药物有效性较低、安全性低、用药周期长,使患者难以长期坚持。手术干预存在风险及禁忌症。肠道菌群干预的有效性和安全性还未被广泛验证。对肥胖有效疗法的开发和验证仍需要长期持续进行。
中医药认识肥胖主要认为以“痰湿”为主,病久则入络成瘀,最终形成痰瘀互结的证候。治疗主要包括中药复方治疗、针灸治疗以及其他疗法如埋线、推拿、刮痧等。较多文献报道阳性结果,但因其组方选穴等具有较强的个体特异性,可复制性和推广性相应较低。
随着现代医学科技手段的进步,人们对肥胖的认识更加深入,肥胖造成的后果严重,目前也仍有大量的肥胖患者未被彻底治愈。因此,运用祖国传统医学丰富的经验和理论,开发出对发病环节有针对性的中药制剂,为进一步提高临床疗效提供科学依据将是今后研究的重中之重。
目前同类产品在市场的状况:轻身减肥片、轻身消胖丸、减肥通圣片等是目前在市场上较常见的同类产品。组方主要以大黄、山楂、泽泻等消导通便类药物为主,对于“痰湿”这一主要病理因素重视不足,且脾胃功能较差者难以适应。
发明内容
本发明的目的是针对现有治疗和预防肥胖的产品存在的技术问题,提供一种中药组合物在预防、治疗肥胖方面的性能和功效,特别是提供中药组合物治疗痰瘀互结型肥胖的新用途,即在治疗痰瘀互结型肥胖的药物或保健品中的新应用。
为实现本发明的目的,本发明一方面提供一种中药组合物在制备预防或/和治疗肥胖的药物或保健品中的应用,其中,所述的中药组合物包括如下原料:黄连,乳香,没药,血竭,法半夏,全瓜蒌,绞股蓝,茯苓,穿山龙。
其中,所述肥胖为痰瘀互结型肥胖。
在筛选具有预防和治疗肥胖(尤其是痰瘀互结型肥胖)的天然药物的过程中,发明人发现本发明的中药组合物具有明显的抑制血管血脂沉积、降低血管内壁血脂斑块的功效,维持血管内径或增大血管在收缩和舒张末期血管内径,防止血管阻滞的作用。
特别是,所述原料药的重量份配比为:黄连3-12,乳香6-15,没药6-15,血竭3-12,法半夏3-9,全瓜蒌6-15,绞股蓝10-30,茯苓10-30,穿山龙10-30。

特别是,所述乳香选择炒乳香,优选为醋炒乳香;所述没药选择炒没药,优选为醋炒没药;所述半夏选择法半夏;所述瓜蒌选择全瓜蒌。
法半夏在现代炮制工艺中为生半夏用甘草、石灰等炮制而成,能够显著降低生半夏的毒性,并增强燥湿化痰的功效。相对于瓜蒌皮、瓜蒌仁、瓜蒌根各具偏性,全瓜蒌更能够从整体角度宽胸散结、涤痰导滞。乳香、没药生品常外用,炒后更适宜于内服,用于调气活血。
在临床中,肥胖患者往往痰浊与血瘀同在,且经脉阻滞,容易出现其它并发症。本着痰瘀同治的治则,本发明以“化痰活血、降浊通络”为思路,组方命名为“骐龙血脉健方”。其中用擅长活血的血竭和通络作用较强的穿山龙作为君药,血竭古称“骐竭”,与穿山龙的“龙”字共称“骐龙”。瓜蒌宽胸理气,消痰开结,乳香善理气活血,没药重化瘀理血,助君药活血之力,共为臣药。半夏苦辛温燥,燥湿化痰,和胃降逆,助瓜蒌化痰散结之力,茯苓健脾渗湿,助化痰之力;绞股蓝健脾理气、益气活血,增强君臣药性,共为佐药。痰瘀互结,易郁而化热,黄连味苦性寒,燥湿之余,兼能清热,为使药。方中诸药痰瘀并治,共奏化痰活血、降浊通络之功,尤适于痰瘀互结型肥胖的病症。
血竭又称为龙血竭为百合科植物剑叶龙血树含树脂木材的乙醇提取物,具有活血散淤、止痛、止血、生肌敛疮等功效;内服主治淤血肿痛、经闭、痛经,外用主治外伤出血、溃疡不愈、痔漏肿痛,为著名的活血化淤中药之一。血竭中含有多种化合物,不同提取工艺获得的血竭因其纯度及化合物种类不同而功效有所不同。血竭是“活血圣药”,作为一种传统的活血化淤中药在临床上应用也日益增多。临床上近年来采用龙血竭治疗心血管疾病,有较好的疗效。有研究对血竭进行了系统的化学成分研究,分离、鉴定了20种化合物,并证明其中的黄酮类化合物为其活血化淤的主要有效成分。临床应用表明,血竭能改善陈旧性心肌梗塞患者心绞痛症状,改善微循环,降低全血、血浆粘度,调节TXB2/PGF1α平衡,增加心肌供氧,有助于改善心功能,是防治冠心病的有效药物。现代医学研究还证实,血竭具有双向调节止血与活血等作用。
穿山龙味苦,微寒,归肝肺经,主要功效活血通络,为薯蓣科薯蓣属植物穿龙薯蓣的根茎,又称为穿山薯蓣、地龙骨、穿龙骨、野山药、黄姜、龙革薛等。主要用于治疗风湿痹痛;肢体麻木;胸痹心痛;慢性气管炎;跌打损伤;虐疾;痈肿。穿山龙的主要成分为薯蓣皂甙,纤细薯蓣皂甙,穗菝葜甾甙,25-D-螺甾-3,5-二烯,对羟基苄基酒石酸等。穿山龙现代已作为甾体激素药物和抗冠心病皂苷类药物的重要工业原料。现代药理研究证明,其总皂苷类成分具有调节免疫、改善心血管功能、祛痰、抗肿瘤、抗炎镇痛等多种药理作用。民间多用于浸酒或煎服,可治疗岔气闪腰、老年性腰腿痛、风湿性关节炎等病症。
