Description
Arzneimittel aus Pflanzenextrakten als Lioaseinhibitor
Die exzessive Aufnahme von Fett kann zahlreiche Erkrankungen verursachen, wie z. B. Fettleibigkeit (Adipositas), hohe Blutfette und Typ 2 Diabetes, als zentrale Erscheinungen des metabolischen Syndroms. Erhöhte Triglycerid und Glukosewerte im Blut nach der Nahrungsaufnahme sind mit einer Insulinresistenz assoziiert, die eine zentrale Bedeutung in der Entwicklung des metabolischen Syndroms darstellt. Eine Möglichkeit der Reduzierung des postprandialen Triglyceridspiegels ist die Senkung der Aktivität der Enzyme, die das Nahrungsfett hydrolysieren. Die Bauchspeicheldrüsenlipase (e. c. 3.1.1.3) ist wesentlich für den Abbau und die Absorption der durch Nahrungsaufnahme zugeführten Fetten im Intestinaltrakt. Aus diesem Grunde führt deren Verringerung zu einer Vermeidung oder zumindest einer Verzögerung der intestinalen Verdauung der Nahrungsfette und demzufolge zu einer Verringerung der Bluttriglyceride. Die Fettleibigkeit ist ein wichtiger Risikofaktor für den Typ-2- Diabetes. Es ist allgemein anerkannt, dass eine Gewichtsreduktion die Blutzuckerkontrolle verbessert und zur Reduzierung des kardiovaskulären Risikofaktors beiträgt. Im allgemeinen wird es als schwierig angesehen, eine Gewichtreduzierung zu erreichen und das verringerte Gewicht beizubehalten. Tetrahydrolipstatin (THL, Orlistat, Xenical) ist ein käuflich erwerbbarer Lipaseinhibitor, der erhalten wird aus Streptomyces toxystricini. Nach oraler Verabreichung verhindert Orlistat den Fettabbau, indem die aktiven Abschnitte der pankreati- schen Lipase (Bauchspeicheldrüse -Lipase) blockiert werden. In klinischen Versuchen führte Orlistat zu einer Gewichtsreduktion und zu einer Verbesserung des postprandialen (nach Nahrungsaufnahme)
Fettprofiles (reduziert wurden die postprandialen Triglyceride, sowie das Nüchtern-LDL-Cholesterin und das Gesamtcholesterin sowie das Verhältnis LDL zu HDL). Auch konnte das Auftreten von Typ 2 Dia-
betes durch Orlistat reduziert werden. Die Anwendung von Orlistat führt zu unerwünschten Nebeneffekten wie öligem und fettem Stuhl. Eine sanftere Lipasehemmung wird erreicht durch Anwendung von pflanzlichen Extrakten und deren aktive Wirkstoffe, die in einer zeitli- che Verzögerung anstelle einer kompletten Unterbindung der Fettabsorption resultieren und auf diese Weise öligen oder fetten Stuhl vermeiden können.
Systematisch kann dies durch Wirkstoffe erzielt werden, die selbst resorbiert oder während der Verdauung inaktiviert werden und somit aus dem Reaktionsgewicht mit dem Lipase-Coiipase-Fett-komplex entzogen werden.
Die Verabreichung von Lipaseinhibtoren werden als nützliche Medikation für Patienten mit Symptomen des metabolischen Syndroms erachtet. Zusätzlich kann ein lipasehemmender Pflanzenextrakt oder hiervon isolierte Bestandteile als Nahrungszusätze verwendet werden.
Aus mehreren Patenten ist die Anwendung von Pflanzenextrakten als mögliche Inhibitoren für die Lipase im Verdauungstrakt genannt. Es handelt sich hierbei um japanische Anmeldungen mit den folgenden Nummern: JP 2003 026 585, JP 2002 275 077, JP 2002 047 194.
In mehreren Veröffentlichungen wurden die positiven Effekte von Pflanzenextrakten auf das postprandiale Lipidprofil beschrieben, nämlich um folgende Substanzen wie Zylokaria Paliurus, Weintraubenkernen, Salbei, Cassia mimosoides, L. var. Nomame Makine und Alpinia officinarum.
