Abstract

Antecedentes
Campo técnico
La presente descripción se refiere a un instrumento quirúrgico y a un conjunto de empuñadura para su utilización con un instrumento quirúrgico. Más particularmente, la presente descripción se refiere a un conjunto de empuñadura que incorpora un actuador accionado por muelle para su utilización con un instrumento quirúrgico que tiene un efector de extremo distal.
Antecedentes de la técnica relacionada
Durante los procedimientos quirúrgicos se utilizan diversos instrumentos para manipular tejido. Algunos de estos instrumentos incorporan un conjunto de empuñadura que está previsto para transmitir una fuerza a un efector de extremo. Por ejemplo, algunos instrumentos quirúrgicos pueden estar provistos de un par de mordazas en el extremo distal para agarrar o cortar diversos tejidos. La operación del conjunto de empuñadura abre y cierra las mordazas transmitiendo una fuerza desde un actuador o mecanismo disparador asociado con el conjunto de empuñadura hasta las mordazas y, por lo tanto, hasta el tejido. Pueden estar previstos otros tipos de instrumentos quirúrgicos, incluyendo instrumentos que tienen efectores de extremo de aplicación de fijaciones que están configurados para aplicar grapas, clips u otras fijaciones al tejido, e instrumentos que aplican energía electroquirúrgica para sellar y/o fundir tejido.
Durante la realización de determinados procedimientos quirúrgicos con los instrumentos quirúrgicos arriba descritos se pueden producir daños en el propio instrumento. Esto puede ocurrir cuando el tejido sobre el que se está operando es suficientemente rígido o duro, de tal modo que no puede ser comprimido o cortado por el instrumento quirúrgico. Adicionalmente, es posible que determinados tejidos duros no puedan ser penetrados mediante la cantidad de fuerza aplicada a las fijaciones en situaciones en las que se desea grapar o sellar tejido. De modo similar, es posible que algunos instrumentos no puedan comprimir por completo los tejidos cuando se requiere una compresión de tejidos durante la aplicación de clips quirúrgicos.
Además, muchos instrumentos quirúrgicos utilizan conjuntos de cartucho recambiables o desechables para aplicar múltiples clips o grapas al tejido. Un posicionamiento inadecuado de los conjuntos de cartucho en el instrumento quirúrgico, por ejemplo, puede resultar en una resistencia del instrumento quirúrgico a la aplicación de presión sobre el disparador de un conjunto de empuñadura, produciéndose así un daño en el propio instrumento quirúrgico. Esto también puede ocurrir cuando el conjunto de cartucho está desprovisto de fijaciones y el cirujano intenta seguir utilizando o reutilizar el instrumento quirúrgico. Adicionalmente, una vez que las mordazas están posicionadas alrededor de tejido, un cirujano puede desear aflojar las mordazas para corregir la posición del efector de extremo. Además, un cirujano puede desear utilizar el instrumento quirúrgico para agarrar o manipular de otro modo tejido.
El documento US2002/062136 describe un conjunto de empuñadura y un instrumento quirúrgico en los que se basa el preámbulo de las reivindicaciones independientes.
Compendio
Por consiguiente, los aspectos de la invención son tal como se definen en las reivindicaciones independientes, exponiéndose características preferentes en las reivindicaciones subordinadas. La presente descripción se refiere a un conjunto de empuñadura para su utilización con un instrumento quirúrgico que tiene una parte de extremo distal operativa. El conjunto de empuñadura incluye un alojamiento, un elemento de accionamiento, un actuador, un brazo de muelle y un elemento de tensión previa. El elemento de accionamiento está dispuesto al menos parcialmente dentro del alojamiento y se puede trasladar longitudinalmente con respecto al alojamiento. El actuador está dispuesto en cooperación mecánica con el alojamiento. El brazo de muelle está dispuesto al menos parcialmente dentro del alojamiento. Descripción detallada de realizaciones
Ahora se describirá detalladamente una realización del conjunto de empuñadura y el instrumento quirúrgico actualmente descritos, con referencia a los dibujos, en los que los números iguales designan elementos idénticos o correspondientes en cada una de las diversas vistas. Como es habitual en la técnica, el término "proximal" se refiere a la parte o al componente más cercanos al usuario u operador, por ejemplo el cirujano o médico, mientras que el término "distal" se refiere a la parte o al componente más alejados del usuario.
