DESCRIZIONE DESCRIPTION
Il trovato in esame riguarda uno specifico dispositivo a uso chirurgico da impiegare nella gestione delle suture e nei trapianti muscolo/tendinei eseguiti per via mini invasiva e, in particolare, per il trasferimento del gran dorsale assistito in endoscopia. Come noto agli esperti del setore gli interventi sopra citati sono al momento molto delicati esponendo il paziente ad alcuni rischi. Il nervo ascellare è sicuramente tra le strutture più vulnerabili. Esso, infati, incrocia lo spazio attraverso il quale viene trasferita l’unità muscolo-tendinea quindi può essere danneggiato, in special modo se non visualizzato e se si utilizzano strumenti chirurgici appuntiti. Altro aspetto cruciale è la gestione delle suture che armano il trapianto: queste andrebbero protete da eventuali abrasioni durante il loro scorrimento e dovrebbero essere mantenute parallele in modo che non si incrocino e quindi non ruotino l’innesto; dovrebbero altresì poter essere rilasciate singolarmente così da fissare la prima coppia mantenendo le seconde (e il trapianto stesso) il più vicino possibile all’area in cui le stesse saranno successivamente assicurate. The present invention relates to a specific device for surgical use to be used in the management of sutures and in muscle / tendon transplants performed by a minimally invasive way and, in particular, for endoscopic assisted transfer of the great dorsal spine. As known to the experts in the field, the aforementioned interventions are currently very delicate, exposing the patient to some risks. The axillary nerve is certainly among the most vulnerable structures. In fact, it crosses the space through which the muscle-tendon unit is transferred, so it can be damaged, especially if not visualized and if sharp surgical instruments are used. Another crucial aspect is the management of the sutures that arm the transplant: these should be protected from any abrasions during their sliding and should be kept parallel so that they do not cross and therefore do not rotate the graft; they should also be able to be released individually so as to fix the first pair while keeping the second ones (and the transplant itself) as close as possible to the area in which they will subsequently be insured.
Allo scopo della presente domanda di brevetto appare superfluo descrivere i vari rimedi e accessori attualmente utilizzati per ovviare ai problemi citati in quanto il loro funzionamento è familiare a tutti i tecnici del settore, costituendo arte nota, e sono comunque inadeguati a risolvere le difficoltà prospettate. Scopo principale del presente strumento è quello di eliminare o, quanto meno, limitare gli inconvenienti sopra descritti fornendo a tutti i chirurghi del ramo un dispositivo specifico e dedicato, da impiegare nella gestione delle suture e nei trapianti muscolo/tendinei, con particolare riferimento al trasferimento del gran dorsale eseguito per via mini invasiva. A detto risultato si è pervenuti, in conformità dell’invenzione, adottando la soluzione tecnica di realizzare un sistema avente le caratteristiche descritte nelle rivendicazioni indipendenti. Altre particolarità della presente innovazione sono oggetto delle rivendicazioni dipendenti. I vantaggi che derivano dal trovato in esame consistono essenzialmente nel fatto che, adottando l’innovativo attrezzo che andremo a illustrare, si potranno eseguire con più facilità e sicurezza alcune operazioni chirurgiche riguardanti i trapianti muscolo/tendinei (trasferimento del gran dorsale) eseguiti per via mini invasiva. Questi e ulteriori vantaggi e caratteristiche della presente invenzione industriale saranno meglio e in maggior misura compresi da ogni tecnico del ramo dalla descrizione che segue e con l’aiuto degli annessi disegni dati quale esemplificazione del trovato e non vanno considerati in senso limitativo. - La Tavola 1 ci mostra l’oggetto in esame completo di tutte le parti di cui è composto. Nella Fig. 1 è rappresentato il particolare dispositivo (1) da impiegare nella gestione delle suture e nei trapianti muscolo/tendinei, e più in particolare il trasferimento del gran dorsale eseguito per via mini