Darüber hinausgehend hat sich die Erfindung die Ermittlung weiterer
Pflanzen zur Aufgabe gemacht, die als Lipase-Inhibitoren einsetzbar sind.
Zur Lösung dieser Aufgabe werden Extrakte oder Essenzen der nachfolgenden Stoffe sowie deren Mischung hiervon genannt.

10g Pulver des Rohmaterials wurden aufgelöst in e) 50 ml reinem Ethanol bei 37 0C. w) In Wasser von 37 0C h) In Wasser von 95 0C m) In 60%igem Ethanol bei 37 0C
und anschließend in einem Schüttelwasserbad für 2 Stunden inkubiert. Danach wurde das Extrakt in eine Magnetrührerfür 30 Minuten gerührt und bei 6000 Upm für 10 Minuten zentrifugiert. Anschließend wurde das Lösungsmittel durch einen Speedrac concentrator ent- fernt.

Die lipasehemmenden Substanzen in den Pflanzenextrakten konnten bis jetzt noch nicht identifiziert werden.
Beschreibung der Aufbereitung und der Verfahren zur Bestimmung der Enzymaktivitäten.
Vorbereitung der Testlösung
Zur Vorbereitung der Testlösung aus extrahiertem Pulver und flüssigem Material, werden 200 mg des extrahierten Materials in 1 ml eines geeigneten Lösungsmittels aufgelöst und intensiv verrührt. Nach Zentrifugieren bei 3400 Umdrehungen für 5 Minuten wird der erhal- tene flüssige Bestandteil als Testlösung genutzt. Zum Anpassen des
Verhältnisses zwischen der Konzentration der festen Bestandteile und der Enzymmasse im letzten Versuch wurde die Testlösung im
Verhältnis 1 :200 verdünnt um die alkalische Phospatase zu bestimmen.
Zur Zubereitung der Testlösung aus CO2-Auszügen und Ölharzen werden 100 μl öligen Flüssigkeitsextrakt in 900 μl eines geeigneten Enzympuffers gelöst und mit Ultraschall bei 100 Watt für 5 Minuten behandelt.
Pankreatische Lipase (Bauchspeicheldrüsenlipase)
Zwei Lipasebestimmungen wurden durchgeführt um die Aktivität der pankreatischen Lipase in vitro zu ermitteln:
1. Das Assay für die Bestimmung der Lipaseaktivität besteht aus
- 200 μl von einem 50 mM/L BICIN-Puffer (pH 8,0), der 1 % technisches Gummi arabicum enthält.
- 50 μl Pflanzenextraktlösung und - 25 μl einer 0,35 mM/L pankreatischen Lipaselösung
Diese Mischung wurde für 20 Minuten bei 37° inkubiert. Die letzte Messung der pankreatischen Lipaseaktivität wird einem Autoanalyser für klinische Chemie (Konelab, Kone) durchgeführt, unter Verwendung eines Lipasekolorimetertestes mit einer Diacylgly- cerol mit Metylresorufin in dritter Position als Substrat und Colipase und Gallensalze als wesentliche Co-Faktoren. Dieses Verfahren ist für die prankreatische Lipase spezifisch und basiert auf der Spaltung des Methylresorufin vom Substrat durch die Enzyme.
Zur Bestimmung der pankreatischen Lipaseaktivität im Autoanalyzer wurde folgendes Pipettierschema gewählt:
1. 60 μl Reagenz 1: Colipase und Cholat
2. 2 μl Testlösung + 10 μl Waschlösung
3. 180 Sekunden Inkubation
4. 40 μl Reagenz 2: Colorimetrisches Substrat und Cholat 5. 120 Sekunden Inkubation
Die Lipaseaktivität wurde bestimmt anhand von 12 Messpunkten nach 83 Sekunden. Das Methyiresorufin wurde fotometrisch bei 75 Nanometem gemessen.
