Abstract
Un material activable mediante energía para uso en el tratamiento de un trastorno de las glándulas sebáceas, en que a) dicho material activable mediante energía es un cromóforo, adecuado para su administración a la abertura al infundíbulo o a una unidad pilosebácea y se activa mediante una especie de energía que penetra en las capas exteriores de la epidermis, y en que b) dicho material activable mediante energía está compuesto de tal manera que al administrarlo por vía tópica a una sección de la piel, una cantidad suficiente de dicho material infiltra los espacios alrededor del infundíbulo y se concentra selectivamente alrededor de dicho infundíbulo o infiltra la unidad pilosebácea, y en que c) dicho tratamiento es la administración por vía tópica de dicho material activable mediante energía a una sección de la piel afectada por un trastorno de las glándulas sebáceas y la exposición de la sección de la piel a energía pulsada caracterizada por una densidad energética no superior a 100 J/cm2, que hace que dicho material se active fototérmicamente para tratar eficazmente un trastorno de las glándulas sebáceas con una destrucción mínima o ninguna destrucción de las estructuras epidérmicas y dérmicas normales adyacentes.

Para los cromóforos que destruyen a través de la degradación del cromóforo a un producto de reacción tóxico, en general no se desea fotoestabilidad, ya que la descomposición del cromóforo es el proceso que consigue el efecto deseado.
5 En el presente procedimiento, se emplea la fotoactivación selectiva, por la que una fuente de energía (luminosa), por ejemplo, un láser, se hace corresponder con una longitud de onda para el espectro de absorción del material activable mediante energía, preferente un agente cromóforo, por ejemplo, azul de metileno a 661 nm. Por ejemplo, un material activable mediante energía, adaptado para acumularse selectivamente en el infundíbulo y/o la glándula
10 sebácea se administra primeramente a la región de la piel afectada que ha de tratarse. Después de la absorción del material activable mediante energía, el material acumulado se expone a una fuente de energía, por ejemplo, un láser, capaz de producir una longitud de onda fácilmente absorbida por el material activable mediante energía, con lo que se calientan fototérmicamente de manera selectiva aquellas regiones de la dermis de las que se sabe que tienen aceites, bacterias, virus, suciedad, etc. atrapados, es decir, la unidad pilosebácea que incluye la abertura del
15 poro, el infundíbulo y la glándula sebácea. Dado que el material activable mediante energía se concentra selectivamente dentro o alrededor de estos depósitos indeseados, los depósitos se degradan por el calor y/o las especies radicales generadas a partir del material activable mediante energía. La destrucción de las estructuras epidérmicas y dérmicas normales adyacentes es mínima o inexistente.
20 Preferentemente, el tratamiento de la invención modifica la abertura del poro al infundíbulo, de manera que la geometría, por ejemplo, la forma, de la abertura se altera permanentemente. El ajuste de la concentración del material activable mediante energía y de la cantidad de energía aplicada por la fuente de energía efectúa el estrechamiento, cierre o apertura del poro, con lo que se previene la acumulación de suciedad, aceites, bacterias o virus en este folículo. El operador necesitará establecer los parámetros para producir el efecto deseado, que se
25 determinarán de manera individual para cada paciente. Generalmente, lo más deseable es la alteración de la forma del poro, dejándolo agrandado y sin propensión a la acumulación de sebo y/o materiales extraños que podrían causar el taponamiento del poro.
Según se ha expuesto previamente, la presente invención implica el uso de fuentes de energía, por ejemplo, láseres,
