Abstract

Una composición farmacéutica que comprende citarabina y daunorrubicina a una relación molar fija, no antagónica de citarabina con respecto a daunorrubicina de aproximadamente 5:1 para su uso en el tratamiento de la leucemia mieloide aguda (AML) o la leucemia linfoblástica aguda (ALL) en un paciente humano, en donde dicha relación molar no antagónica fija de citarabina con respecto a daunorrubicina está encapsulada en liposomas que comprenden diestearoil fosfatidil colina (DSPC), diestearoil fosfatidil glicerol (DSPG) y colesterol en una relación molar DSPC:DSPG:colesterol de 7:2:1; en donde una dosis de dicha composición se administra por vía intravenosa durante 3 horas o menos; en donde la dosis de dicha composición es 32-134 unidades por m2 , en donde 1 unidad es 1 mg de citarabina y 0.44 mg de daunorrubicina; y en donde dicha composición se administra en un programa de dosificación los días 1, 3 y 5.

Como es típico en los ensayos clínicos de fase I, los pacientes en esta población de fase I eran pacientes muy pretratados que no tenían opciones de tratamiento alternativas. Los resultados para estos pacientes con leucemia en las dosis en que comenzamos a lograr respuestas terapéuticas se resumen a continuación en la Tabla 7. En la Tabla 7, se usan las siguientes abreviaturas: "CR" representa "remisión completa"; "PR" representa "remisión parcial"; "CRp"
5 representa "remisión completa con recuperación incompleta de plaquetas" (es decir, una falla para lograr la recuperación de plaquetas mayor o igual a 100.000/µL para el Día 42 del Estudio); "BMT" representa "trasplante de médula ósea"; "PD" representa el efecto farmacodinámico (PD) que se define como:
• Reducción de la celularidad de la médula ósea (>50% de reducción, con reducción de blastocitos) y/o
• Evento no hematológico emergente del tratamiento posiblemente, probablemente o definitivamente relacionado 10 con CPX-351 (≥grade 2 <DLT).
Como se demostró por los resultados, se observaron respuestas terapéuticas con tan solo 32 unidades/m2 de CPX-351 administradas y se continuó observando sorprendentemente en casi todos los niveles de dosis hasta e incluyendo 134 unidades/m2.
