10 hvori R betegner en phenylgruppe, der eventuelt er substitueret med 1-3 substituenter valgt blandt Cx-C4 alkyl, C1-C4 al-koxy, hydroxy, halogen og trifluoromethyl, eller en 15 isoxazolyl- eller thienylgruppe, der eventuelt er substitueret med 1-3 substituenter valgt blandt Cx-C4 alkyl,
Cx-C4 alkoxy, hydroxy og halogen;

Ved udtrykket lige eller forgrenet Cx-C4 alkyl skal for-30 stås methyl, ethyl, propyl, isopropyl, butyl, isobutyl, sec.butyl, t.butyl, n.pentyl, 3-pentyl, isopentyl, neopentyl, hexyl og isohexyl.
Ved udtrykket C3-C6 cycloalkyl skal forstås cyclopropyl, 35 cyclobutyl, cyclopentyl og cyclohexyl.

Forbindelserne med formlen I indeholder mindst 2 asymmetriske centre, og de kan foreligge i form af stereo-isomere blandinger eller som en enkelt stereoisomer, der kan opnås ved stereoselektiv syntese eller ved at skille 5 den fra den stereoisomere blanding.
Derfor er de stereoisomere blandinger såvel som den enkelte stereoisomer af forbindelser med formlen I omfattet af den foreliggende opfindelse.

Herudover tilvejebringer opfindelsen salte af forbindelserne med formlen I med organiske eller uorganiske syrer, der er velegnede til farmaceutisk anvendelse.
15 Eksempler på velegnede syrer er saltsyre, hydrogenbromid-syre, svovlsyre, citronsyre, ravsyre, para-hydroxybenzoe-syre, maleinsyre og glycolsyre. Foretrukne forbindelser med formlen I er sådanne forbindelser, hvor R betegner phenyl eller isoxazolyl, der eventuelt er substitueret 20 med 1-3 substituenter valgt blandt chlor- og bromatomer, methyl, ethyl, methoxy, ethoxy, hydroxy og trifluorme-thyl; n er 1; Rj og R2 begge er en sec.butyl-gruppe, og R3 er et hydrogenatom.
25 En mere fortrukken udførelsesform for opfindelsen er forbindelserne med formlen I, hvori Rx og R2 begge er en (R)-see.butyl-gruppe.
Forbindelserne med formlen I udviser en bemærkelsesværdig 30 central analgetisk virkning, og de kan anvendes inden for det farmaceutiske område ved balancerede anæstesier og ved behandlingen af smertefulde syndromer af forskellig oprindelse som f.eks. af neoplastisk oprindelse eller efter kirurgi.

Forbindelserne med formlerne II-A og II-B er velkendte eller kan let fremstilles ved kendte metoder.
10 Specielt er forbindelserne med formlen Il-A beskrevet i europæisk patentansøgning nr. 144664, medens forbindelserne med formlen II-B let kan fremstilles ud fra kendte forbindelser ved fremgangsmåden beskrevet i belgisk patentskrift nr. 873766.

En praktisk udførelsesform for den foreliggende opfindelse er følgende. 5-ethenyl-2-pyrrolidinon (II-A; n=l) eller 6-ethenyl-2-piperidon (II-A; n=2) omsættes med et passende halogenid med formlen 20 h

hvori R og R3 har ovenfor anførte betydninger, og X betegner et halogenatom, fortrinsvis chlor eller brom.

Omsætningen udføres i nærværelse af alkalimetaller, som f.eks. natrium og kalium, i nærværelse af alkalimetalhy-drider som f.eks. natriumhydrid eller kaliumhydrid eller andre alkalimetalderivater som f.eks. natriumamid og ka-30 liumamid i et inert organisk opløsningsmiddel som f.eks. dioxan, xylen, toluen, dimethylformamid, dimethylsulfoxid eller tetrahydrofuran ved en temperatur mellem 0 °C og 50 °C.