2. Die Lipaseinhibition unter dem Einfluss von Pflanzenextrakten wurde mit einer zweiten Lipaseaktivitätsmessmethode bestimmt, indem die Freisetzung der freien Fettsäure während der Hydrolyse aus Triolein durch das Enzym gemessen wird. Um das Inkubationsgemisch herzustellen, wird ein gebrauchsfertige
Lösung (Reagenz R1) verwendet. R1 besteht aus:
26 mmol/L Tris Puffer, pH 9,2 19 mmol/L Natriumdesoxycholat 0,1 mmol/L Calciumchlorid
0,3 mmol/L Triolein 300 KU/L Colipase
Die Inkubationsmischung setzt sich wie folgt zusammen:
50 μL 26 mmol/L Tris Puffer, pH 9,2
50 μL 9,7 mmol/L Triolein Emulsion in R1
50 μL 0.134 mol/L pancreatische Lipase in 26 mmol/L Tris Puffer pH
9,2 50 μL Testlösung
1 % Methylzellulose in 26 mmol/L Tris buffer (pH 9.2) wurde bei der Untersuchung der CO2 Auszüge benutzt.
Nach einer Inkubationszeit von 30 Minuten bei 37° in einem Wasser- bad wurde die Reaktion durch Beigabe von 3 ml einer Chloroform-
Heptan-Methanol-Mischung (50:49:1) gestoppt. Die freigesetzten freien Fettsäuren wurden aus dem Reaktionsgemisch durch intensives Rühren für 10 Minuten extrahiert. Nach Zentrifugieren (3400 Umdrehungen, 10 Minuten) wird ein 2 ml Rest werden aus der ChIo- roformphase gewonnen und 1 ml aus einem Kupferreagenz (94 ml reines Wasser, 6ml 1 N NaOH, 6,45 mmol/L pro Cu(NO3^, 0,1 mol/L Triethanolamin, 33 % (w/v) NaCI) wird zugegeben. Diese Mischung wurde intensiv für 10 Minuten gerührt und bei 3400 Umdrehungen für 10 Minuten zentrifugiert. Aus dem flüssigen Überstand wurden 1 ml entnommen und ein 1 ml Chloroform bei 0,1 % Bathocuproin und
0,05 (w/v) 3-Tert~butyl-4-hydroxyanisol wurden hinzugegeben. Der fette Säure- Kupfer- Bathocuproin-Komplex wird schließlich fotometrisch bei 480 nm in einem Standardfotometer bestimmt.
Um sicherzustellen, dass der Rückgang der Lipaseaktivität nicht auf einen unspezifisches Enzymhemmer bzw. Denaturierung des Enzymkomplexes zurückzuführen ist, wurden die Aktivität der α- Amylase und der alkalischen Phosphatase zusätzlich bestimmt. Eine Hemmung der α-Amylase kann sich auswirken als eine Verringerung des postprandialen Blutzuckerspiegels und wird als zusätzlicher positiver Effekt für Diabetes-Patienten betrachtet.
α-Amylase Die Amylaseaktivitätsmessung unter dem Einfluss der Pflanzenextrakte wurden wie folgt durchgeführt:
Der Inkubationsmix für die Bestimmung der Amylaseaktivität besteht aus insgesamt 250 μL:
-200 μL 50 mM/L MOPSO Puffer (pH 8,0) mit 1 % Gummi arabium -25 μL Pflanzenextrakt Lösung und
-25 μL einer 0,35 mM/L α-Amylase Lösung
Diese Mischung wurde für 20 Minuten bei 380C inkubiert. Die abschließliche Messung der pankreatischen Lipaseaktivität wird durchgeführt in einem Autoanalyser für chemische Chemie (Konelab,
Kone).
Prinzip der chemischen Reaktion:

2. p-Nitrophenol-(Glucose)-Reste → p-Nitrophenol (λ= 405 nm) + Glucose
Das Pipettenverfahren im Autoanalyser zur Bestimmung der α- Amylaseaktivitäten wurde wie folgt durchgeführt:
1. 120 μL Reagenz: Substrat + α-Glucosidase
2. 300 Sekunden Inkubation
3. 3 μL Testlösung 4. 180 Sekunden Inkubation
Die α-Amylaseaktivität wird bestimmt durch Messung von 5 Punkten in 120 Sekunden.
