Abstract
【解決方法】赤血球採取および濾過のための方法および装置を提供する。赤血球採取組立体は、赤血球分離および採取手順と同時に或いはそのすぐに白血球減少濾過を行うようになっている。このような手順は、分離済み赤血球を、保存溶液でフィルタから流出させた後および/またはこれに先立って希釈状態で濾過することを含む。かくして、保存溶液は、RBCが濾過される前、RBCと共に及び/またはBCが濾過された後に白血球減少フィルタに通される。赤血球採取/濾過/保存組立体は好ましくは、例えば遠心容器およびドナーから血液を取出すための血液取出し/戻し組立体を有する血液成分分離用使い捨て組立体に予め連結され、血液を遠心容器に通し、血液を諸成分に分離し、分離された赤血球成分を採取し、濾過するようになっている。

Description
本出願は2001年12月10日に出願された(本文と一致しない程度に全体的に参照することによりここに組み入れられる)米国特許出願第60/339653の優先権を請求するものである。
血漿交換方法の始動する種々の他の作動は好ましくは中でも以上に挙げた特許に記載のように行われる。しかしながら、一般に、ここでは図7、図8および図9の概略図を特定的に参照して、並びに図1ないし図5を連続参照して、或る基本特徴を説明する。濾過およびRBC採取は、使用した添加剤溶液組立体および特定の処理器を除いて、図11、図12および図13の変更実施例の場合とほぼ同じである。
前記装置2を利用してドナー4に対する血漿交換手順を行うための1つの好適なプロトコルを以下に要約する。初めに、オペレータが使い捨てプラスチック組立体8を血液成分分離装置6に装填する。このプロトコルに従って、オペレータは種々のバッグ{例えば、採取バッグ954(使用されるなら、94);図7参照、以下に更に説明する}を血液成分分離装置6の夫々のフック(例えば、図7のフック996を参照)に吊るす。1つのバッグを使用する場合、オペレータは次いでカセット組立体110を装置6における遠心ロータ組立体568に設けられている場合のチャンネルハウジング204内の装置6および/または血液処理容器352に装填する。
Claims (22)
- 血液を少なくとも1つの高いヘマトクリット値の赤血球成分に分離して前記高いヘマトクリット値の赤血球成分を採取し且つ白血球減少するための血漿交換装置用の使い捨て組立体であって、
ドナー/患者から血液を取出すための血液取出し組立体と、
前記血液取出し組立体に連結されており、且つドナー/患者から受入れられた血液を高いヘマトクリット値の赤血球を含む血液成分に分離するための分離装置と共同の血液処理容器と、
前記血液処理容器に連結された赤血球採取組立体とを備えており、この赤血球採取組立体は、
前記血液処理容器から高いヘマトクリット値の赤血球を受入れるように配置された白血球減少フィルタと、
前記白血球減少フィルタに連結された赤血球採取容器とを備えており、
かくして、赤血球採取組立体は、前記高いヘマトクリット値の赤血球を前記白血球減少フィルタに押し通し、前記高いヘマトクリット値の赤血球を前記白血球減少フィルタに押し通した後に前記高いヘマトクリット値の赤血球を前記採取容器に採取するようになっている使い捨て組立体。 - 前記採取容器は、更に前記白血球減少フィルタへ高いヘマトクリット値の赤血球の押し通しの完了後、溶液を前記白血球減少フィルタを通して流し、前記高いヘマトクリット値の赤血球との混合のために前記溶液を前記保存容器に採取するようになっている請求項1に記載の使い捨て組立体。
- 前記溶液は、保存溶液である請求項2に記載の使い捨て組立体。
- 前記溶液は、塩水である請求項2に記載の使い捨て組立体。
- 前記溶液は、前記白血球減少フィルタへの前記高いヘマトクリット値の赤血球の押し通し中、前記白血球減少フィルタを通って流れ、それにより前記溶液を前記高いヘマトクリット値の赤血球と混合し、前記溶液および前記高いヘマトクリット値の赤血球を前記赤血球採取容器に採取する請求項2に記載の使い捨て組立体。
- 保存溶液容器と、
前記保存溶液容器と前記赤血球採取容器との間に連結されている第1配管路と、
保存溶液を前記第1配管路を通して前記赤血球採取容器に送るために前記第1配管路を開放するための、前記第1配管路に設けられたクランプとを更に備えている請求項1に記載の使い捨て組立体。 - 前記赤血球採取容器に連結されていて、保存溶液容器に選択的に連結可能である第1配管路と、
保存溶液を通すように前記第1配管路を前記保存溶液容器に連結するための、前記第1配管路に設けられたスパイクコネクタとを更に備えている請求項1に記載の使い捨て組立体。 - 前記赤血球採取容器に連結されていて、保存溶液容器に選択的に連結可能である第1配管路を更に備えており、
前記第1配管路には、前記第1配管路を、保存溶液を圧送するようになっているポンプと関連させるためのポンプループが形成されている請求項1に記載の使い捨て組立体。 - 前記赤血球採取容器に連結されて前記赤血球採取容器から空気を受入れるための空気除去バッグを更に備えている請求項1に記載の使い捨て組立体。
