CAMPO DE LA INVENCIÓNFIELD OF INVENTION

La presente invención se enmarca en el campo de la Medicina, concretamente en el estudio y tratamiento de enfermedades autoinmunes, como la enfermedad de Crohn. La invención se centra en el pronóstico individualizado de la efectividad del tratamiento con los inhibidores del factor de necrosis tumoral alfa (anti-TNF-a) aplicado en dicha enfermedad. ESTADO DE LA TÉCNICA ANTERIORSTATE OF THE PRIOR ART

La enfermedad de Crohn (EC) es una enfermedad intestinal inflamatoria crónica (EII) que resulta de una respuesta inmunitaria anormal a los microbios entéricos en individuos genéticamente predispuestos, y puede afectar a cualquier localización del tracto gastrointestinal. Crohn's disease (CD) is a chronic inflammatory bowel disease (IBD) that results from an abnormal immune response to enteric microbes in genetically predisposed individuals, and can affect any location of the gastrointestinal tract.

La terapia clásica contra la EC consiste en distintas combinaciones de corticosteroides, agentes inmunosupresores y, en algunos casos, cirugía. Sin embargo, en los últimos años se han desarrollado nuevos fármacos biológicos. Entre ellos destacan los inhibidores del factor de necrosis tumoral alfa o anti-TNF-a, también llamados "anti-TNF-a” que corresponde a las siglas en inglés de "Tumoral Necrosis Factor alpha”. Algunos de estos compuestos, como infliximab, adalimumab y biosimilares, se han convertido en una modalidad terapéutica primordial en la EC [1,2]. Classical therapy for CD consists of various combinations of corticosteroids, immunosuppressive agents, and in some cases surgery. However, in recent years, new biological drugs have been developed. Among them, the most notable are tumor necrosis factor alpha inhibitors or anti-TNF-a, also called "anti-TNF-a" which corresponds to the acronym in English for "Tumor Necrosis Factor alpha". Some of these compounds, such as infliximab, adalimumab, and biosimilars, have become a primary therapeutic modality in CD [1,2].

El anti-TNF-a es una citocina ubicua cuyos niveles en sangre aumentan significativamente durante el proceso inflamatorio que tiene lugar en la EII. Los agentes anti-TNF-a se unen al TNF-a soluble y a su precursor transmembrana, bloqueando así la interacción entre el TNF-a y los receptores TNF tipo 1 y 2, y la subsiguiente señalización celular proinflamatoria. El uso de agentes anti-TNF favorece la cicatrización de la mucosa, evita la necesidad de corticoides y reduce la necesidad de cirugía y hospitalizaciones, mejorando así la calidad de vida de los pacientes con EII [3]. Sin embargo, su eficacia en pacientes con EC es heterogénea. La falta de respuesta primaria al tratamiento anti-TNF-a se produce en hasta el 40% de los pacientes con EII en ensayos randomizados, y en el 10-20% de los pacientes en series reales. La pérdida de respuesta secundaria también es frecuente en pacientes con EC, con una incidencia que oscila entre el 23% y el 46% a los 12 meses del inicio del tratamiento anti-TNF-a [4,5]. Además, el uso crónico de inhibidores del TNF-a es caro y conlleva un riesgo significativo de infecciones, acontecimientos autoinmunitarios paradójicos, trastornos linfoproliferativos, enfermedades desmielinizantes y cardiopatías [2]. Anti-TNF-a is a ubiquitous cytokine whose blood levels increase significantly during the inflammatory process taking place in IBD. Anti-TNF-a agents bind to soluble TNF-a and its transmembrane precursor, thus blocking the interaction between TNF-a and TNF type 1 and 2 receptors, and the subsequent proinflammatory cell signaling. The use of anti-TNF agents promotes mucosal healing, avoids the need for corticosteroids, and reduces the need for surgery and hospitalizations, thus improving the quality of life of patients with IBD [3]. However, its efficacy in patients with CD is heterogeneous. Primary lack of response to anti-TNF-a treatment occurs in up to 40% of patients with IBD in randomized trials, and in 10-20% of patients in real-world series. Secondary loss of response is also common in patients with CD, with an incidence ranging from 23% to 46% at 12 months after initiation of anti-TNF-a therapy [4,5]. Furthermore, chronic use of TNF-a inhibitors is expensive and carries a significant risk of infections, paradoxical autoimmune events, lymphoproliferative disorders, demyelinating diseases, and heart disease [2].

