Composiciones que comprenden ácido
6-[3-(1-adamantil)-4-metoxifenil]-2-naftanoico
para tratamiento de enfermedades dermatológicas.
La invención se relaciona con composiciones
farmacéuticas que contienen un 0,3% en peso de ácido
6-[3-(1-adamantil)-4-metoxifenil]-2-naftanoico,
cuya estructura química es como sigue:
en particular composiciones
dermatológicas, para el tratamiento de afecciones dermatológicas con
un componente inflamatorio o
proliferativo.
El ácido
6-[3-(1-adamantil)-4-metoxifenil]-2-naftanoico
(al que en adelante se hará referencia como adapaleno) es un
retinoide derivado del ácido naftoico, que tiene propiedades
antiinflamatorias. Esta molécula ha sido objeto de desarrollo para
el tratamiento tópico del acné común y de dermatosis sensibles a los
retinoides.
El adapaleno ha sido descrito por el Solicitante
en la Patente EP 0.199.636 y este último ha proporcionado también un
método de síntesis de este componente en la Patente EP 0 358
574.
El Solicitante comercializa adapaleno formulado
en una concentración ponderal del 0-1% en forma de
una loción alcohólica, un gel acuoso y una crema. Estas
composiciones están destinadas al tratamiento del acné.
Jamoulle y col., ("Follicular penetration and
distribution of topically applied CD271, a new naphtoic acid
derivative intended for topical acne treatment", Journal of
Investigative Dermatology, 1990, 94(5),
731-2) describen el comportamiento farmacocinético
de CD271 en piel de rata.
Allec y col., ("Skin distribution and
pharmaceutical aspects of adapalene gel", Journal of the American
Academy of Dermatology, Vol. 36, Nº 6, parte 2, 1997,
S119-S125) y Shroot y col., ("A new concept of
drug delivery for acne", Dermatology (Basel), Vol. 196, Nº 1,
1998, 165-170) describen un estudio que compara la
eficacia comedolítica del adapaleno y de la tretinoína.
El Solicitante ha desarrollado una nueva
composición farmacéutica que contiene adapaleno en una concentración
ponderal del 0,3%, destinada al tratamiento de afecciones
dermatológicas con un componente inflamatorio o proliferativo.
Concretamente, el Solicitante ha observado, de manera sorprendente,
que, además de exhibir una mejor eficacia terapéutica en comparación
con las composiciones conocidas, la composición según la invención
exhibe una buena tolerancia, comparable a las de las composiciones
conocidas con una concentración menor de principio activo.
Los resultados concernientes a la tolerancia
observados en pruebas relacionadas con piel fotolesionada
(indicación "fotolesión"), obtenidos en individuos con una
media de 65 años de edad, no pudieron ser explotados en el contexto
de la presente invención. Concretamente, en cuanto al uso de
adapaleno en individuos jóvenes (en particular en relación al acné
con poblaciones de adolescentes o adultos jóvenes), la piel exhibe
características fisiopatológicas muy diferentes (presencia de muchas
lesiones, en particular lesiones inflamatorias, modificación de la
permeabilidad de la piel, hipercornificación del canal folicular,
respuesta inmune, colonización bacteriana de la piel (P.
acnes) e hiperplasia sebácea con hiperseborrea).
Así, un objeto de la presente invención es una
composición farmacéutica que contiene un 0,3% en peso de adapaleno
(ácido
6-[3-(1-adamantil)-4-metoxifenil]-2-naftanoico)
en relación al peso total de la composición, o de sus sales, en
forma de un gel acuoso destinado al tratamiento de afecciones
dermatológicas con un componente inflamatorio o proliferativo.
El término "sales de adapaleno" pretende
significar las sales formadas con una base farmacéuticamente
aceptable, en particular en bases inorgánicas, tales como hidróxido
de sodio, hidróxido de potasio y amoníaco acuoso, o bases orgánicas,
tales como lisina, arginina o N-metilglucamina.
El término "sales de adapaleno" también
pretende significar las sales formadas con aminas grasas, tales como
dioctilamina y estearilamina.
La composición farmacéutica según la invención
es preferiblemente aplicada por vía tópica.
Tópicamente, la composición farmacéutica según
la invención está más particularmente destinada al tratamiento de la
piel y de las membranas mucosas y puede estar en forma de cremas,
geles acuosos o lociones alcohólicas basadas en polietilenglicol.
También puede estar en forma de suspensiones de microesferas o
nanoesferas o de vesículas lipídicas o poliméricas, o de hidrogeles
para liberación controlada. Esta composición para aplicación tópica
puede estar en forma anhidra, en forma acuosa o en forma de una
emulsión.
La composición farmacéutica según la invención
está en forma de un gel acuoso, una crema o una loción alcohólica
basada en polietilenglicol.
En particular, la composición farmacéutica puede
ser un gel acuoso que contenga, en particular, uno o más
ingredientes seleccionados entre Carbómero 940 (BF Goodrich,
Carbopol 980) y propilenglicol, o una crema que contenga, en
particular, uno o más ingredientes seleccionados entre
perhidroescualeno, ciclometicona, sesquiestearato de
PEG-20 metilglucosa y sesquiestearato de
metilglucosa, o una loción alcohólica basada en
polietilenglicol.
