Abstract
Description
本発明は、敗血症患者において呼吸不全、腎不全、又は血小板減少のリスクを予測するための方法及びキットに関する。
敗血症は、重度の感染に合併し、全身性炎症及び広範な組織損傷により特徴付けられ、従って、非冠動脈集中治療室(ICU)における入院及び死亡の主要な原因を表す臨床的な症候群である。一般的な原因は、グラム陰性生物、ブドウ球菌、及び髄膜炎菌を含む。末端臓器不全及び死亡の漸進的に増加したリスクを伴う炎症反応における3つの認識された段階がある:敗血症、重症敗血症、及び敗血症性ショック。重症敗血症は、少なくとも1つの臓器での不全の徴候を伴う敗血症である。心血管不全は、典型的には、低血圧、低酸素血症による呼吸不全、乏尿による腎不全、及び凝血異常による血液不全により顕在化する。
本発明は、敗血症患者において呼吸不全、腎不全、及び血小板減少からなる群より選択される臓器不全を有するリスクを予測するための方法に関し、前記敗血症患者から得られた血液サンプル中のエンドカンの濃度を測定することからなる工程を含む。
本発明者らは、血液エンドカンレベルが、敗血症患者において、呼吸不全、及び/又は腎不全、及び/又は血小板減少を予測するためのツールを表すことを示す。
高レベルの血液エンドカンによって、呼吸不全(Pa02/Fi02<200により定義される)、及び/又は腎不全(クレアチニン>20mg/mLにより定義される)、及び/又は血小板減少を伴う敗血症患者が選択される(表1)。
序論:
21人の患者及び9人の正常なボランティアが、本試験においてプロスペクティブに登録されている。全ての患者が、リール大学病院の集中治療室から来た。組み入れ基準は、SCCM/ACC分類に従った、重症敗血症又は敗血症性ショックを伴う患者であった。非組み入れ基準は、年齢<18歳及び妊娠であった。除外基準は、非敗血症由来の敗血症性ショック、及びICUにおける入院前1ヶ月での免疫抑制治療であった。
エンドカン血漿レベルは、重症敗血症(3,96±3,35ng/mL)において、及び、敗血症性ショック(4,33±5,01ng/mL)において、健常対照(0,67±0,25ng/mL)に対して増加した(p<0.05)(図1)。

- 敗血症患者から得られた血液サンプル中のエンドカンの濃度を測定することからなる工程を含む、敗血症患者において呼吸不全、腎不全、及び血小板減少からなる群より選択される臓器不全を有するリスクを予測するための方法。