本发明中对于肥胖日久,水停血瘀造成的痰郁互结的状态,穿山龙能够疏通经络,有效促使机体恢复正常代谢。
半夏味辛,性温,有毒,归脾、胃、肺经。《名医别录》云:“半夏,消心腹胸膈痰热满结,咳嗽上气,心下急痛坚痞,时气呕逆”,有具有“燥湿化痰、降逆止呕、消痞散结”,外用消肿止痛的功效。半夏之所以以化痰而著称者,即以其有辛开之性味,凡水湿痰涎之结滞而不行者,得此则开通而化之,故医书谓半夏能降逆化痰也。肥胖的发病与“痰湿”的核心病机密切相关,若得半夏之辛开作用,则水饮痰涎便可开而下降。
瓜蒌性味甘,寒,具有清肺化痰,理气宽胸之效。瓜蒌为葫芦科多年草质藤本植物,而中药瓜蒌即为其成熟果皮和种子。秋季果实成熟时连柄剪下,悬挂凉干,即可应用。据分析,瓜蒌含有三萜皂甙、有机酸、树脂、糖类、脂肪油、色素等成分。药理研究表明,瓜蒌能降低血胆固醇及甘油三酯。瓜蒌的化痰功效能够对半夏形成辅助,共同针对肥胖人群“痰湿”的病理要素。
乳香性微寒,味辛、苦、温。归心经、肝经、脾经,有活血行气;通经止痛;消肿生肌之功效。用于心腹诸痛,筋脉拘挛,跌打损伤,疮痈肿痛;外用消肿生肌。乳香的成分主要有树脂、树胶和挥发油,善走窜而行气活血,没药性平,重在苦泄散血化瘀。乳香树脂的主要成分为游离a,β-乳香脂酸、结合乳香脂酸、乳香树脂烃、O-乙酰基-β-乳香脂酸、O-乙酰-a-乳香酯酸等;现代药理研究表明乳香挥发油具有明显的镇痛作用;乳香还能促进多核白血球增加,以吞噬死亡的血球及细胞,改善新陈代谢,从而起消炎防腐作用。
没药味苦,性平,入肝经,具有散瘀止痛,外用消肿生肌的功效,主要用于治疗跌打淤血肿痛,痈疽肿痛,胸腹诸痛;外用治疮口久不收敛。没药的主要成分为树脂、挥发油、树胶等,其中没药树脂的大部分能溶于醚,其中:不溶性部分含α及β罕没药酸,可溶性部分含α,β与γ没药酸、没药尼酸、α与β罕没药酚等;挥发油在空气中易树脂化,含丁香油酚、间苯甲酚、枯醛、藻烯、二戊烯、柠檬烯、桂皮醛、罕没药烯等;树胶水解得阿拉伯糖、半乳糖和木糖。
现代药理研究表明没药的水浸剂对皮肤真菌有抑制作用;没药煎剂能增加血流量,降低血管阻力的作用;而且含油树脂能降低雄兔高胆甾醇血症(饲氢化植物油造成)的血胆甾醇含量,并能防止斑块形成,也能使家兔体重有所减轻。与其他含油树脂的物质相似,没药(一般用酊剂)有某些局部刺激作用,可用于口腔洗剂中,也可用于胃肠无力时以兴奋肠蠕动。。
乳香、没药配伍应用始见于“乳香止痛散”(《证治准绳》)。乳香性微寒,味辛、苦、温。归心经、肝经、脾经,有活血止痛,消肿生肌之功效。它是外伤科要药,用于心腹诸痛,筋脉拘挛,跌打损伤,疮痈肿痛;现代药理作用:镇痛、消炎等。乳香辛温芳香,善走窜而行气活血,没药性平,重在苦泄散血化瘀。《本草纲目》言:乳香活血,没药散血,皆能止痛、消肿、生肌,故二药每每相兼而用。《本草述》:久服舒筋膜,通血脉,固齿牙,长须发。乳香、没药同为活血化瘀类中药,乳香性温,没药性平,二者皆味辛、苦,气芳香,入心、肝、脾经,二者为临床上常用药对,对改善心腹疼痛、风湿痹痛、经闭痛经、跌打损伤瘀痛等具有较好的疗效。对心胃腹肋及其肢体骨节诸痛皆能治之。
天然中草药绞股蓝,又名七叶胆,甘茶蔓、七叶参。性味甘、甘、微寒。入肺、脾、胃、心、肾、大肠经。具有健脾理气、益气活血、化痰止咳等功效。常用于气虚血瘀、脉络瘀阻所致的胸痹心痛等证。现代用治冠心病的心绞痛、心肌梗塞、缺血性中风、脑出血后遗症、血管性头痛及肿瘤等。绞股蓝的有效成份是绞股蓝皂甙,是原人参二醇和原人参三醇的异构体。有明显降低胆固醇、甘油三酯、低密度脂蛋白和升高对人体有好处的高密度脂蛋白作用,同时能促进人体内氧化酶作用,使细胞活性上升,有较明显的扩张血管作用。因其有调节脂肪代谢、阻止脂质在血管壁的沉积作用。所以对保护血管内皮细胞,防止动脉粥样硬化有很好的防治作用。现代药理表明,绞股蓝总甙有抑制白胶原诱导的血小板释放S-HT及释放血栓素A2,提高血小板内CAMP水平,降低血液粘稠度。所以,该品对防治冠心病,脑血栓形成具有十分重要意义。经国内外学者研究,绞股蓝总甙具有抗血小板聚集,减少血小板释放血栓素B2(TXB2)、5羟色胺,抗氧自由基,抗脂质过氧化及保护缺血性心肌损伤,降低血脂和血糖等作用。经研究证实绞股蓝皂甙与人参皂甙为同一化合物,绞股蓝改善心肌缺血、促使缺血心肌恢复的作用与含人参皂甙的成分有关。
