Abstract
Instrumento bipolar (3) de coagulación que comprende un conjunto (12) de electrodos de cuchilla que comprende una cara plana superior (14) y una cara plana inferior (15) separadas por una capa intermedia (28) de material aislante entre ambas, comprendiendo la cara superior (14) un primer y un segundo electrodos (16, 17) separados por un primer elemento aislante (18), y comprendiendo la cara inferior (15) un tercero y un cuarto electrodos (22, 23) separados por un segundo elemento aislante (24), estando el primer y el segundo elementos aislantes (18, 24) no interrelacionados entre sí, de manera que existe por lo menos una zona en la que el primer electrodo (16) recubre el tercer electrodo (22), y caracterizado porque existe por lo menos otra zona en la que el primer electrodo (16) recubre el cuarto electrodo (23).
Description
Instrumento quirúrgico.
Esta invención se refiere a un instrumento
electroquirúrgico bipolar tal como una cuchilla de escalpelo. Dichos
instrumentos se utilizan normalmente para la coagulación y/o el
corte de tejido en intervenciones quirúrgicas, más comúnmente en
"laparoscopia" o cirugía mínimamente invasiva, pero también en
cirugía "abierta".
En general los dispositivos electroquirúrgicos
se pueden clasificar en dos categorías, monopolar y bipolar. En un
dispositivo monopolar, se suministra una señal de radiofrecuencia a
un electrodo activo que se utiliza para tratar tejido en una zona
objetivo, completándose el circuito eléctrico mediante una
almohadilla de masa que es, generalmente, una almohadilla de área
grande acoplada al paciente en una posición alejada de la zona
objetivo. Por el contrario, en una disposición bipolar, en el
instrumento están presentes tanto un electrodo activo como un
electrodo de retorno, y la corriente fluye del electrodo activo al
electrodo de retorno. La presente invención se refiere a un
dispositivo bipolar.
El documento
US-B-4 850 353 da a conocer un
instrumento, según el preámbulo de la reivindicación 1.
En la cirugía mínimamente invasiva, es esencial
poder coagular tejido para contener o impedir la hemorragia. Por
definición, el procedimiento tiene lugar en un espacio limitado, y a
menudo es difícil o poco conveniente tener que reorientar
repetidamente el instrumento quirúrgico para conseguir dicha
coagulación. La presente invención da a conocer un instrumento
quirúrgico capaz de coagular tejido, disponiendo el instrumento de
una serie de zonas en las que se puede llevar a cabo la
coagulación.
Por consiguiente, se da a conocer un instrumento
bipolar de coagulación que comprende un conjunto de electrodos del
tipo de cuchilla que comprende una cara plana superior y una cara
plana inferior separadas por una capa intermedia de material
aislante entre ambas, comprendiendo la cara superior el primer y el
segundo electrodos separados por un primer elemento aislante, y
comprendiendo la cara inferior el tercer y el cuarto electrodos
separados por un segundo elemento aislante, no estando el primer y
el segundo elementos aislantes interrelacionados entre sí, de
manera que existe por lo menos una zona en la que el primer
electrodo recubre el tercer electrodo, y por lo menos otra zona en
la que el primer electrodo recubre el cuarto electrodo.
El término "del tipo de cuchilla" significa
un instrumento en el que el grosor del conjunto de electrodos es
considerablemente menor que su anchura. Típicamente, el grosor del
conjunto de electrodos es menor de 5 mm, y preferentemente menor de
3 mm. La relación de aspecto del conjunto de electrodos es por lo
menos de 1,5, es decir, el conjunto de electrodos es por lo menos
1,5 veces más ancho que grueso. Preferentemente, la relación de
aspecto es por lo menos de 2.
Al disponer dicha por lo menos otra zona en la
que el primer electrodo recubre el cuarto electrodo, el instrumento
ofrece al usuario tres zonas diferentes en el conjunto de
electrodos, desde las cuales puede llevarse a cabo la coagulación.