Alkalische Phosphatase
Die Alkalischθ-Phosphatase-Aktivitätsmessung unter dem Einfluss von Pflanzenextrakten wurde wie folgt durchgeführt:
Der Inkubationsmix für die Bestimmung der Lipaseaktivität bestand aus
- 200 μL einer 50 mM/L HEDTA Puffer (pH 8,0) enthaltend,
- 25 μL Pflanzenextrakttestlösung und
- 25 μL einer 0,014 mM/L alkalische Phosphatase Lösung
Die Mischung wurde inkubiert für 20 Minuten bei 22°C. Die abschließende Messung der pankreatischen Lipaseaktivität wurde durchgeführt in einem Autoanaylser für klinische Chemie (Kone- lab, Kone).
Reaktionsgleichung
2-Amino-2-methyl-1-propanol + 4-Nitrophenylphosphatester → (2-Amino-2-methyl-1-Propanoö) Phosphat + 4-Nitrophenoxid (λ= 409 nm)
Das Pipettenverfahren im Autoanalyser zur Bestimmung der Aktivität der Alkalinen Phospatase war wie folgt:
1. 150 μL Reagenz
2. 300 sec. Inkubation
3. 3μL Testlösung
4. 120 sec. Inkubation
Die Aktivität der alkalische Phosphatase wurde bestimmt mittels 5
Messpunkten in 22 Sekunden.
Verifizierung im Tierversuch
Zur weiteren Überprüfung und Verifizierung der in vitro gefundenen Ergebnisse wurde im Tierexperiment die Triglycerid senkende Wir- kung überprüft.
Dazu wurde ein Lipidbelastungstest bei Ratten mit Triton WR-1339 zur Inhibierung der Triglyceridelimination aus dem Blut durchgeführt und der postprandiale Anstieg der Plasmatriglyceridwerte über vier Stunden hinweg gemessen. Dabei wurden die folgenden Testsubstanzen verwendet:
Proteine und Peptide
- Protamin: stark basisches Kernprotein (von 32 AS 21 x Arg), aus Fischsperma gewonnen, Triglycerid senkend (Tsujita et al., 1996)
- Peptid (D4 Globin Digest): enzym. Hydrolysat aus bovinem E- rythrozytenglobin, Triglycerid senkend (Kagawa et al., 1996, 1998)
- WYP: f32-35 der ß-Kette bovinen Härmoglobins, Triglycerid sen- kend (Kagawa et al., 1996, 1998)
- LVYP: f73-76 bovinen ß-Caseins
Extrakte
- Ethanolische Extrakte - Blätter von Tabebuia impetiginosa (Lapacho)
- Arctium lappa L (Klette)
- Samen von Linum usitatissimum (Lein)
- Samen von helianthus annuus (Sonnenblume)
- Calendula officinalis (Ringelblume) - Oleoresin
- Samen von Carum carvi (Kümmel)
- CO2-Extrakte
- Frucht mit Schale von Elettaria cardamomum (Kardamom)
- Blätter von Thymus vulgaris (Thymian)
- Propolis (Bienenkittharz) Orlistat als Positivkontrolle
Als Ergebnis wurde der Anstieg der postprandialen Triglyceridwerte im Plasma von Ratten nach Gabe der Testsubstanzen erfasst. Dabei zeigte sich, dass weder für Protamin, Peptid D4 noch für die Tetra- peptide ein die Triglyceride senkender Effekt gefunden wurde. Ebenso war auch bei einigen Extrakten in diesem Versuch keine gewünschte Wirkung feststellbar. Dazu zählen Leinsamen, Sonnenblume, Ringelblume und Propolis.
Bei anderen Extrakten konnte jedoch eine teilweise sehr deutliche
Reduzierung der Triglyceride beobachtet werden; dazu zählt Kümmel mit einer Reduzierung um 3 % ± 9 %, Thymian mit einer Verminderung um 7 % ± 7 %, die Klette mit einer Verringerung um 12 % ± 6 %, Kardamon mit einer Verkleinerung um 13 % ± 7 % und insbeson- dere herausragend der Lapachoteeextrakt mit einer signifikanten Absenkung um 16 % ± 7 %. Im Versuch wurde nicht differenziert, welche Inhaltsstoffe der Extrakte die insgesamt beobachtete Wirkung vermitteln. Hier ist noch ein thematischer Raum für die Durchführung weiterer Versuche im Tierbereich, und es ergeht ein Hinweis auf die Erfordernisse von Humanstudien.