Haciendo referencia inicialmente a las FIGURAS 1 y 1A, en ellas se muestra un instrumento quirúrgico 500 que incluye un conjunto 100 de empuñadura, una parte 400 alargada o endoscópica y un efector 450 de extremo. El conjunto 100 de empuñadura es particularmente adecuado para utilizarlo en instrumentos quirúrgicos que incorporan efectores de extremo, tales como instrumentos de aplicación de clips o de grapas, y para utilizarlo en instrumentos de sellado de vasos que utilizan energía electroquirúrgica para sellar tejido. Tal como se ilustra, la parte 400 endoscópica se extiende en dirección distal desde el conjunto 100 de empuñadura, y el efector 450 de extremo está dispuesto en posición adyacente a un extremo distal de la parte 400 endoscópica. El efector 450 de extremo incluye un primer miembro 460 de mordaza y un segundo miembro 470 de mordaza, que se pueden mover (por ejemplo pivotar) uno con respecto al otro entre una posición abierta (FIGURA 1) y una posición aproximada (FIGURA 1A) para sujetar tejido entre los mismos, por ejemplo.
Con referencia específica a las Figuras 2-8, el conjunto 100 de empuñadura incluye en general un alojamiento 120 de empuñadura que tiene un elemento 140 de accionamiento alargado montado para un movimiento longitudinal dentro del alojamiento 120 de empuñadura. El conjunto 100 de empuñadura incluye un actuador o disparador o empuñadura 160 móvil, que está montado de forma móvil (por ejemplo pivotante) con respecto al alojamiento 120 de empuñadura. Al menos una actuación parcial de la empuñadura 160 móvil provoca un movimiento longitudinal del elemento 140 de accionamiento con respecto al alojamiento 120 de empuñadura.
En la realización ilustrada, el alojamiento 120 de empuñadura también incluye una parte 180 de morro con cojinete para el soporte giratorio de la parte de extremo distal de un instrumento quirúrgico. Esto permite girar efectores 450 de extremo o unidades de carga asociados con la parte de extremo distal del instrumento quirúrgico con respecto al conjunto 100 de empuñadura. En la patente de Ee.UU. n° 5,752,644 de Bolanos et al., la patente de EE.UU. n° 5,762,256 de Mastri et al. y la patente de EE.UU. n° 5,865,361 de Milliman et al. se describen ejemplos de unidades de carga para su utilización con un instrumento quirúrgico.
Para mover el elemento 140 de accionamiento, el conjunto 100 de empuñadura incluye un conjunto 200 de accionamiento posicionado entre el elemento 140 de accionamiento y la empuñadura 160 móvil. El conjunto 200 de accionamiento transfiere el movimiento aplicado a la empuñadura 160 móvil por la mano de un operador al elemento 140 de accionamiento para trasladar el elemento 140 de accionamiento longitudinalmente y de este modo actuar un efector 450 de extremo asociado. La actuación del efector de extremo puede incluir el cierre de las mordazas sobre tejido, el disparo de grapas o fijaciones, y/o la realización de otras funciones. El conjunto 200 de accionamiento incluye un soporte 220 y superior un trinquete 240 de accionamiento para mover el elemento 140 de accionamiento dentro del alojamiento 120 de empuñadura en respuesta de una actuación al menos parcial de la empuñadura 160 móvil.
Específicamente, el trinquete 240 de accionamiento está provisto de un pico 260 distal que está configurado para acoplarse con el elemento 140 de accionamiento. El pico 260 se puede acoplar con los dientes 280 formados sobre el elemento 140 de accionamiento o, alternativamente, se puede acoplar con una ranura en el elemento 140 de accionamiento. Los dientes 280 están orientados de tal modo que el pico 260 distal se puede acoplar con los dientes 280 y mover el elemento 140 de accionamiento en dirección distal cuando el trinquete 240 de accionamiento se mueve en la dirección distal, y se puede desacoplar del elemento 140 de accionamiento cuando el trinquete 240 de accionamiento es atraído en una dirección proximal en relación con el elemento 140 de accionamiento.