En primer lugar, la coagulación puede llevarse a cabo utilizando la
cara plana superior, fluyendo la corriente coagulante entre el
primer y el segundo electrodos. En segundo lugar, la coagulación
puede llevarse a cabo utilizando la cara plana inferior, fluyendo
la corriente coagulante entre el tercer y el cuarto electrodos.
Finalmente, la coagulación puede llevarse a cabo entre las caras
superior e inferior del conjunto de electrodos, fluyendo la
corriente coagulante entre el primer y el cuarto electrodos, en la
zona en la que se recubren entre sí.
En una disposición conveniente, el primer
electrodo está en comunicación eléctrica con el tercer electrodo.
Típicamente, el primer electrodo es integral con el tercer
electrodo. Análogamente, el segundo electrodo está convenientemente
en comunicación eléctrica con el cuarto electrodo, y es típicamente
integral con el mismo.
Preferentemente, dicha por lo menos otra zona es
adyacente a un borde del conjunto de electrodos del tipo de
cuchilla, y es convenientemente adyacente a la punta del mismo. De
este modo, la tercera zona desde la que puede llevarse a cabo la
coagulación comprende la punta del conjunto de electrodos.
Existen una serie de maneras en las que el
primer y el cuarto electrodos se pueden fabricar para recubrirse
mutuamente en dicha por lo menos una zona. En una disposición, el
primer y el segundo elementos aislantes son elementos que se
extienden longitudinalmente, estando uno dispuesto a un cierto
ángulo respecto al otro. Alternativa o adicionalmente, el primer y
el segundo elementos aislantes son de formas diferentes entre sí.
En otra disposición alternativa, el primer y el segundo elementos
aislantes son de forma similar, pero están situados en una
orientación diferente entre sí. Un ejemplo de esto es tener cada uno
del primer y el segundo elementos aislantes comprendiendo dos
secciones dispuestas en un ángulo agudo entre sí. Cualquiera que sea
la disposición que se utilice, es esencial que de algún modo el
primer elemento aislante en la cara superior del conjunto de
electrodos no esté correlacionado con el segundo elemento aislante
en la cara inferior del conjunto de electrodos, para asegurar que
existe por lo menos una zona en la que los electrodos de la cara
superior no se corresponden con los de la cara inferior.
Típicamente, bien el primer elemento aislante o
el segundo elemento aislante son integrales con la capa intermedia
de material aislante. Convenientemente, ambos primer y segundo
elementos aislantes son integrales con la capa intermedia.
En una disposición preferente, se dispone
adicionalmente un electrodo de corte, estando aislado el electrodo
de corte de cada uno de los electrodos primero a cuarto. De este
modo, el instrumento puede ser utilizado tanto para cortar como
para coagular tejido, típicamente en un procedimiento tal como una
histerectomía supracervical laparoscópica (LSH, laparoscopic
supracervical hysterectomy). Convenientemente, el electrodo de corte
está situado entre la cara plana superior y la cara plana inferior.
Típicamente, la capa intermedia de material aislante es en dos
partes, y el electrodo de corte está situado entre ambas.
Preferentemente, el electrodo de corte sobresale desde un borde del
conjunto de electrodos del tipo de cuchilla. Por lo tanto, el borde
del conjunto de electrodos puede utilizarse para cortar tejido, y
puede llevarse a cabo la coagulación en cualquiera de las zonas del
instrumento tal como se ha descrito previamente. De este modo,
pueden llevarse a cabo tanto el corte como la coagulación, siendo
necesario solamente un movimiento y una reorientación del
instrumento mínimos, tal como se requiere en un procedimiento
mínimamente invasivo.
A continuación se describirá la invención,
solamente a modo de ejemplo, haciendo referencia a los dibujos
anexos, en los cuales:
La figura 1 es un diagrama esquemático de un
sistema electroquirúrgico que comprende un instrumento
electroquirúrgico construido según la presente invención;
La figura 2 es una vista esquemática en
perspectiva de un instrumento electroquirúrgico construido según la
presente invención; y
La figura 3 es una vista esquemática en
perspectiva del instrumento de la figura 2, ilustrado desde
abajo.