Other studies have focused on mRNA abundance in peripheral blood and intestinal tissue, microbiota analysis or protein markers [6]. However, none of them were sufficiently accurate to be applied in routine clinical practice.

DESCRIPCIÓN DE LA INVENCIÓNDESCRIPTION OF THE INVENTION

La presente invención se centra en identificar biomarcadores fiables para predecirla respuesta el tratamiento con fármacos anti-TNF-aen pacientes con EC. The present invention focuses on identifying reliable biomarkers to predict the response to treatment with anti-TNF-a drugs in patients with CD.

Se clasificaron 113 pacientes con EC según su respuesta clínica a las 12 semanas de tratamiento con anti-TNF-a comoRemisión a Corto Plazo (RCP) o No Remisión a Corto Plazo (NRCP). Se compararon los perfiles de expresión proteica de las muestras de plasma en un subconjunto de pacientes de ambos grupos antes de la terapia anti-TNF-a mediante proteómica SWATH. One hundred and eleventy-three patients with CD were classified according to their clinical response after 12 weeks of anti-TNF-a treatment as Short-Term Remission (STR) or Short-Term Non-Remission (SNR). Protein expression profiles of plasma samples in a subset of patients from both groups were compared before anti-TNF-a therapy using SWATH proteomics.

De entre las 18 proteínas plasmáticas expresadas de forma diferencial anteriormente citadas destaca lavinculina (VICN), que resultó ser una de las proteínas más desreguladas (p<0,001), junto con ENOA. La expresión diferencial de vinculina fue posteriormente confirmada por ELISA (p=0,054). Se utilizó una fuente ESI NanoSpray III (Sciex) para la interfaz entre nLC y MS, con una aplicación de voltaje de 2.600 V. El modo de adquisición consistió en un barrido MS de sondeo de 250 ms de 350 a 1.250 m/z, seguido de un barrido MS/MS de 230 a 1.700 m/z (tiempo de adquisición de 60 ms, energía de colisión rodante) de los 65 iones precursores superiores del barrido de sondeo, para un tiempo de ciclo total de 4,2 s. La fragmentación de los iones precursores en el barrido de sondeo de 250 ms se realizó en un tiempo de ciclo de 4,2 s. El tiempo de ciclo total fue de 1,5 s. The choice of anti-TNF-a agent was at the discretion of the treating physician and drugs were prescribed according to the approved dosing regimen. The study protocol was carried out in accordance with the ethical principles contained in the Declaration of Helsinki and the research protocol was approved by the Biomedical Ethics Committee of Andalusia. All patients were informed and gave their written consent to participate. ;;Study population ;One hundred and thirteen adult patients with CD, naive to anti-TNF-a drugs and with an indication for anti-TNF-a therapy due to clinical, endoscopic or radiological activity, were recruited in three academic hospitals: Hospital Universitario Reina Sofía (Córdoba, Spain), Hospital Universitario de la Princesa (Madrid, Spain) and Hospital Universitario de Galdakano (Vizcaya, Spain). ;;Therapeutic response was assessed 12 weeks after initiation of anti-TNF-a drug, and patients were classified as having Short-Term Remission (STR) or No Short-Term Remission (NRTR). The primary response in the study was absence of clinical response, as assessed by the Crohn's Disease Activity Index (CDAI) in luminal disease and by the Fistula Drainage Assessment (FDA) in perianal disease. Criteria for absence of response were CDAI>150; endoscopic or radiological activity; purulent drainage upon gentle finger compression; new fistula; dose increase; change to a different drug; addition of other immunosuppressants; surgery or endoscopic dilatation. ;;Patients with Short-Term Remission (STR) were further stratified into patients with sustained remission (SR) and patients with loss of response (PR) after 12 months of treatment. ;;Plasma samples ;;Plasma samples were collected within 2 days prior to the first dose of anti-TNF-a agent into 9 mL EDTA-K3 tubes (K3E VACUETTE®, Greiner Bio-One, Kremsmünster, Austria). All samples were centrifuged within 1 hour after collection at 3,000 rpm and 4°C for 10 min. Plasma was recovered immediately and stored at -80°C until analysis. ;;Sample preparation for analysis Liquid chromatography-mass spectrometry (LC-MS) ;;Proteomic analysis was performed in a subset of 20 randomly selected patients classified according to the primary outcome of the study as either RCP patients (n=16; 8 RS and 8 PR) or NRCP patients (n=4). ;;The 