Abstract

Cette prothèse articulée comprend au moins un élément fixe (12) destiné à être rendu solidaire d'un organe du corps humain, notamment d'un os, au moins un élément mobile (4) apte à se déplacer par rapport à cet élément fixe, un premier élément (4) choisi parmi le groupe constitué de l'élément fixe et de l'élément mobile, possédant une tête (8) reçue dans un logement (16) correspondant d'un élément de réception (12), en possédant au moins un degré de liberté en rotation par rapport à ce logement, et il est prévu des moyens permettant la solidarisation en translation de cette tête par rapport au logement dudit élément de réception, ces moyens de solidarisation en translation comprenant le pourtour du débouché (14) sensiblement rigide, dudit logement.

La présente invention concerne une prothèse articulée. L'invention vise plus particulièrement une telle prothèse qui, une fois implantée, relie deux partie du corps humain, entre lesquelles s'exercent des contraintes mécaniques. Il peut s'agir notamment, d'une part, du cotyle de la hanche ou de l'épaule et, d'autre part, du canal médullaire du fémur ou de l'humérus.
L'invention se propose de réaliser une prothèse articulée, dont la structure est simple, dont le montage est aisé et qui est implantée de façon fiable dans les organes qu'elle relie.
A cet effet, elle a pour objet une prothèse articulée, caractérisée en ce qu'elle comprend au moins un élément fixe destiné à être rendu solidaire d'un organe du corps humain, notamment d'un os, au moins un élément mobile apte à se déplacer par rapport à cet élément fixe, un premier élément choisi parmi le groupe constitué de l'élément fixe et de l'élément mobile, possédant une tête reçue dans un logement correspondant d'un élément de réception, en possédant au moins un degré de liberté en rotation par rapport à ce logement, et en ce qu'il est prévu des moyens permettant la solidarisation en translation de cette tête par rapport au logement dudit élément de réception, ces moyens de solidarisation en translation comprenant le pourtour du débouché sensiblement rigide, dudit logement.
Selon d'autres caractéristiques de l'invention : - la tête possède une surface extérieure sphérique apte à coopérer avec une surface intérieure sphérique correspondante du logement ; - la surface extérieure de la tête est munie de moyens d'introduction dans le volume intérieur dudit logement ; - les moyens d'introduction comprennent un méplat équatorial s'étendant à la périphérie de la surface extérieure sphérique de la tête ; - l'élément de réception est l'autre élément choisi parmi le groupe constitué des éléments mobile et fixe ; - l'élément de réception est un élément intermé-

diaire, distinct de l'élément mobile et de l'élément fixe - l'élément intermédiaire est reçu dans le volume intérieur de l'autre élément choisi parmi le groupe composé des éléments mobile et fixe, en possédant au moins un degré de liberté par rapport à cet autre élément, au moins en rotation ; - il est prévu des moyens de solidarisation en translation de l'autre élément par rapport à l'élément intermédiaire, ces moyens de solidarisation comprenant un pourtour rigide du débouché du volume intérieur de cet autre élément ; - la prothèse est destinée à être ancrée dans un os long, en particulier l'humérus ou le fémur, l'élément fixe étant destiné à être rendu solidaire du cotyle de l'épaule ou de la hanche, alors que l'élément mobile comporte une tige apte à être introduite dans le canal médullaire de l'humérus ou du fémur, ainsi qu'une tête prolongeant cette tige et apte à être introduite dans le logement de l'élément de réception.
L'invention va être décrite ci-dessous, en référence aux dessins annexés, donnés uniquement à titre d'exemples non limitatifs et dans lesquels : - la figure 1 est une vue de côté, avec coupe partielle, illustrant les différents éléments constitutifs d'une prothèse articulée conforme à l'invention ; - les figures 2 et 3 sont des vues analogues à la figure 1, illustrant deux étapes du montage de la prothèse de la figure 1 ; et - la figure 4 est une vue analogue à la figure 1, illustrant une variante de réalisation de la prothèse conforme à l'invention.
La figure 1 illustre une prothèse articulée conforme à l'invention, qui est une prothèse d'épaule. A titre de variante, l'homme du métier pourra réaliser une prothèse conforme à l'invention, dont le type est différent, par exemple une prothèse de hanche.
Cette prothèse 2, illustrée sur la figure 1, comprend un élément mobile, désigné dans son ensemble par la référence 4, qui comporte une tige 6, de profil connu, destinée à être

