Abstract

lmplant prothétique de l'articulation métatarsophalangienne du gros orteil, caractérisé en ce qu'il comporte: - une prothèse phalangienne (1), formée d'un piton (3), recouvert par un plateau (6) démontable, l'ensemble étant enfoncé ou vissé dans la phalange de l'orteil; - une prothèse métatarsienne (2), conformée en une surface gauche, ladite prothèse métatarsienne coiffant une zone osseuse du métatarse; - ces prothèses ayant leur surface externe respective en contact de manière à assurer l'articulation.

La présente invention est relative à un implant prothétique de l'articulation métatarsophalangienne du gros orteil.
On sait que de tels implants sont utilisés chez les patients atteints d'hallux rigidus ou d'arthrite rhumatoïde afin de reconstituer leur articulation, en lui redonnant de la mobilité sans présence de douleurs
Les implants connus comportent généralement un élément prothétique solidarisé à la phalange. La partie métatarsienne en regard doit être travaillée par le chirurgien pour s'adapter à la géométrie de l'implant prothétique utilisé.
Avec d'autres implants, le chirurgien dispose de deux pièces, I'une étant destinée à être enfoncée dans la phalange, I'autre devant être adaptée sur le métatarse. II s'agit souvent d'éléments en forme de plateau, dont l'application et la fixation définitives des surfaces de contact sur les parties osseuses nécessitent des interventions importantes et parfois des endommagements irréversibles de la phalange etlou du métatarse. Pour obtenir une fixation satisfaisante des éléments prothétiques, le praticien doit rechercher des zones saines, ce qui l'oblige parfois à surdimensionner les prothèses ou à implanter ces dernières dans des zones plus éloignées de l'articulation à reconstituer.
En outre, I'ensemble de ces dispositifs antérieurs se détériore relativement rapidement de par l'usure des pièces en contact due aux frottements, nécessitant leur Remplacement à plus ou moins long terme. Ces nouvelles interventions sont bien évidemment dommageables aux parties osseuses à traiter, et constituent une souffrance supplémentaire pour le patient.
La présente invention vise donc à pallier ces inconvénients en proposant un nouveau type d'implant prothétique de l'articulation métatarsophalangienne, dont seulement l'un des éléments doit être remplacé en cas d'usure, ce qui limite considérablement l'ampleur de l'intervention.
A cet effet, I'implant prothétique de l'articulation métatarsophalangienne du gros orteil, se caractérise en ce qu'il comporte: - une prothèse phalangienne, formée d'un piton, recouvert par un plateau démontable, I'ensemble étant enfoncé ou vissé dans la phalange de l'orteil; - une prothèse métatarsienne, conformée en une surface gauche, ladite prothèse métatarsienne coiffant une partie distale du métatarse; ces prothèses ayant leur surface externe respective en contact de manière à assurer l'articulation.
D'autres caractéristiques et avantages de la présente invention ressortiront de la description faite ci-après, en référence aux dessins annexés qui en illustrent un exemple de réalisation dépourvu de tout caractère limitatif.
Sur les figures:
- la figure 1 est une vue en coupe et en élévation frontale d'un implant prothétique de l'articulation métatarsophalangienne selon l'invention;
- la figure 2 est vue en coupe et de dessus de la figure 1.
Selon un mode préféré de réalisation de l'implant prothétique de l'articulation métatarsophalangienne du gros orteil, il comporte deux parties une prothèse phalangienne 1 et une prothèse métatarsienne 2.
La prothèse phalangienne 1 est constituée de deux éléments 3, 4 : une partie 3, en forme de piton notamment tronconique ou cylindrique, enfoncée ou vissée dans la phalange du gros orteil, et réalisée dans un matériau biocompatible. Ce piton, dans sa zone la plus large, est coiffé par une embase 5 de manière à recevoir l'autre partie de la prothèse phalangienne constituée par un plateau 6. Au droit de ce plateau et sur la face dirigée vers la phalange, on dispose sur cette zone une couche d'hydroxyapatite pour faciliter le contact entre cet élément et l'os de la phalange.
Cette deuxième partie constitue un plateau 6, avantageusement réalisé en matière plastique, notamment du polyéthylène, cette partie est rendue solidaire de l'élément tronconique par des moyens connus (sertissage, rivetage, assemblage du type tenon mortaise...).
La face externe du plateau 6 est plane sur toute sa surface selon un premier mode de réalisation, ou selon un deuxième mode de réalisation présente une zone concave.
Par ailleurs, en fonction des applications visées et de la morphologie du patient, on adaptera l'épaisseur du plateau 6.
La prothèse métatarsienne 2 comme son nom l'indique est destinée à être fixée dans le métatarse en regard de l'orteil. Cet élément est composé d'une partie métallique, notamment en titane. Cette partie dont le profil s'étend sous la forme d'une portion d'enveloppe hémisphérique, comprend principalement deux zones 8, 9
- une zone de contact 8 entre le plateau 6 de la prothèse phalangienne et la surface externe de ladite enveloppe couvrant la tête du métatarse;
- une zone d'accrochage 9 entre ladite enveloppe et la paroi latérale du métatarse.
Ainsi, pour permettre la fixation de la prothèse métartasienne sur l'os, on prévoit de disposer d'une part, dans une zone en dehors de la zone de contact 8, préférentiellement sur la paroi latérale 10 de ladite enveloppe, un orifice 11, éventuellement pourvu d'un lamage, pour le passage d'un organe de solidarisation 12 (vis, rivet...) et d'autre part, à l'intérieur de l'enveloppe hémisphérique et dans une zone située sensiblement au droit de la zone de contact 8, deux ergots 13 faisant saillie.
Selon une autre caractéristique avantageuse, ladite enveloppe est pourvue intérieurement, sur la face devant être positionnée en regard du métatarse, on dispose sur cette zone une couche d'hydroxyapatite pour favoriser la fixation et éviter tout glissement.
L'invention telle que décrite précédemment offre de multiples avantages:
- elle assure une excellente mobilité de l'articulation;
- le plateau de la prothèse phalangienne peut être changé, en cas d'usure, sans nécessiter le remplacement des autres éléments;
- l'épaisseur variable du plateau réalise une meilleure adaptation selon le patient à traiter, étant aisément adaptable;
- l'implantation de la prothèse phalangienne et de la prothèse métatarsienne ne nécessite pas de multiples interventions destructrices sur les parties osseuses;
- ces prothèses peuvent être aisément adaptées, même sur des patients souffrant d'hallux rigidus, ou d'arthrite rhumatoïde sérieuse.
II demeure bien entendu que la présente invention n'est pas limitée aux exemples de réalisation décrits et représentés ci-dessus, mais qu'elle en englobe toutes les variantes.

