Description

  • Dieser Implantattyp dient in vollkommen bekannter Weise dazu, das Azetabulum (Gelenkpfanne) des Os ilii zu ersetzen, in dem der Oberschenkelkopf gelenkig gelagert ist. In der Regel besteht der Ring aus Metall, während der Kern aus Polyethylen gefertigt ist.
  • Die Befestigung des Rings im Azetabulum kann auf verschiedene Weise erfolgen. Die erfindung betrifft im engeren Sinne die mit Schrauben angeschlagenen Ringe.
  • Als Beispiel läßt sich das Patent FR 2598908 anführen, das eine Gelenkpfannenprothese beschreibt, die zementfrei an die Pfanne angeschlagen und verschraubt wird. Diese Prothese umfaßt einen Ring, der einen breiten bogenförmigen Ausschnitt aufweist, welcher auf einer Seite vom äußeren Rand bis ungefähr zum Polbereich reicht. Außerdem weist der Ring Unebenheiten auf, die geeignet sind, sich in der Knochenpfanne festzusetzen.
  • Der vorliegenden Erfindung liegt u.a. die Aufgabenstellung zugrunde, das Anschlagen und die Befestigung des Rings im Azetabulum zu erleichtern und zu verbessern.
  • Zur Lösung der Aufgabenstellung, für eine Primärstabilität zu sorgen, die das Einsetzen der Schrauben erleichtert, dann um bei Bewahrung einer guten Sicht auf den Azetabulumboden eine leichte und schnelle Implantation durch ein Einsetzgerät zu gewährleisten und schließlich um dem Ring im Sinne eines leichteren Anschlagens eine gewisse Elastizität zu verleihen, wurde ein Implantat entwickelt und hergestellt nach Art der Implantate mit einem Ring, der einen breiten bogenförmigen Ausschnitt aufweist, welcher auf einer Seite vom äußeren Rand bis zum Polbereich reicht, wobei der Außenrand in der Nähe des Ausschnitts symmetrisch angeordnete Unebenheiten aufweist, die geeignet sind, sich in den Hörnern der Knochenpfanne festzusetzen, um eine primäre Stabilität zu gewährleisten, dadurch gekennzeichnet, dar die innere Aushöhlung des Rings gegenüber der äußeren Aushöhlung exzentrisch versetzt ist.
  • Vorteilhafterweise hat der bogenförmige Ausschnitt die Form eines Schlosses, wobei er im Polbereich einen kreisförmigen Teil aufweist, der symmetrisch durch breite Krümmungsradien mit dem Außenrand in Verbindung steht und dabei einen verschmälerten Schlitz bildet.
  • Es erscheint dadurch möglich, die Lage des Rings am Boden der Pfanne zu kontrollieren, um sich vom einwandfreien Kontakt zwischen dem Ring und dem subchondralen Knochen zu überzeugen.
  • Eine andere Aufgabenstellung, die der Erfindung zugrundeliegt, besteht darin, die Anschlagschrauben im Winkel beliebig ausrichten zu können, um sie je nach Pfannenzustand in die gewünschten Teile der Pfanne zu dirigieren.
  • Diese Aufgabenstellung wird dadurch gelöst, dar der Kopf der Anschlagschrauben halbkugelförmig ausgebildet ist und in komplementär ausgeformten Löchern im dickeren Teil des Rings sitzt.
  • Zur Lösung der Aufgabenstellung der Knochenregeneration ist der Außenrand des Rings mit einer porösen Titanschicht in Form von Plasmaspray überzogen.
  • Zur Lösung der Aufgabenstellung, ein eventuelles Drehen des Kerns im Ring zu vermeiden, weist der Ring über den inneren Außenrand schräg hinausragende Nasen auf, die geeignet sind, mit dem Kern zusainmenzuwirken.
  • Vorteilhafterweise ist der Kern so ausgebildet, um in den Ring einschnappen zu können.
  • Nach einem anderen erfindungsgemäßen Merkmal kann der so beschriebene Ring Standardkerne oder Kerne mit einem seitlichen Vorsprung aufnehmen. Im letzteren Fall wird die Ausrichtung des Kerns vor dem Einschnappen festgelegt.
  • Die Erfindung wird nachstehend anhand der beigefügten Zeichnungen näher erläutert:
  • Die Abbildung 1 ist eine Draufsicht des Rings.
  • Die Abbildung 2 ist eine Schnittansicht gemäß der Linie 2-2 der Abbildung 1.
  • Die Abbildung 3 ist eine Teil- und Schnittansicht in grobem Maßstab zur Illustrierung der Vorkehrungen des Kerns für die Aufnahme der Anschlagschrauben.
  • Die Abbildung 4 ist eine Vorderansicht einer Ausführungsform des Kerns.
  • Die Abbildung 5 zeigt in der Vorderansicht eine andere Ausführungsform des Kerns.
  • Wie die Abbildung 1 zeigt, weist der insgesamt mit (1) bezeichnete Ring auf einer Seite einen breiten bogenförmigen Ausschnitt (1a) auf, der vom äußeren Rand des Rings bis zum Polbereich reicht. Der bogenförmige Ausschnitt (1a) ist in Form eines Schlüssellochs profiliert, um bei einem Eingriff für gute Sichtverhältnisse zu sorgen. Zu diesem Zweck weist der bogenförmige Ausschnitt (1a) im Polbereich einen kreisförmigen Teil (1a1) auf, der symmetrisch durch breite Krümmungsradien (1a2) mit dem oberen Außenrand des Rings in Verbindung steht und dabei einen verschmälerten Schlitz bildet.
  • Es ist auch darauf hinzuweisen, daß der Profilausschnitt (1a) dem gesamten Ring eine gewisse Elastizität verleiht und damit das Anschlagen im Azetabulum begünstigt. Die Elastizität kann außerdem dadurch verbessert werden, daß die innere Aushöhlung (1c) des Rings gegenüber der äußeren Aushöhlung (1d) des Rings exzentrisch versetzt ist.
  • Nach einem anderen Merkmal weist der Außenrand des Rings vor allem auf der Seite des bogenförmigen Ausschnitts (1a) Verankerungsunebenheiten (1b) auf, die geeignet sind, sich in den Hörnern der Knochenpfanne festzusetzen. Diese Unebenheiten (1b) ragen über den Außenrand des Rings hinaus und bilden vor allem dreieckige Profilstücke.
  • Vorteilhafterweise sind diese Verankerungsstücke (1b) gegenüber dem bogenförmigen Ausschnitt (1a) symmetrisch angeordnet. Sie haben die Aufgabe, für eine primäre Stabilität des Rings zu sorgen, um ein leichtes und schnelles Einsetzen der Anschlagschrauben (2) zu ermöglichen.
  • Die Befestigung des Rings erfolgt mit Gewindeformschrauben (2), die z.B. in bekannter Weise aus einer Titanlegierung wie TA6V4 bestehen.
  • Der halbkugelförmige Kopf (2a) der Schrauben (2) sitzt in komplementär ausgebildeten Löchern (1e) im dickeren Teil des Rings. Die Kugelform der Schraubenköpfe ermöglicht deren Verschiebbarkeit in einem Winkel von etwa 30º. Man beachte, daß die Löcher (1e) im dickeren Teil des Rings in zwei parallelen Reihen angeordnet sind.
  • Der Außenrand (1d) des Rings ist mit einer porösen Titanschicht in Form von Plasmaspray überzogen, wodurch in bekannter Weise für eine allmähliche Knochenregeneration gesorgt wird.
  • Nach einem anderen Merkmal weist der Ring, wie insbesondere die Abbildung 1 zeigt, über den inneren Außenrand schräg hinausragende Nasen (1f) auf, die geeignet sind, mit dem Kern (3) zusammenzuwirken. Diese Nasen gewährleisten die Blockierung des Kerns (3) in der Drehbewegung.
  • Der in bekannter Weise aus Polyethylen gefertigte Kern (3) ist profiliert und so ausgebildet, um in die innere Aushöhlung des Kerns einschnappen zu können. Je nach der zu behandelnden Pathologie kann es sich bei dem Kern (3) um einen Standardkern handeln (Abbildung 4) oder um einen Kern mit seitlichem Vorsprung (3a) (Abbildung 5). Der Vorsprung (3a), der gegenüber der horizontalen Ebene einen Winkel von ca. 10º bildet, begrenzt die Luxationsgefahr.
  • Aus der Beschreibung gehen die Vorteile eindeutig hervor; hervorzuheben ist insbesondere:
  • - eine primäre Stabilität, die das Einsetzen der Anschlagschrauben erleichtert;
  • - eine gute Sicht für das einfache und schnelle Einsetzen des Rings mit dem Einsetzgerät;
  • - ein Plasmaspray-Überzug für die Knochenregeneration;
  • - der Einsatz verschiedener Kerntypen, wobei einige einen Vorsprung aufweisen, der die Luxationsgefahr begrenzt.

