Den foreliggende opfindelse angår kirurgiske indretninger for tilvejebringelse af dræn i intra-abdominal viscera.
Indføringen af drænrør i maven, galdeblæren eller andet intra-abdominal viscera medfører ifølge sagens natur en risiko for 5 at spilde mavesaft, galde eller infekterede væsker i bughulen, hvis viscera bliver invaginateret under tragtudvidelse, eller wireføringen bliver snoet inde i bughulen og drænet ikke kan genindføres. Derfor er et formål med denne opfindelse at tilvejebringe en visceral forankringsanordning, som atraumatisk 10 mobiliserer og internt tamponerer mavens, tarmens eller galdeblærens væg og/eller udefra tilvejebringer adgang gennem bugeller brystvæggen før indføring af en større fastholdelseslykke eller andet dræn.
Mange og forskellige indretninger og fremgangsmåder har været 15 fremme med det formål at gøre det nemmere at indføre dræn i intra-abdominal viscera. Et mavekateter-placeringsssystem er beskrevet US Patent nr. 3.961.632 af Moossun, 8. juni 1976. Moossun-systemet tilvejebringer mobilisering af mavevæggen ved anvendelse af en magnetisk nasogastrisk intubationsindretning.
20 Efter at mavevæggen er ført ind i nærheden af bugvæggen, føres et kateter af Foley-typen gennem bugvæggen ind i maven. Maven holdes på plads ved hjælp af en oppustelig ring på kateteret og en ydre låseskive. En fremgangsmåde og et apparat til dræn af urin, er beskrevet i US Patentskrift nr. 3.598.124 af An-25 derson den 10. august 1971. Anderson anvender et kateter af Foley-typen til fastholdelse af dræn i blæren.
Der kendes flere typer af tilbagetrækkelige mekaniske indretninger til indføring, fastholdelse og fjernelse igen af perku-tane transportrør. Eksempler udgøres f.eks. af indretninger 30 beskrevet i US Patentskrift nr. 4.393.873 af Nawash, 19. juli 1983; US Patentskrift nr. 3.039.468 af Price, 19. juni 1962, og i US Patentskrift nr. 3.835.863 af Goldberg, 17. september 1974. Nawash beskriver, at utilsigtet fjernelse af transportrøret placeret i maven, forhindres ved hjælp af en eftergive- 2 DK 169986 B1 lig spids, som indtager en udadbulende konfiguration efter opløsning af en retentionsbinding. Price beskriver et trocar med sammentrækkelige fingre. Fingrene indgriber med mavevæggen og en glidende klemme indgriber med den ydre hud for at trække « 5 mavevæggen og huden sammen om kanylen. Goldberg-indretningen er et T-formet drænrør, som kirurgisk implementeres i en ka- * nal. På grund af fleksibiliteten af tværrøret, folder tværrørets arme sig til en i hovedsagen parallel position, når røret trækkes tilbage gennem åbningen i kanalen.
10 Anvendelsen af wirestyr ved kirurgiske metoder med indføring af katetre i vasculære systemer er beskrevet i US Patentskrift nr. 4.534.363 af Gold den 26. april 1984. I Gold's patent er beskrevet en forbedret belægning til angiografiske wireførin-ger og en fremgangsmåde til fremstilling af wireføringen.
15 Som det fremgår af den foreliggende opfindelse, er der endnu ikke før nu blevet tilvejebragt en simpel indretning til atraumatisk mobilisering og intern tamponering af væggen af en intra-abdominal viscera mod bug- eller brystvæggen.
Det er opfindelsens formål at tilvejebringe en visceral for-20 ankringsindretning til percutan indføring i en hul viscus.
Den viscerale forankringsindretning omfatter en biokompatibel langstrakt tværstang og en fleksibel biokompatibel sutur fastgjort til en af stangens ender, nær dens center. En metode til visceral vægmobilisering ved brug af den viscerale foran-25 kringsindretning skal nu beskrives. Et første trin er at tilvejebringe en tragt eller en passage uden for legemet gennem huden og den viscerale væg til visceral lumen. Et yderligere trin er at indføre den viscerale forankringsindretning gennem tragten og ind i det viscerale lumen. Under indføringen af den 30 langstrakte tværstang, rettes denne ind langs tragtens langs- * gående akse. Suturen er ladt sig strækkende gennem tragten med dens frie ende forblivende uden for legemet. Et næste trin udgør trækning i suturen, indtil tværstangen er rettet ind 3 DK 169986 B1 vinkelret på tragtens langsgående akse og indtil tværstangen indgriber med den viscerale væg. Når der trækkes yderligere i suturen, vil den viscerale væg mobiliseres.
