Abstract

Изобретение относится к медицине, а именно к сосудистой хирургии, и направлено на уменьшение времени пережатия брюшной аорты при хирургической реконструкции неосложненной аневризмы инфраренального отдела брюшной аорты и инфраренальной аневризмы с неполным разрывом в отделениях сосудистой и общей хирургии у лиц пожилого и старческого возраста, отягощенных различной сопутствующей патологией с высоким риском оперативного лечения. Поставленная задача достигается тем, что в первую очередь накладывают дистальный анастомоз между периферической браншей сосудистого протеза с подвздошной или бедренной артерией на одной стороне, затем аневризматический мешок отсекается между двумя зажимами в области шейки ниже почечных артерий, тромботические массы выдавливаются из мешка путем открытия нижнего зажима над аневризматическим мешком, зажим снова закрывают, после чего аневризматический мешок максимально гофрируют путем вворачивания его стенок продольно внутрь с последующим наложением сверху и стягиванием П-образных швов, которые сводят стенки аневризматического мешка друг к другу, во время стягивания П-образных швов над аневризматическим мешком зажим над ним временно открывают, далее на проксимальный конец аневризматического мешка накладывают гемостатические П-образные швы вдоль всего периметра, нижний зажим над аневризматическим мешком снимают, формируют проксимальный анастомоз между центральной браншей сосудистого протеза и брюшной аортой, открывают магистральный кровоток по брюшной аорте через сосудистый протез и первый дистальный анастомоз к ноге и органам таза, затем формируют второй дистальныйThe invention relates to medicine, namely to vascular surgery, and is aimed at reducing the time of compression of the abdominal aorta during surgical reconstruction of uncomplicated aneurysm of the infrarenal abdominal aorta and infrarenal aneurysm with an incomplete rupture in the vascular and general surgery departments of elderly and old people burdened with various concomitant pathology with a high risk of surgical treatment.
