L'invention concerne un dispositif oculaire anti-photophobie temporaire, et son procédé de préparation. La photophobie correspond à une intolérance à la lumière.
Les personnes atteintes de photophobie sont très sensibles à une forte luminosité et peuvent être atteintes de maux de tête. Dans des cas extrêmes, la lumière peut être irritante.
C'est un symptôme souvent associé : - à des troubles physiologiques : inflammation cornéenne, uvéite, kératite, conjonctivite, détachement rétinien, troubles au niveau du système nerveux central.
- à une chirurgie réfractive ou à une chirurgie de la cataracte.
A l'heure actuelle, la recommandation des médecins à leurs patients atteints de photophobie, et particulièrement des chirurgiens après une opération de la cataracte, est le port de lunettes de soleil.
L'un des buts de cette invention est de proposer une alternative au port de lunettes après chirurgie de la cataracte, qui assure un meilleur confort au patient sans pour autant réduire sa vision des couleurs et des contrastes à long terme. Un autre but est de fournir un dispositif (lentille de contact par exemple) palliant une photophobie d'une autre origine.
Dans le premier cas, l'invention consiste en un implant coloré temporairement afin que le patient s'adapte plus facilement au changement des capacités de transmission de la lumière entre le cristallin naturel - qui a jauni au cours du temps et qui est éliminé lors de la chirurgie - et la lentille nouvellement implantée. Le colorant intégré à l'implant se caractérise par des capacités de filtration de la lumière dans les longueurs d'ondes comprises entre 400 et 575 nm. Ce colorant est libéré progressivement, après la mise en place de l'implant, dans les fluides environnants. Le patient est ainsi protégé d'une hypersensibilité à la lumière immédiatement après la chirurgie et retrouvera peu à peu, grâce à une augmentation progressive du pourcentage de transmission des longueurs d'ondes préalablement absorbées, une vision optimale des couleurs et des contrastes ; cet aspect est d'autant plus important que la luminosité est de plus en plus recherchée chez les personnes âgées et qu'une bonne vision des couleurs et des contrastes est indispensable pour une bonne qualité de vie. Dans le second cas, l'invention consiste en une lentille de contact colorée temporairement dans les mêmes conditions que précédemment.
Il existe déjà des dispositifs oculaires colorés définitivement pour bloquer la lumière bleue-violette, l'implant AcrySof Natural d'Alcon, ou l'implant Hoya AFl(uy). Leur objectif principal est de protéger l'œil de la dégénérescence maculaire que peuvent induire les longueurs d'ondes fortement énergétiques, même si cela est sujet à controverse. Ils filtrent uniquement les faibles longueurs d'ondes, et ceci de façon définitive. Mais :
1. ils modifient irréversiblement la vision des couleurs et des contrastes du patient (réduction de la performance scotopique). 2. la photophobie et la phototoxicité ne sont pas seulement dues aux faibles longueurs d'ondes mais aussi à la lumière bleue-verte.
Il est donc fourni selon l'invention un dispositif oculaire permettant d'éviter le phénomène de photophobie, consécutif notamment à une opération chirurgicale de l'œil, le dispositif oculaire contenant un ou plusieurs chromophores absorbant la lumière dans les longueurs d'ondes de 400 à 575 nm et relarguables, après implantation, dans les fluides environnants en quelques semaines à quelques mois en fonction de la concentration en chromophores et de l'affinité du chromophore pour le matériau constitutif du dispositif.
Le dispositif oculaire est avantageusement une lentille intraoculaire en polymère hydrogel (comme par exemple le poly(hydroxyéthylméthacrylate), les copolymères acryliques ou méthacryliques dont l'un des monomères est avantageusement Phydroxyéthylméthacrylate, les matrices hydroxyéthylméthacrylate/silicone, les polymères à base de N-vinylpyrrolidone, etc)..
Le ou les chromophores utilisés sont bien entendu choisis parmi les chromophores non toxiques et biocompatibles, susceptibles d'être extraits du dispositif oculaire par dissolution ou entraînement par les fluides biologiques environnants. Ils absorbent la lumière dans les longueurs d'ondes de 400 à 575 nm et à titre d'exemples préférés, on peut citer la riboflavine en solution aqueuse inférieure ou égale à 3 % (m/v) et la fluorescéine en solution aqueuse inférieure ou égale à 0,6 % (m/v).
Le ou les chromophores sont fixés temporairement dans le polymère hydrogel, soit par absorption/diffusion, soit par association covalente ou ionique selon la nature du chromophore concerné.
L'invention concerne également le procédé de préparation du dispositif oculaire, qui comprend l'étape consistant à placer un dispositif oculaire non coloré dans une solution aqueuse du ou des chromophores, à l'abri de la lumière, la durée d'imprégnation étant fonction de la température.
La description va maintenant être faite sous forme d'exemples non limitatifs.