invasiva, con in primo piano: le cannule forate (2, 2B) solidali all’asta centrale (3), che le separa e raccorda nel contempo, il cui apice è convenientemente smussato così da evitare danni ai muscoli, nervi e vasi sanguigni quando funge da sonda; l’impugnatura sagomata (4) la cui finitura zigrinata migliora la presa anche se si indossano guanti bagnati e di conseguenza scivolosi. Nella Fig. 2 è visibile la superficie posteriore dell’impugnatura (4B) sulla quale sono evidenti due distinte zone cromatiche delle quali una perfettamente levigata (5) e l’altra (5B), invece, opaca così rifinite perché dovranno essere immediatamente identificabili dall’operatore che grazie a questo espediente potrà agevolmente prevenire l’involontaria rotazione dello strumento durante il suo uso impedendo, in questo modo, l’incrocio delle suture e quindi la conseguente rotazione del trapianto stesso. Nella Fig. 5 è nuovamente raffigurato lo specifico dispositivo (1C) introdotto attraverso un piccolo foro (10) definito correntemente “portale artroscopico posteriore”. Lo strumento, visualizzato grazie a un comune artroscopio, a sua volta introdotto in un differente “portale” (non visibile in figura), viene sospinto dall’operatore verso l’ascella (11). Questa fase chirurgica, eseguita fino a oggi a cielo aperto (estendendo l’incisione cutanea, muscolare e tendinea per tutto il tratto compreso tra il sito di definitiva fissazione del trapianto e l’ascella medesima), potrà essere evitata grazie ed trovato in oggetto. Esso, infatti, può raggiungere per via mini invasiva l’ascella (11), dove dovrà “caricare” il trapianto (9B), percorrendo l’intero spazio in modo sicuro per le strutture vasculo-nervose (12) che incrocia nel tratto, grazie alla sua particolare forma e all’assenza di margini taglienti, acuminati o abrasivi. Raggiunta l’ascella (11) il dispositivo (1C) emergerà attraverso l’incisione, eseguita in precedenza, per la preparazione dell’unità muscolotendinea del gran dorsale, venendosi a trovare nel medesimo campo chirurgico con il trapianto (9B), opportunamente armato, alla cui estremità emergono i capi delle suture (8E, 8F). Il successivo ed essenziale passo chirurgico vede un contributo fondamentale da parte del dispositivo (1, 1B, 1C) già citato in precedenza. Le suture (8E, 8F) che emergono dall’apice del trapianto (9B) sono facilmente trasportate all’interno delle cannule (21, 2L), con fili metallici asolati di comune impiego in chirurgia, per essere quindi avvolte attorno ai quattro elementi cilindrici (non visibili in figura) presenti sul dorso del manico dello strumento e infine bloccate. In tal modo il trapianto muscolo-tendineo è reso solidale allo strumento, che lo trae all’intemo dello spazio subacromiale (lo spazio non violato dalle incisioni chirurgiche che sono, viceversa, necessarie in chirurgia tradizionale aperta) e lo trascina in avanti fino a raggiungere l’area in cui il trapianto stesso, attraverso le proprie suture (8E, 8F), dovrà essere definitivamente fissato. Raggiunta l’area desiderata tali suture (8E, 8F) sono facilmente rilasciate sempre dai quattro elementi cilindrici (non visibili in figura) a cui erano state in precedenza bloccate. - Table 3 shows the present invention still in use so as to better understand its operation. The sutures (8E, 8F) that emerge from the apex of the graft (9B) are easily transported inside the cannulas (21, 2L), with slotted metal wires commonly used in surgery, to be then wrapped around the four cylindrical elements (not visible in the figure) present on the back of the instrument handle and finally blocked. In this way the muscle-tendon transplant is made integral with the instrument, which draws it into the subacromial space (the space not violated by the surgical incisions which are, conversely, necessary in traditional open surgery) and drags it forward until it reaches the area where the graft itself, through its sutures (8E, 8F), will have to be definitively fixed. Once the desired area has been reached, these sutures (8E, 8F) are always easily released from the four cylindrical elements (not visible in the figure) to which they had previously been blocked.