Generell konnte mit diesem Tierversuch nachgewiesen werden, dass von den in vitro als erfindungsgemäß wirksam gekennzeichneten Wirkstoffen tatsächlich auch einige eine gleiche Wirkung in einem Tierversuch zeigten. Ebenso wurde deutlich, dass diese Wirkung keinesfalls linear übertragbar ist und in einzelnen Fällen sogar von anderen Einflüssen überlagert wird, die im Extremfall zu einer Inver-
tierung der Wirkung führen können. Aus diesem Versuch ist jedoch abzuleiten, dass eine weitere Überprüfung der hier als schützenswert im Sinne einer spezifischen Lipasehemmung aufgeführten Stoffe sinnvoll ist und sehr konkrete Ergebnisse erwarten lässt. Im Folgen- den sollen weitere Einzelheiten und Merkmale der Erfindung anhand von zwei Beispielen näher erläutert werden. Diese sollen die Erfindung jedoch nicht einschränken sondern nur erläutern. Es zeigt in schematischer Darstellung:

Die Figuren zeigen im Einzelnen:
In Figur 1 ist tabellenmäßig die Auswirkung der den Ratten etwa 0,05 Stunden nach der Nahrungsaufnahme verabreichten Substanzen in Bezug auf den Triglyceridwert verzeichnet. Dabei wird unterschieden zwischen den Proteinen und Peptiden sowie den Extrakten von natürlichen Wirkstoffen. Signifikant ist, dass die Proteine und Peptide in dieser Versuchskonfiguration nicht zu der gewünschten Reduzierung sondern teilweise zu einer deutlichen Erhöhung der
Triglyceridwerte führten. Dies gilt auch für einige natürliche Wirkstoffe. Andere natürliche Substanzen sind jedoch in der gewünschten Richtung wirksam. Insbesondere Signifikant ist hier die Wirkung des Lapachotees aus der inneren Rinde des Taheebo-Baumes (Tabe buia impetiginosa).
In Figur 2 sind die Kurven der Triglyceridwerte in Abhängigkeit von der Einwirkzeit nach der Nahrungsaufnahme aufgetragen. Erwartungsgemäß sind die Triglyceridwerte bei der Gabe von Orlistat am niedrigsten. Für die bereits in Figur 1 aufgeführten, positiv wirkenden Extrakte konnte jedoch auch in diesem Versuchszeitraum von vier
Stunden eine wunschgemäße Reduzierung der Triglyceride erreicht werden. Als wirksam nachgewiesen sind die Klette, der Lapachotee und das Kardamom. Der vergleichsweise besonders geringe Anstieg der Triglyceridwerte bei der Gabe von Orlistat entspricht den Erwar- tungen und weist nach, dass der Tierversuch in der durchgeführten
Spezifikation für die Erfindung relevant ist.
Claims
1. Verwendung der nachfolgenden Stoffe, Stoffextrakte oder Stoff- essenzen als Einzelkomponente oder als Gemisch zur Herstellung eines oral zu applizierenden Arzneimittels, das als Lipase- Inhibitor wirkt:
Cardamom und/oder
Majoran und/oder Thymian und/oder
Chili (Capsicum frutescens) und/oder
Abendprimel und/oder
Oreganoblätter (oreganum vugare) und/oder
Kamilleblätter (Chamomilla recutica) und/oder Muskatnuss (Myristica fragrans) und/oder
Apfelmus (pomace) und/oder
Kornkeim und/oder
Mandel und/oder
Haselnuss und/oder Walnuss und/oder
Kürbiskeim und/oder
Cuphea wrightii und/oder
Abendprimel und/oder
Ölflachs (Linum usitatissimum) und/oder große Klette (Arctium lappa L) und/oder
Flachs und/oder
Keime des Helianthus annuus (sunflower) und/oder
Ringelblume (Calendula officinalis) und/oder
Echinops banaticus und/oder Mallotus philippinensis und/oder
Raps, und/oder