Haciendo referencia a la figura 1, un generador
(1) tiene una conexión hembra (2) de salida que proporciona una
salida de radiofrecuencia (RF) para un instrumento (3) a través de
un cable de conexión (4). La activación del generador (1) se puede
llevar a cabo desde el instrumento (3) a través de una conexión en
el cable (4), o por medio de una unidad (5) de interruptor de
pedal, tal como se muestra, conectada a la parte trasera del
generador mediante un cable (6) de conexión con el interruptor de
pedal. En la realización ilustrada, la unidad (5) de interruptor de
pedal tiene dos interruptores de pedal (5A) y (5B) para seleccionar
un modo de coagulación y un modo de corte del generador (1),
respectivamente. El panel frontal del generador tiene pulsadores (7)
y (8) para ajustar respectivamente niveles de potencia de
coagulación y de corte, que se indican en una pantalla (9). Se
disponen pulsadores (10) como un medio alternativo para la selección
entre los modos de coagulación y de corte.
Haciendo referencia a las figuras 2 y 3, se
muestra el instrumento (3) con una varilla (11) que se extiende
longitudinalmente y un conjunto (12) de electrodos del tipo de
cuchilla. El conjunto (12) de electrodos tiene la forma de una
espátula (13), inclinada según un ángulo de unos 20 grados
aproximadamente con respecto al eje de la varilla (11), y tiene una
cara ancha superior (14), una cara ancha inferior (15) y una sección
transversal relativamente fina. La cara superior (14) está formada
por un primer electrodo (16) (denominado el electrodo superior
izquierdo), y un segundo electrodo (17) (denominado el electrodo
superior derecho). El primer y el segundo electrodos (16) y (17)
están fabricados de cobre, recubierto con oro duro sobre níquel.
Entre el primer y el segundo electrodos (16) y
(17) hay un primer elemento aislante (18), fabricado de cerámica.
El primer elemento aislante (18) tiene forma acodada, con una parte
de pie (19) que se extiende longitudinalmente, una parte de codo
(20), y una parte extrema (21) que se extiende a un ángulo de unos
45 grados. La parte extrema (21) forma ángulo hacia el segundo
electrodo (17), de manera que el primer electrodo (16) tiene un área
superficial mayor que el segundo electrodo.
La cara inferior (15) está constituida por un
tercer electrodo (22) (denominado electrodo inferior izquierdo), y
un cuarto electrodo (23) (denominado electrodo inferior derecho), de
nuevo fabricados de cobre recubierto como anteriormente. Entre el
tercer y el cuarto electrodos (22) y (23) hay un segundo elemento
aislante (24), de nuevo fabricado de cerámica. El segundo elemento
aislante (24) también tiene forma acodada, con un pie (25) que se
extiende longitudinalmente, una parte de codo (26) y una parte
extrema (27). La parte extrema (27) está, análogamente, en ángulo a
unos 45 grados, pero en orientación opuesta a la del primer elemento
aislante (18), de manera que el cuarto electrodo (23) tiene un área
superficial mayor que el tercer electrodo (22). A lo largo de un
lado del conjunto (12) de electrodos, el primer electrodo (16)
recubre el tercer electrodo (22). A lo largo del lado opuesto del
conjunto (12) de electrodos, el segundo electrodo (17) recubre el
cuarto electrodo (23). Sin embargo, la orientación diferente del
primer y del segundo elementos aislantes (18) y (24) significa que,
hacia la punta del conjunto (12) de electrodos, el primer electrodo
(16) recubre el cuarto electrodo (23). Esto es importante, tal como
se explicará después en mayor detalle.
Entre la cara superior (14) y la cara inferior
(15) hay una parte intermedia (28), que comprende una estructura de
tres capas que consiste en una capa superior (29) de cerámica, un
electrodo de corte plano (30) y una capa inferior (31) de cerámica.