茯苓,俗称云苓、松苓、茯灵,为寄生在松树根上的菌类植物,形状像甘薯,外皮黑褐色,里面白色或粉红色。其原生物为多孔菌科真菌茯苓的干燥菌核,多寄生于马尾松或赤松的根部。产于云南、安徽、湖北、河南、四川等地。不管寒、温、风、湿诸疾,都能发挥其独特功效。茯苓味甘、淡、性平,入药具有利水渗湿、益脾和胃、宁心安神之功用。现代医学研究:茯苓能增强机体免疫功能,与诸药合用共用达理气活血、健脾化痰之效。
黄连性味苦、寒,归心、胆、脾、胃、大肠经,《珍珠囊》曰:“泻心火,心下痞。”《仁斋直指方》言:“能去心窍恶血”,具有“清热燥湿,泻火解毒”之功。近代病理研究表明黄连具有抗脂质氧化作用。研究表明,黄连具在体内、外都具有较强的抑制脂质过氧化作用,其抑制脂质过氧化作用可能通过清除羟自由基来实现。高脂饲料喂食大鼠发现黄连能有效降低实验大鼠血脂、高敏C反应蛋白水平,抑制炎症反应。黄连的化学成分有生物碱和木脂素,此外还包括有酚酸、挥发油、香豆素、萜类、甾体、多糖、树脂、蛋白质、淀粉、色素以及一些微量元素。其中研究最多是有丰富药理活性的生物碱类。小檗碱是黄连的主要化学成分,研究表明,小檗碱具有显著的抑制肥胖的作用。主要包括抑制前脂肪细胞分化及脂肪生成、诱导白色脂肪细胞向棕色脂肪细胞的转化、增加能量消耗以及调节肠道菌群。另外,小檗碱对肥胖的相关并发症也有一定的作用,如降血糖、降血脂、调节慢性炎症、改善胰岛素抵抗等。
其中,所述的中药组合物按照如下方法制备而成:
1)将原料黄连,乳香,没药,半夏,瓜蒌,绞股蓝,茯苓,穿山龙混合均匀后加水,对混合物进行加热,加热提取2-4次,过滤、合并滤液,获得混合提取液;
2)对混合提取液进行浓缩处理,制成混合浓缩液;
3)将血竭进行粉碎处理,制成血竭粉末;
4)将混合浓缩液与血竭粉末混匀,即得。
其中,步骤1)、3)中所述原料的重量份配比为:黄连3-12,乳香6-15,没药6-15,血竭3-12,法半夏3-9,全瓜蒌6-15,绞股蓝10-30,茯苓10-30,穿山龙10-30。
特别是,所述原料的重量份配比为:黄连3-9,乳香6-12,没药6-12,血竭3-9,法半夏6-9,全瓜蒌9-15,绞股蓝15-30,茯苓15-30,穿山龙15-30,优选为:黄连6,乳香12,没药12,血竭6,法半夏9,全瓜蒌15,绞股蓝30,茯苓30,穿山龙30。
其中,步骤1)中每次加热提取过程中所述原料混合物的总重量与加入的水的重量之比为1∶1~15。
特别是,加热提取温度为60~100℃,优选为80~100℃;加热提取时间为1~5h/次。
特别是,原料混合物加水混合均匀后,浸泡20~90min后,再进行所述的加热提取。
尤其是,所述的浸泡时间优选为30~60min,进一步优选为30min。
其中,步骤1)中所述加热提取按照如下步骤进行:
1A)将原料黄连,乳香,没药,半夏,瓜蒌,绞股蓝,茯苓,穿山龙混合后加水浸泡,加入的水的重量与混合原料的重量之比为(1-15):1,浸泡20-90min后进行第一次加热提取,加热提取温度为60-100℃,加热提取时间为1-5h;
1B)对第一次提取混合体系进行过滤,制得第一次提取液;然后向滤渣中至少加水1次,继续进行至少1次的加热提取,其中,每次加入的水的重量与混合原料的重量之比为(1-15):1,加热提取温度为60-100℃,加热提取时间为1-5h;
1C)过滤,将滤液合并,获得所述的混合提取液。
特别是,步骤1A)中所述的浸泡时间优选为30-60min,进一步优选为30min;加入的水的重量与混合原料的重量之比优选为(5-15):1,进一步优选为(6-12):1更进一步优选为(6-9):1;加热提取时间优选为2-5h;加热提取温度优选为80-100℃;步骤1B)中每次加入的水的重量与混合原料的重量之比优选为(1-5):1;每次加热提取时间优选为1-3h;加热提取温度优选为80-100℃。
其中,步骤2)中所述的浓缩处理是将混合提取液在真空状态下进行减压蒸发浓缩,去除水分,其中浓缩温度为80-100℃,优选为80℃,浓缩时的相对压力为-0.05~-0.1MPa,优选为-0.05~-0.095MPa,进一步优选为-0.07MPa。
特别是,步骤2)中所述的浓缩处理是将混合提取液在常压下的加热蒸发处理,即在绝对压力为1atm下,加热合并后的水提取液,使其中的水分蒸发,达到浓缩目的。
其中,步骤2)中所述混合浓缩液的相对密度为1.05-1.5,优选为1.1~1.5,进一步优选为1.1~1.3;步骤3)中血竭粉末的粒径小于15μm。
