Claims (1)
- 【特許請求の範囲】 1. 末梢動脈音の変化を監視するステップと、患者の生理学的状態の変化は末 梢動脈音の特定の変化が検出された時に発生したと決定するステップとからなる こと特徴とする患者の生理学的状態変化の非侵襲型検出方法。 2. 上記生理学的状態は心筋虚血、レム睡眠状態の兆候、および睡眠無呼吸の 一つであることを特徴とする請求項1記載の方法。 3. 測定位置に圧力を加えるステップを更に有し、該圧力は(a)該測定位置で 静脈滞留を実質的に起こさず、(b)該測定位置で制御できない静脈逆流を実質的 に起こさず、(c)該測定位置が心臓レベルより下にある時に該測定位置の動脈の 壁の緊張を閉塞を起こさず部分的に取り除くに十分であることを特徴とする請求 項1記載の方法。 4. 患者の健康状態の変化を検出するための装置において、 患者の指に適用し、その指の末梢動脈音を検知するためのプローブと、 該プローブからの信号を受信し、該指の末梢動脈音の変化を示す出力を提供す るプロセッサとを有し、該患者の生理学的状態あるいは医学的状態を示すことを 特徴とする装置。 5. 該プローブは、(a)該指に静脈滞留を実質的に起こさず、(b)該指の動脈の 壁の緊張を、閉塞を起こさず部分的に取り除くに十分な圧力を付与するための膜 を有することを特徴とする請求項4記載の装置。 6. さらに該指の血液容量の変化を監視するためのセンサを有することを特徴 とする請求項5記載の装置。 7. 該プローブはさらに加圧バンドを有することを特徴とする請求項4記載の 装置。 8. 末梢血管収縮を検知するため指に適用するプローブにおいて、該プローブ は、 該指の動脈の壁の緊張を閉塞を起こさず部分的に取り除くに十分な圧力を付与 するための膜と、 該指に静脈滞留を実質的に起こさないための手段とを有することを特徴とする プローブ。 9. 上記プローブはさらに該指の血液容量の変化を監視するためのセンサを有 することを特徴とする請求項8記載のプローブ。 10.患者の生理学的疾患の非侵襲型検出方法において、 指の最先端を含む、被験者の指の末端の周囲に静圧域を付与することを有し、 該静圧域は(a)該末端に静脈滞留を実質的に起こさず、(b)該末端に制御できない 静脈逆流を実質的に起こさず、(c)指が心臓レベルにある時に該末端の動脈の壁 の緊張を閉塞を起こさず部分的に取り除くに十分であることを特徴とする方法。 11.上記容量関連の変化は該末端の容量の変化を測定することにより測定され ることを特徴とする請求項10記載の方法。 12.上記容量関連の変化は該末端の光学密度の変化を測定することにより測定 されることを特徴とする請求項10記載の方法。 13.上記測定変化は患者の心筋虚血の存在を示すために監視されることを特徴 とする請求項10乃至12のいずれかに記載の方法。 14.上記測定変化は、レム睡眠の兆候および持続時間、また睡眠無呼吸の少な くとも一つを示すために監視されることを特徴とする請求項10乃至12のいず れかに記載の方法。 15.上記静圧域は約30mmHgから患者の心拡張期圧力より約10%上の範囲にあ ることを特徴とする請求項10乃至12のいずれかに記載の方法。 16.上記静圧域は、指の最先端を含み指の心臓に近い部分の周囲に付与するこ とを特徴とする請求項10乃至12のいずれかに記載の方法。 17.上記静圧域は、最も末端の指骨を少なくとも囲むエンドキャップと、隣接 した環状加圧バンドとを有するシンブル形状のプローブにより加えられることを 特徴とする請求項16に記載の方法。 18.末梢動脈音の変化を検出するプローブにおいて、 患者の指での静脈血液の滞留を防止するため、最も末端の指骨の両側に圧力を かけるように形成した圧力室を有する、患者の指を受け入れるエンドキャップと 、 上記指での静脈血液の滞留を防止するため、指の末端部分の前の指骨に圧力を かけるように形成した膜を有する加圧バンドと、 上記最も末端の指骨の動脈の脈動容量変化を検出するためのセンサとを有する ことを特徴とするプローブ。 19.さらに加温機構を有することを特徴とする請求項18記載のプローブ。 