Abstract
Description
Claims (17)
- REVENDICATIONS1. Instrument de guidage (1), pour guider au moins une broche de guidage (2) destinée à guider un implant, par exemple une vis creuse (4), pour la compression d'une articulation (6) entre un premier fragment osseux, par exemple une phalange (7), et un deuxième fragment osseux, par exemple un métatarse (8), l'instrument de guidage (1) comprenant au moins : - un organe d'appui (11) configtfré pour être placé en appui sur une portion dorsale (6.1) de l'articulation (6), et - un organe de guidage (12) qui est lié à l'organe d'appui (11) et qui présente au moins un passage (14) configuré pour guider une broche de guidage (2) ; l'instrument de guidage (1) étant caractérisé en ce que le passage (14) est configuré pour s'étendre suivant une direction oblique (0) par rapport à 15 l'organe d'appui (11) de façon à s'étendre transversalement à l'articulation (6) lorsque l'organe d'appui (11) est placé en appui contre la portion dorsale (6.1) de l'articulation (6), de sorte que ladite au moins une broche de guidage (2) peut traverser le premier fragment osseux, par exemple la phalange (7), et le deuxième fragment osseux, par exemple le métatarse (8), suivant une direction 20 oblique (0) par rapport à l'organe d'appui (11), et le passage (14) étant en outre configuré pour autoriser des déplacements de ladite au moins une broche de guidage (2) dans un plan de guidage (P14) incluant la direction oblique (0). 25
- 2. Instrument de guidage (1) selon la revedication 1, dans lequel le passage (14) est configuré pour autoriser des déplacements uniquement dans le plan de guidage (P14).
- 3. Instrument de guidage (1) selon l'une quelconque des 30 revendications précédentes, tiens lequel le plan de guidage (P14) est défini par la direction oblique (0) et par une direction planto-dorsale (Y).
- 4. Instrument de guidage (1) selon l'une quelconque des revendications précédentes, dans lequel la direction oblique (0) est orientée 35 vers une région centrale 6.Q) de l'articulation (6) lorsque l'organe d'appui (11) est placé en appui contre la portion dorsale (6.1) de l'articulation (6).
- 5. Instrument de guidage (1) selon l'une quelconque des revendications précédentes, dans lequel l'organe d'appui (11) présente sur son côté dorsal : un premier repère (11.1), par exemple un premier trait, destiné à être sensiblement aligné avec l'axe longitudinal (X8) du deuxième fragment osseux lorsque l'organe d'appui (11) est placé en appui contre la portion dorsale (6.1) de l'articulation (6), - un deuxième repère (11.2), par exemple un deuxième trait, 10 destiné à être sensiblement aligné avec l'axe longitudinal (X7) du premier fragment osseux lorsque l'organe d'appui (11) est placé en appui contre la portion dorsale (6.1) de l'articulation (6), un troisième repère (11.3), par exemple un troisième trait, destiné à être sensiblement aligné avec la ligne de fusion (6.5) de l'articulation 15 (6) lorsque l'organe d'appui (11) est placé en appui contre la portion dorsale (6.1) de l'articulation (6), et un quatrième repère (11.4), par exemple un quatrième trait, aligné avec la direction oblique (0). 20
- 6. Instrument de guidage (1) selon l'une quelconque des revendications précédentes, dans lequel le passage (14) a globalement la forme d'un rectangle en section dans le plan de guidage (P14).
- 7. Instrument de guidage (1) selon l'une quelconque des 25 revendications précédentes, dans lequel le passage (14) a une profondeur (D14), mesurée parallèlement à la direction oblique (0), comprise entre 6 mm et 10 mm.
- 8. Instrument de guidage (1) selon l'une quelconque des 30 revendications précédentes, dans lequel le passage (14) a une hauteur (H14) comprise entre 10 mm et 20 mm, de préférence comprise entre 10 mm et 15 mm.
- 9. Instrument de guidage (1) selon l'une quelconque des 35 revendications précédentes, dans lequel le passage (14) a une largeur (W14),mesurée perpendiculairement au plan de guidage (P14), qui est substantiellement constante le long de la direction oblique (0).
- 10. Instrument de guidage (1) selon l'une quelconque des 5 revendications précédentes, dans lequel l'organe d'appui (11) présente au moins : - un trou proximal (23) traversant configuré pour l'insertion d'une broche proximale (25) destinée à immobiliser l'organe d'appui (11) sur le deuxième fragment osseux, par exemple le métatarse (8), et 10 - un trou distal (24) traversant configuré pour l'insertion d'une broche distale (26) destinée à immobiliser l'organe d'appui (11) sur le premier fragment osseux, par exemple la phalange (7).
- 11. Instrument de guidage (1) selon l'une quelconque des 15 revendications précédentes, dans lequel l'organe d'appui (11) comprend une plaque proximale (21) et une plaque distale (22), la plaque proximale (21) et la plaque distale (22) étant agencées pour s'étendre respectivement sur une portion dorsale (8.1) du deuxième fragment osseux et sur une portion dorsale (7.1) du premier fragment osseux. 20
- 12. Instrument de guidage (1) selon la revendication 11, dans lequel la plaque proximale (21) a globalement une forme allongée suivant une direction proximale (X21) qui est parallèle à l'axe longitudinal (X8) du deuxième fragment osseux, la plaque distale (22) a globalement une forme allongée 25 suivant une direction distale (X22) qui est parallèle à l'axe longitudinal (X7) du premier fragment osseux, la direction proximale (X21) et la direction distale (X22) formant un angle (A21.22) compris entre 160 degrés et 180 degrés, de préférence compris entre 167 degrés et 173 degrés. 30
- 13. Instrument de guidage (1) selon l'une quelconque des revendications 11 à 12, dans lequel la direction oblique (0) forme avec la direction distale (X22) un angle (A14.22) compris entre 35 degrés et 70 degrés, de préférence compris entre 50 degrés et 60 degrés. 35
- 14. Instrument de guidage (1) selon l'une quelconque des revendications précédentes, comprenant en outre un organe de préhension (31) fixé à l'organe d'appui (11) de sorte que l'organe de préhension (31) permet à un opérateur de placer l'organe d'appui (11) en appui contre la portion dorsale (6.1) de l'articulation (6).
- 15. Instrument de guidage (1) selon la revendication comprenant en outre des moyens de fixation (32) configurés pour fixer de façon à amovible l'organe de préhension (31) sur l'organe d'appui (11).
- 16. Instrument de guidage (1) selon la revendication 15, dans 10 lequel les moyens de fixation (32) comprennent au moins : un plot 3) de forme globalement cylindrique à base circulaire, un alésage (34) configuré pour recevoir le plot (33), et un élément de friction configuré pour générer des frottements 15 entre le plot (33) et l'alésage (34) lorsque l'organe de préhension (31) est fixé sur l'organe d'appui (11), de sorte que l'organe de préhension (31) peut transmettre un couple à l'organe d'appui (11),
- 17. Instrument de guidage (1) selon l'une quelconque des 20 revendications précédentes, dans lequel une portion plantaire (11.6) de l'organe d'appui (11) présente un avivement (11.7).