特别是,步骤3)中所述粉碎选择为超微粉碎;粉碎后的血竭粉末的粒径优选为1-15μm,进一步优选为10-15μm。
特别是,还包括将混合浓缩液干燥后制成混合药材粉末后再与血竭粉混匀,制成治疗痰瘀互结型肥胖的中药组合物。

本发明的用于痰瘀互结型肥胖的中药组合物可以和药剂学上可接受的辅料按传统或现代生产工艺制成临床可以接受的口服剂型。如胶囊剂、片剂、颗粒剂、丸剂、口服液等剂型的药物。
所述的药剂学上可接受的辅料作为药剂的非活性成分,可以是选自《药物赋形剂手册》(HandbookofPharmaceuticalexcipients,第2版,由A.Wade和P.J.Weller编辑;AmericanPharmaceuticalAssociation出版,WashingtonandThePharmaceuticalPress,London,1994)等工具书中药学上可接受的载体,例如淀粉、糊精、乳糖、微晶纤维、明胶、蔗糖、硬脂酸镁等。
临床上需要长期用药,制成固体制剂贮存方便,患者服用便利,也更适应市场需求。优选的剂型是胶囊剂、颗粒剂、片剂、丸剂、散剂、口服液等。
特别是,所述中药组合物的重量与所述药物或保健品的总重量之比为0.01-100:100,即中药组合物占药物或保健品的0.01-100%。
所述药物可以采用本领域公知的方法制成各种剂型,如片剂、胶囊剂、丸剂、散剂、颗粒剂、糖浆剂、汤剂等。
本发明还提供了一种治疗痰瘀互结型肥胖的方法,每天口服1-2次,每次3-6克(组合物,干燥),孕妇禁止服用,用药期间禁止食用辛辣食物以及饮酒,连续使用2-4周后,体重/BMI显著改善。
本发明的用于痰瘀互结型肥胖的中药组合物具有如下优点:
1、本发明开发、挖掘了中药组合物(骐龙血脉健方)的新的药用价值,将其用于预防、缓解和治疗肥胖和肥胖导致的并发症等病症的治疗,尤其是针对痰瘀互结型肥胖的发病原因,对症治疗,疗效显著。
2、本发明的中药组合物由多种中药成分复合而成,其中的药物的活性成分,相互协同作用,直接调控体内脂肪生成、堆积的病理机制,针对痰瘀互结型肥胖的形成原因,采取多环节综合治疗,有效防止肥胖和由肥胖引发的其他疾病和并发症,治疗效果快,作用迅速,减肥效果稳定,不反弹。
3、本发明的中药组合物(骐龙血脉健方)对肥胖,尤其是痰瘀互结型肥胖的疗效显著,并且具有“通络”效果,尤其适合于痰瘀互结、经脉阻滞的状态。
4、本发明制备的用于痰瘀互结型肥胖的中药组合物(骐龙血脉健方)无毒副作用,制备工艺简单,产品质量稳定效果明显,并且使用方便,填补治疗痰瘀互结型肥胖的中成药市场的不足。
5、本发明的治疗肥胖的中药组合物(骐龙血脉健方)通过化痰活血,降浊通络达到改善肥胖的目的,而且更注重对痰瘀互结证型的改善作用,能够有效防止肥胖进一步引发的其他疾病和并发症。
6、本发明的中药组合物(骐龙血脉健方)原料来源丰富、价廉、临床使用安全,制备工艺简单,可制成各种剂型,且服量小,使用方便,费用低,降低肥胖的治疗成本,节约临床卫生医疗成本,适宜市场推广应用,使更多患者或减肥者从中获益,因此易于推广。

下面结合具体实施例来进一步描述本发明,本发明的优点和特点将会随着描述而更为清楚。但这些实施例仅是范例性的,并不对本发明的范围构成任何限制。本领域技术人员应该理解的是,在不偏离本发明的精神和范围下可以对本发明技术方案的细节和形式进行修改或替换,但这些修改和替换均落入本发明的保护范围内。
以下通过试验例来进一步阐述本发明所述药物的有益效果,这些试验例包括了本发明药物的药效学试验。
实施例1骐龙血脉健颗粒制备
1)按照以下重量配比准备原料药(单位:×10g)
黄连6、乳香12、没药12、血竭6、法半夏9、全瓜蒌15、绞股蓝30、茯苓30、穿山龙30,其中,乳香为醋炒乳香;没药为醋炒没药;
2)将原料药材黄连,乳香,没药,半夏,瓜蒌,绞股蓝,茯苓,穿山龙清洗后,置于中药多功能提取釜中并混合均匀,加入自来水浸泡30min(通常为20-90min),其中,加入的自来水的重量与药材混合物的重量(干重)之比为9:1(通常为1-15:1);
3)开启多功能提取釜电源进行加热,加热至100℃,保持在此温度下恒温提取3h后过滤,得到第一次提取液备用;
4)接着向滤渣中加入1次自来水,加热提取1次,其中每次加入的自来水的重量与药材混合物的重量(干重)之比为5:1(通常为1-15:1),加热提取,温度为100℃,加热提取3h,过滤得到第二次提取液;