30 para dirigirlas a glándulas sebáceas y causar su destrucción fototérmica. Las glándulas sebáceas se componen fundamentalmente de material lípido amorfo y no contienen ningún cromóforo endógeno evidente. Con el fin de conseguir una fotocoagulación selectiva de las glándulas sebáceas y limitar la extensión de la lesión térmica en el tejido circundante, puede utilizarse un material activable mediante energía con distribución selectiva a la unidad pilosebácea. La introducción de un material activable mediante energía en las glándulas sebáceas seguida de su
35 exposición a una energía (luminosa) con una longitud de onda que corresponde al pico de absorción del cromóforo aumentará la absorción local de luz en el tejido y conducirá a un daño térmico selectivo de las glándulas sebáceas.
El infundíbulo es un sitio crítico en la patogénesis de muchos de los estados patológicos, especialmente el acné. Existe evidencia de que la proliferación y la desescamación anormal de los queratinocitos del infundíbulo conducen 40 a la formación de microcomedones y, después, a los “tapones” foliculares clínicamente visibles o comedones. Desde el punto de vista clínico, parece que algunos folículos sebáceos son más propensos que otros a la aparición de lesiones de acné, posiblemente debido a una diferencia estructural inherente o una anormalidad funcional del infundíbulo que les predispone a formar tapones y a obstruirse. La naturaleza de autorresolución del acné en la mayoría de los pacientes puede reflejar la eliminación de tales folículos “propensos al acné”, que se sustituyen
45 eventualmente por piel normal o una fibrosis después de repetidos brotes de la inflamación. Si la arquitectura del infundíbulo es importante en la patogénesis del acné, la destrucción selectiva de esta porción del folículo a través de la dirección de energía, asistida por un material activable mediante energía, por ejemplo, láser, puede ayudar a eliminar o corregir el sitio “patológico” al crear una abertura folicular distendida que es capaz de expulsar cualquier material de obstrucción.
50 El procedimiento de activación selectiva mediante energía de acuerdo con la presente invención usa fuentes de energía, por ejemplo, luminosa, por ejemplo, láseres, que se corresponden con un material activable mediante energía concreto. En el caso de la activación fototérmica, para facilitar el aumento de la temperatura, el tiempo de duración del pulso debe ser menor que el tiempo de relajación térmica para el material activable mediante energía.
55 El tiempo de relajación térmica se define como el tiempo que necesita una estructura para enfriarse al 50 % de su temperatura pico inmediatamente después de la exposición a una fuente luminosa capaz de suministrar la energía suficiente para la fotoactivación del cromóforo. 1 a 50 milisegundos (ms) fotoactiva adecuadamente materiales activables mediante energía, como partículas de carbono, partículas de óxido de hierro y colorantes sensibles a láser, por ejemplo, materiales cromóforos, depositados dentro de la matriz del folículo piloso, por ejemplo, alrededor del infundíbulo y la glándula sebácea. Los diferentes tipos de materiales activables mediante energía requieren variaciones en la dosis energética aplicada y el
5 tipo de fuente de energía necesario para efectuar el tratamiento del área de la piel afectada. Cuando se administra sobre la piel de la región que ha de tratarse, el material activable mediante energía se absorbe dentro de la matriz del folículo piloso y durante la exposición, la energía se concentrará en aquellas áreas críticas de la matriz folicular en las que se ha recogido el material activable mediante energía, por ejemplo, la unidad pilosebácea que incluye la glándula sebácea, el infundíbulo y la abertura del poro.
10 El suministro del material activable mediante energía, preferentemente azul de metileno u otros colorantes aprobados por la FDA, a la matriz folicular puede conseguirse mediante administración por vía tópica, inyección, tecnología de encapsulación mediante liposomas, masaje, iontoforesis o tecnología ultrasónica u otros modos de suministro de compuestos a la región dérmica de la piel, por ejemplo, vehículos farmacéuticamente aceptables.