14 most abundant proteins in blood were depleted using the Hu-14 Multiple Affinity Removal System kit (Agilent Technologies, Wilmington, DE, USA) following the manufacturer's instructions. Non-depleted proteins were concentrated using 5,000 molecular weight cut-off centrifugation concentrators (Agilent Technologies). Samples were then cleaned to remove any contaminants by TCA/acetone protein precipitation and solubilized in 50 µl of 0.2% RapiGest SF (Waters, Mil-ford, MA, USA) in 50 mM ammonium bicarbonate. Total protein content was measured using the Qubit Protein Assay kit (Thermo Fisher Scientific, Waltham, MA, USA) and 50 µg of proteins were digested with trypsin following a protocol adapted from Vowincke et al. Briefly, protein samples were incubated with 5 mM dithiothreitol at 60°C for 30 min and then with 10 mM iodoacetamide at room temperature in the dark for 30 min. Modified sequencing grade trypsin (Promega, Madison, WI, USA) was added at a ratio of 1:40 trypsin:protein and incubated at 37°C for 2 h; the same amount of trypsin was added again and incubated at 37°C for another 15 h. RapiGest was then precipitated by centrifugation after incubation with 0.5% trifluoroacetic acid at 37°C for 1 h. The final volume was adjusted with milli-Q water and acetonitrile to a final concentration of 0.5 ^g peptide/^L, 2.25% acetonitrile, and 0.2% trifluoroacetic acid. ;;Spectral library creation ;;To create the MS/MS spectral libraries, the peptide solutions were analyzed by a shotgun data-dependent acquisition (DDA) approach using nano-LC-MS/MS. To obtain a good representation of the peptides and proteins present in the 20 samples, pooled vials, consisting of equivalent mixtures of the original samples, were prepared. One ^g of each pooled sample (2 ^L) was separated on an Ekspert nLC415 nano-LC system (Eksigent, Dublin, CA, USA) using an Acclaim PepMap C18 column (75 ^m x 25 cm, 3 ^m, 100 Å) (Thermo Fisher Scientific) at a flow rate of 300 nL/min. Water and acetonitrile, both containing 0.1% formic acid, respectively, were used as solvents A and B. The gradient was from 5% to 30% B over 120 min, 10 min to 90% B, and finally 20 min to equilibrate the column at 5% B, for a total time of 150 min. ;;Once the peptides were eluted, they were directly injected into a Triple TOF 5600+ hybrid quadrupole-TOF mass spectrometer (Sciex, Redwood City, CA, USA), operated with a "top 65" data-dependent acquisition system in positive ion mode. A NanoSpray III ESI source (Sciex) was used for the interface between nLC and MS, with an applied voltage of 2,600 V. The acquisition mode consisted of a 250 ms probing MS scan from 350 to 1,250 m/z, followed by a MS/MS scan from 230 to 1,700 m/z (60 ms acquisition time, rolling collision energy) of the top 65 precursor ions from the probing scan, for a total cycle time of 4.2 s. Fragmentation of precursor ions in the 250 ms probing scan was performed in a cycle time of 4.2 s. The total cycle time was 1.5 s. Fragmented precursors were then added to a dynamic exclusion list for 15 s. All singly charged ions were excluded from MS/MS analysis. ;;Peptide and protein identifications were performed using Protein Pilot software (version 5.0.1, Sciex) with a Swiss-Prot concatenated targetreverse decoy human database (downloaded March 2016), containing 20,200 target protein sequences, specifying iodoacetamide as Cys alkylation. The false discovery rate (FDR) was set at 0.01 for both peptides and proteins. The MS/MS spectra of the identified peptides were then used to generate the spectral library for SWATH peak extraction using the MS/MSALL with SWATH Acquisition MicroApp (version 2.0, Sciex) plug-in for PeakView software (version 2.1, Sciex). Peptides with a confidence score greater than 99% (obtained from searching the Protein Pilot database) were included in the spectral library. ;;Relative quantitation by SWATH acquisition ;;Individual samples were analyzed using a Data Independent Acquisition (DIA) method. Each sample (2 ^L) was analyzed using the LC-MS equipment and LC gradient described above to construct the spectral library instead of using the SWATH-MS acquisition method. The method consisted of repeating a run with 60 TOF MS/MS scans (300 to 1600 m/z, high sensitivity mode, 90 ms acquisition time) of sequential overlapping precursor isolation windows of varying width (1 m/z overlap) covering the mass range 350 to 1250 m/z with a previous TOF MS scan (350 to 1250 m/z, 50 ms