20.さらに垂直移動センサを有することを特徴とする上記請求項19記載のプ ローブ。 21.血圧測定装置を較正するための方法において、心臓に対して測定位置の垂 直位置を変更することにより動脈血管内の流体静力学的圧力の変化を導き出し、 監視すること、また対応する容量およびこの動脈血管の特徴に関連した他の容量 を測定し、この測定値と導き出した流体静力学的変更をプロットすることにより 測定動脈血管のコンプライアンス曲線の生成を特徴とする方法。 22.上記圧力室は変形可能な筒状膜により形成され、また上記プローブはプロ ーブ内にある指を押し出すのを避けるように、加圧された時に該膜の軸方向の動 きを防止する手段をさらに有することを特徴とする請求項18に記載のプローブ 。 23.上記変形可能な筒状膜は指の直径方向で対向する側に2つの部分に分ける ことを特徴とする上記請求項22記載のプローブ。 24.上記エンドキャップは複数の容器部分に分割され、各分割部分はそれに接 続した膜部分を有し、また該複数の容器部分は互いに接合されて該エンドキャッ プを形成することを特徴とする請求項22に記載の方法。 25.上記膜の軸方向への移動防止手段は、上記容器に固定し、該膜の軸方向で 内側への移動を抑制するように筒状膜内に入れられた中間部分を持つ抑制物を有 することを特徴とする請求項22記載のプローブ。 26.被験者の動脈血圧を測定する方法において、 該被験者の指の末端を所定温度に加温し該指の動脈血管を最大に膨張させるこ とと、 上記指の末端の血圧波を伴った容量関係の変化を測定することと、 該測定変化を動脈血圧に変換すること、から成ることを特徴とする動脈血圧測 定方法。 27.上記静圧域は、静脈血液の滞留を実質的に防止し、指を心臓レベル以下に した時に指の動脈を閉塞させることなく動脈壁の緊張を部分的にとるに十分であ るように、指の末端部の周りに加えることを特徴とする請求項26に記載の方法 。 28.上記測定した変化は血圧波を伴う指の末端部の容量の変化であることを特 徴とする請求項26若しくは27のいずれかに記載の方法。 29.上記測定した変化は血圧波を伴う指の末端部の光学密度の変化であること を特徴とする請求項26若しくは27のいずれかに記載の方法。 30.上記指の末端部の測定した変化は、 該指に所定の外部反対圧力として上記静圧域をかけることと、 上記被験者の心臓に対して複数の垂直位置に該指を垂直移動させることと、 上記被験者の心臓に対して該被験者の指の各垂直位置により該被験者の血圧に 加えた流体静力学的圧力を測定することと、 上記被験者の動脈血圧について該被験者の指の末端部の上記測定した変化を較 正するため、および該測定変化を動脈血圧に変換するため、上記外部反対圧力と 上記流体静力学的圧力測定値を利用すること、により動脈血圧に変換することを 特徴とする請求項27乃至29のいずれかに記載の方法。 31.被験者の末梢動脈音の変化を測定するためのプローブにおいて、 開口端と閉口端を有する堅い容器と、 上記容器の内側に設け、該容器の開口端に密封接続した柔軟な膜と、 上記容器の内側に設け、上記膜を該容器の内壁に対して押すように位置決めし た環状の加圧バンドと、 上記容器内部に設け、上記膜の内部にある指の押し出しを避けるために該膜を 膨張させた時に該膜の軸方向の移動を防止するように、該膜に対向して位置決め した概略U字状の抑制棒とを有することを特徴とするプローブ。 32.上記加圧バンドは上記膜を2つの密封した圧力チャンバに分割することを 特徴とする上記請求項31記載のプローブ。 33.光学センサをさらに有することを特徴とする上記請求項31記載のプロー ブ。 34.被験者の動脈脈動波を監視するための装置において、 該被験者の指の最先端を含む末端部の所定長さを受け入れるための筒状ソケッ トを含む圧力アプリケータと、 該被験者の指が上記筒状ソケット内に入れられた時に該指の末端部の周りに、 静脈血液の滞留を実質的に防止し、該指を心臓レベルにした時に該指の動脈を閉 塞させることなく動脈壁の緊張を部分的にとるに十分であるような静圧域を加え るための圧力源と、 血圧波を伴った上記指の末端部における変化を測定するための測定装置とを有 することを特徴とする装置。 