En foretrukket udførelsesform ifølge opfindelsen er en genud-5 trækkelig visceral forankringsindretning. Den genudtrækkelige viscerale forankringsindretning omfatter en biokompatibel langstrakt tværstang, en fleksibel biokompatibel sutur, hvis ende er fastgjort nær tværstangens center, og en anden fleksibel biokompatibel sutur, hvis ene ende er fastgjort til den 10 ene ende af tværstangen. Ved et første trin føres en tragt udefra gennem legemets hud og viscerale væg til visceral lumen. Tragten har en langsgående akse, der strækker sig fra uden for legemet og til visceral lumen. Ved et næste trin, indføres den viscerale forankringsindretning gennem tragten og 15 ind i visceral lumen. Ved indføringen er tværstangen rettet ind langs tragtens længdeakse. Suturerne er ladt sig strækkende gennem tragten med deres frie ender forblivende uden for legemet. Ved næste trin trækkes der i suturen, fastgjort til tværstangen center, indtil tværstangen er rettet ind vinkelret 20 på tragtens længdeakse og tværstangen indgriber med den viscerale væg. Når der trækkes yderligere i den første sutur, vil den viscerale væg mobiliseres. Ved endnu et trin genudtrækkes den viscerale forankringsindretning ved at slække den første sutur, således at ankret frigøres fra indgreb med den visce-25 rale væg. Derefter trækkes der i suturen fastgjort til enden af tværstangen, indtil tværstangen er rettet ind langs med tragtens længdeakse, hvorefter stangen trækkes gennem tragten.
Ifølge opfindelsen opnås i forhold til den kendte teknik en simpel visceral forankringsindretning til atraumatisk mobili-30 sering og indvendig tamponering af en intraabdominal viscus imod abdominal- eller brystvæggen.
Opfindelsen forklares nedenfor under henvisning til tegningen, hvor 4 DK 169986 B1 fig. 1 viser i forstørret målestok den viscerale forankrings-indretning, fig. 2 et langsgående snit gennem den i fig. 1 viste indretning, t 5 fig. 3 skematisk første trin ved fremgangsmåde til visceral vægmobilisering ifølge opfindelsen, fig. 4 skematisk et andet trin ved fremgangsmåden, hvor den viscerale forankringsindretning ses efter indføringen i en hul viscera, 10 fig. 5 skematisk et yderligere trin ved fremgangsmåden, hvor ankret ses beliggende vinkelret på tragtens længdeakse og indgribende med den viscerale væg, fig. 6 endnu et trin ved fremgangsmåden, hvor den viscerale væg ses trukket ind mod bugvæggen, og et drænrør er ført ind i 15 visceral lumen, fig. 7 skematisk et trin ifølge fremgangsmåde til fjernelse af forankrings indretningen, og hvor den viscerale forankringsindretning ses frigjort fra indgreb med den viscerale væg og endesuturen strækker sig gennem en kanyle, 20 fig. 8 et yderligere trin ifølge fremgangsmåden til fjernelse af forankrings indretningen, hvor den viscerale forankrings indretning ses rettet ind langs kanylens længdeakse, før den fjernes med kanylen, fig. 9 i perspektiv en anden udførelsesform af den viscerale ♦ 25 forankringsindretning ifølge opfindelsen, * fig. 10 skematisk et trin ifølge en alternativ udførelsesform af fremgangsmåden, og DK 169986 B1 s fig. 10A den i fig. 10 viste nål set fra en 90° drejet vinkel.
Under henvisning til tegningen beskrives nu en bestemt udførelsesform ifølge opfindelsen. Dette skal dog ikke opfattes som et forsøg på at begrænse opfindelsens ide, idet der kan fore-5 tages mange ændringer og modificeringer, specielt af en fagmand inden for området, uden at man derved afviger fra selve opfindelsens ide.