Las capas de cerámica superior e inferior (29) y (31) sirven para
aislar el electrodo de corte (30) con respecto a los electrodos
primero a cuarto (16), (17), (22) y (23) descritos previamente. El
electrodo de corte (30) está fabricado de acero inoxidable, y se
extiende ligeramente más allá de la periferia del resto del conjunto
(12) de electrodos.
El primer y el tercer electrodos (16) y (22)
(los electrodos superior e inferior del lado izquierdo) están
fabricados de un elemento metálico unitario, y están conectados a
través de un conductor (no mostrado) a una salida del generador
electroquirúrgico (1). Análogamente, el segundo y el cuarto
electrodos (17) y (23) (los electrodos superior e inferior del lado
derecho) están asimismo fabricados de un elemento metálico unitario,
y están conectados a través de un conductor (no mostrado) a otra
salida del generador (1). El electrodo de corte (30) está asimismo
conectado a través de un conductor (no mostrado) a una salida del
generador (1). El generador (1) tiene una salida conmutable, como la
descrita en nuestra solicitud de patente europea
co-pendiente EP 1 287 788.
A continuación se describirá el funcionamiento
del instrumento (3). El instrumento (3) es introducido en el cuerpo
del paciente a través de un trocar quirúrgico (no mostrado). Cuando
el cirujano desea utilizar el instrumento (3) para cortar tejido,
el interruptor de pedal (5B) es presionado enviando al instrumento
una señal de corte electroquirúrgico. Esta señal de corte es
suministrada al electrodo de corte (30), estando los electrodos
primero al cuarto (16), (17), (22) y (23) conectados entre sí y
actuando como el electrodo de retorno para la operación de corte.
El corte electroquirúrgico está escrito en mayor detalle en nuestra
solicitud de patente PCT co-pendiente
WO03/055402.
Cuando el cirujano desea coagular tejido, el
interruptor de pedal (5A) es presionado enviando al instrumento (3)
una señal de coagulación. Esta señal de coagulación es suministrada
entre los electrodos (16) y (22) del lado izquierdo por una parte,
y los electrodos (17) y (23) del lado derecho por la otra. Esto
proporciona al cirujano tres opciones diferentes para la
coagulación. En primer lugar, el cirujano puede utilizar la cara
superior del conjunto (12) de electrodos, coagulando tejido entre el
primer y el segundo electrodos (16) y (17) a través del primer
elemento aislante (18). En segundo lugar, el cirujano puede utilizar
la cara inferior del conjunto (12) de electrodos, coagulando tejido
entre el tercer y el cuarto electrodos (22) y (23) a través del
segundo elemento aislante (24). Finalmente, el cirujano puede
utilizar la punta del conjunto (12) de electrodos, coagulando
tejido entre el primer y el cuarto electrodos (16) y (23) a través
de la parte intermedia (28). Esto es posible solamente dadas las
orientaciones diferentes de los primer y segundo elementos aislantes
(18) y (24), que permiten que el primer y el cuarto electrodos (16)
y (23) se recubran entre sí en la zona del conjunto (12) de
electrodos. De este modo, el cirujano puede coagular tejido
utilizando tres partes diferentes del conjunto (12) de electrodos,
minimizando por lo tanto la necesidad del movimiento y de la
reorientación del instrumento (3). Además, la utilización de las
caras superior e inferior (14) y (15) del instrumento (3) es
adecuada para la coagulación de un área extensa, mientras que la
utilización de la zona de la punta del instrumento es más adecuada
para la coagulación puntual. Por supuesto, las tres partes
diferentes del conjunto (12) de electrodos utilizado para la
coagulación pueden ser utilizadas en combinación. Por ejemplo, la
coagulación con punta descrita anteriormente puede reforzarse
mediante una contribución de la coagulación entre el primer y el
segundo, y/o el tercer y el cuarto electrodos (16), (17), (22) y
(23). Esto proporciona una zona mayor de coagulación cuando se
utiliza la zona de la punta, si se desea.