acquisition time) for each run. The total run time was 5.5 s. The width of the 60 variable windows was optimized based on the ion density found in previous DDA analyses using a Sciex SWATH variable window spreadsheet. ;;SWATH data analysis;;Selective data extraction from fragment ion chromatogram traces of SWATH series was performed with PeakView (version 2.1) using the MS/MSALL microapplication with SWATH Acquisition (version 2.0). This application processed the data using the spectral library created from the shotgun data. Up to 10 peptides per protein and 12 fragments per peptide were selected based on signal intensity; shared and modified peptides were excluded from processing. Ten-minute windows and 20 ppm widths were used to extract ion chromatograms; SWATH quantitation was attempted for all proteins in the ion library that were identified by ProteinPilot with an FDR less than 1%. The retention times of the peptides that were selected for each protein were realigned in each run according to 118 endogenous Apolipoprotein B-100 peptides eluted along the entire time axis. Extracted ion chromatograms were then generated for each selected fragment ion; the peak areas of the peptides were obtained by summing the peak areas of the corresponding fragment ions. PeakView calculated an FDR and score for each assigned peptide based on the chromatographic and spectral components; only peptides with an FDR less than 1% were used for protein quantification. Protein quantification was calculated by summing the peak areas of the corresponding peptides. MarkerView (version 1.2.1, Sciex) was used for signal normalization; Differential abundance was checked by applying the T-test at the protein level, using SPSS version 25.0 (IBM, Chicago, IL, USA). ;;Functional pathway analysis ;;The functional pathways of the identified candidate biomarkers were analyzed using DAVID Bioinformatics Resources 6.7. ;;Enzyme-linked immunosorbent assay ;;The Enzyme-Linked Immunosorbent Assay (ELISA) was used to quantify the protein concentration of human alpha-enolase (ENOA) (Abnova, Taipei, Taiwan) and vinculin (VINC) (Antibodies-online, Aachen, Germany) in plasma samples from 113 patients (97 RCP and 16 NRCP). The assay was performed following the manufacturer's instructions and the data obtained were used to validate the proteomics results. ;;Statistical analysis ;;Categorical variables were presented in frequency tables, while continuous variables were expressed as mean and standard deviation or as median and interquartile range (IQR) for those with an asymmetric distribution. The Kolmogorov-Smirnov test was used to check the normality of the distribution. Los pacientes más jóvenes (NRCP: 53,5 (43,0-59,3) años frente a RCP: 39,0 (27,5-50,0) años, p=0,005) y con un diagnóstico más reciente de EC (NRCP: 22,0 (2,5-26,0) frente a RCP: 4,0 (1,0-13,0), p=0,021), tenían más probabilidades de presentar una RCP a los fármacos anti-TNF-a. Se cuantificaron 300 proteínas plasmáticas en el análisis proteómico. Como biomarcadores potenciales de la respuesta primaria identificamos 18 proteínas expresadas diferencialmente (p<0,009 y cambio de pliegue >2,4): ENOA, VINC, PDZ y LIM dominio proteína 1 (PDLI1), Rho GDP-inhibidor de la disociación 2 (GDIR2), Zyxin (ZYX), Fructosa-bisfosfato aldolasa A (ALDOA), Moesin (MOES), Glutatión S-transferasa omega-1 (GSTO-1), Al-pha-actinina-1 (ACTN1), Transgelina-2 (TAGL2), Proteína 2 asociada a la proteína de respuesta a la privación de suero/Caveolae (SDPR), Inhibidor beta de la disociación de Rab GDP (GDIB), Cofilina-1 (COF1), Proteína S100-A4 (S10A4), Proteína supresora de Ras 1 (RSU1), Pleckstrin (PLEK), Proteína 14-3-3 zeta/delta (1433Z) y Triosafosfato isomerasa (TPIS) (Tabla 2). 300 plasma proteins were quantified in the proteomic analysis. As potential biomarkers of primary response we identified 18 differentially expressed proteins (p<0.009 and fold change >2.4): ENOA, VINC, PDZ and LIM domain protein 1 (PDLI1), Rho GDP-dissociation inhibitor 2 (GDIR2), Zyxin (ZYX), Fructose-bisphosphate aldolase A (ALDOA), Moesin (MOES), Glutathione S-transferase omega-1 (GSTO-1), Al-pha-actinin-1 (ACTN1), Transgelin-2 (TAGL2), Serum deprivation response protein/Caveolae-associated protein 2 (SDPR), Rab GDP-dissociation inhibitor beta (GDIB), Cofilin-1 (COF1), Protein S100-A4 (S10A4), Ras suppressor protein 1 (RSU1), Pleckstrin (PLEK), Protein 14-3-3 zeta/delta (1433Z) and Triosephosphate isomerase (TPIS) (Table 2).