La composición farmacéutica según la invención
puede contener también aditivos inertes o combinaciones de estos
aditivos, tales como
- -
- agentes humectantes,
- -
- potenciadores del sabor,
- -
- agentes conservantes tales como ésteres del ácido parahidroxibenzoico,
- -
- estabilizadores,
- -
- reguladores de la humedad,
- -
- reguladores del pH,
- -
- modificadores de la presión osmótica,
- -
- emulsores,
- -
- agentes de pantalla para el UV-A y el UV-B y
- -
- antioxidantes, tales como el \alpha-tocoferol, el butilhidroxianisol o el butilhidroxitolueno, la superóxido dismutasa, el ubiquinol o ciertos agentes quelantes de metales.
Por supuesto, los expertos en la técnica
cuidarán de seleccionar el/los compuesto(s)
eventual(es) que se ha(n) de añadir a estas
composiciones de tal forma que las propiedades ventajosas
intrínsecamente ligadas a la presente invención no resulten
afectadas, o no lo sean substancialmente, de forma adversa por la
adición contemplada.
La composición farmacéutica según la invención
está destinada al tratamiento de afecciones dermatológicas con un
componente inflamatorio o proliferativo, seleccionadas entre:
- -
- acné común, comedones, acné polimorfo, acné noduloquístico, acné conglobata y acné secundario, tal como el acné solar, el relacionado con fármacos o el ocupacional,
- -
- formas extendidas y/o severas de psoriasis, ictiosis y estados ictiosiformes,
- -
- enfermedad de Darier,
- -
- queratosis actínicas,
- -
- queratodermia palmoplantar y queratosis pilaris,
- -
- leucoplasias y estados leucoplasiformes, liquen plano,
- -
- cualquier proliferación dermatológica benigna o maligna, grave y extensa.
La composición según la invención es
particularmente adecuada para el tratamiento del acné, tal como el
acné común, y en particular para el tratamiento del acné común de
intensidad moderada a moderadamente grave.
Se darán ahora diversas formulaciones de
composiciones que contienen un 0,3% de adapaleno, a modo de
ilustración, al igual que resultados que muestran la forma
terapéutica de la composición según la invención y la buena
tolerancia a la misma por los pacientes tratados.

En este ejemplo, se ilustran diversas
formulaciones tópicas concretas que contienen un 0,3% de
adapaleno.
El adapaleno del presente ejemplo es
suministrado por la compañía Sylachim, División Finorga (referencia
del producto CF9611996).
Se realizaron pruebas en una población
consistente en pacientes que sufrían de acné. En esta población, se
diferenciaron tres grupos; el primero recibió una aplicación tópica
diaria del gel de adapaleno al 0,3%, el segundo una aplicación
tópica diaria del gel de adapaleno al 0,1% en el mismo vehículo y el
tercero es un grupo control que recibe una aplicación tópica diaria
del gel correspondiente a la composición de los dos primeros geles,
pero que no contenía agente activo.
Las Figuras 1 a 3 dan los resultados obtenidos
en términos de regresión del número de lesiones según su
naturaleza.
Estas observaciones conducen a las siguientes
conclusiones:
- el gel de adapaleno al 0,3% actúa más
rápidamente que el gel de adapaleno al 0,1%; concretamente, desde la
cuarta semana de tratamiento, se observa una diferencia entre la
efectividad del gel de adapaleno al 0,1% y el de adapaleno al
0,3%;
- el gel de adapaleno al 0,3% produce un efecto
terapéutico claramente mayor tras 8 semanas de tratamiento.
Se tratan ocho individuos que sufren de acné
común de intensidad media a moderadamente grave durante 10 días con
2 g de gel de adapaleno al 0,3% aplicado diariamente sobre 1.000
cm^{2} de piel que ha de ser tratada (cara, pecho y espalda).
Se toman muestras de sangre los días 1, 2, 4, 6,
8 y 10. Durante el día 10, y después de la aplicación final, se
toman muestras a las 1, 2, 6, 8, 10, 12, 16 y 24 horas.
Se determina la concentración en plasma del
adapaleno total (libre y conjugado) en estas muestras usando el
siguiente protocolo:
- hidrólisis enzimática con una mezcla de
\beta-glucurodinasa y arilsulfatasa;
- extracción
líquido-líquido;
- paso a través de HPLC (cromatografía líquida
de alto rendimiento);
- detección fluorométrica después.
Este método hace posible la detección de una
concentración mínima de 0,15 ng/ml y permite la cuantificación del
adapaleno para una concentración mínima de 0,25 ng/ml.
Las concentraciones plasmáticas de adapaleno
medidas tras 10 días de tratamiento son muy bajas y confirman la
seguridad del uso diario del gel de adapaleno al 0,3%.