Adicionalmente, el trinquete 240 de accionamiento es sometido a una tensión previa a través del elemento 242 de tensión previa en la dirección general de la flecha "A" (véase la FIGURA 2).
Está previsto que la actuación de la empuñadura 160 móvil aproxime los miembros 460, 470 de mordaza del efector 450 de extremo, y que una continuación de la actuación de la empuñadura 160 móvil (o actuaciones sucesivas de la empuñadura 160 móvil) provoque el disparo de fijaciones desde uno de los miembros 460 de mordaza, por ejemplo. En la patente de EE.UU. n° 6,953,139 de Milliman et al. se explican otros detalles sobre el modo en que la actuación de la empuñadura 160 móvil provoca el avance distal del elemento 140 de accionamiento y la actuación del efector 450 de extremo.
Con referencia adicional a las Figuras 2-8, el conjunto 100 de empuñadura también incluye un elemento 190 de tensión previa y un brazo de muelle o brazo 300 de muelle de actuador. Una primera parte 192 del elemento 190 de tensión previa está acoplada mecánicamente con una parte del actuador 160, y una segunda parte 194 del elemento 190 de tensión previa está acoplada mecánicamente con un brazo 300 de muelle. El elemento 190 de tensión previa aplica una tensión previa sobre una parte 162 de agarre del actuador 160 en dirección distal (por ejemplo, hacia una posición preactuada o posición abierta; véase la FIGURA 2).
El brazo 300 de muelle, tal como se muestra en la FIGURA 8, por ejemplo, está dispuesto dentro del alojamiento 120 de empuñadura, y se puede montar en el alojamiento 120 de tal modo que sea giratorio con respecto al alojamiento 120 de empuñadura. Específicamente, el brazo 300 de muelle incluye una abertura 310 que se encaja alrededor de un buje 122 en el alojamiento de empuñadura. Adicionalmente, el brazo 300 de muelle incluye una abertura 320 configurada para recibir la segunda parte 194 del elemento 190 de tensión previa a través de la misma. Con referencia específica a las Figuras 2 y 3, cuando el actuador 160 está en su posición preactuada, una parte 330 de cola del brazo 300 de muelle está en contacto con una parte 142 inferior del elemento 140 de accionamiento. Aquí está previsto que la parte 330 de cola ejerza una fuerza hacia arriba (por ejemplo, en la dirección general de la flecha "C" en la FIGURA 3) contra el elemento 140 de accionamiento. Dicha fuerza hacia arriba puede ayudar a estabilizar la ubicación longitudinal del elemento 140 de accionamiento. Por ejemplo, la fuerza hacia arriba sobre el elemento 140 de accionamiento proporcionada por la parte 330 de cola del brazo 300 de muelle puede ayudar a mantener los miembros 460, 470 de mordaza en una posición aproximada o parcialmente aproximada cuando los miembros 460, 470 de mordaza están siendo utilizados como una pinza. Como se puede apreciar, en instrumentos quirúrgicos sin el brazo 300 de muelle descrito, el elemento 190 de tensión previa empuja la empuñadura 160 móvil hacia su posición abierta, y por lo tanto puede empujar los miembros 460, 470 de mordaza hacia su posición abierta. El elemento de tensión previa puede ser cualquier muelle de tensión u otro miembro resiliente.