Claims (7)

1 - Azetabulumimplantat bestehend aus Anschlagschrauben (2), einem mit besagten Schrauben (2) angeschlagenen Ring (1) und einem Kern (3), wobei der Ring (1) einen breiten bogenförmigen Ausschnitt (1a) aufweist, der auf einer Seite vom äußeren Rand bis zum Polbereich reicht, während der Außenrand in der Nähe dieses Ausschnitts symmetrisch angeordnete Unebenheiten (1b) aufweist, die geeignet sind, sich in den Hörnern der Knochenpfanne festzusetzen, um eine primäre Stabilität zu gewährleisten, dadurch gekennzeichnet, daß die innere Aushöhlung (1c) des Rings gegenüber der äußeren Aushöhlung (1d) exzentrisch versetzt ist.
2 - Implantat nach Anspruch 1, dadurch gekennzeichnet, daß der bogenförmige Ausschnitt (1a) im Polbereich einen kreisförmigen Teil (1a1) aufweist, der symmetrisch durch breite Krümmungsradien (1a2) mit dem Außenrand auf der Seite des verjüngten Ringbereichs in Verbindung steht und dabei einen verschmälerten Schlitz bildet.
3 - Implantat nach Anspruch 1, dadurch gekennzeichnet, daß der Kopf (2a) der Anschlagschrauben (2) halbkugelförmig ausgebildet ist und in komplementär ausgeformten Löchern im dickeren Teil des Rings sitzt.
4 - Implantat nach Anspruch 1, dadurch gekennzeichnet, daß der Außenrand (1d) des Rings mit einer porösen Titanschicht in Form von Plasmaspray überzogen ist.
5 - Implantat nach Anspruch 1, dadurch gekennzeichnet, daß der Ring (1) über den inneren Außenrand schräg hinausragende Nasen (1f) aufweist, die geeignet sind, mit dem Kern (3) zusammenzuwirken.
6 - Implantat nach irgendeinem der Ansprüche 1 und 5, dadurch gekennzeichnet, daß der Kern (3) so ausgebildet ist, um in den Ring (1) einzuschnappen.
7 - Implantat nach Anspruch 6, dadurch gekennzeichnet, daß der Kern (3) einen seitlichen Vorsprung (3a) aufweist.