Se pudieron hacer dos tipos de observación:
- primeramente, la monitorización de los
pacientes tratados con el marco del punto 1 del presente ejemplo 3
hizo posible observar que la tolerancia al gel de adapaleno al 0,3%
era buena para todos los pacientes. Todos mostraron signos de
sequedad de la piel y de descamación con un máximo al séptimo día de
tratamiento; estos síntomas disminuyen después hasta finalizar el
tratamiento. 2b) Más aún, también se puede hacer referencia a las
pruebas descritas en el Ejemplo 2 anterior.
Paralelamente a las mediciones de efectividad,
los experimentadores registraron los posibles efectos colaterales
causados, en primer lugar, por aplicación tópica del gel de
adapaleno al 0,3%, y los causados en segundo lugar por la aplicación
del gel de adapaleno al 0,1%; finalmente, se hicieron las mismas
observaciones sobre una población control a la que se había
administrado un gel sin principio activo.
En la tabla siguiente se dan las
observaciones.
Al leer esta tabla, se observa que la aparición
de efectos colaterales no deseados es estadísticamente la misma para
los dos geles con las diferentes concentraciones de agente activo.
La intensidad de los efectos colaterales no deseados es media, lo
que conduce a la conclusión de que los dos geles son bien tolerados
por los pacientes.
En base a estas observaciones, se puede concluir
que los pacientes que sufren de acné común pueden ser tratados con
un gel de adapaleno al 0,3%, describiéndose dicha exposición al
adapaleno como débil o muy débil en condiciones clínicas.
Se deduce, por lo tanto, de estos diversos
estudios que una composición farmacéutica que contenga un 0,3% de
adapaleno exhibe una razón de beneficios/riesgos que la hace
particularmente adecuada para el tratamiento de elementos
dermatológicos con un componente inflamatorio o proliferativo, y en
particular el acné común.
Claims (13)
1. Composición farmacéutica que contiene un 0,3%
en peso de adapaleno (ácido
6-[3-(1-adamantil)-4-metoxifenil]-2-naftanoico)
en relación al peso total de la composición, o de sus sales
farmacéuticamente aceptables, caracterizada por estar en
forma de gel acuoso.
2. Composición según la reivindicación 1, que
contiene uno o más ingredientes seleccionados entre carbómero 940 y
propilenglicol.
3. Composición según una de las reivindicaciones
1 a 2, donde la composición de gel tiene la fórmula:
4. Composición farmacéutica que contiene un 0,3%
en peso de adapaleno (ácido
6-[3-(1-adamantil)-4-metoxifenil]-2-naftanoico)
en relación al peso total de la composición, o de sus sales
farmacéuticamente aceptables, caracterizada por estar en
forma de crema.
5. Composición según la reivindicación 4, que
contiene uno o más ingredientes seleccionados entre
perhidroescualeno, ciclometicona, sesquiestearato de
PEG-20 metilglucosa y sesquiestearato de
metilglucosa.
6. Composición según una de las reivindicaciones
4 ó 5, donde la composición de la crema tiene la fórmula:

7. Composición farmacéutica que contiene un 0,3%
en peso de adapaleno (ácido
6-[3-(1-adamantil)-4-metoxifenil]-2-naftanoico)
en relación al peso total de la composición, o de sus sales
farmacéuticamente aceptables, caracterizada por estar en
forma de una loción alcohólica basada en polietilenglicol.
8. Composición según cualquiera de las
reivindicaciones 1 a 7, que además incluye aditivos inertes o
combinaciones de estos aditivos, siendo seleccionados dichos
aditivos entre el grupo consistente en:
- -
- agentes humectantes,
- -
- potenciadores del sabor,
- -
- agentes conservantes tales como ésteres del ácido parahidroxibenzoico,
- -
- estabilizadores,
- -
- reguladores de la humedad,
- -
- reguladores del pH,
- -
- modificadores de la presión osmótica,
- -
- emulsores,
- -
- agentes de pantalla para el UV-A y el UV-B y
- -
- antioxidantes.
9. Composición farmacéutica según cualquiera de
las reivindicaciones 1 a 8, destinada al tratamiento de afecciones
dermatológicas con un componente inflamatorio o proliferativo.
10. Composición según la reivindicación 9,
destinada al tratamiento de afecciones dermatológicas con un
componente inflamatorio o proliferativo seleccionadas entre:
- -
- acné común, comedones, acné polimorfo, acné noduloquístico, acné conglobata y acné secundario, tal como el acné solar, el relacionado con fármacos o el ocupacional,
- -
- formas extendidas y/o severas de psoriasis, ictiosis y estados ictiosiformes,
- -
- enfermedad de Darier,
- -
- queratosis actínicas,
- -
- queratodermia palmoplantar y queratosis pilaris,
- -
- leucoplasias y estados leucoplasiformes, liquen plano,
- -
- cualquier proliferación dermatológica benigna o maligna, grave y extensa.
11. Composición farmacéutica según la
reivindicación 10, destinada al tratamiento del acné común.
12. Composición farmacéutica según la
reivindicación 11, destinada al tratamiento del acné común de
intensidad moderada a moderadamente grave.
13. Composición farmacéutica según cualquiera de
las reivindicaciones 1 a 12 para aplicación tópica.