Alternativt omsættes ved fremstillingen af forbindelserne med formlen III, hvori R3 er forskellig fra hydrogen, udgangsforbindelsen II-A med en alkohol med formlen 5 i3

10 Herefter oxideres mellemproduktet med formlen III for at opnå det tilsvarende epoxid med formlen IV.
Epoxideringen udføres direkte på mellemproduktet III under anvendelse af persyrer, oxygen eller peroxider som 15 oxiderende midler.
Eksempler på velegnede persyrer er m-chlorperbenzoesyre, pereddikesyre, perbenzoesyre, trifluoropereddikesyre eller 3,5-dinitroperoxybenzoesyre.

Eksempler på velegnede peroxider er hydrogenperoxid, t-butylperoxid og titan-tetraisopropoxid.
Udelukkende af praktiske og økonomiske grunde anvendes m-25 chlorperbenzoesyre i et organisk opløsningsmiddel som f.eks. chloroform, dichlormethan eller dichlorethan fortrinsvis.
Alternativt, specielt i nærværelse af let oxiderbare 30 grupper i mellemproduktet III, kan epoxidet IV fremstilles på følgende måde. Mellemproduktet III oxideres først til det tilsvarende diolderivat under anvendelse af f.eks. osmiumtetroxid eller kaliumpermanganat, og herefter omdannes diolderivatet til epoxidet IV ved behandling 35 med en base. Epoxidet IV giver forbindelser med formlen I ifølge opfindelsen ved omsætning med en passende amin med 7 DK 172008 B1 formlen V. Eksempler på passende aminer med formlen V er dimethylamin, methylethylamin, dibutylamin, di-n.propyl-amin, di-isopropylamin, dibutylamin, di-isobutylamin, di-sec.butylamin, (R,R)-di-sec.butylamin, (S,S)-di-sec.bu-5 tylamin, (R,S)-di-sec.butylamin, dicyclopropylamin, di-cyclobutylamin, dicyclopentylamin, piperidin, dicyclo-hexylamin, di-(3-pentyl)amin, N-(2-butyl)-N-isopropyl-amin.
10 Aminen med formlen V anvendes sædvanligvis i overskud og kan også virke som opløsningsmiddel.
Eventuelt kan omsætning udføres under anvendelse af organiske opløsningsmidler som f.eks. alifatiske alkoholer, 15 alifatiske og aromatiske carbonhydrider eller ethere.
En alternativ metode til fremstilling af forbindelserne med formlen I, hvori n=l, består i at anvende 3-carboxy- 5-ethenyl-2-pyrrolidinonerne med formlen II-B som ud-20 gangsforbindelser.
Disse forbindelser kan fremstilles ud fra estere af 2-ethenylcyclopropan-1,1-dicarboxylsyre således som beskrevet i belgisk patentskrift nr. 873766, idet de direkte 25 kan decarboxyleres, uden isolering ved opvarmning, hvilket giver 5-ethenyl-2-pyrrolidinonerne med formlen III.
Ved oxidering og kondensation med en amin V ifølge det tidligere beskrevne fås forbindelserne med formlen I, 30 hvori n=l.
Forbindelserne med formlen I indeholder mindst to asymmetriske centre, hvilket er angivet ved * i reaktionsskema 1, og de kan derfor foreligge i form af enkelte ste-35 reoisomere eller i form af stereoisomere blandinger.

Yderligere asymmetriske centre kan eventuelt forefindes på substituenterne R, Rlr R2 og R3.
Dersom forbindelserne med formlen I opnås som en stereo-5 isomer blanding, kan de enkelte stereoisomere adskilles ved almindelige metoder som f.eks. udkrystallisering og kromatografi.
Analogt kan adskillelsen udføres på et af mellemproduk-10 terne ved syntesen og specielt på epoxidet med formlen IV.
Det er klart for fagmanden, at ved reaktionen angivet i reaktionsskema 1 kan forbindelserne med formlen I opnås 15 stereoselektivt.
Idet man f.eks. går ud fra en forbindelse med formlen II-A eller II-B med en forudbestemt konfiguration, kan epoxidet med formlen IV opnås som en enkelt stereoisomer 20 ved en stereoselektiv epoxidering. Herefter giver omsætningen af epoxidet, fremstillet ved stereoselektiv syntese eller ved adskillelse fra en stereoisomer blanding med aminen med formlen V de stereoisomert rene forbindelser med formlen I.

