Abstract
Una composición de apósito para heridas que comprende un vehículo farmacéuticamente aceptable y un lisado de células enteras no viable, o una fracción de sedimento obtenida sometiendo el lisado de células enteras a centrifugación, o una fracción del sobrenadante obtenida sometiendo el lisado de células enteras a centrifugación; siendo el lisado de células enteras derivado de macrófagos, en el que dicho vehículo farmacéuticamente aceptable está en forma de una hoja sólida, un ungüento semisólido, una hoja sólida con aberturas, una banda, una tela tejida, una tela de punto, una tela no tejida, una espuma hidrófila, una esponja liofilizada o una esponja secada con disolvente

final a las 2,5 h, se incubó la placa de microtitulación a 37ºC, CO2 al 5% durante este tiempo. El efecto proliferativo de cada condición de ensayo se evaluó comparando las lecturas de absorbancia medidas. Utilizando los valores de control positivo (10% FBS/DMEM) y negativo (DMEM libre de suero) como 100% y 0% respectivamente, todas las otras condiciones se pudieron calcular como % de proliferación.
Las muestras de ensayo incluyeron fluido de la herida agudo y crónico, obtenido de pacientes con una herida del sitio del donante o una úlcera venosa crónica, respectivamente. El líquido de la herida se recogió por aspiración en ambos casos.
Se realizaron experimentos adicionales sobre un fluido de herida simulado, que representa las condiciones hostiles de una herida crónica. El fluido de la herida simulado comprendía PBS + 2% de BSA + elastasa (5 g/ml).
Todos los fluidos de la herida se diluyeron 1:2 con muestras de ensayo apropiadas, es decir liberados de macrófagos, lisado de macrófagos, liberado de monocitos y se incubaron durante 2 horas a 37ºC antes de la adición a los fibroblastos. En este punto, las muestras de ensayo se diluyeron adicionalmente a 1:2 con DMEM libre de suero y se añadieron directamente a la monocapa de células para su evaluación en el ensayo de proliferación celular como se ha descrito anteriormente.
Haciendo referencia a la FIG. 1, se puede observar que la proliferación de fibroblastos para las muestras de células dérmicas (a) y (b) cultivadas en el fluido crónico de la herida era mucho menor, de hecho negativa, en comparación con las células (c) cultivadas en el fluido agudo de la herida. Se cree que esto se debe a la presencia de altos niveles de enzimas proteasas en el fluido crónico de la herida que degradan los factores de crecimiento e interfieren de otro modo con la proliferación celular.
Haciendo referencia a la FIG. 2, que tiene fines ilustrativos, las células dérmicas se incubaron en (a) 10% de suero como un control positivo que estimula las células; (b) medio libre de suero como control negativo que mantiene las células pero no da crecimiento; (c) fluido crónico de la herida solo; (d) el medio en el que se preparó el liberado; (e) tanto el fluido crónico de herida como un liberado de células monocíticas; y (f) tanto el fluido crónico de la herida como el liberado de macrófagos.
Los resultados muestran que el liberado de macrófagos estimula significativamente el crecimiento celular y puede revertir el efecto negativo del entorno de la herida crónica. Este efecto es significativamente mejor que el efecto observado con el liberado de monocitos.
Haciendo referencia a la FIG. 3, las muestras son (a) 10% de suero como un control positivo que estimula las células; (b) medio libre de suero como control negativo que mantiene las células pero no da crecimiento; (c) fluido crónico de la herida solo; y (d) el fluido crónico de herida y un lisado de macrófagos. Se puede observar que la muestra del lisado de macrófagos (d) también fue capaz de superar el efecto negativo del entorno de herida crónica y estimular el crecimiento celular.
Un efecto similar se lograría con hepatocitos. Esto se debe a que los macrófagos y los hepatocitos producen una combinación de factores de crecimiento e inhibidores de proteasa, principalmente alfa-1-antitripsina.
Las realizaciones anteriores se han descrito únicamente a modo de ejemplo. Muchas otras realizaciones que caen dentro del alcance de las reivindicaciones adjuntas serán evidentes para el lector experto.