introduite dans le canal médullaire de l'humérus non représenté. Cette tige 6 est prolongée par une collerette 7, ainsi que par une tête 8 sphérique de diamètre d, qui est pourvue d'un méplat équatorial 10 s'étendant de façon inclinée. Ceci signifie que ce méplat n'est pas perpendiculaire à l'axe principal A de la collerette 7. La dimension transversale, ou largeur, de ce méplat 10, qui est notée d', est inférieure au diamètre d précité.
La prothèse illustrée à la figure 1 comprend également un élément fixe 12, destiné à être solidarisé au cotyle non représenté de l'épaule. Cet élément 12 possède une surface extérieure sphérique, ainsi qu'une surface intérieure sphérique, dont le diamètre est égal au diamètre extérieur d de la tête 8.
La surface intérieure de cet élément 12 s'étend selon un secteur angulaire supérieur à 1800, de sorte que la dimension transversale, ou largeur, du débouché 14 du volume intérieur 16 de l'élément 12, formant logement, est inférieure au diamètre d précité. Cette dimension transversale est égale à la largeur d'du méplat 10. En variante, elle peut également être légèrement supérieure.
Le montage de la tête 8 dans le logement 16 va être décrit maintenant en référence aux figures 2 et 3.
Il s'agit tout d'abord d'incliner la tête 8, de sorte que le méplat 10 s'étende au voisinage du débouché 14. On rapproche alors l'élément 4 par rapport à l'élément fixe 12, de manière à introduire la tête 8 dans le logement 16.
Le pourtour du débouché 14 est sensiblement rigide, c'est-à-dire qu'il est réalisé en un matériau non déformable, qui n'a pas subi de modification substantielle de sa géométrie depuis son usinage. A cet effet, l'élément mobile 10 peut être réalisé entièrement en un matériau rigide, notamment métallique. En variante, il peut être réalisé en un matériau déformable, tel que du polyéthylène, une bague rigide étant alors rapportée au voisinage de ce débouché.
L'emploi d'un tel débouché rigide est avantageux en termes de tenue mécanique de la prothèse. Cette tenue est en effet notablement supérieure à celle d'une prothèse faisant

appel à la déformation d'une embouchure, par exemple par sertissage ou encliquetage.
Ensuite, on fait à nouveau pivoter la tête 8 à l'intérieur du logement 16, de manière que le méplat 10 ne se trouve plus en regard du débouché 14. La tête 8 est alors libre de pivoter à l'intérieur de ce logement, mais ne possède aucun degré de liberté en translation par rapport à l'élément fixe 12. Ceci est dû au fait que la largeur du débouché 14 est inférieure au diamètre d de cette tête 8.
Une fois la prothèse dans sa configuration de la figure 3, on peut rapporter sur le méplat 10 un moyen de butée avantageusement amovible, telle qu'une vis 18. Cette dernière, en limitant le pivotement relatif de la tête 8 et de l'élément fixe 12, empêche cette tête de recouvrer sa position illustrée à la figure 2, ce qui évite toute désolidarisation intempestive entre les éléments mobile 4 et fixe 12.
A titre de variante, on peut prévoir que l'élément fixe, solidaire du cotyle, est pourvu d'une tête analogue à celle 8, alors que l'élément mobile comprend un logement de réception de la tête, analogue à celui référencé 16 sur les figures 1 à 3.
La figure 4 illustre une variante de réalisation de l'invention, dans laquelle il est fait appel à un élément intermédiaire 20.
L'élément fixe 112 possède une surface extérieure sphérique, ainsi qu'une surface intérieure sphérique, de diamètre D. Contrairement à l'exemple de réalisation de la figure 1, la surface intérieure de l'élément fixe 112 ne s'étend pas selon un secteur angulaire supérieur à 1800.
L'élément intermédiaire 20 possède une surface extérieure 22, définissant une portion de sphère tronquée, dont le diamètre est identique à celui D du logement 116 de l'élément fixe 112. Cet élément intermédiaire possède une surface intérieure 24 également sphérique, dont le diamètre est égal à celui d de la tête 8. Cette surface intérieure 24 s'étend selon un secteur angulaire supérieur à 1800, de sorte que la dimension transversale du débouché 26 du logement 28 de cet élément 20 est sensiblement égale à la dimension transversale