Dersom yderligere asymmetriske centre forefindes på substituenterne Rx og R2, fås stereoisomert rene forbindelser med formlen I ved at anvende aminer med formlen V med forudbestemt konfiguration som f.eks. (R,R)-di-sec.butyl-30 amin, (S,S )-di-sec.butylamin, (R,S)-di-sec.butylamin og N-2(R)-butyl-N-isopropylamin.
Foretrukne forbindelser er sådanne, hvori carbonatomerne mærket med * i formlen I angivet i reaktionsskema 1 begge 35 udviser S konfiguration.

Saltene af forbindelserne med formlen I med farmaceutisk acceptable syrer fremstilles på almindelig måde. Den farmakologiske bedømmelse af forbindelserne ifølge opfindelsen har vist, at de udviser en høj affinitet og en bemær-5 kelsesværdig aktivitet over for opioide receptorer, og at de udviser bemærkelsesværdig antinociceptive egenskaber, hvilket gør dem specielt velegnet til anvendelse ved terapi som central analgetika, der er mere aktive og har mindre bivirkninger end morfin og beslægtede forbindel-10 ser, fordi de ikke inducerer fysisk afhængighed. Specielt er farmakologisk virkning blevet vist i forhold til morfin ved et vitro forsøg, hvor man har bedømt konkurrencen med hensyn til binding af 3H-dihydromorphin (DHM) til opioide receptorer og bedømmelsen af den morfinlignende 15 virkning på isolerede marsvineileum (beskrevet i eksempel 12) såvel som in vivo forsøg, som f.eks. varmepladeforsøget på mus (beskrevet i eksempel 13) og springforsøget (beskrevet i eksempel 14).
20 Toxicitetsundersøgelser blev udført på mus ved subcutan administrering af stigende doser af forbindelser med formlen I for at bedømme den akutte toxicitet udtrykt som LDS0.
25 Forbindelserne med formlen I viste LD50 værdier på mere end 300 mg/kg s.c. med et ret favorabelt terapeutisk index mellem 100 og 10 000.
Forbindelserne ifølge opfindelsen kan administreres i te-30 rapeutiske doser mellem 1 og 500 mg/dag peroralt og mellem 1 og 500 pg/dag intravenøst eller intramuskulært.
Opfindelsen angår også farmaceutiske præparater med anal-getisk virkning indeholdende en terapeutisk effektiv 35 mængde af en forbindelse med formlen I ifølge opfindelsen som angivet i krav 9. Disse præparater kan indeholde den 10 DK 172008 B1 aktive bestanddel sammen med passende faste eller flydende farmaceutiske hjælpestoffer, og de kan administreres peroralt eller parenteralt.
5 De farmaceutiske præparater fremstillet på kendt måde kan være faste stoffer som f.eks. tabletter, overtrukne tabletter, kapsler, pulvere eller frysetørrede pulvere der skal fortyndes i anvendelsesøjeblikket og granulater eller væsker som f.eks. opløsninger, suspensioner eller 10 emulsioner.
Herudover kan de yderligere indeholde sædvanlige hjælpestoffer som f.eks. konserveringsmidler, stabilisatorer, befugtningsmidler, emulgerende midler, salte til regule-15 ring af det osmotiske tryk, pufre, farvestoffer og smagsstoffer.
Opfindelsen illustreres nærmere i de efterfølgende eksempler, idet dog eksemplerne 1-6 og 9-10 angår fremstilling 20 af mellemprodukter.
EKSEMPEL 1
Fremstilling af (5S )-1-(2-chlorphenyl)methvl-5-ethenyl-2-25 pvrrolidinon 80% NaH i olie (3,57 g; 0,1188 mol) blev portionsvis sat til en opløsning af (5S)-5-ethenyl-2-pyrrolidinon (12 g; 0,108 mol) i dimethylformamid (120 ml), idet man holdt 30 temperaturen ved ca. 20 °C. Efter ca. 30 minutters forløb blev Nal (0,8 g; 0,0054 mol) tilsat og herefter ved stuetemperatur tildryppet 2-chlorbenzylchlorid (19,13 g; 0,1188 mol).
35 Reaktionsblandingen blev omrørt ved stuetemperatur i 3 timer og herefter udhældt i en blanding af vand og is DK 172008 B1 11 (320 ml). Efter ekstraktion med ethylether blev de opsam-lede organiske faser vasket med vand og tørret over natriumsulfat.
5 Efter afdampning af opløsningsmidlet blev opnået en olie, der blev destilleret ved 125-130 °C/0,3 mmHg, hvilket gav (5S)-l-[(2-chlorphenyl)methyl]-5-ethenyl-2-pyrrolidinon (24,4 g; 96% udbytte) [a]D20= + 131,7° (c=2,4%, ethanol) 10 1H-NMR (200 MHz, CDC13-TMS): δ (ppm): 1,71-1,87 (m, IH); 2,13-2,61 (m, 3H); 3,92 (m, IH); 4,54 (AMq, 2H, δΑ=4,88, δΜ=4,20, JAM= 15,6 Hz); 5,06-5,19 (m, 2H); 5,55-5,73 (m, 1H);7,15-7,35 (m, 4H).