El análisis de las vías funcionales mostró que 17 de estas 18 proteínas identificadas están reguladas por la acetilación. Functional pathway analysis showed that 17 of these 18 identified proteins are regulated by acetylation. Furthermore, 14 of these potential anti-TNF-a response biomarkers are involved in platelet hemostasis/function (VINC, ALDOA, MOES, ACTN1, TAGL2, SDPR, COF1, and PLEK) and/or are involved in cytoskeletal organization and/or cell adhesion (VINC, PDLI1, GDIR2, ZYX, ALDOA, MOES, ACTN1, TAGL2, SDPR, COF1, S10A4, RSU1, PLEK, and 1433Z), 3 of them are related to the inflammatory response (GSTO1, GDIB, and S10A4), and 3 proteins are involved in the glycolytic process (ENOA, ALDOA, and TPIS).

Validación de ENOA y VINC como marcadores potenciales de remisión a corto plazo Validation of ENOA and VINC as potential markers of short-term remission

ENOA y VINC fueron seleccionadas para realizar la validación de los resultados proteómicos mediante pruebas ELISA, debido a su mayor nivel de significación (Tabla 2). ENOA and VINC were selected to perform the validation of proteomic results using ELISA tests, due to their higher level of significance (Table 2).

Según ELISA, ambas proteínas mostraron un aumento no significativo en las muestras de plasma de los pacientes con RCP(Figura 1 A y B). Sin embargo, en el caso de VINC, la expresión diferencial rozó la significación estadística (NRCP: 0,8 (0,2-1,2) vs. RCP: 1,2 (0,6 2,0), p=0,054) (Figura 1B). According to ELISA, both proteins showed a non-significant increase in plasma samples from patients with CPR (Figure 1 A and B). However, in the case of VINC, the differential expression bordered on statistical significance (NRCP: 0.8 (0.2-1.2) vs. CPR: 1.2 (0.6-2.0), p=0.054) (Figure 1B).

Posteriormente, los pacientes del grupo RCP se estratificaron en pacientes con RS (Respuesta Sostenida) y pacientes con PR (Pérdida de Respuesta) tras 12 meses de tratamiento. En la comparativa de los valores obtenidos, encontramos diferencias significativas en los niveles plasmáticos de VINC entre los grupos de RS (1.3 (0.8-2.1)) y PR (0.7 (0.2-1.8)) (p=0.019), y entre RS y NRCP (0.8 (0.2-1.2)) (p=0.014) (Figura 2A). Estas diferencias concordaban con los resultados anteriores obtenidos en el análisis proteómico para VINC solo para los grupos RS (492127.1 ± 219002.2) y NRCP (106569.3 ± 65803.9) (p=0.001) (Figura 2B). Subsequently, patients in the RCP group were stratified into patients with RS (Sustained Response) and patients with PR (Loss of Response) after 12 months of treatment. In the comparison of the values obtained, we found significant differences in the plasma levels of VINC between the RS (1.3 (0.8-2.1)) and PR (0.7 (0.2-1.8)) groups (p=0.019), and between RS and NRCP (0.8 (0.2-1.2)) (p=0.014) (Figure 2A). These differences were in agreement with the previous results obtained in the proteomic analysis for VINC only for the RS (492127.1 ± 219002.2) and NRCP (106569.3 ± 65803.9) groups (p=0.001) (Figure 2B).