加热提取温度不限于100℃,其他温度(如60~100℃,优选为90~100℃)均适用于本发明。
5)将2次提取液合并,制成混合提取液,然后置于真空高能蒸发浓缩器内进行减压蒸发浓缩,制得浓缩浸膏,其中,减压浓缩的相对压力为-0.07MPa(即以一个大气压为0,控制减压浓缩时的压力相对于一个大气压为-0.07MPa),浓缩温度为80℃,将混合提取液浓缩成相对密度为1.2的混合浓缩液;
提取液浓缩过程中除了采用减压蒸发浓缩之外,本领域中现有已知的浓缩方式均适用于本发明,例如常压下的加热、蒸发。混合浓缩液的相对密度为1.05-1.5均适用于本发明。
6)将混合浓缩液于45-50℃下进行干燥处理(烘干),干燥至含水率低于5%(通常为1-4%)后进行粉碎并过180目筛,制成混合药材粉末;
7)将血竭采用超微粉碎机进行超微粉碎处理,制成粒径小于15μm(通常为10-15μm)的血竭超微粉末;
本发明中血竭粉碎以超微粉碎为例进行说明,其他粉碎处理也适用于本发明。
8)将混合药材粉末、血竭超微粉末、辅料糊精混合,乙醇作黏合剂,制粒,干燥,即制得本发明用于治疗肥胖的中药骐龙血脉健颗粒。
实施例2骐龙血脉健丸的制备
1)按照以下重量配比准备原料药(单位:×10g)
黄连3、乳香6、没药12、血竭9、法半夏6、全瓜蒌9、绞股蓝15、茯苓15、穿山龙30
2)将药材黄连、乳香、没药、半夏、瓜蒌、绞股蓝、茯苓、穿山龙漂洗后,置于中药多功能提取釜中并混合均匀,加入自来水浸泡60min,其中,加入的自来水的重量与药材混合物的重量(干重)之比为6:1;
3)开启多功能提取釜电源进行加热,加热至80℃,保持在此温度下恒温提取5小时,然后过滤,得到第一次提取液备用;
4)接着向滤渣中分2次加入自来水,加热提取2次,其中每次加入的自来水的重量与药材混合物的重量(干重)之比为4:1(通常为1-15:1),加热提取温度为80℃,加热提取时间2h,过滤得到2次提取液;
5)将3次提取液合并,制成混合提取液,然后置于真空高能蒸发浓缩器内进行减压蒸发浓缩,制得浓缩浸膏,其中,减压浓缩的相对压力为-0.05MPa(即以一个大气压为0,控制减压浓缩时的压力相对于一个大气压为-0.05MPa),将混合提取液浓缩成相对密度为1.3的混合浓缩液;
6)将混合浓缩液进行喷雾干燥,制得混合药材粉末;
7)将血竭采用超微粉碎机进行超微粉碎处理,制成粒径小于15μm(通常为1-15μm)的血竭超微粉末;
8)将混合药材粉末、血竭超微粉末、辅料炼蜜混合,搅拌均匀后置于制丸机制中进行制丸,然后干燥,即得本发明用于治疗肥胖的中药骐龙血脉健浓缩丸。
实施例3骐龙血脉健片的制备
1)按照以下重量配比准备原料药(单位:×10g)
黄连9、乳香12、没药6、血竭3、法半夏9、全瓜蒌15、绞股蓝30、茯苓30、穿山龙15
2)将原料药材黄连,乳香,没药,瓜蒌,绞股蓝,茯苓,穿山龙清洗后,置于中药多功能提取釜内,加入自来水浸泡20min(通常为20-90min),其中,加入的自来水的重量与药材混合物的重量(干重)之比为12:1(通常为1-15:1);
3)开启多功能提取釜电源进行加热,加热至80℃,保持在此温度下恒温提取5h,然后过滤,得到第一次提取液备用;
4)接着向滤渣中分3次加入自来水,加热提取3次,其中每次加入的自来水的重量与药材混合物的重量(干重)之比为1:1(通常为1-15:1),加热提取温度为90℃,加热提取时间1h,过滤得到3次提取液;
5)将4次提取液合并,制成混合提取液,然后置于真空高能蒸发浓缩器内进行减压蒸发浓缩,制得浓缩浸膏,其中,减压浓缩的相对压力为-0.095MPa(即以一个大气压为0,控制减压浓缩时的压力相对于一个大气压为-0.095MPa),将混合提取液浓缩成相对密度为1.5的混合浓缩液;
6)将混合浓缩液于45-50℃下进行干燥处理(烘干),干燥至含水率低于5%(通常为1-4%)后进行粉碎并过100目筛,制成混合药材粉末;
7)将血竭采用超微粉碎机进行超微粉碎处理,制成粒径小于15μm(通常为1-10μm)的血竭超微粉末;
8)将混合药材粉末、血竭超微粉末、辅料糊精混合,乙醇制粒,干燥,压片机压片,即制得本发明用于治疗痰瘀互结型肥胖的中药骐龙血脉健片。
实施例4骐龙血脉健胶囊的制备
1)按照以下重量配比准备原料药(单位:×10g)
黄连3、乳香15、没药15、血竭3、法半夏3、全瓜蒌6、绞股蓝30、茯苓10、穿山龙30