Eremstillinq af den racemiske blanding af 1-Γ(2-chlorphe-m?l)methvll-5-ethenvl-2-pvrrolidinon 35 En opløsning af 2-chlorbenzylamin (15,8 g; 0,05 mol) i absolut ethanol (23,4 ml) blev hurtigt sat til 6,6-dime- DK 172008 B1 14 thyl-2-ethenyl-5,7-dioxaspiro[2,5]octan-4,8-dion (9,81 g; 0,05 mol), afkølet i isbad under omrøring.
Under tilsætningen steg temperaturen indtil ca. 35 °C, og 5 herefter vendte den hurtigt tilbage til stuetemperatur.
Reaktionsblandingen henstod uomrørt natten over og blev herefter opvarmet 1,5 time med tilbagesvaling.
10 Efter afdampning af opløsningsmidlet blev remanensen opløst i ethylether (240 ml), vasket med 10% HC1 og herefter ekstraheret med NaHC03 opløsning. Den vandige fase blev vasket med ethylether, syrnet med 10% HC1 og ekstraheret med ethylether.

De forenede organiske ekstrakter blev vasket med vand og tørret over natriumsulfat.
Efter afdampning af opløsningsmidlet blev den opnåede re-20 manens opvarmet 15 minutter ved 185-195 °C.
Ved destillation ved 125-130 °C/0,3 mmHg blev opnået 1-[ ( 2-chlorphenyl)methyl]-5-ethenyl-2-pyrrolidinon (8,8 g; 75% udbytte) som en racemisk blanding.

Fremstilling af (5S)-i-(diphenvlmethvl.I-5-ethenvl-2-Pvr-rolidinon 5
En opløsning af (5S)-5-ethenyl-2-pyrrolidinon (2,2 g; 0,02 mol), diphenylmethanol (7,37 g; 0,04 mol) og p-tolu-ensulfonsyre-monohydrat (7,61 g; 0,04 mol) i eddikesyre (25 ml) blev opvarmet 1,5 time med tilsvaling.

Efter afkøling blev opløsningen udhældt i vand (100 ml) og ekstraheret med ethylether.