A continuación, se evaluó el valor pronóstico de los parámetros clínicos y patológicos para predecir la respuesta del paciente al tratamiento anti-TNF-a. Edad [RM: 1,1 (1,0-1,1), p=0,009)], duración de la enfermedad [RM: 1,1 (1,0-1,2), p=0,006], inducción con corticosteroides [RM: 5,1 (1,7-15,5), p=0. 004] y principalmente, la resección intestinal [RM: 6,4 (2,1-19,8), p=0,001] y la puntuación CDAI basal [RM: 1,0 (1,0-1,0), p=0,000], fueron factores independientes de NRCP (Tabla 1). The prognostic value of clinical and pathological parameters in predicting patient response to anti-TNF-a therapy was then evaluated. Age [OR: 1.1 (1.0-1.1), p=0.009)], disease duration [OR: 1.1 (1.0-1.2), p=0.006], corticosteroid induction [OR: 5.1 (1.7-15.5), p=0. 004] and mainly, bowel resection [OR: 6.4 (2.1-19.8), p=0.001] and baseline CDAI score [OR: 1.0 (1.0-1.0), p=0.000], were independent factors of NRCP (Table 1).

In multivariate analysis, corticosteroid induction [OR: 8.6 (1.7-43.5), p=0.009], bowel resection [OR: 10.5 (2.1-52.0), p=0.004] and baseline CDAI score [OR: 1.0 (1.0-1.0), p=0.003] were predictors of NRCP. Inclusion of baseline LVNC levels in the analysis increased the predictive ability of the model (Table 3).

Se obtuvo una muy buena capacidad discriminante del modelo ajustado que incluye la inducción con corticosteroides, la resección intestinal y la puntuación basal del CDAI, sin los niveles basales de VINC [AUC (IC 95%) = 0,904 (0,838-0,970)]. Los resultados fueron e incluso mejores incluyendo los niveles basales de [AUC (IC 95%) = 0,919 (0,862-0,977)] (Tabla 4yFigura 3). A very good discriminant capacity was obtained from the adjusted model including corticosteroid induction, bowel resection and baseline CDAI score, without baseline LVNC levels [AUC (95% CI) = 0.904 (0.838-0.970)]. The results were even better including baseline levels of [AUC (95% CI) = 0.919 (0.862-0.977)] (Table 4 and Figure 3).

Valoración del pronóstico de pacientes con EC sometidos a terapia anti-TNF-a a largo plazo Assessment of the prognosis of patients with CD undergoing long-term anti-TNF-a therapy

Una vez trascurridos 12 meses desde el inicio del tratamiento con anti-TNF-a se rehacen los grupos con el fin de comparar el grupo de RS (respuesta sostenida a los 12 meses, n=75) frente al nuevo grupo formado por PR (pérdida de respuesta a los 12 meses) y NRCP (no respuesta a corto plazo) (n=38). After 12 months from the start of treatment with anti-TNF-a, the groups were re-established in order to compare the RS group (sustained response at 12 months, n=75) against the new group consisting of PR (loss of response at 12 months) and NRCP (no response in the short term) (n=38).

Se detecta que existen diferencias significativas entre ambos grupos tanto para VINC (p=0,002) como para ENOA (p=0,031) (Tabla 5), cuando en la respuesta a corto plazo, solo VINC rozaba la significación estadística (Figura 1B). Significant differences were detected between both groups for both VINC (p=0.002) and ENOA (p=0.031) (Table 5), whereas in the short-term response, only VINC was close to statistical significance (Figure 1B).

En el modelo univariante, tanto la resección intestinal (2,6 (1,1 -6,2), p=0,029) como el CDAI basal (1,0 (1,0-1,0), p=0,011) y los niveles de expresión de VINC basales (0,7 (0,5-1,0), p=0,038), serían predictores de respuesta por sí solos, a diferencia del modelo anterior donde VINC no era predictor por si solo, pero sí la Inducción con corticoides, que ahora no lo es (1,2 (0,5-3,2), p=0,651). In the univariate model, both intestinal resection (2.6 (1.1-6.2), p=0.029) and baseline CDAI (1.0 (1.0-1.0), p=0.011) and baseline VINC expression levels (0.7 (0.5-1.0), p=0.038) would be predictors of response on their own, unlike the previous model where VINC was not a predictor on its own, but corticosteroid induction was, which is not now (1.2 (0.5-3.2), p=0.651).

The discriminant capacity of the adjusted model including bowel resection, baseline CDAI score and baseline VINC expression levels was statistically significant although discrete [AUC (95% CI) = 0.696 (0.592-0.800)] (Table 6).
