I de ovennævnte kendte forbindelser er imidazolringen således substitueret med en benzoyl- eller aromatisk hetero-cycloylgruppe (-CO-Ar), medens de ifølge opfindelsen anvendte forbindelser i den tilsvarende stilling har en derivatiseret 15 carboxylgruppe, og der er ikke påvist ækvivalens med hensyn til cardiotonisk (eller anden farmakologisk) virkning mellem sådanne forbindelsestyper.
Fra Chemical Abstracts 82:152201a (1977) kendes imid-azolderivater med bl.a. hypotensiv og blodpladeaggregations-20 inhiberende virkning. Disse kendte forbindelser adskiller sig fra de ifølge opfindelsen anvendte forbindelser ikke blot ved forskellig virkningsretning, men også strukturelt, idet de i 2-stillingen har svovl og i 4-stillingen har en phenylsubstituent i stedet for 0^.4-alkyl.
25 De mest foretrukne forbindelser, der fremstilles ved fremgangsmåden ifølge opfindelsen, er sådanne hvor R2 er methoxy eller ethoxy.

hvor % og R2 har de for formlen I anførte betydninger, og M er et pharmaceutisk acceptabelt alkalimetal, såsom natrium eller kalium, jordalkalimetal, såsom calcium eller magnesium, et overgangsmetal, såsom zink eller jern, eller et hoved-5 gruppemetal.
Ved omsætningen af et aminoketocarboxylat med formlen 10 NH2 hvor Ri og R2 er som defineret for formlen I, er cyanatelier thiocyanatsaltet fortrinsvis et natrium- eller kalium-cyanat eller -thiocyanat. Denne omsætning sker ved at blande ca. 1 molækvivalent af et tilsvarende aminoketocarboxylat 15 med ca. 1 til ca. 5 molækvivalenter, fortrinsvis ca. 1 molækvivalent, af et cyanat- eller thiocyanatsalt i et passende opløsningsmiddel. Reaktionen får lov at forløbe i ca. 5 minutter til 10 timer afhængigt af reagenserne, opløsningsmidlet og temperaturen, der kan være fra ca. -10 til ca.
20 50°C, fortrinsvis 0°C. Egnede opløsningsmidler til denne reaktion er et ikke-reaktionsdygtigt opløsningsmiddel, fortrinsvis et med vand blandbart opløsningsmiddel, f.eks. en organisk syre, såsom eddikesyre, en alkohol, såsom methanol eller ethanol, eller en ether, såsom tetrahydrofuran eller 25 p—dioxan. Fortrinsvis blandes opløsningsmidlet med vand. Det foretrukne opløsningsmiddel er vandig ethanol.
Produktet af denne reaktion kan isoleres ved en hvilken som helst kendt metode, såsom omdannelse til det tilsvarende natrium- eller kaliumsalt og genfældning med carbon-30 dioxid eller en mineralsyre, såsom fortyndet saltsyre.

nol, n-propanol eller n-butanol, dimethylformamid eller dimethylsulfoxid. Imidazolcarboxylsyrederivatet og base får lov at reagere i ca. 1 minut til ca. 24 timer afhængigt af reagenserne og temperaturen, der kan være fra ca. -78°C til 5 ca. 150°C, fortrinsvis fra ca. 0 til ca. 25°C.
Aminoketocarboxylatet med formlen III kan fremstilles ved reduktion af en tilsvarende oxim med formlen 10 ^

30 Forbindelserne med formlen I kan anvendes til be handling af hjerteinsufficiens', herunder'på grund af kongestipn, baglæns hjertesvigt, forlæns hjertesvigt, svigtende funktion af venstre hjertekammer eller svigtende funktion af højre hjertekammer eller til behandling af en hvilken som helst 35 anden tilstand, der kræver styrkelse af hjertefunktionen med et cardiotonisk middel. I mange henseender har disse forbindelser en digitalis-lignende virkning.