Den i fig. 1 viste viscerale forankringsindretning 10 omfatter en tværstang 11 og sutur 12. Tværstangen 11 har en lang-10 strakt og cylindrisk form og omfatter en centerpart 15, beliggende mellem enderne 13 og 14. Suturen 12 omfatter en fastgjort ende 17 og en fri ende 18. Enden 17 er fastgjort til tværstangen li, et sted i hovedsagen ved dennes center. Forankringsindretningen 10 omfatter endvidere en sutur 20 med en 15 fri ende 21 og en ende 22 fastgjort til tværstangens 11 ende 13. Suturerne 12 og 20 er typisk fremstillet af almindeligt suturmateriale. Ved den foretrukne udførelsesform anvendes der f.eks. 4-0 TEVDEK suturmateriale. Suturerne 12 og 20 er omkring 40 cm lange.
20 Den indre konstruktion af tværstangen 11 er vist i fig. 2. Tværstangen 11's centerpart 15 omfatter en stiv stang 30, omsluttet af et spiralfjederhylster 16. Ved den foretrukne udførelsesform udgøres hylstret 16 af et standard rustfrit stål-kirurgisk wireføringsmateriale med en omtrentlig yderdiameter 25 på 0,87 mm.· Stangen 30 kan udgøres af en rustfri stålkanyle eller stang. Ved den foretrukne udførelsesfom, er stangen 30 omtrent 10 mm lang. Hylstret strækker sig ud over enderne af stangen 30 til dannelse af fleksible endeparter 32 og 33. Det er ønskværdigt at tilvejebringe fleksible endeparter således, 30 at forankringsindretningen 10 atraumatisk kan indgribe med de indre vægge af intra-abdominal viscera. Ved denne i fig. 2 viste udførelsesform strækker suturerne 12 og 20 sig ind i det indre af hylstret 16 og er indbyrdes forbundet rundt om stangen 30 ved 31. Suturerne 12 og 20 kan udgøres af en enkelt su- 6 DK 169986 B1 tur for nemmere fremstilling.
Tværstangens li ender 13 og 14 er afrundede, og strækker sig ud over enderne af det skruefjederformede hylster 16. Enderne 13 og 14 kan fremstilles af ethvert passende biokompatibelt 5 materiale, som er i stand til at "binde" med hylstret 16. Der kan f.eks. anvendes epoxy til fastgørelsen eller også kan en lille kugle af metal være fastsvejset til hylstret 16. Ved den foretrukne udførelsesform, anvendes dog epoxy til formgivning af enderne 13 og 14 og til at fastgøre disse til hylstret 16.
10 Suturen's 20 fastgjorte ende 22 er ført gennem endedelen 13, til fastgørelse af suturen 20 til tværstangen 11. Suturen 20 er derved ført gennem epoxymaterialet for at være fastgjort ved 31, som tidligere beskrevet.
Fremgangsmåden med visceral vægmobilisering ved anvendelse af 15 den viscerale forankringsindretning, skal beskrives under henvisning til fig. 3-7.
Som vist i fig. 3, tilvejebringes fra ydersiden af legemet en tragt eller passage gennem huden og den viscerale væg til visceral lumen ved hjælp af nålepunktering. Ved den foretrukne 20 udførelsesform af fremgangsmåden, anvendes til punkteringen en 15 cm lang, størrelse 22, nål 39, hvorover en kanyle eller et størrelse 16 plasthylster 40 er monteret. Efter at den udspilede visceral lumen er forsynet med den udragende nål, føres den ydre kanyle eller hylstret ind over nålen og anbringes på 25 plads for at udgøre tragten. Når hylstret er på plads, fjernes nålen. I fig. 3 er vist hvorledes plasthylstret 40 udgør tragten, som strækker sig fra udenfor legemet gennem bugvæggen 41 og den viscerale væg 43 ind i viscera's 42 visceral lumen 44.

Fig. 4 viser et yderligere trin ved fremgangsmåden, hvor tvær-30 stangen 11 er ført ind gennem tragten, som udgøres af hylstret * 40 og ind i visceral lumen 44. Tværstangen 11 er rettet ind langs tragtens længdeakse ved indføringen, og med suturerne 12 og 20 sig strækkende gennem hylstret 40 med deres frie ender 7 DK 169986 B1 18 og 21 forblivende uden for legemet. Tværstangen 11 trækkes gennem hylstret 40 ved hjælp af en 0,95 mm "J" bukket wirefø-ring 45. I fig. 4, er vist tværstangen 11 inde i visceral lumen 44 efter at den er blevet trukket gennem hylstret 40 ved 5 hjælp af wireføringen 45.