Con referencia específica a las Figuras 4 y 5, en el conjunto 100 de empuñadura mostrado la empuñadura 160 móvil ha sido actuada hacia una empuñadura 170 estacionaria. Tal como se describe más arriba, y como se muestra al comparar las Figuras 2 y 4, la actuación de la empuñadura 160 móvil ha provocado una traslación distal del elemento 140 de accionamiento. En esta posición, el brazo 300 de muelle ha girado en la dirección general de la flecha "B" (FIGURA 3), de tal modo que la parte 330 de cola del brazo 300 de muelle está situada en dirección proximal junto a un extremo 144 proximal del elemento 140 de accionamiento. (Antes de la traslación distal del elemento 140 de accionamiento, el propio elemento 140 de accionamiento impedía la rotación del brazo 300 de muelle, tal como se puede ver en la FIGURA 3.) En esta posición, la parte 330 de cola del brazo 300 de muelle está aplicando una tensión previa sobre el elemento 140 de accionamiento en dirección distal, por ejemplo para ayudar a que los miembros 460, 470 de mordaza y/o la empuñadura 160 móvil permanezcan en una posición deseada. El brazo de muelle se acopla con el elemento de accionamiento para mantener las mordazas del instrumento quirúrgico en una posición pinzada sobre tejido, por ejemplo. Está previsto que la geometría específica del brazo 300 de muelle y/o la constante de muelle del elemento 190 de tensión previa puedan ser diseñadas para contrarrestar al menos algunas de las fuerzas (por ejemplo, de tejido grueso o del propio elemento 190 de tensión previa) que pueden hacer que los miembros 460, 470 de mordaza se muevan hacia una posición abierta, o para proporcionar fuerzas en dirección distal suficientes para ayudar a mantener los miembros 460, 470 de mordaza en una posición aproximada.
Con referencia ahora a las Figuras 6 y 7, en el conjunto 100 de empuñadura mostrado la empuñadura 160 móvil ha sido actuada al menos una vez (tal como se muestra en la FIGURA 4) y después ha sido movida de vuelta hacia su posición abierta. Aquí, el brazo 300 de muelle no está en contacto con el elemento 140 de accionamiento y, por lo tanto, el brazo 300 de muelle no ejerce ninguna fuerza sobre el elemento 140 de accionamiento en esta posición. Un tope 124 (FIGURA 7) del alojamiento 120 de empuñadura está posicionado para limitar la rotación del brazo 300 de muelle. El brazo 300 de muelle puede incluir una superficie 331 para acoplarla con el tope 124.
Como se puede apreciar, cuando el instrumento 100 quirúrgico está configurado para ser utilizado múltiples veces, una traslación proximal suficiente del elemento 140 de accionamiento hace que el elemento 140 de accionamiento entre en contacto con el brazo 300 de muelle, lo que provoca una rotación del brazo 300 de muelle en la dirección general de la flecha "D" en la FIGURA 6, contra la tensión previa del elemento 190 de tensión previa.
Adicionalmente está previsto que el instrumento 500 quirúrgico pueda ser alimentado por una fuente de energía y/o
motor. En la Publicación de Patente de EE.UU. n° 2008/0255607 se incluyen detalles adicionales de un instrumento quirúrgico con motor de este tipo.
Adicionalmente, aunque las figuras representan un instrumento quirúrgico de aplicación de fijaciones lineales, la presente descripción abarca otros tipos de instrumentos quirúrgicos endoscópicos y éstos pueden ser utilizados con 5 el conjunto 100 de empuñadura descrito y con el brazo 300 de muelle y el elemento 190 de tensión previa descritos. En la Publicación de Patente de EE.UU. n° 2010/0179540 de Marczyk et al., y en la Solicitud de Patente de EE.UU. n° de Serie 12/718,143 de Marczyk et al., y en la Patente de EE.UU. n° 7,988,028, del mismo cesionario, se describen detalles adicionales de fórceps endoscópicos. En la Publicación de Patente de EE.UU. n° 2009/0173767 de Milliman et al., del mismo cesionario, se describen detalles adicionales de un instrumento quirúrgico de aplicación 10 de fijadores circulares.
Se entenderá que es posible realizar diversas modificaciones a la realización aquí descrita. Por ejemplo, los muelles helicoidales ilustrados pueden ser sustituidos por diferentes tipos de elementos de tensión previa para aplicar una tensión previa sobre el brazo 300 de muelle descrito e introducir el mismo en el elemento 140 de accionamiento. Por lo tanto, la descripción precedente no ha de ser interpretada como limitativa, sino simplemente como 15 ejemplificaciones de una realización particular. Los expertos en la técnica imaginarán otras modificaciones dentro del alcance de las reivindicaciones adjuntas.