Anvendeligheden af forbindelser med formlen I som cardiotoniske midler kan bestemmes ved at indgive prøveforbindelsen (0,1-10 mg/kg) intravenøst, intraperitonealt, in-traduodenalt eller intragastrisk i et passende bærestof til 5 en bastardhund (ligegyldigt hvad køn). Forsøgshundene anæ-stetiseres og forberedes ved at isolere en passende arterie (f.eks. femoral eller halspulsåre) og vene (f.eks. femoral eller ydre halsvene), indføres polyethylenkatetere fyldt med 0,1% heparin-Na for at måle arterieblodtryk og indgive 10 forbindelser. Brystkassen åbnes ved at spalte brystbenet i midterlinien eller ved indskæring ved venstre femte ribbensmellemrum. En Walton-Brodie spændingsmåler sys fast til højre eller venstre ventrikel for at måle myocardiets sammentrækningsstyrke. En elektromagnetisk flydesonde kan anbrin-15 ges rundt om roden af den opadgående aorta for at måle hjertekapaciteten minus coronarblodstrømmen. Aorta og vena cava kan forbindes for at måle venereturmængden til hjertet. Alternativt kan hjerte og lunger isoleres vaskulært fra resten af kredsløbssystemet. Der fremkaldes hjertesvigt ved at ind-20 give natriumpentobarbital (20-40 mg/kg injektion efterfulgt af konstant infusion af 0,25 mg/kg/min), ved at indgive propa-nalol-hydrochlorid (4 mg/kg injektion efterfulgt af konstant infusion på 0,18 mg/kg/min) intervenøst eller ved at indgive natriumpentobarbital (0,15 mg/ml) i blodet, der gennem-25 strømmer hjertet. Efter indgivelse af et af disse hjerte-funktionsnedsættende midler stiger højre arterietryk dramatisk og hjertekapaciteten reduceres alvorligt. Afbødning af disse virkninger ved hjælp af prøveforbindelsen viser den cardiotoniske aktivitet.

Forbindelserne kan indgives på forskellige måder for at opnå den ønskede virkning. Forbindelserne kan indgives alene eller i form af pharmaceutiske præparater til den patient, der behandles, enten oralt eller parenteralt, dvs.
intravenøst eller intramuskulært. Den mængde forbindelse, der indgives, varierer afhængigt af hvor alvorlig den svigtende hjertefunktion er og af indgivelsesmåden.