35.上記測定装置は血圧波を伴う上記指の末端部の体積の変化を測定すること を特徴とする請求項34記載の装置。 36.上記測定装置は血圧波を伴う上記指の末端部の光学密度の変化を測定する ことを特徴とする請求項34記載の装置。 37.上記圧力アプリケータは、上記変化を測定する位置から所定距離だけ被験 者の心臓方向へ向けて延ばした上記指の末端部の周りに静圧を加えることを特徴 とする請求項34乃至36のいずれかに記載の装置。 38.上記圧力アプリケータは、 変形可能な筒状膜を内部に設け、筒状チャンバをその間に形成する容器と、 上記被験者の指が上記筒状膜内に入れられた時に該指の末端部を流れる血液流 の容量の変化により該筒状膜を変形させるために、該筒状膜に流体圧を加えるた めの流体圧力源とを有し、 上記変形可能な筒状膜は複数の部分に分割されていることを特徴とする、上記 請求項34乃至36のいずれかに記載の装置。 39.上記変形可能な筒状膜は、上記被験者の指が入れられた時に該指の直径方 向で対向する側に2つの部分に分割されることを特徴とする請求項38記載の装 置。 40.上記容器は上記膜の分割部分に対応した複数の部分に分割され、互いに接 合させて該膜の分割部分と上記筒状チャンバを形成することを特徴とする請求項 38記載の装置。 41.上記筒状膜は、上記容器に固定させ、かつ上記筒状膜内に入れた中間部分 を有した抑制棒により上記複数の部分に分割し、上記流体圧を該筒状膜に加える 際に各膜部分の外側周辺部の内部への移動を抑制するようにしたことを特徴とす る請求項38記載の装置。 42.上記圧力源は、30mmHgから約70mmHgの範囲の静圧域を加えることを特徴と する請求項34乃至41のいずれかに記載の装置。 43.上記装置はさらに、上記指の末端部の測定した変化を該被験者の動脈血圧 に変換するための変換装置を有することを特徴とする請求項34乃至41のいず れかに記載の装置。 44.上記変換装置は、 上記被験者の心臓に対して複数の垂直位置を通じて該指を垂直に移動させる時 の該被験者の血圧に加えた流体静力学的圧力を測定するための測定装置と、 該被験者の指の末端部の該測定変化を該被験者の動脈血圧について較正するた め、および該測定変化を動脈血圧に変換するため、上記静圧域と上記流体静力学 的圧力測定値を利用するコンピュータとを有することを特徴とする請求項43記 載の装置。 45.上記装置はさらに、上記被験者の指の動脈血管を最大膨張させるため所定 温度に該指を事前加温するためのプレヒータを有することを特徴とする請求項4 3記載の装置。 46.上記圧力アプリケータは被験者が着用する手袋の少なくとも1本の指に組 み込まれ、該手袋の指の中に入れた該被験者の手の指の末端部の血圧波を伴う変 化を測定することを特徴とする請求項34記載の装置。 47.被験者の血流力学的状態を監視するための装置において、 上記被験者の指の動脈血管を最大膨張させるため所定温度に該被験者の指の末 端部を加温するためのヒータと、 血圧波を伴った上記指の末端部の変化を測定するための測定装置とを有するこ とを特徴とする装置。 48.上記指の末端部の周りに、静脈血液の滞留を実質的に防止し、該指を心臓 レベルにした時に該指の動脈を閉塞させることなく、動脈壁の緊張を部分的にと るに十分な静圧域を加えるための圧力アプリケータをさらに有することを特徴と する請求項47に記載の装置。 49.上記測定装置は血圧波を伴う上記指の末端部の体積の変化を測定すること を特徴とする請求項47若しくは48のいずれかに記載の装置。 50.上記測定装置は血圧波を伴う上記指の末端部の光学密度の変化を測定する ことを特徴とする請求項47若しくは48のいずれかに記載の装置。 51.