Description

  • Die vorliegende Erfindung betrifft eine Druckhaltevorrichtung, insbesondere für den Einsatz bei einem zementierten femoralen Hüftschaftersatz. Die Druckhaltevorrichtung ist ebenfalls geeignet für den Einsatz in anderen zementierten prothetischen Anwendungen.
  • Ein femoraler Knochen wird für eine Hüftschaftersatzprothese hergerichtet durch Aufweiten des inneren Hohlraums des femoralen Knochens mittels Einsatz eines Stecheisens oder einer Feile. Der aufgeschnittene Hohlraum wird mit Knochenzement entweder vor oder nach der Einführung der Hüftschaftprothese gefüllt.
  • Bei dem Verfahren, bei dem eine Hüftschaftprothese eingeführt wird, nachdem der Zement in den aufgebrochenen Hohlraum eingebracht worden ist, ist es wichtig, den Schaft in dem erhaltenen femoralen Hohlraum zu zentrieren und den Druck des die Hüftschaftprothese umgebenden Zements aufrechtzuerhalten. Die Zentralisierung einer Hüftschaftprothese und die Druckbeaufschlagung des Zements sind in Verbindung gebracht worden mit der dauerhaften Überlebensfähigkeit des Schafts beim chirurgischen Einsetzen eines künstlichen Hüftgelenks.
  • Bekannte Druckhaltevorrichtungen werden entweder vor der Einführung des Schafts entfernt oder ein Teil der Druckhaltevorrichtung wird entfernt, oder in eine separate Komponente umgewandelt, wodurch der Zement und die Druckerzeugung gestört werden. Es ist eine Aufgabe der vorliegenden Erfindung, den Druck auf dem Zement ohne irgendwelche Störungen aufrechtzuerhalten.
  • Die EP 0 320 138 offenbart eine proximale femorale Abdichtvorrichtung für den Einsatz bei der Anwendung von Knochenzement unter Druck auf einen chirurgisch hergerichteten Knochenmarkskanal des Oberschenkelknochens vor der Implantierung einer Hüftprothese. WO 90/003 75 offenbart eine Vorrichtung zum Abdichten des Knochenmarkkanals, wenn Knochenzement eingesetzt wird.
  • Die Haube ist zwischen zwei Positionen bewegbar, einer ersten Position, in der der Kragen die Öffnung abdeckt und einer zweiten Position, in der die Öffnung nicht abgedeckt ist.
  • Vorzugsweise verfügt der Kragen über eine äußere Oberfläche, die der Form eines Knochenhohlraums entspricht, in den die prothetische Komponente einzusetzen ist.
  • Vorteilhafterweise besitzt die Haube einen Reifen, der sich über die Öffnung in dem Kragen erstreckt. Insbesondere erstreckt sich ein Körper der Haube von dem Reifen zu dem Kragen.
  • Vorzugsweise paßt sich der Reifen an die Form der prothetischen Komponente an. Der Reifen kann mit dem Kragen über ein Federgelenk verbunden sein, gegen das er sich seitwärts falten läßt.
  • Optimalerweise korrespondiert der Körper der Haube zu der Form der zu invertierenden prothetischen Komponente. Die Haube kann an dem Kragen entlang an den vorderen und hinteren Seiten des Kragens befestigt sein.
  • Vorzugsweise ist die Haube über eine zerreißbare und flexible Membran befestigt.
  • Vorteilhafterweise ist die Haube mit einem Einsatz ausgekleidet, um die Haube zu versteifen. Der Einsatz kann sich ebenfalls an die Form der prothetischen Komponente anpassen.
  • Eine Ausführungsform der Druckhaltevorrichtung gemäß der vorliegenden Erfindung wird nun beschrieben mit Hilfe eines Beispiels unter Bezugnahme auf eine Abbildung:
  • 1 stellt eine perspektivische Ansicht einer Druckhaltevorrichtung gemäß der vorliegenden Erfindung dar.
  • Unter Bezugnahme auf die Abbildung ist eine Druckhaltevorrichtung 1 vorgesehen, die in zwei Teilen gebildet ist, einem Kragen 4 und einer Haube 3. Der Kragen 4 ist aus einem flexiblen Material gebildet, wie z.B. Silikon von medizinischer Qualität, und die Haube 3 wird gebildet aus Silikon mit einem Einsatz aus Polyethylen hoher Dichte. Der Körper 2 der Druckhaltevorrichtung 1 weist die Form eines Kragens 4 mit einer oberen Fläche 6, Seiten 8 und einer Bodenfläche 12 auf.
  • Der Kragen 4 definiert eine zentrale Öffnung 14. Die zentrale Öffnung 14 des Kragens 4 korrespondiert mit der Form der prothetischen Komponente, in diesem Beispiel einem Hüftschaft. Die Öffnung 14 verfügt über interne Wände 16, die zu den proximalen Abschnitten des Hüftschafts passen.
  • Der Körper 2 verfügt über einen unteren Abschnitt 10 mit externen Wänden 18. Die externen Wände 18 des unteren Abschnitts 10 korrespondieren zu einem ausgeschnittenen femoralen Kanal, in den die Druckhaltevorrichtung 1 in der Weise eingeführt wird, daß der Körper 2 in dem Einlaß des ausgeschnittenen Kanals des Patientenknochens sitzt.
  • Die Druckhaltevorrichtung 1 verfügt über eine Haube 3 mit einem Reifen 20, der sich in einem Bogen entlang der Öffnung 14 des Kragens 4 erstreckt. Da die Öffnung 14 des Kragens 4 zu dem proximalen Abschnitt des Hüftschafts korrespondiert, verfügt sie im allgemeinen über eine ovale Form, und der Reifen 20 erstreckt sich zentral entlang der schmalen Breite der ovalen Form. Der Reifen 20 korrespondiert zu der Form des Hüftschaftes. Ein Federgelenk 22 verbindet den Reifen 20 mit dem Kragen 4. Der Reifen 20 läßt sich seitlich gegen das Federgelenk 22 falten.
  • Die Haube 3 ist mit einem Polyethyleneinsatz in der Form der Haubenauskleidung 24 ausgekleidet, welche die Einführung des Schaftes unterstützt mittels Versteifung der Haube 3, wodurch gestattet wird, den Schaft auf einfache Weise einzuführen. Die Haubenauskleidung 24 paßt sich ebenfalls an die Form des Hüftschaftes an.
  • Die Haube 3 ist mit der Druckhaltevorrichtung 1 entlang der äußeren und inneren Seiten der oberen Fläche 6 des Kragens 4 über eine zerreißbare und flexible Membran befestigt, die gestaltet ist, zu reißen oder sich zu strecken bei Öffnung der Haube 3.
  • Der Zweck der vorliegenden Erfindung ist es, die Einführung von PMMA-Knochenzement in einen aufgeschnittenen femoralen Kanal unter Verwendung einer unter Druck befindlichen Spritze zu ermöglichen. Die Druckhaltevorrichtung 1 ermöglicht den Druckaufbau des Zements in dem femoralen Hohlraum.
  • Die Druckhaltevorrichtung 1 ermöglicht die Einführung des Schafts ohne die Entfernung oder den Austausch jeglicher Druckhaltekomponenten. Die Kontrolle der endgültigen proximalen Position des Schafts in dem femoralen Hohlraum ist möglich.
  • Der Vorteil der Druckhaltevorrichtung 1 der vorliegenden Erfindung liegt darin, daß der Druck auf dem Zement von der Einführung des Zements bis zum Aushärten aufrechterhalten werden kann ohne jegliche Störungen durch die Druckhaltevorrichtung.
  • Verbesserungen und Modifikationen können von dem vorhergehend Gesagten gemacht werden, ohne von der vorliegenden Erfindung abzuweichen.