2)将原料药材黄连,乳香,没药,瓜蒌,绞股蓝,茯苓,穿山龙清洗后,置于中药多功能提取釜内,加入自来水浸泡90min(通常为20-90min),其中,加入的自来水的重量与药材混合物的重量(干重)之比为6:1(通常为1-15:1);
3)开启多功能提取釜电源进行加热,加热至80℃,保持在此温度下恒温提取4h,然后过滤,得到第一次提取液备用;
4)接着向滤渣中加1次自来水,加热提取1次,其中每次加入的自来水的重量与药材混合物的重量(干重)之比为5:1(通常为1-15:1),加热提取温度为80℃,加热提取时间3h,过滤得到第二次提取液;
5)将2次提取液合并,制成混合提取液,然后置于真空高能蒸发浓缩器内进行减压蒸发浓缩,制得浓缩浸膏,其中,减压浓缩的相对压力为-0.07MPa(即以一个大气压为0,控制减压浓缩时的压力相对于一个大气压为-0.07MPa),将混合提取液浓缩成相对密度为1.1的混合浓缩液;浓缩温度为80℃。
6)将混合浓缩液进行冷冻干燥,然后进行粉碎并过100目筛,制成混合药材粉末;
7)将血竭采用超微粉碎机进行超微粉碎处理,制成粒径小于15μm(通常为1-10μm)的血竭超微粉末;
8)将混合药材粉末、血竭超微粉末、辅料微晶纤维混合后,装入明胶硬胶囊,即制得本发明用于治疗痰瘀互结型肥胖的中药骐龙血脉健胶囊。
实施例5骐龙血脉健口服液体剂的制备
1)按照以下重量配比准备原料药(单位:×10g)
黄连12、乳香6、没药6、血竭12、法半夏9、全瓜蒌15、绞股蓝10、茯苓30、穿山龙10
2)将原料药材黄连,乳香,没药,瓜蒌,绞股蓝,茯苓,穿山龙清洗后,置于中药多功能提取釜内,加入自来水浸泡60min(通常为20-90min),其中,加入的自来水的重量与药材混合物的重量(干重)之比为9:1(通常为1-15:1);
3)开启多功能提取釜电源进行加热,加热至100℃,保持在此温度下恒温提取3h,然后过滤,得到第一次提取液备用;
4)接着向滤渣中分2次加入自来水,加热提取2次,其中每次加入的自来水的重量与药材混合物的重量(干重)之比为5:1(通常为1-15:1),加热提取温度为100℃,加热提取时间3h,过滤得到2次提取液;
5)将3次提取液合并,制成混合提取液,然后置于真空高能蒸发浓缩器内进行减压蒸发浓缩,制得浓缩浸膏,其中,减压浓缩的相对压力为-0.07Pa(即以一个大气压为0,控制减压浓缩时的压力相对于一个大气压为-0.07MPa),将混合提取液浓缩成相对密度为1.1的混合浓缩液;浓缩温度为100℃。
6)将血竭采用超微粉碎机进行超微粉碎处理,制成粒径小于小于15μm(通常为1-10μm)的血竭超微粉末;
7)将血竭超微粉末加入到混合浓缩液中,搅拌,混匀后分装于塑料复合膜包装中,制成本发明用于治疗肥胖的中药骐龙血脉健口服液体剂(骐龙血脉健汤)。
试验例1对高脂饮食动物模型的体重影响
1.材料
1.1实验动物及饲养
选取48只8周龄APOE-/-雄性小鼠,体质量(20±2)g(购自北京维通利华实验动物技术有限公司,实验单位许可证编号:SYXK(京)2014-0041,实验动物生产许可证号:SCXK(京)2016-0006)。适应性喂养(SPF级)1周后开始实验。在广安门医院Ⅱ级动物房饲养,雌雄分笼,每笼5只,给予规律光照(昼夜各12小时),饮用无菌水,环境温度为20±2℃,相对湿度为45%-50%,良好的通风条件。
40只APOE-/-小鼠以高脂饲料喂养14周;8只APOE-/-小鼠以普通饲料喂养14周;高脂饲料购于北京华阜康生物科技有限公司,普通饲料由广安门医院实验动物中心提供。
高脂饲料(又称西方饮食)的配方:酪蛋白、蛋氨酸、玉米淀粉、麦芽糖糊精、蔗糖、纤维素、玉米油、无水奶油、矿物质混合物、碳酸钙、维生素混合物、酒石酸氢胆碱、胆固醇、抗氧化剂等,营养成分如表1。
维持饲料(即普通饲料)的原料组成:玉米、豆粕、鱼粉、面粉、麸皮、食盐磷酸铵钙、石粉、多种维生素、多种微量元素、氨基酸等,营养成分如表1。

1.2主要药品及试剂
(1).骐龙血脉健方:黄连6g(成品批号:17020003)、炒乳香12g(成品批号:17010011)、炒没药12g(成品批号:17080101)、法半夏9g(成品批号:17110160)、瓜蒌15g(成品批号:17050141)、穿山龙30g(成品批号:17090117)、绞股蓝30g(成品批号:17100026)、茯苓30g(成品批号:17100101)、血竭6g(成品批号:1701006)。骐龙血脉健方配方由中国中医科学院广安门医院药剂科提供,制成干燥的活性组合物颗粒。