被験者の心肺疾病を検出するための装置において、 該被験者の指の最末端を含む末端部の所定長さを受け入れるための筒状ソケッ トを含む圧力アプリケータと、 該被験者の指が上記筒状ソケット内に入れられた時に該指の末端部の周りに、 静脈血液の滞留を実質的に防止し、該指を心臓レベルにした時に該指の動脈を閉 塞させることなく動脈壁の緊張を部分的にとるに十分な静圧域を加えるための圧 力源と、 血圧波を伴った上記指の末端部における変化を測定するための測定装置と、 上記測定変化が上記被験者の心肺疾病がない時の通常の既知状態と比較した際 に、所定パーセントを超える時に、心肺疾病の存在を知らすためのインディケー タとを有することを特徴とする装置。 52.睡眠中の被験者を監視するための装置において、 該被験者の指の最先端を含む末端部の所定長さを受け入れるための筒状ソケッ トを含む圧力アプリケータと、 該被験者の指が上記筒状ソケット内に入れられた時に該指の末端部の周りに、 静脈血液の滞留を実質的に防止し、該指を心臓レベルにした時に該指の動脈を閉 塞させることなく動脈壁の緊張を部分的にとるに十分な静圧域を加えるための圧 力源と、 血圧波を伴った上記指の末端部における変化を測定するための測定装置と、 上記被験者の睡眠中の状態を示すため上記測定変化を利用するインディケータ とを有することを特徴とする装置。 53.上記測定装置は血圧波を伴う上記指の末端部の体積の変化を測定すること を特徴とする請求項51若しくは52のいずれかに記載の装置。 54.上記測定装置は血圧波を伴う上記指の末端部の光学密度の変化を測定する ことを特徴とする請求項51若しくは52のいずれかに記載の装置。 55.上記圧力アプリケータは、30mmHgから約70mmHgの範囲の圧力を加えること を特徴とする請求項51若しくは52のいずれかに記載の装置。 56.上記圧力アプリケータは、上記変化を測定する位置から所定距離だけ被験 者の心臓方向へ向けて延ばした上記指の末端部の周りに静圧を加えることを特徴 とする請求項51若しくは52のいずれかに記載の装置。 57.上記圧力アプリケータは、 変形可能で被験者の指の末端部を受け入れるための筒状膜を内部に設け、筒状 外チャンバをその間に形成する容器と、 上記被験者の指が上記筒状膜内に入れられた時に該指の末端部の血圧波を伴っ た該指の体積の変化により該筒状膜を変形させるために、該筒状外チャンバに静 圧域を加えるための圧力源とを有し、 上記変形可能な筒状膜は被験者の指がその中に入れられた時に該指の周りで複 数の部分に分割され、該部分の各々は中央領域と外周部とを含み、該各部分の外 周部は内側への移動を抑制されており、該筒状外チャンバ内の圧力が該筒状膜の 各部分の中央領域をその外周部より大きく内側に移動し、それにより複数の膜部 分の内側に移動した中央領域を装置本体にしっかり押しつけて、身体の末端に対 する装置の軸方向運動および回転運動に対し被験者の指が抗することを特徴とす る請求項51若しくは52のいずれかに記載の装置。 58.上記変形可能な筒状膜は、上記被験者の指が入れられた時に該指の直径方 向で対向する側に2つの部分に分割されることを特徴とする請求項57記載の装 置。 59.上記容器は上記膜の分割部分に対応した複数の部分に分割され、互いに接 合させて該膜の分割部分と共に上記筒状外チャンバを形成し、該膜の分割部分の 各々はその外周端において、それぞれ容器の外周端に固着させたほぼ4面の膜で 構成することを特徴とする請求項57記載の装置。 60.上記筒状膜は、上記容器に固定させ、かつ上記筒状膜内に入れた中間部分 を有した抑制棒により上記複数の部分に分割し、上記流体圧を該筒状外チャンバ に加える際に各膜部分の外側周辺部の内部への移動を抑制するようにしたことを 特徴とする請求項57記載の装置。 61.上記末梢動脈音の変化を監視するステップは、皮膚動脈の血液流の変化を 監視することを含む請求項1記載の方法。 62.上記末梢動脈音の変化を監視するステップは、指の動脈の血液流の変化を 監視することを含む請求項1記載の方法。 63.上記静圧域は70mmHgであることを特徴とする請求項10乃至12のいずれ かに記載の方法。 64.上記圧力アプリケータは70mmHgの圧力を加えることを特徴とする請求項3 4乃至41、及び51,52のいずれかに記載の装置。