Til oral eller parenteral indgivelse ligger den dardiotonisk effektive mængde forbindelse fra ca. 0,01 mg/kg af patientens' legemsvægt pr. dag op til ca. 500 mg/kg af patientens legemsvægt pr. dag og fortrinsvis fra ca. 0,1 mg/kg 5 af patientens legemsvægt pr. dag op til ca. 50,0 mg/kg af patientens legemsvægt pr. dag.
Til oral indgivelse kan en enhedsdosering f.eks. indeholde fra 1 til 500 mg aktivt stof. Til parenteral indgivelse kan en enhedsdosis f.eks. indeholde fra 1 til 50 mg 10 aktivt stof. Gentagne daglige indgivelser af forbindelserne kan være ønskelige og vil variere alt efter patientens tilstand og indgivelsesmåden.
Som anvendt her betyder udtrykket "patienter" varmblodede dyr, f.eks. fugle såsom høns og kalkuner, samt patte-15 dyr såsom primater, mennesker, får, heste, kvæg, svin, hunde, katte, rotter og mus.
Til oral indgivelse kan forbindelserne sammensættes til faste eller flydende præparater såsom kapsler, piller, tabletter, pastiller, puddere, opløsninger, suspensioner el-20 ler emulsioner. De faste doseringsformer kan være en kapsel, der kan være af den almindelige gelatinetype, der f.eks. indeholder smøremidler og inaktivt fyldstof såsom lactose, saccharose og majsstivelse kombineret med bindemidler såsom acacie, majsstivelse eller gelatine, disintegreringsmidler 25 såsom kartoffelstivelse eller algininsyre og et smøremiddel såsom stearinsyre eller magnesiumstearat.
Til parenteral indgivelse kan forbindelserne indgives som injicerbare doser af en opløsning eller suspension af forbindelsen i et fysiologisk acceptabelt fortyndingsmid-30 del med et pharmaceutisk bærestof, der kan være en steril væske såsom vand og olier med eller uden tilsætning af et overfladeaktivt stof og andre pharmaceutisk acceptable hjælpestoffer. Som eksempel på olier, der kan anvendes i disse præparater, er sådanne af jordolie, animalsk, vegetabilsk eller 35 syntetisk oprindelse, f.eks. jordnøddeolie, sojabønneolie og mineralolie. vandig dekstrose og beslægtede sukkeropløsninger, ethanol og glycoler såsom propylenglycol eller polyethylenglycol som flydende bærestoffer, især til injicerbare opløsninger.
Forbindelserne kan indgives i form af en depotinjek-5 tion eller et implanteringspræparat, der kan sammensættes på en sådan måde, at det tillader protraheret frigivelse af det aktive stof. Det aktive stof kan komprimeres til pellets eller små cylindre og implanteres subcutant eller intramuskulært som depotinjektioner eller implantater. Implantater kan væ-10 re kombineret med inaktive materialer såsom bionedbrydelige polymere eller syntetiske siliconer, f.eks. "Silastic", en siliconegummitype fremstillet af Dow-Coming Corporation.
Opfindelsen vil i det følgende blive nærmere belyst ved hjælp af eksempler, idet eksempel 1 illustrerer fremstil-15 lingen af forbindelserne med formlen I ved fremgangsmåden ifølge opfindelsen, medens eksempel 2 illustrerer disse forbindelsers cardiotoniske virkning.
Eksempel 1 20 l/l~Dimethylethyl-2,3-dihydro-5-methyl-2-oxo-lH-imidazol-4-carb- oxylat
Til en opløsning af 31,6 g (0,2 mol) tert.buty1-acetoacetat i 30 ml eddikesyre, der omrøres og afkøles i et bad af is og methanol, tilsættes dråbevis i løbet af 70 minut-25 ter en opløsning af 15,3 g (0,22 mol) natriumnitrit i 50 ml vand. Blandingen omrøres i 2 timer ved 0°C, og der tilsættes 500 ml ethylether. Den etheriske opløsning vaskes med vand, natriumbicarbonatopløsning, natriumchloridopløsning og tørres over magnesiumsulfat. Opløsningsmidlet afdam-30 pes, hvilket efterlader 36,4 g af en olie, der er tert.buty 1-2-(oximino)-3-oxobutanoat.
Olien opløses i 300 ml ethanol og 200 ml 2N saltsyre. Der tilsættes 2,0 g 10%'s palladium på trækul, og blandingen rystes under hydrogengas i en Parr-ryster, indtil der 35 er forbrugt 2 molækvivalenter hydrogen (ca. 2 timer). Katalysatoren fjernes ved filtrering, hvilket giver en sur opløsning af tert.butyl-2-amino-3-oxo-butanoat, der deles i 2 lige store portioner.

Til den ene halvdel af ovennævnte opløsning tilsættes 16/2 g (0,2 mol) kaliumcyanat, og blandingen opvarmes på dampbad i en time. Opløsningen, der bliver neutral, syrnes med 2N saltsyre, og produktet krystalliserer ved afkøling.

Bastardhunde med en legemsvægt på 1-3 kg anæsteti-seres med 35 rag/kg pentobarbitalnatrium intravenøst. Deres brystkasse åbnes kirurgisk, og der dannes en pericardievugge 15 til understøtning af hjertet. En Brodie-Walton deformationsmåler sutureres til venstre ventrikel til overvågning af myo-cardiets sammentrækningskraft. Apparater til overvågning af hjertehastighed og arterieblodtryk tilsluttes ligeledes.
