Description

La présente invention a pour objet une prothèse de genou hémi-compartirnentale à glissement avec plateau tibial blindé et elle se rapporte plus particulièrement à la protection du blindage du plateau tibial.
Lorsque lton est amené en chirurgie osseuse à remplacer l'articulation naturelle du genou par une prothèse calquant anatomiquement la nature, il est important que sa résection soit minimale et conserve tous les ligaments.
Il est également nécessaire de permettre une angu laiton de plus de 100 et d'obtenir la rotation naturelle.
Ces interventions chirurgicales sont effectuées à la suite d'arthrite, d'arthrite rhumastismale,d'arthrose,de lésion polyarthritique,de genou instable et douloureux mais également à la suite de traumatismes particulièrement chez les grands sportifs. Par conséquent, il ess indispensable que ces prothèses permettent la correction des valgus ou varus anormaux par l'implantation de plaseaux de différentes épaisseurs mais en utilisant une quantité minimale de ciment.
De plus, cette prothèse doit permettre d'obtenir des surfaces parfaitement lisses supprimant ainsi tout risque de rupture.
Finalement, elle autorise sans problème la réimplan talion d'autres prothèses.
Conformément à l'invention, l'articulation du genou, devant etre constituée, comporte une prothèse définitive condylienne, un plateau de blindage et un élément tibial en général réalisé en une matière synthétique assemblable telle que le polyéthylène de très haute densité (RCH 1000) combinant un repérage radiographique, arsicu- laiton dans laquelle le plateau de blindage destiné à maintenir un élément tibial en matière appropriée acceptable, par exemple en polyéthylène de très haute densité, muni d'un organe de repérage radiographiqueet est caractérisée en ce que le plateau ou coupelle de blindage présente une forme en plan aux deux tiers circulaire dont le rebord périphérique a une hauteur variable et dont le fond comporce, d'une part, des stries coopérant avec le ciment orthopédique ei, d'autre part, une fenêtre longitudinale destinée à recevoir les picots d'accrochage adaptés pour accrocher l'élément tibial à la coupelle de blindage et à ancrer cet élément tibial dans la partie supérieure du tibia.
Diverses autres caractéristiques de 1 'invention ressortent d'ailleurs de la description détaillée qui suit.
Une forme de réalisation de l'objet de l'invention est représentée, à titre d'exemple non limitatif, au dessin annexé.
La fig. 1 est une vue en perspective partielle d'un genou muni d'une prothèse.

La fig. 5 est une vue en perspective du blindage des éléments, côsé fémur, vue de dessus, pour coopérer avec la prothèse condylienne.
La fig. 6 est une vue en perspective arrière du blindage maintenant l'élément tibial.
A la fig. l,-on a représenté partiellement un fémur 1 muni de sa prothèse condylienne 2. De plus, le tibia 3, après avoir été entaillé convenablement, a reçu le plateau de blindage 4 contenant intérieurement une prothèse tibiale 5 en matière synthétique.Bien entendu, après la miseen place de ce blindage, l'intervention chirurgicale permet la remise en place des ligaments et des muscles de façon à reconstituer complètement l'articulation du genou en assurant au patient une annulation normale de plus de 100 avec rotation nacurelle.
Le plateau de blindage 4, tel que vu aux fig. 2 et 3, se compose d'une coupelle normalement en alliage d'acier (316 L austénitique) dont le fond 6 est lisse et est percé d'une fenêtre lo-ngitudinale 7 destinée à recevoir des picots d'accrochage 8, 9, 10 de grandeurs diverses mais adapté pour, d'une part, accrocher l'les ment tibial en matière plastique li et, d'autre part, servir d'ancrage dans la partie supérieure du tibia 3 après perchage correct de ce dernier. La coupelle 4 est tronquée de façon à laisser libre la partie arrière de l'extrémité supérieure du tibia en dégageant ainsi la partie supérieure légèrement hémisphérique de l'élément tibial 11 destiné à coopérer avec la prothèse condylienne 2 fixée d'une manière connue en soi sur la tête de fémur 1.Comme cela vient d'être décrit, la prothèse tibiale, formant le plan de rotation, est simplement encliquetée dans le plateau de blindage 4 qui forme une coquille aux deux tiers circulaire à fond plat et qui présente, sur sa face postérieure 4a, des stries parallèles ou cannelures 13 augmentant -ainsi l'ancrage du ciment orthopédique dans la liaison os-tibia-implant,
De même, l'élément tibial 11 présente sur sa face avant une rainure 14 pour l'ancrage du ciment orthopédi- que.
Les prothèses condyliennes et tibiales 2 et 5 étant réalisées le plus souvent en trois dimensions pour concorder avec la conformation du genou de chaque patient, la coquille ou plateau de blindage 4 est réalisée égale- ment en trois dimensions correspondant aux cotes de la prothèse. De même, les épaisseurs peuvent varier suivant la morphologie du patient.
L'ensemble plateau de blindage 4 es prothèse tibial 5 ainsi monté devient Triécaniquement souhaite el monobloc offrant au praticien chirurgien un ensemble cohérent d'une longévité remarquable et d'un emploi facile.

Claims (2)

REVENDICATIONS
1 - Prothèse de genou constituée par un élément condylien, un plateau ou c#oupelle de blindage destinés à maintenir un élément tibial en matière appropriée acceptable par exemple en polyéthylène de très haute densité, muni d'un organe de repérage radiographique, caractérisée en ce que le plateau ou coupelle de blindage(4)présente une forme en plan aux deux tiers circulaire dont le rebord périphérique a une hauteur variable et dont le fond comporte > d'une part, des stries (4a) coopérant avec le ciment orthopédique et,J'autre part, une fenetre longitudinale (7) destinée à recevoir les picots d'accrochage (8,9,10) adaptés pour accrocher l'élément tibial (11) à la coupelle de blindage (4) et à ancrer cet élément tibial dans la partie supérieure du tibia (3).
2 - Prothèse de genou suivant la revendication 1, caractérisée en ce que l'épaisseur de la coupelle de blindage varie suivant la morphologie du genou du patient et en ce que cette pièce (4) est réalisée en alliage d'acier -(316 L austénitique).
