Abstract

Prothèse d'articulation de genou comportant un ou plusieurs éléments composés d'oxyde d'alumine ou de zirconium, fixés sans ciment chirurgical.Le ou les surfaces en contact avec l'os sont recouvertes d'un dérivé phosphocalcique, appliqué pendant l'une des phases de traitement du ou des éléments en céramique, par dépôt d'une barbotine composée de phosphate de calcium et de liants thermorésorbables ou par application de poudre phosphocalcique sur la pièce enduite d'une barbotine adhésive thermorésorbable.

Description

La présente invention concerne une prothèse d'articulation de genou comportant un ou plusieurs éléments composés d'oxyde d'alumine ou de zirconium, fixés sans ciment chirurgical. Les prothèses de genou sont constituées classiquement . - d'un élément fémoral ancré dans l'extrémité inférieure du fémur, reproduisant le condyle fémoral, dans le cas d'un implant uni-compartimental, ou les condyles fémoraux et la trochlée de l'articulation naturelle, pour l'implantation d'une prothèse totale. L'ancrage de cet élément peut être favorisé par présence, d'un ou plusieurs plots et/ou d'un tige médullaire. - d'un élément tibial ancré par une tige médullaire et/ou des plots, dans l'extrémité supérieure du tibia, comprenant dans sa partie supérieure un plateau horizontal d'appui, d'une surface correspondant à l'ensemble de la surface osseuse tibiale, dans le cas d'une prothèse totale du genou, ou dans le cas d'une prothèse uni-compartimentale, d'une surface égale à environ la moitié de la surface tibiale supérieure, respectant le massif des épines. - d'un ou deux éléments intermédiaires comprenant, du côté de l'élément fémoral, une ou deux cavités glénoïdes recevant le ou les condyles fémoraux, et une face inférieure plane, venant en appui sur la face supérieure de l'élément tibial. Ce ou ces éléments peuvent être montés fixes ou mobiles sur l'élément tibial selon le type de prothèse. L'élément fémoral et/ou l'élément tibial peuvent présenter sur leurs faces en contact avec l'os, un revêtement favorisant la repousse osseuse et l'ancrage secondaire de ces pièces, afin d'éviter une fixation par ciment chirurgical. Ce revêtement peut être fait de microbilles, de micro fils, de madrépores et/ou d'un composé phosphocalcique, dont le plus courant est l'hydroxyapatite. Les éléments fémoral et tibial sont généralement métalliques, tandis que l'élément intermédiaire est en matériau favorisant le glissement et le roulement tel que le polyéthylène de haute densité. De plus, l'élément tibial, dans le cas d'une prothèse totale du genou avec conservation du ligament croisé antérieur et/ou du ligament croisé postérieur, présente une échancrure postérieure, laissant le passage du ou des ligaments conservés. Le ou les éléments intermédiaires respectent, également, le passage ce ou ces ligaments.
Une prothèse uni-compartimentale ou totale du genou, à plateau mobile, avec ou sans conservation du ou des ligaments croisés, peut présenter un ou plusieurs plots et/ou un plateau rapporté, assurant la mobilité de ou des éléments intermédiaires.
Ce type de prothèse, en raison des matériaux utilisés, présente l'inconvénient d'entraîner une usure importante de l'élément intermédiaire.
La présente invention vise à remédier à cet inconvénient, en fournissant une prothèse qui induit une usure minimale, tout en conservant une mobilité qui corresponde à celle de l'articulation naturelle, grâce à l'emploi de matériaux performants.
Cette prothèse est du type possédant - un élément fémoral globalement anatomique, extérieurement, reproduisant le condyle fémoral, dans le cas d'un implant uni-compartimental, les condyles fémoraux et la trochlée de l'articulation naturelle, pour l'implantation d'une prothèse totale, entièrement composé de céramique d'oxyde d'alumine ou de zirconium, présentant sur les surfaces en contact avec le fémur un revêtement de composé phosphocalcique de synthèse de type hydroxyapatite, déposé par enduction sur pièce prothétique, à l'état d'ébauche, après pression isostatique, ou après pré-frittage ou frittage, pour être implanté sans ciment chirurgical.