Claims (11)
- 510152025303540REIVINDICACIONES1. Un conjunto (100) de empuñadura para su utilización con un instrumento (500) quirúrgico que tiene una parte (450) de extremo distal operativa, comprendiendo el conjunto de empuñadura:un alojamiento (120);un elemento (140) de accionamiento que está dispuesto al menos parcialmente dentro del alojamiento y que se puede trasladar longitudinalmente con respecto al alojamiento;un actuador (160) dispuesto en cooperación mecánica con el alojamiento;un brazo (300) de muelle dispuesto al menos parcialmente dentro del alojamiento; y caracterizado porun elemento (190) de tensión previa para aplicar una tensión previa sobre el actuador, una primera parte del elemento de tensión previa dispuesta en cooperación mecánica con el actuador y una segunda parte del elemento de tensión previa dispuesta en cooperación mecánica con el brazo de muelle, en donde al menos una actuación parcial del actuador traslada el elemento de accionamiento en dirección distal desde una posición proximal, en la que una parte del elemento de accionamiento está en contacto con el brazo de muelle, hasta una posición distal, en la que el elemento de accionamiento no está en contacto con el brazo de muelle.
- 2. El conjunto de empuñadura de la reivindicación 1, en donde una superficie (142) inferior del elemento de accionamiento incluye múltiples dientes (280), y en donde una parte del brazo de muelle está en contacto con la superficie inferior del elemento de accionamiento cuando el elemento de accionamiento está en la posición proximal.
- 3. El conjunto de empuñadura de la reivindicación 1 o la reivindicación 2, en donde una parte del brazo de muelle está en contacto con una superficie (144) del elemento de accionamiento orientada en dirección proximal, cuando el elemento de accionamiento está entre la posición proximal y la posición distal.
- 4. El conjunto de empuñadura de cualquiera de las reivindicaciones precedentes, en donde el brazo de muelle ejerce una fuerza distal sobre el elemento de accionamiento cuando el elemento de accionamiento está entre la posición proximal y la posición distal.
- 5. El conjunto de empuñadura de cualquiera de las reivindicaciones precedentes, en donde el brazo de muelle se puede mover con respecto al alojamiento.
- 6. El conjunto de empuñadura de la reivindicación 5, en donde el brazo de muelle es giratorio con respecto al alojamiento.
- 7. El conjunto de empuñadura de la reivindicación 6, en donde una cantidad predeterminada de rotación del brazo de muelle con respecto al alojamiento provoca un acoplamiento de una parte (331) del brazo de muelle con una parte (124) del alojamiento de la empuñadura, lo que evita sustancialmente que continúe la rotación del brazo de muelle con respecto al alojamiento.
- 8. El conjunto de empuñadura de cualquiera de las reivindicaciones precedentes, en donde la segunda parte del elemento de tensión previa se extiende a través de una abertura (320) en el brazo de muelle.
- 9. Un instrumento (500) quirúrgico que comprende:el conjunto de empuñadura de cualquiera de las reivindicaciones precedentes;una parte (400) endoscópica que se extiende en dirección distal desde el conjunto de empuñadura; yun efector de extremo (450) que está dispuesto en posición adyacente a un extremo distal de la parte endoscópica y que incluye un primer miembro (460) de mordaza y un segundo miembro (470) de mordaza, pudiendo moverse el primer miembro de mordaza con respecto al segundo miembro de mordaza desde una posición abierta hasta una posición aproximada.
- 10. El instrumento quirúrgico de la reivindicación 9, en donde el brazo de muelle está en contacto con el elemento de accionamiento cuando los miembros de mordaza están en la posición abierta.
- 11. El instrumento quirúrgico de la reivindicación 9 o 10, en donde el brazo de muelle no está en contacto con el elemento de accionamiento cuando los miembros de mordaza están en la posición aproximada.
