Claims (1)
- 【特許請求の範囲】 1.血管内閉塞装置において、 外面と、基端と、前記基端から前記閉塞装置に進入する流体が内部に保持され るように閉鎖された先端とを有する、閉塞装置本体と、 前記本体の前記基端に設けられたコネクタと、 前記本体の前記外面に前記閉鎖された先端に隣接して設けられ、収縮状態と膨 張状態との間で膨張するように構成された、第一膨張可能部材と、 基端と、前記第一膨張可能部材に流体を流すように連結された先端とを有し、 前記本体内に形成された、第1膨張内腔とを有し、 前記閉塞装置は、前記第一膨張可能部材が収縮状態で血管の側壁に形成された 孔に挿入されるように構成され、その後、前記第一膨張可能部材を膨張させ、前 記第一膨張可能部材を引っ張って前記血管の内部に当接させ、前記血管を通って 流れる流体から前記側壁の孔を実質的にシールするように構成したことを特徴と する、血管内閉塞装置。 2.前記本体の前記外面に設けられた第二膨張可能部材であって、前記第二膨張 部材は収縮状態と膨張状態との間で膨張自在であり、前記第一膨張可能部材より も先端側に配置された、前記第二膨張可能部材と、 基端と、前記第一膨張可能部材に流体を流すように連結された先端とを有し、 前記本体に形成された、第二膨張内腔とを更に有し、 前記第二膨張可能部材は、前記第一膨張可能部材が破損した場合に前記血管内 で選択的に膨張させることができるように構成された、請求項1に記載の血管内 閉塞装置。 3.前記装置の前記本体の前記第一及び第二膨張可能部材の基端側に設けられた 、潅注孔と、 前記潅注孔から前記本体の前記基端まで前記本体を貫通した灌注内腔とを更に 有し、 前記装置の使用時に、前記潅注孔に付近に存在する流体をその場所から潅注す ることを可能とし、これによって手術場所の汚染を最小にするように構成さ れた、請求項2に記載の血管内閉塞装置。 4.前記装置の前記本体の前記第一膨張可能部材の基端側に設けられた、潅注孔 と、 前記潅注孔から前記本体の前記基端まで前記本体を貫通した、潅注内腔とを更 に有し、 前記装置の使用時に、前記潅注孔の付近に存在する流体をその場所から潅注す ることが可能であり、これによって手術場所の汚染を最小にするように構成され た、請求項1に記載の血管内閉塞装置。 5.前記装置の前記本体の前記第一及び第二膨張可能部材の基端側に設けられた 、第三膨張可能部材と、 前記第三膨張可能部材から前記本体の前記基端まで前記本体を貫通し、前記第 三膨張可能部材を膨張させる、第三膨張内腔とを更に有し、 前記第三膨張可能部材は、ステント状吻合装置を膨張させるために選択的に膨 張させることができるように構成された、請求項2に記載の血管内閉塞装置。 6.前記装置の前記本体の前記第一膨張可能部材の基端側に設けられた、第三膨 張可能部材と、 前記第三膨張可能部材から前記本体の前記基端まで前記本体を貫通し、前記第 三膨張可能部材を膨張させる、第三膨張内腔とを更に有し、 前記第三膨張可能部材は、ステント状吻合装置を膨張させるために選択的に膨 張させることができるように構成された、請求項1に記載の血管内閉塞装置。 7.血管の側壁に孔を形成するように構成された血管パンチ部材を更に有し、前 記パンチは、 一端に切断フランジが設けられた中空本体部材と、 前記中空本体部材内において伸長位置と引込み位置との間で摺動移動するよう に取り付けられた、ヘッド部材とを有し、前記中空本体部材は、前記閉塞装置の 前記先端の少なくとも一部と前記ヘッド部材とを内部に同時に受け入れるように 構成されている、請求項1に記載の血管内閉塞装置。 8.冠状動脈バイパス移植術の実施方法において、 第1端部及び第2端部を有する移植血管を提供する工程と、 閉塞装置を提供する工程であって、前記閉塞装置は、 外面と、基端と、前記基端から前記閉塞装置に進入する流体が内部に保持され るように閉鎖された先端とを有する、閉塞装置本体と、 前記本体の前記基端に設けられたコネクタと、 前記本体の前記外面に前記閉鎖された先端の付近に設けられ、収縮状態と膨張 状態との間で膨張するように構成された、第一膨張可能部材と、 基端と、前記第一膨張可能部材とに流体を流すことができるように連結された 先端とを有し、前記本体内に形成された、第一膨張内腔とを有し、 大動脈の側壁に切開部を形成する工程と、 前記閉塞装置本体の前記先端と収縮状態の前記第一膨張可能部材とを、前記切 開部を通して、前記大動脈に挿入する工程と、 前記第一膨張内腔を通して前記第一膨張可能部材に流体を供給し、前記第一膨 張可能部材を大動脈の内側で膨張させる工程と、 膨張させた前記第一膨張可能部材が大動脈の前記側壁の内側と接触するまで前 記閉塞装置本体を引込み、大動脈を通って流れる流体から前記切開部を実質的に シールする工程と、 前記閉塞装置本体の外部に前記移植血管を入れ子式に取り付ける工程と、 前記移植血管の第1端部を前記切開部に亘って大動脈の前記側壁に縫い付ける 工程と、 前記第一膨張可能部材から流体を抜くことによって前記第一膨張可能部材を収 縮させる工程と、 大動脈と前記移植血管とから前記閉塞装置本体を引抜く工程と、 を含むことを特徴とする、冠状動脈バイパス移植術実施方法。 9.前記移植血管をクランプして閉鎖することによって、前記移植血管を通る流 体の流れを阻止する工程を更に有する、請求項8に記載の冠状動脈バイパス移植 術実施方法。 10.前記閉塞装置本体が前記移植血管から完全に引抜かれる前に、前記移植血 管をクランプして閉鎖する工程を更に有する、請求項9に記載の冠状動脈バイパ ス移植術実施方法。 11.前記移植血管の前記第2端部を冠状動脈に縫付ける工程を更に有する、請 求項10に記載の冠状動脈バイパス移植術実施方法。 12.前記移植血管は、前記本体の前記先端を前記大動脈切開部に挿入する前に 、前記閉塞装置本体に入れ子式に受け入れられる、請求項8に記載の冠状動脈バ イパス移植術実施方法。