Fremstilling af diastereolsomere (5S.2'S) og 5S.2'R) af 1-Γ(2-chlorphenvlImethvll-5-oxiranvl-2-pyrrolldinon 5
En blanding af (5S)-1-(2-chlorphenyl)methyl-5-ethenyl-2-pyrrolidinon (29,2 g; 0,1239 mol) fremstillet som beskrevet i eksempel 1 og 85% 3-chlorperbenzoesyre (55,6 g; 0,2740 mol) i chloroform (302 ml) henstod 24 timer ved 40 10 °C.
Efter afkøling blev den organiske fase ekstraheret med 10% NaOH (5 x 30 ml), vasket med vand og tørret over natriumsulfat .

Fremstilling af 1-Γ(5S)-2-oxo-l-(2-thienvlmethvl)-5-Pvr-rolidinvll-1.2-ethandiol diastereoisomerblanding
En opløsning af osmiumtetroxid (0,032 g; 0,000126 mol) i 25 t.butanol (3 ml) blev tilsat til en opløsning af (5S)-5-ethenyl-l-(2-thienylmethyl)-2-pyrrolidinon (6,22 g; 0,03 mol), fremstillet som beskrevet i eksempel 1, og N-methylmorpholin-N-oxid-monohydrat (4,46 g; 0,033 mol) i acetone (15 ml) og vand (6 ml) ved stuetemperatur og un-30 der nitrogen.
Reaktionsblandingen blev omrørt ved stuetemperatur ca. 16 timer under nitrogen; herefter blev den fortyndet med vand (25 ml), natriumhydrosulfit (0,4 g), og magnesium-35 trisilicat (4,5 g) blev tilsat.

Efter 10 minutters omrøring blev reaktionsblandingen filtreret og vasket med acetone: vand»l:l.
Filtratet blev behandlet med IN svovlsyre indtil pH 7 og 5 det blev inddampet til tørhed under vacuum ved 60 °C.
Remanensen blev opløst i vand, opløsningen blev syrnet til pH 2 med IN H2S04, mættet med natriumchlorid og ekstraheret med ethylacetat (9 x 40 ml).

Fremstilling af (5S.2'S ) og (5S.2'R)-5-oxlranvl-l-(2-thi-enylmethyl)-2-pyrrolidinon 30 En opløsning af methansulfonylchlorid (4,19 g; 0,0366 mol) i dichlormethan (28 ml) blev dråbevis tilsat til en blanding af 1-[(5S)-2-oxo-l-(2-thienylmethyl)-5-pyrroli-dinyl]-1,2-ethandiol (6,68 g; 0,02768 mol), fremstillet som beskrevet i eksempel 5, benzyltriethylammoniumchlorid 35 (0,28 g; 0,00124 mol), dichlormethan (28 ml) og 20 DK 172008 B1 23 vægt/vægt-% natriumhydroxidopløsning (33,4 g; 0,167 mol) under kraftig omrøring og med tilbagesvaling.
Herefter henstod reaktionsblandingen under omrøring ved 5 samme temperatur i 1,5 time.
Efter afkøling og fortynding med vand (55 ml) blev den organiske fase fraskilt, vasket med vand indtil neutral pH, tørret over natriumsulfat og inddampet til opnåelse 10 af en blanding af (5S,2'S) og (5S,2'R)-5-oxiranyl-l-(2-thienylmethyl)-2-pyrrolidinon (5,8 g; 94% udbytte) i et forhold (5S,2'S):(5S,2'R)=2:1 som en olie.
De to diastereoisomere blev adskilt ved kromatografi 15 (Jobin Yvon Chromatospac, silica gel, elueringsmiddel ethylacetat:hexan=8:2)

En opløsning af (5S,2'R)-l-[(2-chlorphenyl)methyl]-5-oxi-ranyl-2-pyrrolidinon (9,5 g; 0,03774 mol), fremstillet som beskrevet i eksempel 4, og (R,R)-disec.butylamin 10 (6,34 g; 0,04906 mol) i n.butanol (19,5 ml) blev opvarmet 70 timer med tilbagesvaling.
Efter afdampning af opløsningsmidlet blev remanensen opløst i 5% afkølet HC1 (50 ml) og ekstraheret med ethylet-15 her (2 x 50 ml). Den vandige fase blev indstillet til basisk reaktion med K2C03 og ekstraheret med ethylether, der herefter blev vasket med vand, tørret over natriumsulfat og inddampet.
20 Remanensen blev renset ved kromatografi (silicagel, elue-ringsmiddel CHC13:1N methanolisk ammoniak=98:2) til fremstilling af forbindelse 1 (7,2 g; 50% udbytte).