15 La expresión “vehículo farmacéuticamente aceptable”, según se usa en este documento, indica un material, composición o vehículo farmacéuticamente aceptable, como una carga, diluyente, excipiente, disolvente o material de encapsulación líquido o sólido, implicado en la conducción o transporte de un material activable mediante energía de la presente invención dentro de o al sujeto, de tal manera que puede llevar a cabo su función propuesta. Los
20 vehículos deben ser “aceptables” en el sentido de ser compatibles con los otros ingredientes de la formulación y no ser lesivos para el paciente. Algunos ejemplos de materiales que pueden servir como vehículos farmacéuticamente aceptables incluyen: azúcares, como lactosa, glucosa y sacarosa; almidones, como almidón de maíz y almidón de patata; celulosa y sus derivados, como carboximetilcelulosa de sodio, etilcelulosa y acetato de celulosa; tragacanto en polvo; malta; gelatina; talco; excipientes, como manteca de cacao y ceras de supositorios; aceites, como aceite
25 de cacahuete, aceite de semillas de algodón, aceite de cártamo, aceite de sésamo, aceite de oliva, aceite de maíz y aceite de soja; glicoles, como propilenglicol; polioles, como glicerina, sorbitol, manitol y polietilenglicol; ésteres, como oleato de etilo y laurato de etilo; agar; agentes tamponantes, como hidróxido de magnesio e hidróxido de aluminio; ácido algínico; agua apirógena; disolución salina isotónica; disolución de Ringer; alcohol etílico; disoluciones tampón de fosfato; y otras sustancias compatibles no tóxicas empleadas en formulaciones farmacéuticas. Los vehículos
30 preferidos incluyen aquellos que son capaces de entrar en un poro por tensión superficial y transporte de disolventes, de tal manera que el material activable mediante energía se transporta al interior o alrededor del poro, por ejemplo, a la glándula sebácea, al tapón, al infundíbulo y/o a la glándula sebácea y el infundíbulo.
En las composiciones también puede haber presentes agentes humectantes, emulsionantes y lubricantes, como
35 laurilsulfato de sodio y estearato de magnesio, así como agentes colorantes, agentes de liberación, agentes de recubrimiento, agentes edulcorantes y aromatizantes, conservantes y antioxidantes. Las formas de dosificación líquidas para la administración por vía tópica de los compuestos de la invención incluyen emulsiones, microemulsiones, disoluciones, cremas, lociones, pomadas, suspensiones y jarabes farmacéuticamente aceptables. Además del principio activo, las formas de dosificación líquidas pueden contener diluyentes inertes de uso común en
40 la técnica como, por ejemplo, agua u otros disolventes, agentes solubilizantes y emulsionantes, como alcohol etílico, alcohol isopropílico, carbonato de etilo, acetato de etilo, alcohol bencílico, benzoato de bencilo, propilenglicol, 1,3butilenglicol, aceites (en particular, los aceites de semillas de algodón, cacahuete, maíz, germen, oliva, ricino, melocotón, almendras y sésamo), glicerol, alcohol tetrahidrofurílico, polietilenglicoles y ésteres de ácidos grasos de sorbitán, y mezclas de los mismos.
45 Las suspensiones pueden contener, además de los principios activos, agentes de suspensión como, por ejemplo, alcoholes isostearílicos etoxilados, polioxietilensorbitol y ésteres de sorbitán, celulosa microcristalina, metahidróxido de aluminio, bentonita, agar-agar y tragacanto, y mezclas de los mismos.
50 Las pomadas, pastas, cremas y geles pueden contener, además de un compuesto activo de esta invención, excipientes, como grasas animales y vegetales, aceites, ceras, parafinas, almidón, tragacanto, derivados de celulosa, polietilenglicoles, siliconas, bentonitas, ácido silícico, talco y óxido de cinc, o mezclas de los mismos.
El término “crema” está aceptado en la técnica y pretende incluir sistemas de emulsiones semisólidas que contienen
55 un aceite y agua. Las cremas de aceite en agua son miscibles con agua y se absorben bien en la piel, por ejemplo, crema acuosa BP. Las cremas de agua en aceite (aceitosas) son inmiscibles con agua y, por lo tanto, más difíciles de eliminar de la piel. Estas cremas son emolientes, lubrican y humedecen, por ejemplo, crema oleosa BP. Ambos sistemas requieren la adición de un tensioactivo o emulsionante natural o sintético.