Se apreciará que son posibles otros diseños y
formas de los elementos aislantes (18) y (24), mientras sigan
proporcionando, como mínimo, una zona en la que el primer electrodo
(16) recubra el cuarto electrodo (23). Por ejemplo, el primer y el
segundo elementos aislantes (18) y (24) pueden ser sustancialmente
idénticos, y aún así extenderse a ángulos diferentes con respecto
al eje longitudinal de la varilla (11). Alternativamente, cada uno
de los elementos de aislamiento (18) y (24) puede tener una forma
diferente. Cualquiera que sea el diseño utilizado, la no
correlación entre los elementos de aislamiento (18) y (24) sirve
para proporcionar la zona de coagulación adicional en la que el
primer electrodo (16) recubre el cuarto electrodo (23).
Claims (17)
1. Instrumento bipolar (3) de coagulación que
comprende un conjunto (12) de electrodos de cuchilla que comprende
una cara plana superior (14) y una cara plana inferior (15)
separadas por una capa intermedia (28) de material aislante entre
ambas, comprendiendo la cara superior (14) un primer y un segundo
electrodos (16, 17) separados por un primer elemento aislante (18),
y comprendiendo la cara inferior (15) un tercero y un cuarto
electrodos (22, 23) separados por un segundo elemento aislante
(24), estando el primer y el segundo elementos aislantes (18, 24)
no interrelacionados entre sí, de manera que existe por lo menos una
zona en la que el primer electrodo (16) recubre el tercer electrodo
(22), y caracterizado porque existe por lo menos otra zona en
la que el primer electrodo (16) recubre el cuarto
electrodo (23).

6. Instrumento bipolar de coagulación, según
cualquier reivindicación precedente, en el que dicha por lo menos
otra zona es adyacente a un borde del conjunto (12) de electrodos
del tipo de cuchilla.
7. Instrumento bipolar de coagulación, según la
reivindicación 6, en el que dicha por lo menos otra zona es
adyacente a la punta del conjunto (12) de electrodos del tipo de
cuchilla.
8. Instrumento bipolar de coagulación, según
cualquier reivindicación precedente, en el que el primer y el
segundo elementos aislantes (18, 24) son elementos que se extienden
longitudinalmente, estando uno dispuesto con cierto ángulo respecto
al otro.
9. Instrumento bipolar de coagulación, según
cualquiera de las reivindicaciones 1 a 7, en el que el primer y el
segundo elementos aislantes (18, 24) son de formas diferentes entre
sí.
10. Instrumento bipolar de coagulación, según
cualquiera de las reivindicaciones 1 a 7, en el que el primer y el
segundo elementos aislantes (18, 24) son de forma similar, pero
están situados en una orientación diferente entre sí.
11. Instrumento bipolar de coagulación, según la
reivindicación 10, en el que cada uno del primer y el segundo
elementos aislantes (18, 24) comprende dos secciones dispuestas en
un ángulo agudo entre sí.
12. Instrumento bipolar de coagulación, según
cualquier reivindicación precedente, en el que el primer elemento
aislante (18) es integral con la capa intermedia (28) de material
aislante.
13. Instrumento bipolar de coagulación, según
cualquier reivindicación precedente, en el que el segundo elemento
aislante (24) es integral con la capa intermedia (28) de material
aislante.
14. Instrumento bipolar de coagulación, según
cualquier reivindicación precedente, que comprende además un
electrodo de corte (30), estando aislado el electrodo de corte
respecto a cada uno de los electrodos primero a cuarto (16, 17, 22,
23).
15. Instrumento bipolar de coagulación, según la
reivindicación 14, en el que el electrodo de corte (30) está situado
entre la cara plana superior (14) y la cara plana inferior (15).
16. Instrumento bipolar de coagulación, según la
reivindicación 15, en el que la capa intermedia (23) de material
aislante es de dos partes, y el electrodo de corte (30) está situado
entre ambas.
17. Instrumento bipolar de coagulación, según
cualquiera de las reivindicaciones 14 a 16, en el que el electrodo
de corte (30) sobresale de un borde del conjunto (12) de electrodos
del tipo de cuchilla.