Fremstilling af (1R)-1- Γ ( 5S )-l- Γ ( 2-chlomhenvl )methvl 1 10 oxo-S-pyrrolidinvll-2-piperidinoethanol (forbindelse 31ΐ
En opløsning af (5S,2,S)-l-[(2-chlorphenyl)methyl]-5-oxi-ranyl-2-pyrrolidinon (4 g; 0,016 mol), fremstillet som beskrevet i eksempel 4, og piperidin (2,72 g; 0,032 mol) 15 i ethanol (8,3 ml) blev opvarmet 20 timer med tilbagesvaling.
Efter afdampning af opløsningsmidlet blev remanensen opløst i 5% HC1 og ekstraheret med ethylether.

Den vandige fase blev indstillet til basisk reaktion med K2C03 og ekstraheret med ethylether, hvorefter der blev vasket med vand, tørret over natriumsulfat og inddampet.

1) Bedømmelse af konkurrencen for binding af r3Hl-dihv-dromorphin (DHM) til opioide receptorer i rotteh-jerneho-20 mogenat.
Forsøgene blev udført som beskrevet af Simon et al. [Proc. Nat. Acad. Sci. USA, 70, 1947-1949, (1973)].
25 Sprague Dawley rotter der vejede ca. 150 g blev anvendt. Dyrene fik hovedet hugget af, og efter fjernelse af cerebellum blev hjernerne homogeniseret i 50 nM Tris-HCl puffer (pH 7,4) indeholdende 0,32 M saccharose. Homogenatet blev centrifugeret ved 1000 omdrejninger pr. minut i 10 30 minutter og den supernatante del blev atter centrifugeret ved 20000 omdrejninger pr. minut i 20 minutter.
Den opnåede pellet blev opslæmmet i en homogeniseringspuffer uden saccharose indtil en proteinkoncentrati-35 on på 2 mg/ml. Inkubationssystemet bestod af: 500 μΐ homogenat (1 mg proteiner), 390 μΐ 50 mM Tris-HCl puffer 41 DK 172008 B1 (pH 7,4) 100 μΐ af en opløsning af forskellige koncentrationer af forbindelserne med formlen I eller 10 μΜ na-loxon til bestemmelse af den aspecifikke binding og 10 μΐ af 3H-DHM (specifik aktivitet 87,7 Ci/mol, 1 nM slutkon-5 centration). Blandingen blev inkuberet 30 minutter ved 25 °C efter tilsætningen af den radioaktive binder.
Herefter blev prøverne filtreret gennem Whatman GF/B filtre, vasket to gange med 3 ml puffer og tørret i en ovn 10 ved 70 °C. Filtrene blev anbragt i scintillationsampuller med 15 ml "Filter Count" (Packard) scintillationscock-tail, og radioaktiviteten blev målt med en Packard Tri-carb 4530 scintillationstæller med 60% effektivitet.
15 Aktiviteten blev udtrykt som hæmmende koncentration (IC50) 50% af bindingen af 3H-DHM.
I tabel 1 er angivet IC50 værdierne for repræsentative forbindelser med formlen I.

Fysiologisk opløsning 20 1 25 Opløsningen af den aktive bestanddel i fysiologisk opløsning blev filtreret gennem 0,22 pm membranfilter og fordelt i ampuller, idet der blev anbragt 1 ml i hver ampul. Ampullerne blev lukket og steriliseret i autoklav ved 121 °C i 25 minutter.