La estimulación del material activable mediante energía 28 hará que se produzca una activación, por ejemplo, fototermólisis y/o reacciones fotoquímicas, para alterar las células, sebo, bacterias, ácaros, etc. atrapados

Una ventaja de este procedimiento es que solo el tejido con el material activable mediante energía experimentará las reacciones fototérmicas o fotoquímicas. Los cromóforos naturales presentes en los folículos sebáceos o los tapones foliculares no se distinguen
35 suficientemente de otros cromóforos de la dermis y la epidermis para permitir una absorción específica. Sin embargo, el infundíbulo y las glándulas sebáceas son directamente accesibles desde la superficie de la piel a través de un “poro” (la abertura del folículo) que permite la entrada en estas estructuras de sustancias administradas por vía tópica, como materiales activables mediante energía. Por consiguiente, pueden usarse materiales activables mediante energía o suspensiones de partículas para proporcionar una alta absorción local y específica después de
40 su entrada en el infundíbulo y/o la glándula sebácea.
Los materiales activables mediante energía que entran en los folículos sebáceos, como el azul de metileno (un colorante lipófilo catiónico aprobado por la FDA es absorbido por los folículos sebáceos humanos y distribuido con el tiempo a las glándulas sebáceas), pueden usarse para dirigirlos a los infundíbulos o a las glándulas sebáceas

Los materiales activables mediante energía administrados por vía tópica entran inicialmente en el infundíbulo a después se distribuyen a las glándulas sebáceas. Es posible conducir activamente estos materiales o partículas cromóforas a los folículos mediante masaje, presión, ultrasonidos o iontoforesis, después de la administración del 50 cromóforo por vía tópica a la superficie de la piel. Por ejemplo, el azul de metileno puede conducirse rápidamente al interior de los folículos sebáceos y los conductos sudoríparos ecrinos por iontoforesis. El frotamiento de la piel con o sin un disolvente después del suministro a los folículos puede usarse para eliminar el material residual de la superficie de la piel. Por lo tanto, después de la administración y frotamiento adecuados, el material activable mediante energía, por ejemplo, un cromóforo, puede localizarse preferentemente en los folículos, dentro de los
55 infundíbulos o en los infundíbulos y las glándulas sebáceas.

Experimento con el uso de azul de metileno para la tinción de glándulas sebáceas en tejido ex vivo.
5 Un cirujano plástico proporcionó piel humana escindida recientemente en un procedimiento de estiramiento facial. La piel procedía del área periauricular y la línea el cabello anterior de una mujer de mediana edad y piel clara. Las muestras se almacenaron a 4 °C durante la noche. El día del experimento, el tejido se afeitó con una cuchilla y se desengrasó frotando la superficie con hisopos de etanol durante un minuto. Después de cortar la piel en piezas más

El azul de metileno, un colorante hidrófilo catiónico, se disolvió en agua destilada, etanol y alcohol propílico a una concentración del 5 % y se administró sobre la superficie de la piel en una capa gruesa a 31 °C. Después de una hora, el exceso de colorante se eliminó con una gasa absorbente seca, lo que dio lugar a una epidermis ligeramente teñida con acentuación de la tinción en los poros foliculares en todas las muestras. La microscopía óptica de las secciones histológicas mostró una densa tinción azul de la epidermis y de algunas glándulas sebáceas y folículos pilosos completos (figura 9). Sólo hubo una mínima tinción inespecífica de la dermis intersticial.

Típicamente, se combinó una cantidad suficiente de azul de metileno (50 ml del colorante disuelto en 500 mg de gel) con hidroxipropilcelulosa, hidroxitolueno butilado y etanol y se administró
40 sobre la epidermis. La penetración del colorante azul de metileno en las glándulas sebáceas de la piel humana recién escíncida se valoró mediante microscopía óptica, según se describe anteriormente.