Claims (12)

  1. Druckhaltevorrichtung (1), die einen Kragen (4), der eine Öffnung (14) definiert, um eine prothetische Komponente aufzunehmen, und eine Haube (3) aufweist, die an dem Kragen (4) befestigt ist, die zumindest einen Abschnitt der zentralen Öffnung (14) des Kragens umschließen kann, dadurch gekennzeichnet, dass die Haube (3) zwischen zwei Positionen bewegbar ist, einer ersten Position, in der die Haube die Öffnung (14) bedeckt, und einer zweiten Position, in der die Öffnung (14) unbedeckt ist.
  2. Druckhaltevorrichtung (1) nach Anspruch 1, dadurch gekennzeichnet, dass der Kragen (4) eine äußere Oberfläche aufweist, die der Form eines Knochenhohlraums entspricht, in den die prothetische Komponente einzusetzen ist.
  3. Druckhaltevorrichtung (1) nach Anspruch 1 oder Anspruch 2, dadurch gekennzeichnet, dass die Haube (3) einen Reifen (20) aufweist, der sich über die Öffnung in dem Kragen erstreckt.
  4. Druckhaltevorrichtung (1) gemäß Anspruch 3, dadurch gekennzeichnet, dass der Reifen (20) sich an die Form der prothetischen Komponente anpasst.
  5. Druckhaltevorrichtung (1) nach Anspruch 3 oder Anspruch 4, dadurch gekennzeichnet, dass der Reifen (20) an dem Kragen (4) durch ein Federgelenk (22) befestigt ist, gegen das er sich seitwärts falten kann.
  6. Druckhaltevorrichtung (1) gemäß einem der Ansprüche 3 bis 5, dadurch gekennzeichnet, dass ein Körper der Haube (3) sich von dem Reifen zu dem Kragen erstreckt.
  7. Druckhaltevorrichtung (1) gemäß einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass der Körper der Haube (3) der Form der einzusetzenden prothetischen Komponente entspricht.
  8. Druckhaltevorrichtung (1) gemäß einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Haube (3) an dem Kragen (4) entlang den vorderen und hinteren Seiten des Kragens (4) befestigt ist.
  9. Druckhaltevorrichtung (1) nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Haube (3) durch eine zerreißbare und flexible Membran befestigt ist.
  10. Druckhaltevorrichtung (1) gemäß einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Haube (3) mit einem Einsatz (24) ausgekleidet ist, um die Haube zu versteifen.
  11. Druckhaltevorrichtung (1) nach Anspruch 10, dadurch gekennzeichnet, dass der Einsatz (24) sich an die Form der prothetischen Komponente anpasst.
  12. Druckhaltevorrichtung (1) im wesentlichen wie oben beschrieben mit Referenz zu den beiliegenden Zeichnungen.
