Die Erfindung betrifft einen Festphasenimmunoassay zur Erkennung viraler Infektionen. Sie dient dem kombinierten Nachweis mehrerer Infektionserreger, wobei man sich der Diagnostik zirkulierenden Antigens und zirkulierender Antikörper bedient. Sie kann auf dem Gebiet der Humanmedizin, in der Klinischen Chemie und Laboratoriumsdiagnostik, in der mikrobiologischvirologischen und der klinisch-immunologischen Diagnostik eingesetzt werden. Ferner findet sie Anwendung in Hygieneeinrichtungen zur Aufklärung epidemiologischer Situationen hinsichtlich der Verbreitung der nachzuweisenden Erreger sowie in Blutspendezentralen und der Pharmaindustrie bei der Elimination erregerhaltiger biologischer Flüssigkeiten oder Organe.The invention relates to a solid phase immunoassay for the detection of viral infections. Festphasenimmunoassaytechniken sind die am häufigsten angewendeten Methoden zum Nachweis infektiöser Erreger (Bakterien, Viren). Da die Infektion mit einem Erreger in dem befallenen Organismus die Erzeugung spezifischer Antikörper hervorruft, wird deren Nachweis als indirekter Beweis eines stattgefundenen Kontaktes mit dem entsprechenden Erreger gewertet und überall dort angewendet, wo sich virales oder bakterielles Antigen aufgrund sehr geringer Konzentration in dem Untersuchungsgut nicht sicher nachweisen lassen. Kommen letztere in den für die Nachweistechniken ausreichenden Konzentrationen vor, wird ihrem Nachweis als direkter Beweis für die Infektion der Vorzug gegeben (Literaturübersicht bei Porstmann, T. und G. Porstmann, Dt. Gesundheitswesen 36, 1986-1990 [1981]). Damit werden Antikörper- und Antigennachweis als Immunoassayvarianten nebeneinander in verschiedenen Testen praktiziert. Die ständige Zunahme der sich als pathogen herausstellenden Infektionserreger führt zu einer steigenden Zahl unterschiedlicher Teste bei der Diagnostik, vor allem aber bei der Überwachung der für die Therapie vorgesehenen biologischen Produkte. Bisher wurde für den Nachweis eines Erregers mindestens ein Test benötigt. Dabei bedient man sich beim Antigentest bekannter Verfahren wie des kompetitiven Festphasenimmunoassays mit insolubilisiertem Antigen oder Antikörper sowie des Zwei-Seiten-Bindungstestes (Schuurs, A. H. W. M. und B. K. van Weemen: CMn. Chim. Acta 81, 1-40 [1977]). Für den Nachweis spezifischer Antikörper sind vier verschiedene Verfahren - Antiglobulin- oder Sandwichtechnik, Kompetitionstechnik, Capturetechnik und Brückentechnik - beschrieben worden (R. Tedder: in J. C. Gluckman und E. Vilmer (Hrsg.): Acquiered Immuno Deficiency Syndrome; Elsevier, Amsterdam, New York, Oxford, Paris, 1986, S. 173-181).Solid phase immunoassay techniques are the most widely used methods for detecting infectious agents (bacteria, viruses). Der Erfindung liegt die Aufgabe zugrunde, einen Festphasenimmunoassay zum simultanen Nachweis von Antigen und spezifischen Antikörpern zu entwickeln. Die Aufgabe wurde dadurch gelöst, daß Antikörper zur Bindung nachzuweisenden Antigens und Antigen zur Bindung spezifischer Antikörper koimmobilisiert werden bzw. daß Antigen an den Antikörper vor seiner Immobilisierung kovalent gebunden wird. Bei vorhandenem Erregerantigen oder/und Antikörper gegen ein Antigen eines anderen Erregers bindet sich entweder ein markierter Antikörper oder ein markiertes Antigen; bei Doppelinfektionen binden sich beide parallel. Durch Kovalentbindung unterschiedlicher Peptide sowie durch deren Koimmobilisierung und Markierung lassen sich spezifische Antikörper gegen mehr als einen Erreger gleichzeitig in einem Ansatz nachweisen. Das Wesen der Erfindung zielt darauf ab, die Aussagen von zwei oder mehreren Testen in einem zu vereinigen, wobei in dem Fall auf die exakte Erregerdiagnose verzichtet werden kann, wenn bereits einer der nachgewiesenen Erreger definierte antiepidemische Maßnahmen oder die Sperrung des biologischen Produktes nach sich zieht. Erfindungsgemäß wird das dadurch erreicht, daß der Zwei-Seiten-Bindungsassay zum Antigennachweis mit der Brückenantikörpertechnik zum Antikörpernachweis kombiniert wird. Dazu wird bakterielles oder virales Antigen (z. B. gereinigt aus Viruslysat, rekombiniertes Antigen oder synthetische Peptide) entweder mit dem Antikörper zur Bindung des zusätzlich nachzuweisenden Antigens nach bekannten Verfahren an der festen Phase koimmobilisiert oder aber an den Antikörper kovalent gebunden, und nur dieser wird an der festen Phase nach bekannten Verfahren insolubilisiert. Im Falle der Kovalentbindung von Peptiden an den Antikörper wird dessen Reaktivität kaum beeinflußt, so daß mehrere (unterschiedliche) Peptide, die Sequenzen aus Proteinen verschiedener Erreger (z. B. HIV-1 und HIV-2) simulieren, koimmobilisiert werden können. Bei vorhandenem Antigen und/oder spezifischem Antikörper in der zu untersuchenden Probe wird ersteres von dem insolubilisierten Antikörper gebunden, während sich letzterer an das koimmobilisierte bzw. kovalent an den Antikörper fixierte Antigen bindet (Abb. 1 u. 2). Nach bekannten Verfahren werden die Proben zwischen 30-120 min bei 20-37 °C inkubiert und nach deren Entfernung durch Absaugen unspezifisch gebundene Bestandteile durch Spülung mit Waschpuffer bekannter Zusammensetzung entfernt. Im Anschluß daran erfolgt die Inkubation mit einer Kombination von markierten Antikörpern und markiertem Antigen oder markierten Antikörpern unterschiedlicher Spezifität (Abb. 1) bzw. die Inkubation mit einem markierten Antigen-Antikörperkonjugat (kovalente Bindung von Antigen an den Antikörper vor oder nach Markierung) über einen Zeitraum von 30-120 min bei 20-37 0C.