Herefter trækkes der i suturen 12, idet der holdes i hylstret 40, hvilket medfører at tværstangen 11 positioneres vinkelret på tragten's længdeakse og derved vil tværstangen li indgribe med den hule viscera's indre væg. Der trækkes i hovedsagen i 10 suturen 12 i den med pilen A viste retning. Hylstret 40 fjernes udover suturerne 12 og 20, som vist i fig. 5. I fig. 5 er vist, tværstangen 11 rettet ind vinkelret på nåltragten 48 og indgribende med den indre viscerale væg 47. Længden af tværstangen 11 er større end diameteren af nåltragten 48, og der-15 for indgriber tværstangen 11 med den indre viscerale væg 47. Herefter trækkes der yderligere i suturen 12, indtil den viscerale væg har bevæget sig ind i tæt nærhed af bugvæggen. Med den viscerale væg trukket tæt ind mod den parietale væg, er der en meget lille chance for intraperitoneal lækage. Trækket 20 i suturen 12 bibeholdes ved at fastgøre suturen 12 til huden ved anvendelse af en kirurgisk standardnål.
Wireføringen anvendes nu til yderligere udvidelse af nåltragten 48, ved at skubbe en eller flere forøgende større dilato-rer over wireføringen og ind i visceral lumen. Sådanne dila-25 torer er almindelig kendte og skal derfor ikke vises eller beskrives yderligere. Drænrøret 50 kan derefter indsættes uden fare for skade og for at tilvejebringe forbindelsen mellem visceral lumen 44 og uden for legemet, I fig. 6 er vist drænrøret 50 indsat, mens tværstangen 11 fastholder den viscerale 30 væg 43 mod bugvæggen 41. Den løse sutur 20, er fasttapet til drænrøret 50.
Få dage efter at drænet er blevet tilvejebragt, er den viscerale forankringsindretning 10 måske ikke længere nødvendig, og kan derfor nemt fjernes. Idet der herefter henvises til fig. 7 8 DK 169986 B1 og 8 afskæres suturen 12, hvorved trækket på tværstangen 11 aflastes. En kanyle 55, som f.eks. kan være en 5 "French" kanyle, anvendes til at fjerne den viscerale forankringsindretning 10. Kanylen 55 er trykket ind over suturen 20, som vist i ? 5 fig. 7 og ført ind ved siden af drænrøret 50 i visceral lumen 44. Når kanylen 55 distale ende 56 er inde i visceral lumen 44, kan et svagt træk i den med pilen B viste retning udøves på suturen 20. Tværstangen 11 vil rette sig selv op langs med kanylens 55 længdeakse, som vist i fig. 8 og kan derefter nemt 10 trækkes ud sammen med kanylen 55. Tvær s tangen's 11 ende 13 holdes i berøring med kanylens distale ende 56 ved træk i suturen 20, hvorved disse forbliver rettet koaksialt ind med hinanden, mens kanylen 55 og tværstangen 11 trækkes ud. Suturen 12 vil følge med tværstangen 11, når tværstangen og kany-15 len trækkes ud.
Opfindelsen er blevet beskrevet under henvisning til en bestemt udførelsesform. Der kan dog foretages mange ændringer u-den at man derved afviger fra opfindelsens ide. F.eks. viser fig. 9 en alternativ udførelsesform af den viscerale foran-20 kringsindretning 10', som er identisk med den i fig. 1 og 2 viste udførelsesform, dog bortset fra at suturen 20 ikke er tilstede. Den i fig. 9 viste udførelsesform med kun en enkelt sutur, anvendes når forankringsindretningen 10' ikke skal trækkes tilbage, d.v.s. i modsætning til den ovenfor beskrevne 25 forankringsindretning 10, der kan trækkes tilbage. En sådan visceral forankringsindretning 10' er monteret nær den distale ende af indføringsnålen, som vist i fig. 10 og anbringes under starten af punkteringen. Som vist i fig. 10 har nålen 100 i dens hule indre forankringsindretningen 10' og en wireføring 30 101, som anvendes til at trække forankrings indretningen 10' ud af nålen, når penetrering af viscera er opnået.