(2).阿托伐他汀钙片(立普妥):规格为20mg/片×7片/盒,辉瑞制药有限公司,批准文号:国药准字J20120049;
1.3主要仪器
YB502精密电子天平(上海精密仪器仪表有限公司);3-18K高速冷冻离心机(Sigma,Germany);-80℃超低温冰箱(Sanyo,Japan);BCD-190TMPK4℃冰箱(中国海尔公司);Milli-Q超纯水净化装置(MerckMillipore,USA);SIM-F140AY65自动制冰机(Sanyo,Japan)。上海寺冈DS-788电子秤。
2.方法
2.1实验动物分组及给药方法
2.1.1实验动物分组
40只8周APOE-/-雄性小鼠,随机分为模型组、阿托伐他汀钙组、中药骐龙血脉健方大、中、小剂量组,共5组,每组8只,以高脂饲料喂养;8只APOE-/-雄性小鼠,为正常组(空白组),以普通饲料喂养。
2.1.2药品配置
(1).骐龙血脉健方(颗粒剂),研磨,临用时用生理盐水配制成混悬液,4℃冰箱保存。
(2).阿托伐他汀钙片,研磨成粉,临用时用生理盐水配制成混悬液,4℃冰箱保存。
2.1.3给药方法及剂量
正常组、模型组予生理盐水灌胃,阿托伐他汀钙组予阿托伐他汀钙溶液灌胃,中药骐龙血脉健方大、中、小剂量组分别给予相应剂量的该方中药颗粒剂配制成的混悬液灌胃。灌胃时间为14周。灌胃剂量为0.1ml/10g/d,实验过程中的灌胃剂量依照每周测得的体重进行调整。
(3).正常组:给予小鼠生理盐水灌胃,每次0.1ml/10g/d,每日一次;
(4).模型组:给予小鼠生理盐水灌胃,每次0.1ml/10g/d,每日一次;
(5).阿托伐他汀钙组:给予小鼠阿托伐他汀钙溶液(人(按60kg)用量20mg/d,用生理盐水配制)灌胃,每次0.1ml/10g/d,每日一次;
(6).骐龙血脉健方大剂量组:给予中药骐龙血脉健方灌胃,按照临床用量20倍,每次0.1ml/10g/d,每日一次;
(7).骐龙血脉健方中剂量组:给予中药骐龙血脉健方灌胃,按照临床用量10倍,每次0.1ml/10g/d,每日一次;
(8).骐龙血脉健方小剂量组:给予中药骐龙血脉健方灌胃,按照临床用量5倍,每次0.1ml/10g/d,每日一次。
2.2小鼠体重测量方法
自0周开始,对所有小鼠每隔一周称重一次,选用清洁容器放置于动物电子秤上,去皮后对小鼠逐一称重,随后进行准确记录直至全部测完,小鼠体重测量结果如表2。
2.3小鼠附睾脂肪分离及称重
14周后,沿腹中线剪开腹部及胸腔,注射器针尖插入左心室,同时剪开右心耳,用预冷的冰生理盐水灌流,完整取下两侧附睾脂肪,生理盐水中清洗血渍,滤纸吸干多余水分,称重,小鼠附睾脂肪称重结果如表3。
2.4数据分析
实验数据采用SPSS24.0软件包进行统计学分析,计量资料用均数±标准差表示:组间比较,符合正态分布时用单因素方差分析,p<0.05为差异有统计学意义;非正态分布时,采用非参数检验,p<0.05为差异有统计学意义。
3.结果
3.1骐龙血脉健方对ApoE-/-小鼠体重的影响

由表2的测定结果可知:第0周,实验各组与模型组相比无差别(P>0.05)。随着时间推移,各组体重均上涨,并且模型组的体重增长幅度大于空白组。各实验组体重增长幅度小于模型组。第14周,模型组体重进一步上涨,体重均数大于空白组(P<0.01)。阿托伐他汀钙组与骐龙血脉健方大、中剂量组体重明显低于模型组(P<0.05),骐龙血脉健方小剂量组体重均数皆低于模型组,但差异无统计学意义(P>0.05)。
3.2骐龙血脉健方对ApoE-/-小鼠附睾脂肪重量的影响

根据上表,ApoE-/-小鼠进食高脂饮食后出现脂肪堆积,附睾脂肪重量相比对照组显著增重(P<0.01),阿托伐他汀钙组给药后重量显著下降(P<0.05),骐龙血脉健方大剂量组降低附睾脂肪重量更为明显(P<0.01),中小剂量体现出一定的降低趋势,但未体现出统计学意义(P>0.05)。可能与附睾脂肪一定程度的个体差异以及每组样本量相对较小有关。但本实验结果能够证明骐龙血脉健方对减轻附睾脂肪重量具有显著效果。
试验例2骐龙血脉健方治疗肥胖症验案
患者刘某,男,45岁,主因“体重升高10余年”,于2014年8月12日就诊。
现病史:患者既往体重80kg左右,10余年前因生活规律紊乱、情绪焦虑,体重渐进增加,至95kg左右渐趋稳定。未就诊及处理。2013-7-15体检示血生化TG 3.12,腹部超声提示脂肪肝(轻度)。近1年调整饮食节律,清淡饮食,适度活动,曾服用益气健脾中药,但体重未见明显变化。