d'du méplat 10.
La solidarisation mutuelle en translation, de l'élément mobile 4 et de l'élément intermédiaire 20, s'opère de façon analogue à ce qui a été décrit en référence aux figures 1 à 3, à propos de la solidarisation des éléments mobile et fixe.
Par ailleurs, étant donné que la surface intérieure de l'élément fixe 112 ne s'étend pas selon un secteur angulaire de 1800, la surface extérieure de l'élément intermédiaire 20 est reçue dans le logement 116 en possédant des degrés de liberté, à la fois en rotation et en translation. En d'autres termes, l'élément intermédiaire peut être extrait du logement 116, quelle que soit l'orientation angulaire de l'élément intermédiaire 20 par rapport à l'élément fixe 112.
De la sorte, par exemple lors d'un choc d'une grande violence, l'élément intermédiaire 20 peut être désolidarisé de l'élément fixe, quelle que soit sa position. Ceci permet d'éliminer sensiblement tout risque d'arrachement de l'élément fixe 112, qui provoquerait une destruction des tissus adjacents du patient. L'agencement de la figure 4 convient tout particulièrement à une prothèse de hanche.
En variante, l'élément intermédiaire 20 peut être rendu solidaire en translation de l'élément fixe 112, comme dans l'agencement illustré dans la demande de brevet français NO 00 08521, déposée en date du 30 juin 2000.
Selon une variante supplémentaire, l'élément fixe peut être pourvu d'une tête, analogue à celle 8 illustrée sur les figures, coopérant avec l'élément intermédiaire 20. Les surfaces extérieures de ce dernier coopérent alors avec un logement ménagé dans l'élément mobile, qui est analogue à celui 16.
A titre de variante supplémentaire, on peut prévoir qu'au moins l'un des éléments décrits ci-dessus est fabriqué en un matériau composite.
Dans la description ci-dessus, il a seulement été fait mention d'un méplat équatorial 10, permettant l'introduction de la tête 8 dans le volume intérieur de l'élément fixe ou de l'élément intermédiaire. D'autres moyens d'introduction peuvent être prévus. On citera notamment, à titre d'exemple,

la présence de deux ergots disposés sur le pourtour du débouché du logement, coopérant avec deux joues ménagées à la surface extérieure de la tête sphérique.

Claims (9)

REVENDICATIONS
1. Prothèse articulée, caractérisée en ce qu'elle comprend au moins un élément fixe (12 ; 112) destiné à être rendu solidaire d'un organe du corps humain, notamment d'un os, au moins un élément mobile (4) apte à se déplacer par rapport à cet élément fixe, un premier élément (4) choisi parmi le groupe constitué de l'élément fixe et de l'élément mobile, possédant une tête (8) reçue dans un logement (16 ; 28) correspondant d'un élément de réception (12 ; 20), en possédant au moins un degré de liberté en rotation par rapport à ce logement, et en ce qu'il est prévu des moyens permettant la solidarisation en translation de cette tête par rapport au logement dudit élément de réception, ces moyens de solidarisation en translation comprenant le pourtour du débouché (14 ; 26) sensiblement rigide, dudit logement.
2. Prothèse articulée selon la revendication 1, caractérisé en ce que ladite tête (8) possède une surface extérieure sphérique apte à coopérer avec une surface intérieure sphérique correspondante du logement (16 ; 28).
3. Prothèse articulée selon la revendication 1 ou 2, caractérisée en ce que la surface extérieure de la tête (8) est munie de moyens (10) d'introduction dans le volume intérieur dudit logement (16 ; 28).
4. Prothèse articulée selon les revendications 2 et 3, caractérisée en ce que les moyens d'introduction comprennent un méplat équatorial (10) s'étendant à la périphérie de la surface extérieure sphérique de la tête.
5. Prothèse articulée selon l'une des revendications précédentes, caractérisée en ce que ledit élément de réception est l'autre élément (12) choisi parmi le groupe constitué des éléments mobile (4) et fixe (12).
6. Prothèse articulée selon les revendications 1 à 4, caractérisée en ce que l'élément de réception est un élément intermédiaire (20), distinct de l'élément mobile (4) et de l'élément fixe (112).

risée en ce que l'élément intermédiaire est reçu dans le volume intérieur (116) de l'autre élément (112) choisi parmi le groupe composé des éléments mobile et fixe, en possédant au moins un degré de liberté par rapport à cet autre élément, au moins en rotation.
8. Prothèse articulée selon la revendication 7, caractérisée en ce qu'il est prévu des moyens de solidarisation en translation de l'autre élément par rapport à l'élément intermédiaire, ces moyens de solidarisation comprenant un pourtour rigide du débouché du volume intérieur de cet autre élément.

Prothèse articulée selon l'une quelconque des revendications précédentes, caractérisée en ce qu'elle est destinée à être ancrée dans un os long, en particulier l'humérus ou le fémur, l'élément fixe (12 ; 112) étant destiné à être rendu solidaire du cotyle de l'épaule ou de la hanche, alors que l'élément mobile (4) comporte une tige (6) apte à être introduite dans le canal médullaire de l'humérus ou du fémur, ainsi qu'une tête (8) prolongeant cette tige et apte à être introduite dans le logement de l'élément de réception.