d'un élément tibial, également, entièrement composé de céramique d'oxyde d'alumine ou de zirconium, présentant, sur la surface en contact avec le tibia, un revêtement de composé phosphocalcique déposé selon la même technique pour être implanté sans ciment chirurgical. Selon l'invention, le revêtement phosphocalcique est appliqué par l'intermédiaire d'une barbotine à base du ou des composés phosphocalciques et d'un ou plusieurs liants thermorésorbables sur la pièce même, avant ou après frittage, ou stade de -frittage de ces pièces, sur les surfaces de contact avec le fémur et le tibia.
La fixation et la liaison du revêtement phosphocalcique sur les pièces sont assurées par frittage de l'ensemble.
Selon l'invention, l'élément tibial et/ou l'élément fémoral sont destinés à être implantés sans ciment, par une fixation primaire mécanique et un ancrage secondaire par ostéogenèse de l'os néoformé à l'intérieur des pores de la ou des surfaces de contact.
Ainsi dans la prothèse selon l'invention - le roulement et le glissement de l'élément fémoral sur le ou les éléments intermédiaires sont assurés par des matériaux à coefficient de frottement faible, minimisant l'usure de l'élément intermédiaire.
- le frottement, dans le plan horizontal, de l'élément intermédiaire sur l'élément tibial, dans le cas d'utilisation de prothèses à plateau mobile,est également diminué par l'emploi de ces matériaux.
Avantageusement, cette prothèse permet d'envisager l'utilisation couple de frottement céramique/céramique à base d'oxyde d'alumine pour les deux pièces en contact ou oxyde d'alumine et oxyde de zirconium.
Selon des modes particuliers de réalisation - l'élément fémoral et/ou l'élément tibial en alumine ou zirconium peuvent présenter une tige médullaire et ou plusieurs plots d'ancrage recouverts ou non de phosphate de calcium, assurant une fixation primaire par pénétration dans des cavités en regard creusées dans le fémur et/ou le tibia.
- le ou les surfaces en contact avec l'os peuvent être creusées de madrépores et/ou de stries de fixation.
- le dérivé phosphocalcique peut être déposé sous forme de poudre sur le ou pièces préalablement enduites d'une barbotine adhésive thermorésorbable. - des montages mixtes peuvent etre envisagés tels que - l'élément fémoral est alumine ou zirconium et l'élément tibial est métallique fixé ou non par du ciment chirurgical, l'élément intermédiaire étant ou non mobile.
- l'élément fémoral est métallique et l'élément tibial est en alumine ou zirconium, l'élément intermédiaire étant mobile ou fixe.

Claims (1)

  1. REVENDICATIONS 1- Prothèse d'articulation du genou composée d'un ou plusieurs éléments en oxyde d'alumine ou de zirconium possédant, sur les surfaces en contact avec l'os, un revêtement phosphocalcique, éléments destinés à être implantés sans ciment chirurgical; 2- Prothèse selon la revendication 1 caractérisée en ce que le revêtement phosphocalcique est déposé par enduction d'une barbotine composée ou plusieurs phosphates de calcium et d'un ou plusieurs liants thermorésorbables le ou les éléments concernés. La fixation et la liaison du revêtement au(x) dit(s) élément(s) sont assurées par frittage de l'ensemble. 3- Prothèse selon la revendication 1 caractérisée en ce que revêtement phosphocalcique est appliqué par poudrage sur une pièce enduite d'une barbotine adhésive thermorésorbable. La fixation et la liaison du revêtement à la pièce sont assurées par frittage de l'ensemble. 4- Prothèse selon la revendication 1 et l'une des revendications 2 ou 3 caractérisée en ce que le revêtement phosphocalcique est appliqué sur une pièce avant frittage. 5- Prothèse selon la revendication 1 et l'une des revendications 2 ou 3 caractérisée en ce que le revêtement phosphocalcique est appliqué sur une pièce pré-frittée- 6- Prothèse selon la revendication 1 et l'une des revendications 2 ou 3 caractérisée en ce que le revêtement phosphocalcique est appliqué sur une pièce frittée. 7- Prothèse selon la revendication 1 caractérisée en ce que l'élément en oxyde d'alumine ou de zirconium est l'élément fémoral. 8- Prothèse selon la revendication 1 caractérisée en ce que l'élément en oxyde d'alumine ou de zirconium est l'élément tibial. 9- Prothèse selon la revendication 1 caractérisée ce que les éléments en oxyde d'alumine ou de zirconium consistent en lélément fémoral et l'élément tibial. 10- Prothèse selon la revendication 1 caractérisée en ce que le ou les éléments qui la composent, comprennent un ou plusieurs plots et/ou une tige d'ancrage médullaire qui assurent une immobilisation primaire par pénétration dans des cavités en regard. 11- Prothèse selon la revendication 10 caractérisée ce que le ou les plots et/ou la tige médullaire d'ancrage sont recouverts de composé phosphocalcique. 12- Prothèse selon la revendication 1 caractérisée en ce que les surfaces de contact avec l'os sont creusées de madrépores et/ou de stries de fixation.