Claims (7)

REVENDICATIONS
1 - Implant prothétique de l'articulation métatarsophalangienne du gros orteil, caractérisé en ce qu'il comporte:
- une prothèse phalangienne (1), formée d'un piton (3), recouvert par un plateau (6) démontable, I'ensemble étant enfoncé ou vissé dans la phalange de l'orteil;
- une prothèse métatarsienne (2), conformée en une surface gauche, ladite prothèse métatarsienne coiffant une zone osseuse du métatarse; ces prothèses ayant leur surface externe respective en contact de manière à assurer l'articulation.
2 - Implant prothétique de l'articulation métatarsophalangienne du gros orteil selon la revendication 1, caractérisé en ce que la face externe du plateau (6) est plane sur toute sa surface.
3 - Implant prothétique de l'articulation métatarsophalangienne du gros orteil selon la revendication 1, caractérisé en ce que la face externe du plateau (6) présente une zone concave.
4 - Implant prothétique de l'articulation métatarsophalangienne du gros orteil selon l'une quelconque des revendications précédentes, caractérisé en ce que la prothèse métatarsienne (2) comprend principalement deux zones (8, 9):
- une zone de contact (8) entre le plateau (6) de la prothèse phalangienne (1) et la surface externe de ladite enveloppe couvrant la tête du métatarse
- une zone d'accrochage (9) entre ladite enveloppe et la paroi latérale (10) du métatarse.
5 - Implant prothétique de l'articulation métatarsophalangienne du gros orteil selon l'une quelconque des revendications précédentes, caractérisé en ce qu'on prévoit de disposer d'une part, dans une zone en dehors de la zone de contact (8), préférentiellement sur la paroi latérale (10) de ladite enveloppe, un orifice (11), éventuellement pourvu d'un lamage, pour le passage d'un organe de solidarisation (12) et d'autre part, à l'intérieur de l'enveloppe hémisphérique et dans une zone située sensiblement au droit de la zone de contact (8), deux ergots (13) faisant saillie.
6 - Implant prothétique de l'articulation métatarsophalangienne du gros orteil selon la revendication 1, caractérisé en ce qu'on dispose entre l'embase (5) du piton et sur la face en regard de la phalange, une couche d'hydroxyapatite.
7 - Implant prothétique de l'articulation métatarsophalangienne du gros orteil selon l'une quelconque des revendications précédentes, caractérisé en ce que l'enveloppe est pourvue intérieurement, sur la face devant être positionnée en regard du métatarse, d'une couche d'hydroxyapatite.