Dans tous les exemples, on utilise comme pastille de départ une pastille de polymère hydroxyl d'une épaisseur de 0,8 mm et d'un diamètre de 10 mm.

On prépare une solution aqueuse de riboflavine à 1 % (m/v). Une pastille de polymère hydrogel (Benz 25UV), ayant un taux d'hydratation de 25 % (m/v), est conditionnée dans la solution de riboflavine 7 jours à température ambiante et à l'abri de la lumière. La pastille avant conditionnement avait le spectre de transmission représenté sur la figure 1.
La pastille colorée est ensuite conditionnée dans un flacon contenant 5 ml d'une solution aqueuse de chlorure de sodium à 0,9 % (m/v) (sérum physiologique) à température ambiante et à l'abri de la lumière. La solution de NaCl à 0,9 % est renouvelée tous les 3 jours au cours de l'expérimentation.
La figure 2A donne le spectre de transmission de la pastille colorée après sept jours de conditionnement dans la solution de riboflavine et on constate une absorption nette des longueurs d'ondes entre environ 400 et 520 nm qui permet de diminuer significativement la quantité de lumière transmise à la rétine tout en ne réduisant pas à néant l'efficience scotopique.
Les figures 2B, 2C, 2D correspondent aux spectres de transmission respectivement après 1 jour, 30 jours et 60 jours dans la solution de NaCl et montrent l'élimination ou relarguage progressif de la riboflavine, le polymère retrouvant progressivement ses propriétés initiales de transmission de la lumière.
Exemple 2
On prépare une solution aqueuse de fluorescéine à 0,6 % (m/v). Une pastille de polymère hydrogel, identique à celle utilisée dans l'exemple 1, est conditionnée dans la solution de fluorescéine 7 jours à température ambiante et à l'abri de la lumière. Après 7 jours, la pastille est conditionnée dans un flacon contenant 5 ml d'une solution aqueuse de NaCl à 0,9 % (m/v) à température ambiante et à l'abri de la lumière. La solution de NaCl est renouvelée tous les 3 jours pendant l'expérimentation.
Les figures 3 A à 3E représentent les spectres de transmission aux jours 0, 1, 30, 60 et 90 jours de conditionnement dans la solution de NaCl. Initialement, l'absorption est supérieure à 80% pour les longueurs d'ondes inférieures à 520 nm. On constate que le polymère retrouve peu à peu ses capacités initiales de transmission de la lumière, toutefois plus lentement que dans l'exemple 1.
Exemple 3 On prépare une solution aqueuse de fluorescéine à 0,1 % (m/v). Une pastille de polymère hydrogel identique à celle utilisée dans les exemples précédents est conditionnée dans la solution de fluorescéine pendant 7 jours à température ambiante et à l'abri de la lumière. Après 7 jours, la pastille est conditionnée dans un flacon contenant 5 ml d'une solution aqueuse de NaCl à 0,9 % (m/v), à température ambiante et à l'abri de la lumière. La solution de NaCl est renouvelée tous les 3 jours pendant l'expérimentation.
Les figures 4 A à 4D représentent les spectres de transmission aux jours 0, 15, 45 et 60 de conditionnement dans la solution de NaCl. On constate que le spectre de transmission au jour 0 est similaire à celui obtenu avec une solution de fluorescéine à 0,6 % (exemple 2), (transmission très légèrement supérieure aux alentours de 400nm quand la concentration initiale est plus faible). Claims

4REVENDICATIONS
1. Dispositif oculaire permettant d'éviter le phénomène de photophobie, caractérisé en ce que le dispositif contient un ou plusieurs chromophores absorbant la lumière dans les longueurs d'ondes comprises entre 400 et 575 nm et relarguables après la pose dans ou sur l'œil dans les fluides environnants en quelques semaines à quelques mois en fonction de la concentration en chromophores et de l'affinité du chromophore pour le matériau constitutif du dispositif.
2. Dispositif oculaire selon la revendication 1, caractérisé en ce qu'il s'agit d'une lentille intraoculaire en polymère hydrogel.
3. Dispositif oculaire selon l'une ou l'autre des revendications 1 et 2, caractérisé en ce qu'il est coloré par de la riboflavine ou de la fluorescéine.
4. Procédé de préparation d'un dispositif oculaire selon l'une quelconque des revendications 1 à 3, caractérisé en ce qu'il consiste à placer un dispositif oculaire non coloré dans une solution aqueuse du ou des chromophores et à l'abri de la lumière.
5. Procédé selon la revendication 4, caractérisé en ce que la solution de chromophore est solution aqueuse de riboflavine inférieure ou égale à 3 % (m/v).
6. Procédé oculaire selon la revendication 4, caractérisé en ce que la solution de chromophore est une solution aqueuse de fluorescéine inférieure ou égale à 0,6 % (m/v).