刻下症:气短,乏力,倦怠,眠差梦多,小便可,大便溏结不调。
既往史:高血压病3年,最高150/100mmHg左右,平时在130/86mmHg左右,未规律监测及服药。
家族史:父母均肥胖。
否认外伤史及过敏史。
个人史:嗜食油腻,偶有饮酒,否认吸烟史。
查体及中医四诊:身高168cm,体重96.3kg,神志清楚,表情自如,形体对称,对答切题,语声清晰,唇色暗,舌紫暗胖大,有齿痕及瘀斑,苔黄厚腻,脉弦滑。
西医诊断:1.肥胖症;2.高脂血症;3.脂肪肝;4.高血压。
中医诊断:肥胖(痰瘀互结证)。
治则:化痰活血,降浊通络。
处方:骐龙血脉健方。
药物组成及煎服法:
黄连6g,乳香10g,没药10g,血竭粉分冲6g,法半夏9g,全瓜蒌30g,绞股蓝30g,茯苓15g,穿山龙30g。28剂,水煎服。嘱继续保持饮食清淡,适度活动。
二诊(2014年9月9日):体重91.3kg,患者气短、乏力、倦怠较前好转,倦怠仍稍明显,睡眠稍好转,做梦频次较前减少,大便成型次数增多,时有稀溏。舌紫暗胖大,有齿痕及瘀斑,苔黄腻,脉弦滑。上方略作加减,28剂,水煎服,注意事项同前。
三诊(2014年10月14日):假期停药1周,体重87.6kg,气短、乏力明显好转,仍稍倦怠,睡眠偶有做梦,大便偶有稀便。舌暗红胖大,有瘀斑,苔薄黄,脉弦滑。上方略作加减,28剂,水煎服,注意事项同前。
四诊(2014年11月11日):体重84.5kg,诸症基本消失,依据上方加减继续调治3个月余,体重渐降至75kg左右,嘱保持良好生活习惯,饮食清淡,规律运动。随访未再复发。
试验例2所示病例因体重升高就诊,符合肥胖症的诊断,中医辨证符合痰瘀互结证,经过较长时间服用骐龙血脉健方后,体重逐渐下降,临床症状显著好转,说明骐龙血脉健方能够有效改善肥胖症,尤适于痰瘀互结证。并且在长期使用的过程中,未发现显著的不良反应。
本发明上述实施例仅是范例性的,并不对本发明的范围构成任何限制。本领域技术人员应该理解的是,在不偏离本发明的精神和范围下可以对本发明技术方案的细节和形式进行修改或替换,但这些修改和替换均落入本发明的保护范围内。
Claims (9)
1.一种中药组合物在制备预防或/和治疗肥胖的药物或保健品中的应用,其中,所述中药组合物由如下原料药制备而成:黄连,乳香,没药,血竭,法半夏,全瓜蒌,绞股蓝,茯苓,穿山龙。
2.如权利要求1所述的应用,其特征是,所述药物由所述中药组合物和药学上可接受的载体组成。
3.如权利要求1或2所述的应用,其特征是,所述原料药的重量份配比为:黄连3-12,乳香6-15,没药6-15,血竭3-12,法半夏3-9,全瓜蒌6-15,绞股蓝10-30,茯苓10-30,穿山龙10-30。
4.如权利要求1或2所述的应用,其特征是,所述原料药的重量份配比为:黄连3-9,乳香6-12,没药6-12,血竭3-9,法半夏6-9,全瓜蒌9-15,绞股蓝15-30,茯苓15-30,穿山龙15-30。
5.如权利要求1或2所述的应用,其特征是,所述中药组合物按照如下方法制备而成:
1)将原料黄连,乳香,没药,半夏,瓜蒌,绞股蓝,茯苓,穿山龙混合均匀后加水,对混合物进行加热,加热提取2-4次,过滤、合并滤液,获得混合提取液;
2)对混合提取液进行浓缩处理,制成混合浓缩液;
3)将血竭进行粉碎处理,制成血竭粉末;
4)将混合浓缩液与血竭粉末混匀,即得。
6.如权利要求5所述的应用,其特征是,步骤1)中所述加热提取按照如下步骤进行:
1A)将原料黄连,乳香,没药,半夏,瓜蒌,绞股蓝,茯苓,穿山龙混合后加水浸泡,加入的水的重量与混合原料的重量之比为(1-15):1,浸泡20-90min后进行第一次加热提取,加热提取温度为60-100℃,加热提取时间为1-5h;
1B)对第一次提取混合体系进行过滤,制得第一次提取液;然后向滤渣中至少加水1次,继续进行至少1次的加热提取,其中,每次加入的水的重量与混合原料的重量之比为(1-15):1,加热提取温度为60-100℃,加热提取时间为1-5h;
1C)过滤,将滤液合并,获得所述的混合提取液。
7.如权利要求5所述的应用,其特征是,步骤2)中所述混合浓缩液的相对密度为1.05-1.5,优选为1.1~1.5,进一步优选为1.1~1.3。
8.如权利要求5所述的应用,其特征是,步骤3)中血竭粉末的粒径小于15μm。
9.如权利要求1或2所述的应用,其特征是,所述药物以片剂、胶囊剂、丸剂、散剂、颗粒剂、糖浆剂、溶液剂形式存在。