- 前記赤血球採取容器は、第1赤血球採取容器であり、前記赤血球採取組立体は、前記白血球減少フィルタに連結され且つ前記白血球減少フィルタの後に配置されている第2赤血球採取容器を更に備えており、前記使い捨て組立体は、更に、高いヘマトクリット値の赤血球を前記分離装置から前記白血球減少フィルタに通し、且つこれを通して前記第1および第2赤血球採取容器に同時に通すようになっている請求項1に記載の使い捨て組立体。
- 前記赤血球採取容器は、第1赤血球採取容器であり、前記赤血球採取組立体は、前記白血球減少フィルタに連結され且つ前記白血球減少フィルタの後に配置されている第2赤血球採取容器を更に備えており、前記使い捨て組立体は、更に、高いヘマトクリット値の赤血球を前記分離装置から前記白血球減少フィルタに通し、且つこれを通してまず前記第1および第2赤血球採取容器のうちの一方に、次いで前記第1および第2赤血球採取容器のうちの他方に通すようになっている請求項1に記載の使い捨て組立体。
- 前記赤血球採取容器は、第1赤血球採取容器であり、前記赤血球採取組立体は、第2赤血球採取容器および第2白血球減少フィルタを備えており、前記第2白血球減少フィルタは、前記第2赤血球採取容器と連通して配置されており、前記使い捨て組立体は、更に、高いヘマトクリット値の赤血球を前記分離装置から前記第1および第2白血球減少フィルタに同時に通し、かくして前記第1および第2赤血球採取容器に同時に通すようになっている請求項1に記載の使い捨て組立体。
- 前記赤血球採取容器は、第1赤血球採取容器であり、前記赤血球採取組立体は、第2赤血球採取容器および第2白血球減少フィルタを備えており、前記第2白血球減少フィルタは、前記第2赤血球採取容器と連通して配置されており、前記使い捨て組立体は、更に、高いヘマトクリット値の赤血球を前記分離装置から前記第1白血球減少フィルタに通してまず前記第1赤血球採取容器に、次いで前記第2白血球減少フィルタに通して前記第2赤血球採取溜め部に入れるようになっている請求項1に記載の使い捨て組立体。
- 血小板を採取しようとするときに、前記カセット組立体に連結されて、分離された血小板を受入れるための血小板採取バッグを備えている請求項1に記載の使い捨て組立体。
- 血漿を採取しようときに前記カセット組立体に連結される血漿採取バッグを備えている請求項1に記載の使い捨て組立体。
- 前記血液取出し組立体と、前記流入組立体と、前記処理容器と、前記赤血球採取組立体との間でそれらに連結されているカセット組立体を備えており、前記カセット組立体は血液および血液成分を通すための一体の流体通路を備えている請求項1に記載の使い捨て組立体。
- 血液を少なくとも1つの高いヘマトクリット値の赤血球成分に分離し、この高いヘマトクリット値の赤血球成分を採取し且つ白血球減少させるための使い捨て組立体であって、
ドナー/患者から血液を受入れるための血液取出し組立体と、
前記血液取出し組立体に連結されていて、受入れられた血液の流入流れを通す流入組立体と、
受入れられた血液の流入流れを受入れるために前記流入組立体に連結されていて、ドナー/患者から受入れられた血液を高いヘマトクリット値の赤血球を含む血液成分に分離するための分離装置と共同の血液処理容器と、
前記血液処理容器に連結されている赤血球採取組立体とを備えており、この赤血球採取組立体は、
前記血液処理容器から高いヘマトクリット値の赤血球を受入れるように配置されている白血球減少フィルタと、
前記白血球減少フィルタに連結されている赤血球採取容器とを備えており、
それにより、前記高いヘマトクリット値の赤血球は前記血液処理容器への受入れられた血液の流入流れにより前記血液処理容器から前記白血球減少フィルタを通して押出されるようになっている使い捨て組立体。 - 前記血液取出し組立体と、前記流入組立体と、前記処理容器と、前記赤血球採取組立体との間でそれらに連結されているカセット組立体を備えており、前記カセット組立体は血液および血液成分を通すための一体の流体通路を備えている請求項17に記載の使い捨て組立体。
- 前記流入組立体は更に、
前記カセット組立体および前記血液処理容器に連結されている流入管路を備えており、
前記カセット組立体は更に、
流入ポンプと協働するようになっている流入配管ループと、
前記流入配管ループおよび前記流入配管路に連結されている一体の流体通路とを備えており、それにより受入れられた血液は前記流入ポンプの圧送作用下で前記流入配管ループ、カセットおよび流入管路を通って流れて分離された赤血球を血液処理容器から前記白血球減少フィルタを通して押出すようになっている請求項18に記載の使い捨て組立体。 - カセットに設けられ、保存溶液を前記白血球減少フィルタを通して圧送するためのポンプと協働するようになっている配管ループを更に備えている請求項19に記載の使い捨て組立体。
- 前記流入組立体は更に、
前記血液処理容器に連結されている流入配管路を備えており、
前記使い捨て組立体は更に、
流入ポンプと協働するようになっている流入配管路を備えており、それにより、受入れられた血液は前記流入ポンプの圧送作用下で前記流入配管ループおよび流入管路を通って流れて分離された赤血球を血液処理容器から前記白血球減少フィルタを通して押出すようになっている請求項18に記載の使い捨て組立体。 - 保存溶液を前記白血球減少フィルタを通して圧送するためのポンプと協働するようになっている溶液廃刊ループを更に備えている請求項18に記載の使い捨て組立体。
